Startle response: epileptic or non-epileptic? The case for "flash" SMA reflex seizures.

A 19-year-old woman complained of long-standing, frequent, debilitating brusque movements triggered by unexpected stimuli. She was neurologically normal and neuroimaging was also normal. Conspicuous startle reactions were easily reproduced under EEG and video monitoring: the interictal EEG was normal, the ictal recordings doubtful; clinically, the startle reaction was asymmetric, with elevation of the left limbs. The diagnosis of hyperekplexia and startle epilepsy were discussed. We learned that she had been evaluated at age 3-4 months for spontaneous, generalised tonic-clonic seizures and "infantile spasms", in fact for early-onset startle reactions triggered by noise or contact, in association with prominent EEG changes. A full remission had been achieved under ACTH therapy, but the startle reactions had reappeared at around age six. The patient was successfully treated with carbamazepine. The history, clinical and neurophysiological data led us to discuss the diagnosis of hyperekplexia and startle epilepsy. We concluded that the patient had an unusual form of cryptogenic focal epilepsy originating from the supplementary motor area, presenting as strictly stimulus-triggered "flash" seizures.

[The effect of dopamine injected into the lateral brain ventricle on flash visual evoked potential (FVEP) after prenatal intoxication with cadmium].

PURPOSE: Earlier studies demonstrated that exposure, especially prenatal, of mammalians to cadmium (Cd) results in disorders of visual evoked potentials (FVEP). The purpose of this study is to find out any influence of cadmium intoxications on the effect of dopamine (DA) on FVEP. METHOD: 18 Wistar albino rats in 3-6 months of age were divided into 3 groups: control (drinking tap water only), prenatal exposure of 5 ppm, and 50 ppm Cd. Flash visual evoked potentials (FVEP) were recorded before and after injections of dopamine (DA) 100 and 200 nmols into the lateral brain ventricle. The amplitude and latency of N1 and P2 were statistically analysed by the test of t-Student. p < 0.05 was used to indicate significant difference. RESULTS: There were no statistical differences of mean latency of P2 waves between initial records of FVEP and after both doses of DA in all observed groups. The amplitudes of both waves increased after DA injections compared to initial values in all groups. The differences were statistically significant. The mean latencies of N1 peak were prolonged after both DA doses (106-109%) compared to the initial records (100%) in the control group; however, there were no changes of it in the Cd-treated groups before and after DA injections. CONCLUSION: Cadmium blocked the dopamine effect on the latencies of N1 and P2 of FVEP (because it prolonged them alone); however, cadmium increased the stimulative effect of dopamine on their amplitudes.

[Monitor visual function with flash visual evoked potential during orbital surgery].

OBJECTIVE: To evaluate the feasibility and reliability of monitoring visual function with flash visual evoked potential (FVEP) during orbital surgery. METHOD: The visual function of 82 cases was monitored during orbital surgery with FVEP. Of these cases, the intraoperative and postoperative visual functions (IOVF and POVF) were compared. RESULTS: Sixty-nine cases were successfully monitored. Sixty-seven cases obtained the accordance of IOVF and POVF, and there were false positive in 1 case and false negative in 1 case. CONCLUSION: Intraoperative monitoring of visual function with FVEP can show the surgical injury of the optic nerve and unsuitable procedures, and decrease the surgical blind rate.

[The effect of serotonin on flash visual evoked potential in the rat prenatally exposed to cadmium].

AIM: The purpose of this paper was to find out any influence of cadmium (Cd) on the effect of serotonin (5-HT) in the central nervous system (CNS). METHOD: 18 Wistar albino strain female rats were divided into 3 groups: 6 received 5 ppm, 6 one 50 ppm cadmium in tap water since time of conception and during next 21 days after delivery. Control group of 6 rats received tap water only. Newborns were examined when they were 3 to 6 months old. Flash visual evoked potential (FVEP) was recorded before and after injection of 10 microliters saline and then 5-HT into the right lateral brain ventricle. Two doses of 5-HT 125 and 250 nmols were used. Amplitudes of the first deep negative wave (N1) and the next positive one (P1) were measured from isoelectric line to peaks. For statistic analysis the t-test of Student was used with statistical significance by p < 0.05. RESULTS: Shortened latencies (89-99%) of the peaks N1 and P1 of FVEP in the control and Cd 50 ppm groups after both doses of 5-HT were observed, however, they slightly prolonged (103-105%) in the Cd 5 ppm group. The highly significant increase of amplitude of the waves N1 and P1 in all observed groups was received. The only differences were observed in the Cd 5 ppm group; the amplitude of P1 peak was of the same value (100-101%) after 125 nmols and decreased (91%) after 250 nmols of 5-HT compared to the initial values in this group. CONCLUSION: Cadmium increases the serotonin sensibility in the CNS.

