[Carcinoembryonic antigen in the evaluation of the effectiveness of surgical treatment in cancer of the rectum].

The carcinoembryonic antigen content (CEA) was studied in 163 patients who had been subjected to surgical treatment. The content of antigen in blood plasma was determined by the radioassay. It is shown that a high pre- and postoperative CEA level may be indicative of the spread of the neoplastic process, nonradical character of the treatment, and an unfavourable prognosis.

Liver vitamin A concentration in patients who died of cancer.

The concentrations of vitamin A in liver samples obtained at autopsy of patients who died of cancer was investigated. It was found that liver vitamin A levels in cancer patients were significantly lower than in controls. Although our recent results have suggested that the decreased serum retinol level in cancer patients reflects rather a functional impairment of vitamin A release from the liver, the results of this study suggest that this phenomenon can also indicate a true vitamin A deficiency in patients with neoplastic process.

Oncogenic osteomalacia. A case report.

Oncogenic osteomalacia is a rare condition characterized by the development of pain and fractures in a patient with specific laboratory abnormalities consisting of hypophosphatemia, hyperphosphaturia, and decreased 1,25-OH vitamin D levels. The clinical scenario is completed by the association of this osteomalacic state with the finding of a neoplastic process in the afflicted patient. The authors report a patient in whom the diagnosis of oncogenic osteomalacia was established and treatment begun despite the fact that the associated tumor (benign undifferentiated tumor of meschymal origin) escaped detection for many months. Following discovery of the tumor and identification by magnetic resonance imaging, the patient was cured by surgical resection.

Antineoplaston A in cancer therapy. (I).

Twenty-one patients with advanced cancer or leukemia were treated with antineoplaston A and followed for up to nine months. Dosage by intravenous, intramuscular, subcutaneous, rectal, intrapleural, intravesical and/or topical administration ranged from 0.6 to 33 U/m2/24 h. Treatment was well tolerated, although side effects included fever of short duration and elevation of platelet and white blood count. In 18 cases some degree of clinical improvement was observed. Complete remission occurred in 4 cases. More than 50% remission occurred in 4 other cases which, along with another 6 cases, are continuing the treatment with high doses of antineoplaston A and show a continuing regression of the tumors although not yet achieving the criteria for complete remission; 2 of these 6 cases seem unlikely to achieve remission. Two patients temporarily discontinued treatment. During treatment, 5 patients expired; in 2 of them, however, was seen significant regression of the neoplastic process. The deaths were not due to cancer or to any toxicity incurred by the treatment.

[Intestinal metaplasia and fungus involvement in peptic and neoplastic lesions of the mucosa of the stomach].

260 ulcers and 128 cancers were found by means of stomach biopsies. Analyzing the coincidence of ulcer resp. cancer with intestinal metaplasia or fungus contamination about double the frequency of linkage to intestinal metaplasia (26% versus 15%) and likewise to fungus contamination (13% versus 7%) could be ascertained.--The figures in the combinations of cancer resp. ulcer with fungus infection and IM (4% cancer; 2% ulcers) were strikingly low. This is to be explained by an increased immunologic defense function of the intestinalised stomach mucosa.--Following demands of practice any neoplastic process with ulcerated fungi-contaminated lesion of stomach mucosa has to be safely excluded.

[Computerized tomography findings in annular pancreas in an adult].

We present a case of annular pancreas in an adult female studied by computerized tomography. CT demonstrated an enlarged pancreatic head in the center of which was visualized the contrast-filled duodenal lumen. We consider these findings to be sufficiently characteristic of annular pancreas to permit differential diagnosis with neoplastic processes of the pancreatic head without having to resort to more invasive diagnostic methods like endoscopic retrograde cholangiography.

Disseminated adenocarcinoma with ocular involvement in a cat.

Disseminated adenocarcinoma was associated with unilateral uveitis and multiple subcutaneous nodules in an 8-year-old domestic shorthair cat. Erythroleukemia and hind limb paresis developed over a 5-month period. At necropsy, numerous sites of metastases were found that had histologic features similar to the neoplasm in the left eye. Neoplastic cells were identified in the lungs, mediastinum, muscles, subcutaneous tissues, lymph nodes, and vasculature associated with the meninges of the brain and spinal cord. Morphologic associations were not found between erythroleukemia and the neoplastic process.

[The pathology of Meckel's diverticulum].

Sixteen cases of Meckel's diverticulum are reported, 14 of them detected during emergency operative intervention for acute surgical abdomen and 2 during planned operation. The complicated forms of Meckel's diverticulum were 10: acute diverticulitis 4 (25 per cent), intestinal obstruction 4 (25 per cent), neoplastic process 1 (6.25 per cent), intestinal hemorrhage 1 (6.25 per cent). Symptom-free were 43.75 per cent of the diverticuli. The clinical symptoms of acute diverticulitis resembled much those of acute appendicitis. The method of choice for its surgical management is cuneiform resection of the intestinal wall at the site of the diverticulum and transverse reconstruction of the intestinal lumen. Cancer of the diverticulum requires extended intestinal resection.

