Initial management of auricular trauma.

The exposed and unprotected position of the auricle makes it susceptible to injuries. Because many of these injuries are initially managed in a primary care setting, family physicians should have an understanding of the management of auricular hematomas, lacerations, abrasions and thermal injuries. Auricular hematoma requires prompt drainage and pressure applied to the site for several days. Treatment of lacerations and abrasions includes meticulous cleaning and minimal debridement of viable tissue. Thermal injuries require recognition of the extent of injury. Prompt treatment of auricular injuries will help avoid aesthetic and functional complications.

[Local cold injuries; symptoms and therapy].

Etiology, pathophysiology, and clinical picture of local frost-injuries are described. By aid of literature, and by proper experiences in 49 patients, who had been treated at the University-Hospital of Innsbruck during the year of 1966 to 1973, therapy of local frost-injuries is discussed. Moreover the advantage of surgical therapy on the sympathicus over conservative treatment is pointed out.

Cold weather injuries in an arctic environment.

The cases of eight Special Forces soldiers who sustained cold weather-related injuries while conducting winter training as part of Operation Arctic Saber in the Northwest Territories and Arctic Circle are reported. Environmentally related injuries can represent difficult diagnostic and treatment challenges in the field. Moreover, they may compromise the overall mission if they are not identified and treated early. Cold weather injuries can also result in long-term disfigurement and disability that may limit a soldier's future worldwide deployability. Mission requirements, equipment utilization, and environmental exposure place soldiers at particular risk for cold weather-related injuries in such austere settings. Nonetheless, with appropriate education and safety precautions, these potentially life-threatening risks can be greatly minimized.

[Characteristics of collateral circulation after ligation of the femoral artery under conditions of cold injury].

Studies on 52 dogs have shown that ligation of the femoral artery 12-14 hours following ice cooling of the extremity is accompanied with a great number of severe acute pneumonias frequently resulting in death. The leading mechanism of local disorders is spasm of the collaterals, the development of circulatory hypoxia of the tissues and congestive alterations in the venous system.