Comparison of the light-flash phenomena observed in space and in laboratory experiments.

Astronauts on Apollo and Skylab missions have reported observing a variety of visual phenomena when their eyes were closed and adapted to darkness. These observations were studied under controlled conditions during a number of sessions on board Apollo and Skylab spacecraft and the data available to date on these so-called light flashes are in the form of descriptions of the phenomena and frequency of occurrence. Similar visual phenomena have been demonstrated in a number of laboratories by exposing the eyes of human subjects to beams of neutrons, alpha particles, pions and protons. More than one physical mechanism is involved in the laboratory and space phenomena. No direct comparison of the laboratory and space observations has been made by observers who have experienced both. However, the range of visual phenomena observed in the laboratory is consistent with the Apollo and Skylab observations. Measured detection efficiencies can be used to estimate the frequencies with which various phenomena would be observed if that subject was exposed to cosmic rays in space.

Apollo-Soyuz light-flash observations.

While dark adapted, two Apollo-Soyuz astronauts saw eighty-two light flash events during a complete 51 degrees orbit which passed near the north magnetic pole and through the South Atlantic Anomaly. The frequency of events at the polar parts of the orbit is 25 times that noted in equatorial latitudes and no increased frequency was noted in the South Atlantic Anomaly at the 225-km altitude. The expected flux of heavy particles at the northern and southern points is 1-2 min-1 per eye, and the efficiency for seeing HZE particles which were below the Cerenkov threshold is 50%.

[Autoregressive analysis of flash evoked potentials in healthy preterm infants during sleep].

To interpret the flash evoked potential (FVEP) as dynamic high-order responses to natural and experimental stimulation in healthy preterm infants, waveform analysis of FVEP in 36 healthy preterm infants (postconceptional age 28~42 weeks) were performed using an autoregressive analysis. Based on the histogram of damping frequency of different component impulse response waveforms, the waveforms were divided into 4 groups: group I (0 ~ <2 Hz), group II (2 ~ <6.5 Hz), group III (6.5 ~ <12.0 Hz) and group IV (12~25 Hz). The total power, power of component impulse responses (group I~IV), and damping time (group II~IV) changed significantly with increasing postconceptional age (P<0.01 or P<0.05). Identification of an impulse response component with dominant frequency which undergoes a well-identified change with age is considered to be a useful tool for discriminating between normal and abnormal changes in the FVEP with age in healthy preterm infants.

[Flash pulmonary edema: when the kidney causes decompensation of the heart].

Flash pulmonary edema is a paroxismal pulmonary edema associated to a hypertensive crisis, that subsides in hours with the usual therapeutic measures. It occurs in patients with renal artery stenosis. We report two male patients aged 71 and 74 years old, presenting with acute dyspnea and high blood pressure. Diffuse rales were auscultated and arterial blood pressure was high in both. Dyspnea subsided in few hours with diuretics and oxygen. In both, a critical renal arterial stenosis was found and an angioplasty with stent placement was performed. After 5 and 6 months of follow up, the patients remain asymptomatic.

[The contrast study of preoperative flash visual evoked potential test and the postoperative resumed visual acuity in senile cataract patients].

PURPOSE: To study the using value of flash visual evoked potentials (FVEP) in predicting the resumed visual acuities in senile cataract patients. METHODS: Forty-three eyes with senile cataract had been conducted extracapsular cataract extraction combined with posterior chamber intraocular lens implantation. The contrast study had been carried out in this research between preoperative FVEP, light projection and color discrimination tests with the postoperative regained visual acuities. RESULTS: There were significant correlations between the regained visual acuities and the amplitudes of FVEP (X1), the latencies of P1 wave (X2) as well as color discriminations (P < 0.01). The coefficients were 0.7277, -0.5678 and 0.4302 in sequence. There was no correlation between the resumed visual acuities and the light projection test before operation. The formula of linear regression between the visual acuities (Y) and the indexes of FVEP was Y = 0.2255 +0.047X1 -0.0026X2. CONCLUSION: FVEP is an effective method to evaluate the visual function before the cataract operation. It may predict the resumed visual acuity.