[The clinical characteristics of women with uterine hemorrhages in premenopause].

The clinical characteristics was studied on 431 women with uterine bleedings during the premenopaussal phase of the climacterium. The author established that menarche, duration of menstrual interval and the menstruation itself before the bleeding did not differ from those of female population in the country. The same was referred to the reproductive characteristics. Premenopausal bleeding reached its peak between 46 and 48 years of age as its duration was relatively great. It was connected with frequent neuro-endocrine and metabolic pathology. The most frequent manifestations were obesity and hypertension. The number of the former hepatitis patients was comparatively large. Diseases of the uterine body were frequent in the structure of genital pathology. The frequency of benign, precancerous and malignant neoplastic processes (without myoma) was high-12.7%.

[Circulating immune complexes in the blood serum of patients with untreated cervical cancer--their occurrence and quantitative analysis].

Circulating immunological complexes (KI) are one of the parameters of humoral immunity. The occurrence of KI was found in the course of neoplastic diseases both in animals and humans. They are formed as a result of interaction of neoplastic antigens with specific tumor antibodies. Immunological complexes may be favourable or unfavourable to the modulation of effector mechanisms directed against neoplastic cells. In spite of extensive research, the participation of KI in immunological neoplastic response has not been clearly defined. The quantitative analysis of KI in the serum of patients with neoplasms revealed that the levels of KI may correlate with the degree of the progression of the disease, which has been shown for example in malignant melanoma, embryonal neuroma and carcinoma of the breast. The investigation of KI in the serum of patients with cervical carcinoma is rarely reported. The present paper shows 71 patients with squamous epithelial cervical carcinoma in various stages of progression, patients not receiving therapy. The evaluation of KI was made by means of two methods: test of inhibition of forming rosettes EA and inhibition of forming rosettes EAC. The authors of the few reports on the subject agree (which was also confirmed in our research) that the serum of patients with cervical carcinoma reveals the presence of KI. The frequency of occurrence and levels of KI may correlate with the degree of progression of a neoplastic process.

[Occurrence of circulating immune complexes in the blood serum of patients with cervical cancer prior to treatment].

In the blood serum of 71 patients before treatment with cervical carcinoma and in 100 healthy women from the health services working staff, the occurrence of circulating immunologic complexes (CI) has been estimated applying the rosette EA and EAC inhibition test. The presence of CI has been proved and the frequency of positive results increases together with the stage of advancement of the neoplastic process. It appears that the CI examination should be regarded as a significant parameter in monitoring the course of treatment.

[Mucus formation in epithelial derivations of Müller's canal as exemplified by mucinous cancer of the endometrium].

Analysis of the structural features of mucinous adenocarcinoma of the endometrium and of a large reference group including the hyperplastic and neoplastic processes in the endometrium, specimens of human embryos and fetuses, enabled the authors to come to a conclusion that mucus production and deposition in the endometrial cell cytoplasm is embryogenetically predetermined, though it is restricted in health. During a pathological hyperplasia and malignant degeneration the capacity of such production may recover and induce the formation of the foci of endocervical metaplasia as well as a mucinous adenocarcinoma of the endometrium. The existence of this nosological entity may serve as an argument in favour of the ovarian mucinous tumors origination from the epithelium of the Mullerian canal.

Local reaction to gingival injections of MER/BCG in guinea pigs.

The local reaction to gingival injections of methanol extraction residue of BCG (MER/BCG) was investigated in guinea pigs to help determine the potential of this agent in treating oral carcinoma. The drug was used in a standard and diluted form. Both concentrations were well tolerated and caused no ulceration or necrosis. Histological examinations performed at predetermined intervals showed an inflammatory response to the standard and the diluted solution. This reaction was more pronounced when the higher concentration was used. No changes in the alveolar bone were found in either of the groups. Complete healing with scar formation was evident four to six weeks later. The absence of severe reactions after injections of MER should encourage further investigations of this agent in the local treatment of neoplastic processes in the oral cavity.

Calicoblastic neoplasms in Acropora palmata, with a review of reports on anomalies of growth and form in corals.

