[Computed tomography and magnetic resonance in the preoperative staging of the spread of rectal cancer. A correlation with the anatomicopathological aspects].

In order to assess the accuracy of CT and MRI in evaluating the local extent of rectal carcinoma, 22 patients with a rectoscopic diagnosis of rectal cancer were examined before surgery with both techniques. Tumor extent into the perirectal fat, involvement of surrounding structures and local adenopathy were evaluated on both CT and MR images. These findings were compared with the micro- and macrohistologic findings obtained from the surgical specimens. As far as perirectal fat infiltration is concerned, the diagnostic accuracy of CT was 73% and that of MRI was 71%. Sensitivity was 100% for CT and 91% for MRI, while specificity was low for both techniques (45% for CT and 50% for MRI). In evaluating local adenopathy, CT accuracy was 82% and MR accuracy was 90%, with 89% sensitivity for both techniques and 77% specificity for CT and 92% for MRI. The overall accuracy was 59% for CT and 67% for MRI. In agreement with literature data, our study shows a similar accuracy for CT and MRI in evaluating the local extent of rectal carcinomas. Moreover, our results confirm the value of macrohistology in the local staging of rectal carcinoma and its prognostic value, as well as the low specificity of both CT and MRI in evaluating lesion spread into the perirectal fat.

[Diagnostic guidelines in diarrhea].

Acute diarrhea is usually short-lasting; therefore, diagnostic procedures are mainly concerning the degree of dehydration. With longer duration of high fever or bloody diarrhea, microbiologic stool tests are necessary. Proctosigmoidoscopy is indicated in case of dysenteric disease or suspected antibiotic-as-associated pseudomembranous colitis. In chronic diarrhea, the most important diagnostic procedure is a careful history. Side effects of drugs and food-related causes are especially noticeable, as are indices of an organic origin, e.g. unwanted weight loss or blood in the stools. Also, careful history and physical examination are essential for the decision about laboratory tests, tests of gastrointestinal function or endoscopy.

[Initial experience with transanal endoscopic microsurgery].

The authors present their initial experience with transanal endoscopic microsurgery which they introduced as the first department in the Republic. So far they used the method in 10 patients. The purpose of the present paper is to provide information on the surgical approach to adenomas and carcinomas in the area of the rectum by a new surgical technique.

Small, polypoid-appearing carcinoid tumors of the rectum: clinicopathologic study of 16 cases and effectiveness of endoscopic treatment.

Fifteen cases of small-size rectal carcinoid tumors (RCTs) were discovered among a total of 21,522 healthy teachers who received proctosigmoidoscopy. Adding another case seen in the outpatients, a total of 16 RCTs were clinicopathologically studied. The average age of the patients was 48.8 yr, and the incidence was predominantly among males. Prior to endoscopy, digital examination of the rectum revealed a palpable firm nodule in 13 patients. On endoscopy, RCTs were generally round, yellow-discolored polyps with a size less than 13 mm in diameter, covered by a normal-appearing mucosa. An erythematous change or depression was seen in three tumors. Histologically, they showed pure insular, trabecular, or mixed structures, and tumor cells were confined to the mucosa and/or submucosa in all cases. Immunohistochemically, RCTs never failed to show focal or diffuse positivities for chromogranin A and/or neuron-specific enolase. Pancreatic polypeptide was immunostained in 14 tumors to a varying degree. The tumors having been safely treated by endoscopic polypectomy and/or surgical excision, all of the patients are alive and clinically free of disease during the average observation period of 79 months.

[Indications and reliability of rectoscopy (author's transl)].

With reference to 935 rectoscopies carried out from October 1973 to December 1974, the importance of this endoscopic method for the diagnosis of all anorectal conditions is drawn to the attention. In almost 10% of the patients investigated we found a tumorous change. Passing of blood from the rectum is the most frequent (43%) indication for rectoscopy. 40% of the rectal carcinomata diagnosed by rectoscopy were palpable with the finger.

[Amelanotic melanoma of the rectum].

We report the very rare case of an amelanotic anorectal melanoma. Until now there are only three other well documented cases of that tumor-type reported in the literature. The course of disease is described from initial symptoms till 10 months after discharge from hospital and suggestions for staging, therapy and follow-up are made.

Adenomyosis: a déjà vu?