[The dynamic magnetic resonance study of focal liver lesions by FLASH sequences with bolus intravenous gadolinium-DTPA].

Thirty-five patients with hepatic hemangioma (n = 12), metastasis (n = 10), hepatocellular carcinoma (HCC) (n = 10) and focal nodular hyperplasia (n = 3) were examined with the fast low-angle shot (FLASH) technique and an intravenous bolus injection of Gd-DTPA. In order to differentiate the lesions, the following criteria were used: a) pre Gd-DTPA intensity of lesions; b) post Gd-DTPA patterns of contrast enhancement. On the basis of these criteria, an unquestionable differential diagnosis could be made. Hemangiomas were characterized by an hypointense mass before Gd-DTPA, by peripheral contrast enhancement and by subsequent continuous hyperintense fill-in; thus, hemangiomas were visualized as hyperintense lesion during the late phase. Before contrast administration hypovascular metastases appeared as hypointense; they were characterized by delayed uptake of contrast agent. HCCs were hyperintense lesions before contrast administrations; then, quick contrast enhancement and rapid decrease in signal intensity were observed with visualization of a hyperintense ring due to the capsule. Finally, focal nodular hyperplasia appeared isointense or hypointense relative to normal liver on precontrast scans; the lesions were enhanced transiently with subsequent quick dismission of contrast agent. This initial experience suggests dynamic contrast-enhanced MR imaging as an effective method to improve the differential diagnosis among hepatic tumors when precontrast T2-weighted images are equivocal.

[Evaluation of multislice dynamic MR imaging of the whole liver by inversion recovery snap shot FLASH method].

We acquired dynamic images over the whole liver by inversion recovery snap shot FLASH method after a bolus intravenous injection of Gadolinium-DTPA. Each nodule of hepatoma in the liver showed early enhancement and gradually turned to show low intensity. In two cases of hepatoma, small intrahepatic metastases, which were not detected by US, CT and spin-echo image of MRI, were suspected as high intensity nodules on early phase. Also recurrent areas after TAE were enhanced on early phase. This method is practical for improving the detection of lesions and is useful for evaluating the recurrence after TAE.

[Characteristics of normal infants' flash electroretinograms].

PURPOSE: To investigate the characteristic of retinal function of Chinese infants with ages ranging from 4 to 5 months. METHODS: The flash electroetinograms(ERGs) of 17 infants(34 eyes) of 4 months, 13 infants (26 eyes) of 5 months and 39 adults (78 eyes) were tested. Their rod responses, cone responses and maximal combined responses were recorded. The amplitudes and latencies of b waves were compared and analyzed among the 2 infant groups and the adult group. RESULTS: In each of 2 infant groups, of all 3 responses, the means of amplitudes were significantly lower and latencies were significantly longer than those of adults. Between the 2 infants groups, rod responses and maximal combined responses were not significantly different, but the cone responses were significantly different, the infants group with age of 5 months has higher amplitude and shorter latency. The average amplitudes of the rod responses of the 4 months infant group and the 5 months infant group were 38.9% and 36.7% of that of the adult group respectively; the average amplitudes of the cone responses of the 4 months infant group and thd 5 months infant group were 62.6% and 71.5% of that of the adult group respectively. CONCLUSIONS: Between the ages ranging from 4 to 5 months, all of the responses of the infant ERG had not reached the levels of adult. Cone response developed more rapidly and was more mature than rod response.

Multichannel flash spectroscopy of the reaction centers of wild-type and mutant Rhodobacter sphaeroides: bacteriochlorophyllB-mediated interaction between the carotenoid triplet and the special pair.

Multichannel flash spectroscopy (with microsecond time resolution) has been applied to carotenoid (Car)-containing and Car-less reaction centers (RC) of Rhodobacter sphaeroides with a view to investigate the interaction between the Car and its neighboring pigments at room temperature. Under neutral redox potential conditions, where the primary quinone acceptor (QA) is oxidized, the light-induced spectral changes in the 350-1000 nm region are attributed to the photochemical oxidation of the special pair (denoted here as P870), the generation of P870(+)QA(-), and the attendant electrochromism of adjacent chromophores. A bathochromic shift of <1 nm in the visible absorption region of Car reveals the sensitivity of Car to the P870 photooxidation. Under low redox potential conditions, where QA is reduced, P870 triplets (P870(+)) are formed. The time-resolved triplet-minus-singlet (TmS) spectrum of Car-less RC shows a deep bleaching at 870 nm, which belongs to P870(+), and additional (but smaller) bleaching at 800 nm; the entire spectrum decays at the same rate (with a lifetime of about 50 micros). The bleaching at 800 nm arises from the pigment interaction between P870(+) and the accessory bacteriochlorophylls on A and B branches (BA,B). In Car-containing RC, the TmS spectra of Car are accompanied by two smaller, negative signals--a sharp peak at 809 +/- 2 nm and a broad band at 870 nm--which decay at the same rate as the TmS spectrum of Car (ca 10 micros). The former is ascribed to the perturbation, by Car(+), of the absorption spectrum of BB; the latter, to the TmS spectrum of P870(+), a species that appears to be in approximate thermal equilibrium with Car(+). These assignments are consistent with the absorption-detected magnetic resonance spectra obtained by other workers at low temperatures.

