One thousand cases of prostatectomy.

A survey of one thousand transvesical prostatectomies was conducted for studying occasional complications. An improved technique is recommended for the removal of adenomas, 30-40 g in weight or above. The number of complications showed a decline which, together with the possibility to reduce the operating and nursing periods, is attributed to some outstanding features of the method such as the use of continuous running haemostatic synthetic sutures for constricting the edges of the prostatic bed, the use of a three-bore catheter with its balloon located inside the bladder, rinsing the bladder system, ambulation on the first postoperative day, the application of absorbable synthetic sutures and the early removal of the catheter.

[Determination of carcinoembryonic antigen in urine and in gastric secretions].

The diagnostic significance of the identification of carcinoembryonic antigens in the serum, urine and gastric secretions of patients with malignant neoplasms is considered. In the subjects tested, CEA levels in the serum and gastric secretion were certainly diagnostically indicative. Increases in urinary CEA levels were however minimal, even lower than those encountered during urinary inflammations.

[Urinary tract infection in patients with calculi].

The authors review a series of 2,475 cases of patients operated for renal calculi and select 843 files in which the bacteriological study was performed. E. coli is the most common organism (29.8%) compared with Proteus which infects 21% of patients. 71.5% of the patients were infected. Bilateral calculi are the most heavily infected. Kidneys containing calculi which were subjected to a complex operation involving the parenchyma (nephrotomies) were the most difficult to disinfect, even in the absence of recurrence. Conversely, in the absence of recurrence, simple operations (pyelotomy, partial nephrectomy) provide disinfection in the majority of cases. In the authors' series, E. coli remains the organism most frequently encountered, even in cases of recurrence. Proteus and Pyocyanicus are the most difficult organisms to eradicate.

[Urogenital tuberculosis - is it still of topical interest? (author's transl)].

The only slowly regressive fraction of the extrapulmonary forms of tuberculosis are due to the long latent period between the primary infection and the organic manifestations. Recently, shortterm programs which require further clinical trial confront the established longterm chemotherapy carried out increasingly with outstanding success as an outpatient therapy. The support of an effective drug therapy by corticosteroids in the exudative florid initial stage of urogenital tuberculosis for the prevention of specific strictures with threatened organic loss continues to be given. In the indication for timely operative measures in the stabilization phase, the immune status should also be included as well as the morphological criteria.

The importance of microscopic examination of the urinary sediment.

Urinalysis may provide evidence of significant renal disease in asymptomatic patients. The microscopic urinalysis is vital to making diagnoses in many asymptomatic cases, including urinary tract infection, urinary tract tumors, occult glomerulonephritis, and interstitial nephritis. Three-hundred-fifty-three consecutive urinalyses were performed in our Urinalysis Laboratory to discern the incidence of significant urinary findings not disclosed by macroscopic examination. One-hundred-seventy-two urines had negative macroscopic exams, but 52 of these had positive microscopic findings, an incidence of 30% (52/172). Furthermore, 15% (52/353) of all urines studied revealed significant urinary findings microscopically. An adequate examination of the urine must include a microscopic as well as a macroscopic examination.