Adenomyosis is a relatively frequent finding in series of hysterectomies performed for menorrhagia and dysmenorrhea. Evident selection biases of the available studies on adenomyosis have always limited the possibilities of defining the real clinical importance of the condition. Until now the only certain diagnoses have been made by histopathologists on uteri removed at surgery, but recently various sufficiently accurate techniques have been suggested which allow diagnosis on the uterus in situ. With the these methods it might be possible to obtain correct information on the epidemiologic characteristics of adenomyosis and to clarify whether it has a pathogenic role in unexplained ovulatory menorrhagia and juvenile dysmenorrhea. Furthermore, resectoscopic treatment has been proposed in some mild forms of adenomyosis to avoid hysterectomy, whereas it seems improbable that medical treatment can offer a definitive solution. The adoption of standard histologic criteria for adenomyosis seems important. Until this is done, it will be difficult to establish whether adenomyosis is really a disease or merely a paraphysiologic condition.

[Role of the intracavitary irridiation in the curative treatment of cancer of the rectum. 170 cases].

In carcinoma of the rectum occurring in patients in whom surgery represents a high risk, the problem of local treatment is worthy of discussion in certain precisely defined cases. Intracavitary radiotherapy (contact radiotherapy, interstitial curietherapy) proved to be effective in a series of 170 patients, observed over a period of five years. The death rate due to carcinoma was less than 11 per cent. This simple, ambulatory method may be used in fragile and very elderly patients and is without danger. Surgery is still possible in the case of failure but irradiation can be used only in selected patients, in whom very close follow-up is possible.

[Value of rectoscopy in the diagnosis of Schistosoma mansoni bilharziasis. Study at Nkolbisson-Cameroon].

The aim of this study was to investigate endoscopically and histologically rectal mucosa complications in bilharzia patients. Coprology using KATO's technique was carried out in 1406 school children aged 4 to 18 years. 134 (9.53%) were infested. Among the infested school children and the sample population, a random selection of 80 school children (40 infested and 40 of sample population) was made, in whom rectoscopy and biopsy of rectal mucosa were carried out. The lesions found were classified on endoscopic and histologic basis. The prevalence of the disease 9.53% appears to be in constant regression. Inflammatory lesions are more common in bilharzia patients. Histology facilitated the unveiling of a specific rectitis in 72.5% bilharzia patients and 12.5% of the sample population. This study confirm the effectiveness of the several bilharzia eradication campaigns carried out in the Nkolbisson area. Rectoscopy with biopsy of the rectal mucosa should be reserved to symptomatic patients, in whom many coprologies have turned out negative.

[Strengths and weaknesses of endorectal ultrasonography].

Endorectal ultrasound is the most reliable method in staging rectal cancer and is superior to computed tomography. In a prospective series of 152 consecutive patients, comparison of preoperative ultrasound staging and postoperative histopathological staging resulted in an overall accuracy of 90.1%. Overstaging was observed in 9.2%, understaging in 0.7%. Peritumoral inflammatory changes, preoperative radiotherapy and localization of the tumor in the lower rectum were the main reasons for overstaging. Understaging was seen in stenotic or only minimal invasive tumors. Accuracy in staging lymph nodes was 78.5%. Inflammatory changes in the nodes were responsible for either overstaging or understaging. Whether diagnosis will be improved with a higher frequency--mainly in lymph nodes--has yet to be proved. With knowledge of the reasons for misinterpretation, endorectal sonography is--mainly in the lower rectum--a valuable aid in evaluation of surgical procedure.

[Transanal endoscopic microsurgery for local excision of rectal neoplasms].

We removed 30 benign, sessile, rectal polyps by the transanal approach between January 1990 and April 1994. In 16 patients we used transanal endoscopic microsurgery (TEM), while in 14 the adenoma was removed by submucosal excision (SE). There were 3 local recurrences in the SE group, but none as yet in the TEM group. There was no operative mortality. Early complications included myocardial infarction in 1 patient and persistent postoperative fever which responded to antibiotic treatment in another. Late complications included temporary, anal mucous leakage in 10% and 5% of the TEM and SE groups, respectively. TEM was found to be efficient for the removal of polyps in the upper and middle thirds of the rectum, and SE for those in the lower third. This enables safe removal of rectal polyps, avoiding the need for complex operations involving greater risks.