[Flash visual evoked potentials on newborns and infants].

OBJECTIVE: To explore the feasibility of applying flash visual evoked potentials (FVEPs) for visual function test newborns and infants and bring out the consultable laboratory values of FVEPs. The technology of FVEP could be used as diagnostic tests for those who failed the screening and the infants who were cared for in the NICU. METHODS: 41 normal neonates (<or= 4 days) and 18 normal infants (<or= 5 months) were tested with FVEP by Nicolet Viking-IV model evoked potentials apparatus. RESULTS: Get the normal reference range of FVEPs latency in both groups and the consulting value of the difference between the left and right eyes. It has been observed from the paper that the latency of FVEPs has the tendency to shorten with the age increasing. It also shows that there are extreme significant changes of FVEPs (P < 0.05) by different frequency of stimulating light. CONCLUSION: The latency of FVEPs P1 is steady and could be applied to newborns-infants visual function test. While, the references of FVEPs according to different age group should be taken into account when we using this measure in clinic.

Influence of dopamine on flash visual evoked potentials (FVEP) in prenatally mercury intoxicated rats.

The female adult white Wistar rats were given tap water (control) or 50 ppm of methylmercury chloride (MMC) ad libitum throughout their pregnancies. Newborn rats drank mother's milk during the first 21 days after delivery and then only tap water. The study was carried out on three-month old offsprings of white Wistar rats. The flash visual evoked potentials (FVEP) were recorded before and after injecting of 10 microl 0.9% saline, 50 or 100 nmols of dopamine (DA) into the lateral brain ventricle by method used before in our laboratory. The amplitude of the first deep negative (N(1)) peak significantly increased to 109-114% after both doses of DA in the control group and to 138-139% in mercury-treated animals. The amplitude of the next positive (P(1)) wave decreased to 94% and 86% in the control group after 50 and 100 nmols of DA, respectively. In Hg-treated group after 50 nmols of DA, the value dropped down to 91%, but increased to 109% after 100 nmols of DA. The increasing of DeltaN(1)P(1) was observed in the control group to 112% after 50 nmols and to 109% after 100 nmols of DA and in Hg-exposed rats, respectively, to 127% and to 129%. The described changes were statistically significant (p < 0.05). The N(1) and P(1) latencies were prolonged in the control group after both doses of DA. In Hg-treated group, the prolongation of N(1) latency was recorded, while the P(1) latency was not changed. We concluded that prenatal Hg intoxication disturbed the effect of DA on FVEP.

[Study on laser flash spectroscopy of laser-induced interaction of DNA and polynucleotides with SO4*-].

The interactions of DNA and polynucleotides (poly[G],poly[A] and poly[C]) with SO4*- were investigated by 248 nm laser flash photolysis. The results show that, the reactions of poly[G] or poly[A] with SO4*- yield predominantly the corresponding one-electron-oxidated base radicals. With poly[C], it is suggested that SO4*- radical reacts predominantly by addition to the base moiety to produce C5-yl and C6-yl sulphate radical adducts, and subsequently H-abstraction yields C2'-sugar radical. The predominant species resulting from the reaction of DNA with SO4*- is DNA guanyl radical.

[Study on laser flash spectroscopy of processes of photoionization of tyrosine: pH effects].

The processes of photoionization of tyrosine on three different pH value solutions were investigated by 248 nm laser flash photolysis. The results showed that the processes of photoionization of tyrosine were affected by pH value of the solution. With pH value increasing, the molecule number of photoionization increases via a monophotonic process, and decreases via a biphotonic process. Furthermore, the pH effect of photoionization of tyrosine resulting from deprotonation of phenolic hydroxyl group in alkaline solution was demonstrated.
