Modulation of the digestive lysosomal system in Paramecium caudatum. III. Morphological effects of cytochalasin B.

Morphological explanations for the effects of cytochalasin B (CB) on the digestive vacuole system of Paramecium caudatum were sought using electron-microscopic studies. Cytochalasin B (0.3 mM) essentially stopped vacuole release without stopping vacuole growth. Acidosome fusion with the new vacuoles was inhibited and was accompanied by the formation of discoidal vesicle-lined clumps of microfilament-like material next to these vacuoles. Vacuole membrane retrieval at the open cytoproct was inhibited and was accompanied by the loss of the microfilament network typical of this region. At 0.15 mM CB vacuole release and fusion of the acidosomes and lysosomes with the vacuoles were not stopped, but membrane retrieval at the cytoproct was affected. Cytochalasin B at the higher concentration also affected the movement of the horseradish peroxidase (HRP)-labeled vesicles away from the nascent vacuole. Membrane retrieval by the formation of tubules of 35 to 50 nm diameter from membrane of early and late vacuole stages was also affected by CB. It is conceivable that all of these effects were the result of the disruption of membrane associated actin networks. By contrast, several highly organized microfilamentous systems in the cortex and oral region of these cells appeared to be unaffected by CB or dimethyl sulfoxide (DMSO) at the concentrations used. These results show that CB-susceptible structures assume a key role in the normal functioning of the digestive vacuole system and in vacuole membrane retrieval and reutilization in Paramecium.

Glutathione transferases with vanadium-binding activity isolated from the vanadium-rich ascidian Ascidia sydneiensis samea.

Some ascidians accumulate vanadium in vanadocytes, which are vanadium-containing blood cells, at high levels and with high selectivity. However, the mechanism and physiological significance of vanadium accumulation remain unknown. In this study, we isolated novel proteins with a striking homology to glutathione transferases (GSTs), designated AsGST-I and AsGST-II, from the digestive system of the vanadium-accumulating ascidian Ascidia sydneiensis samea, in which the digestive system is thought to be involved in vanadium uptake. Analysis of recombinant AsGST-I confirmed that AsGST-I has GST activity and forms a dimer, as do other GSTs. In addition, AsGST-I was revealed to have vanadium-binding activity, which has never been reported for GSTs isolated from other organisms. AsGST-I bound about 16 vanadium atoms as either V(IV) or V(V) per dimer, and the apparent dissociation constants for V(IV) and V(V) were 1.8 x 10(-4) M and 1.2 x 10(-4) M, respectively. Western blot analysis revealed that AsGSTs were expressed in the digestive system at exceptionally high levels, although they were localized in almost all organs and tissues examined. Considering these results, we postulate that AsGSTs play important roles in vanadium accumulation in the ascidian digestive system.

Contamination of terrestrial gastropods, Helix aspersa maxima, with 137Cs, 85Sr, 133Ba and 123mTe by direct, trophic and combined pathways.

(137)Cs, (85)Sr, (133)Ba and (123m)Te contaminations of terrestrial gastropods, Helix aspersa maxima, by direct deposition, labelled food ingestion or combined (trophic and direct pathways) exposure were carried out under laboratory conditions. The aim of this study was to compare the three contamination pathways: direct, trophic and combined, in terms of individual mortality, radionuclide uptake, depuration and distribution in the tissues. An initial group of 30 snails (2 years old) was exposed to radioactive aerosols during a 20-h period. These aerosols were assumed to be representative of those that would be released during a nuclear accident occurring in a PWR. A second group of 50 snails (same age) was submitted to an ingestion of commercial food contaminated by the same aerosols, twice a week for 21 days (flour at a feeding rate of about 0.2g). A third group of 40 snails was submitted to a combined exposure: exposure to radioactive aerosols (20h), followed by ingestion of flour contaminated by the same aerosols, twice a week for 21 days. No significant difference between the three groups and a reference group of 10 snails was observed, neither in growth nor in mortality. Concerning the direct pathway, at the end of direct deposition (about 1 day after the beginning), cesium was the most bioavailable element, distributed rather homogeneously throughout the whole body (13% of the total Cs in all organs excepting the digestive system and 28% in the muscle). Strontium was measured in the shell (about 70%). Barium was found in the muscle (20%) and in the shell (65%). Tellurium was mainly present in the shell (70%) and in the digestive system (20%). After 21 days of depuration, the faeces eliminated 42% of the Te. As for contamination by ingestion, Te mainly accumulated in the digestive system (72% of Te present in the total body), Ba accumulated in the muscle (75%) and Sr in the shell (70%). Concerning contamination by combined pathways, at the end of the 21-day exposure, the 4 radionuclides had the same tendency as direct deposition. However, the effect of the trophic pathway was significant: it causes an 18% increase of Sr in the shell and an 7% increase of Cs in the digestive system in comparison to direct deposition, resulting in a final 86% in the shell and 27% in the digestive system.