Colonies of the elkhorn coral (Acropora palmata), possessing raised, whitened, irregularly shaped skeletal protuberances, were discovered at Carysfort Reef and Grecian Rocks, Key Largo National Marine Sanctuary, Key Largo, FL. These lesions exhibited relatively rapid growth and spread along the branches, as polyps were overlaid by coenosteal skeletal material. Histopathological examinations of the soft tissues surrounding and extending into the skeletal masses revealed proliferation of gastrovascular canals and associated calicoblastic epidermis, with loss of normal polyp structures and zooxanthellae. The slightly atypical tumor calicoblasts were cuboidal to columnar, resembling those found in the rapidly growing apical tips. Stable carbon isotope ratios of skeletal samples revealed that the tumor skeleton was isotopically lighter than the skeleton in the normal or apical track regions, indicative of higher tissue metabolic rates and lack of carbon isotope fractionation by zooxanthellae. This condition appears to be a neoplasm of the coral, and it is proposed that it be classified "calicoblastic epithelioma." Several different types of abnormal skeletal deposition, possibly the result of neoplastic processes, have been reported to occur in stony corals and are here reviewed.

Watery diarrhea, hypokalemia, achlorhydria syndrome in an infant: effect of the long-acting somatostatin analogue SMS 201-995 on the disease and linear growth.

An 8-week-old infant presented with vomiting and failure to thrive due to small bowel obstruction caused by a diffusely enlarged pancreas. Surgical bypass of the obstruction was followed by secretory diarrhea, hypokalemia, and dehydration. Plasma vasoactive intestinal peptide (VIP) (823pg/ml), pancreatic polypeptide (4,500 pg/ml), and neurotensin (680 pg/ml) concentrations were markedly elevated. No neoplastic process was identified. Therapy with the long-acting somatostatin analogue SMS 201-995 was followed by decline in VIP concentrations (900 to 200-300 pg/ml), decrease in stool frequency, and normalization of serum electrolytes. During 12 months of somatostatin analogue therapy, length and weight progressed along the 3rd percentile on the Tanner growth chart.

Diagnosis of pulmonary diseases.

A broad spectrum of lung disease occurs in association with HIV infection. Included are both infectious and neoplastic processes and idiopathic disorders. To insure prompt, accurate, and efficient diagnosis, a logical, staged sequence of tests should be applied. Chest films and, in some instances, pulmonary function tests and gallium-67 citrate lung scans serve to provide objective indications of lung disease. Each of these tests is sensitive but nonspecific. Specific infecting organisms, particularly P. carinii, can be identified by examining sputum induced by inhalation of 3 per cent saline. Bronchoscopic procedures, including BAL and TBB, are highly sensitive and should be performed in patients having nondiagnostic sputum examinations. Tests involving antigen and antibody detection are of little use in the evaluation of individual patients. Detection of recurrent episodes of PCP is difficult because abnormalities in the usual screening tests may be residual from previous episodes. Finding P. carinii in sputum or bronchoscopic specimens soon (within 2 to 3 months) after a confirmed episode of PCP likely represents residual organisms rather than recrudescence of the infection. Empiric diagnosis of P. carinii should be employed only in limited circumstances when specific diagnostic studies are not available, are contraindicated, or are refused.

[Immunohistochemical tumor diagnosis in breast cancer--use for assessing the stage and biology of so-called "small breast cancer"].

Immunohistochemical methods assure the discrimination between neoplastic and reactive Lesions. Epithelial and mesenchymal neoplastic processes can absolutely be distinguished by antibodies directed to intermediate filaments. Growth activity of breast cancer can be evaluated using antibodies against epidermal growth factor receptor and Ki-67 proliferative protein. With antibodies directed to nuclear steroid receptor proteins semiquantitative determinations (immunoreactive score = IRS) and evaluation of genotypic heterogeneity are possible and may be helpful for therapeutic decisions.

Abnormalities of chromosome 7q and Tac expression in T cell leukemias.

Clonal chromosome abnormalities were demonstrated in 13 cases of T cell leukemia with a mature membrane phenotype. The cases comprised two adult T cell lymphoma leukemia (ATLL), five Sézary syndrome (SS), three T prolymphocytic leukemia (T-PLL), and three T chronic lymphocytic leukemia (T-CLL). The presence of clonal chromosome abnormalities in the three cases of T-CLL supports the view that although clinically benign, this disorder probably represents a neoplastic process. Cells from six of 11 patients were tested and found to be reactive with monoclonal antibody (McAb) anti-Tac in the absence of mitogens (two ATLL, two SS, and two T-PLL). Only the two ATLL patients were serologically positive for the human T cell leukemia virus I (HTLV-I), and their cells showed a higher anti-Tac reactivity. Chromosome 7 abnormalities were observed in seven cases (two ATLL, three SS, two T-PLL), and a strong correlation was seen between the presence of a 7q abnormality and the expression of the Tac antigen (T cell growth factor receptor). These findings are discussed in relation to the localization of T cell receptor genes on chromosome 7 and possible mechanisms for the activation of the Tac gene.
