[The status of immunity in the victims of industrial catastrophes and natural disasters].

The influence of industrial catastrophes or calamities upon human organism is accompanied by the affection of the immunity system. The structure and manifestations of immunity disorders depend on nature of the disturbing factors in every concrete catastrophe. The affections of the immunity system may lead to the formation of secondary immunodeficiency which will require additional medical assistance.

[Tuberculosis and pneumonia].

Analysis of the clinical course of acute pneumonia in pulmonary tuberculosis patients is presented. The lowest rate of acute pneumonia morbidity was found in patients with active tuberculosis during antituberculosis therapy. As soon as the specific process subsides and its activity diminishes acute pneumonia morbidity grows. Specific features of the course of acute pneumonia in patients with different phases of tuberculosis development are shown. Data on the study of liquid-crystal thermography are presented as well as the assessment of the capillary blood flow and the results of immunologic study in the presence of combined pathology. The terms of alleviation of acute pneumonia in tuberculosis patients are delayed, especially in inflammation development in the zones of post-tuberculosis changes.

[Studies on the strain on drivers during high-speed driving].

In modern industrial countries the technological progress has shifted the occupational stress to the psychomental sphere. In the course of this alteration the number of work places with driving, controlling, and monitoring increased considerably. On 10 engine-drivers during high-speed driving from 210 to 250 km/h heart rate and electrocardiogram were monitored continuously. The blood pressure was measured intermittently. The elimination rate of catecholamines in urine, and the levels of cyclic AMP and catechol-O-methyltransferase in plasma were determined in reference to the trial to quantify the psychomental strain by biochemical parameters. In consequence of the individual affective conditions of the engine-drivers it has been attempted to scale the subjective strain.

[Dyspepsia and Helicobacter pylori infection. A study of a population of workers].

We studied 102 workers in a mechanical factory. All subjects were tested by a questionnaire screening dyspeptic symptoms in the last three months, their frequency, absenteeism period, spontaneous drugs intake and a consultant gastroenterologist request. All subjects were tested for specific IgG against Helicobacter pylori with immunoassay Elisa. Dyspepsia was present in 61% of cases. Positivity for Helicobacter pylori infection was in 76%, absenteeism in 82%, consultant request in 21% of dyspeptic workers. Our results suggest, according to literature, the high frequency of Helicobacter pylori infection in dyspeptic patients: furthermore dyspepsia should be considered as an important cause of absenteeism.

[Plasmodium falciparum travels by train from Ethiopia to Djibouti].

To investigate the role of the Djibouti-Ethiopian railway as a potential vehicle for inter-regional spread of malaria vectors and malaria parasites, we performed a double-sided study, both entomological and parasitological, during November 1989, at the frontier post of Guelile where the trains from Ethiopia enter the Republic of Djibouti. No malaria-transmitting mosquitoes were collected either from the daily passenger train or from the weekly vegetables train. One hundred and five passengers entering Djibouti by train from Ethiopia had a thick film examined for malaria parasites. Five smears were positive for Plasmodium falciparum, among them two showed gametocytes. We conclude that the railway may be an effective route for the propagation of the human malaria parasite between Ethiopia and Djibouti. Indeed, passengers infected abroad could import plasmodia into Djibouti and thus become the index cases for local malaria outbreaks, in case the climatic and entomological prerequisites essential for sustaining malaria transmission are present.

[Organization of inpatient care on mass admission of children with burns suffered in a train crash].

The article discusses some experience in organizing medical care for children injured during the crash in Bashkiria in June, 1989. Twenty-eight children with extensive Degrees II--IIIa, IVb burns were treated in the burn department of Children's Clinical Hospital No. 9, Moscow; 24 of them were discharged in a satisfactory condition, 4 children with a burn of more than 50% of body surface died. Simultaneous admission of the injured during a crash creates organizational, therapeutic, and psychological difficulties. The authors consider it necessary to create a special reserve of equipment, instruments, and drugs in the leading therapeutic centers, to train some of the general surgeons in problems of treatment of burn injuries, and engage, without fail, a psychoneurologist in the treatment.

[Epidemiological features and causes of railway traffic accidents].

This article reports on epidemiological features of railway traffic accidents and factors influencing traffic safety. An analysis of various kinds of railway traffic accidents over 30 years in Nanjing Railway Branch, Showed that railway traffic accidents have become a sort of modern social disease, and had particular epidemiological features. The peak of the epidemic curve appeared cyclically and had a close relation to social disturbances, Frequency of accident occurrence was the highest in the first season (period of spring transport) and the third season (period of high temperature) of the year thus most accidents happen in February and August. Most accidents occurred on Fridays and least on Mondays. Distribution of accidents had obvious "antenna" phenomenon in round graph which had a 24-hour cycle. Analysing the multiple factors influencing traffic safety, the results showed that the upper-limit-age of a train driver should not be more than 50 years old; The phenomenon "bathtub" between personage accident rate and age must be taken seriously. More attention should be paid to the psychological aspects when recruiting train drivers. In our country, it is urgent to set up standard of psychomovement function for choosing train locomotive drivers. Fatigue was one of the direct causes resulting in accident occurrences. No statistical correlation was found between biorhythm and accident occurrence.

Finding of a bullet in the cervical column of a body hit by a train.

This case is about the finding of a body of an unidentified male of approximately 70 years of age who was hit by a train. During the carrying out of the corresponding autopsy and after the radiological tests for posterior odontological identification, a foreign body of a cylindrical-cone shape, identical to that of a bullet, was found lodged between the first and second cervical vertebrae. During dissection of the neck, a bullet was found. When the ballistic test was carried out, it was ascertained that the bullet was from a cartridge of 7.92 by 57-mm Mauser caliber, manufactured in Spain in 1936. This ammunition corresponds to that used during the Civil War in Spain (1936-1939). After the identification of the body, it was proved that the bullet was the result of a war wound. However, the victim had been unaware of the existence of the bullet, which had remained in his body for 50 years.

[Several years of observation of the incidence of Trichomonas vaginalis infections among female workers in a single industrial plant].

During 5 years cycle a prophylactic gynaecological, cytological and parasitological examinations of 1918 women were performed. 12.8% of women were infected with T. vaginalis. Decreasing percentage of infections was observed: from 9.92% in 1984 to 2.2% in 1988. In the I. of Papanicolaou cervical smears T. vaginalis has been found in 1.60% of cases, in the II.--in 5.2%, in the III.--in 14.29%, and in the IV.--in 22.22% of cases. In women with normal cervix uteri T. vaginalis have been found in 5.01% of cases, and in women with hypertrophied cervix and partially epidermized ectopy with ovula nabothi in 15.28% of cases.