Performance of temperature-phased anaerobic digestion (TPAD) system treating dairy cattle wastes.

The performance of temperature-phased anaerobic digestion (TPAD) system in the stabilization of dairy cattle wastes at high solids concentrations has never been evaluated, though the process has been established as a feasible alternative to conventional mesophilic processes for the treatment of municipal wastewater sludges. In this study, the TPAD system operating at a retention time of 14 days was subjected to varying total solids (TS) concentrations (3.46-14.54%) of dairy cattle wastes. At TS concentrations lower than 12.20%, corresponding to system volatile solids (VS) loadings in the range of 1.87-5.82 g VS/L/day, the system achieved an average VS removal of 40.2%. The maximum VS destruction of 42.6% was achieved at a TS concentration of 10.35%. Methane recovery from the wastes was consistently within 0.21-0.22 L/g VS fed. There was a drop in the system performance with respect to VS removal and methane recovery at TS concentrations higher than 10.35%. volatile fatty acid/alkalinity ratios less than 0.35 in the thermophilic reactor and 0.10 in the mesophilic reactor were found favorable for stable operation of the system. For the entire range of TS concentrations, the indicator organism counts in the biosolids were within the limits specified by USEPA in 40 CFR Part 503 regulations for Class A designation. After digestion, nearly 80-85% of total phosphorus was associated with the biosolids.

Patterns of infant mortality caused by major congenital anomalies.

BACKGROUND: We assessed the impact of recent advances in perinatal care on infant mortality due to congenital anomaly. METHODS: Analysis of trends in congenital anomaly-attributed infant mortality, using the 1981-1995 Statistics Canada's birth and death records, with a total of 2,878,826 live births, 21,883 infant deaths, and 6, 908 infant deaths due to congenital anomalies. RESULTS: Infant mortality due to major congenital anomaly decreased from 3.11 per 1, 000 live births in 1981 to 1.89 per 1,000 live births in 1995. Cause-specific infant mortality rates for anencephaly, spina bifida, other central nervous system anomalies, cardiovascular system anomalies, respiratory system anomalies, digestive system anomalies, certain musculoskeleton anomalies, urinary system anomalies, chromosomal anomalies, and multiple congenital anomalies were 0.20, 0.23, 0.27, 1.04, 0.24, 0.08, 0.22, 0.16, 0.22, and 0.13 per 1,000 live births, respectively, in 1981-1983, whereas corresponding rates were 0.07, 0.07, 0.18, 0.73, 0.25, 0.03, 0.12, 0.12, 0.26, and 0.06 per 1,000 live births, respectively, in 1993-1995. CONCLUSIONS: Recent Canadian data show that infant deaths caused by major congenital anomalies have decreased significantly, but reductions varied substantially according to specific forms of anomalies.

Were volatile organic compounds the inducing factors for subjective symptoms of employees working in newly constructed hospitals?

This study demonstrated possible relationships between environmental, personal, and occupational factors and changes in the subjective health symptoms of 214 employees after the relocation of a hospital in a region of Japan. Eight indoor volatile organic compounds (VOCs) were detected in at least one of the 19 rooms investigated, and total VOC (TVOC) concentrations in 8 rooms exceeded the advisable value (400 microg/m(3)) established by the Ministry of Health, Labour and Welfare of Japan. Formaldehyde was detected in all the investigated rooms, but none of the results exceeded the guideline value (100 microg/m(3)). Multiple logistic regression analysis was applied to select variables significantly associated with the subjective symptoms that can be induced by sick building syndrome. The results showed that subjective symptoms of deterioration in the skin, eye, ear, throat, chest, central nervous system, autonomic system, musculoskeletal system, and digestive system among employees were associated mainly with gender difference and high TVOC concentrations (>1200 microg/m(3)). Long work hours (>50 h per week) in females and smoking in males were to be blamed for the deterioration of their symptoms. The present findings suggest that to protect employees from indoor environment-related adverse health effects, it is necessary to reduce the concentration of indoor chemicals in new buildings, to decrease work hours, and to forbid smoking.

Expression of neurturin, GDNF, and GDNF family-receptor mRNA in the developing and mature mouse.

The GDNF family of neurotrophic factors currently has four members: neurturin (NRTN), glial cell line-derived neurotrophic factor (GDNF), persephin, and artemin. These proteins are potent survival factors for several populations of central and peripheral neurons. The receptors for these factors are complexes that include the Ret tyrosine kinase receptor and a GPI-linked, ligand-binding component called GDNF family receptor alpha 1-4 (GFRalpha1-4). We have used in situ hybridization to study the mRNA expression of NRTN, GDNF, Ret, GFRalpha1, and GFRalpha2 during embryonic development and in the adult mouse. GDNF receptors were prominently expressed during embryonic development in the nervous system, the urogenital system, the digestive system, the respiratory system, and in developing skin, bone, muscle, and endocrine glands. In some regions, incomplete receptor complexes were expressed suggesting that other, as yet unidentified, receptor components exist or that receptor complexes are formed in trans. NRTN and GDNF were expressed in many trigeminal targets during embryonic development including the nasal epithelium, the teeth, and the whisker follicles. NRTN and GDNF were also expressed in the developing limbs and urogenital system. In the embryo, GDNF factors and receptors were expressed at several sites of mesenchyme/epithelial induction, including the kidney, tooth, and submandibular gland. This expression pattern is consistent with the possibility that the GDNF factors function in inductive processes during embryonic development and with the recently discovered role of NRTN as a necessary trophic factor for the development of some parasympathetic neurons. In the mature animal, receptor expression was more limited than in the embryo. In the adult mouse, NRTN was most prominently expressed in the gut, prostate testicle, and oviduct; GDNF was most prominently expressed in the ovary.

Further characterization of Helix FMRFamide receptors: kinetics, tissue distribution, and interactions with the endogenous heptapeptides.

The biphasic binding of 125I-daYFnLRFamide to crude brain membranes of Helix aspersa is due to two discernible sites (high and low affinity) rather than different agonist-induced states. The tissues in the snail that show the greatest specific 125I-daYFnLRFamide binding are the brain, reproductive system, and digestive system. The heart shows moderate binding levels, whereas low values are obtained in the oviduct and retractor muscles. The N-terminal SAR of the Helix heptapeptides (X-DPFLRFamide) indicates that, although the substitution of Leu for Met accounts for some, the dipeptide X-Asp produces most of the loss in potency at FMRFamide receptors in Helix brain.

A prospective study of adverse drug reactions as a cause of admission to a paediatric hospital.

1. A total of 512 consecutive paediatric hospital admissions of children 2 years old or less were evaluated to assess the extent and pattern of admission caused by suspected adverse drug reactions (ADRs). the proportion of suspected ADRs related to hospital admissions was 4.3%. 2. The organ-systems most commonly implicated were the central nervous system (40.5%), digestive system (16.7%), and skin and appendages (14.3%). Together, they accounted for 71.5% of admissions attributed to ADRs. The most common clinical manifestations inducing admission were convulsions (4 cases), dizziness (4), vomiting (3), and tremor, fever, itching and apnoea (2 cases each). 3. The four classes of drugs most frequently suspected in admissions due to ADRs were respiratory drugs (35%), anti-infective agents (25%), drugs active on the central nervous system (15%) and drugs used in dermatology (10%). The most common drugs related to ADRs were a combination of chlorpheniramine, diphenhydramine, phenylephrine, guaiphenesin and salicylic acid (4 cases), followed by fenoterol, adrenaline, paracetamol, DTP vaccine and antipolio vaccine (2 cases each). 4. There were no significant differences between children older and younger than 1 year (odds ratio 0.89; 95% CI 0.37-2.17) or between the sexes as regards hospital admittance due to suspected ADRs (odds ratio 1.94; 95% CI 0.72-5.42). 5. The results of this kind of study may be influenced by patterns of drug utilization. Nevertheless, the lack of specific studies of drug effects in young children makes it desirable to carry out pharmacoepidemiological studies in this age group.

Academic gaming in nurse education.

This paper describes a study of the usefulness of academic gaming in nurse education. The research has been carried out over the course of 3 years and represents an attempt by a nurse tutor to carry out a study on research lines, during the course of normal working and with a minimum of interference to the normal working of a nurse education department. The study involved psychiatric nurse learners at Winterton Hospital, Sedgefield, Durham and attempted to assess the usefulness of academic gaming by evaluating one academic game which has been designed for nurse learners. This game aims to teach various aspects of the digestive system and digestive processes. The questions which required answers were, can nurse learners learn from academic games? Do they like this method of learning? How does this method compare with other learning methods? Eighty-six learners participated in a test/game/test situation and completed a questionnaire after playing the game. Results suggest that learners can increase their knowledge of the subject by playing the game but it may be most useful as a consolidation method. Learners most often stated that they enjoyed the game and when ranked according to preference, gaming was preferred to film, reading and discussion methods but equally preferred to the lecture method.

CalpA, a Drosophila calpain homolog specifically expressed in a small set of nerve, midgut, and blood cells.

Calpains are calcium-dependent proteases believed to participate in calcium-regulated signal pathways in cells. Ubiquitous calpains as well as tissue-specific calpains have been found in vertebrates. We isolated cDNA clones for a highly tissue-specific calpain gene from Drosophila melanogaster, CalpA, at 56C-D on the second chromosome. The expression of the CalpA gene product was monitored by using a specific antiserum directed against the product expressed by one cDNA clone. The encoded protein is found in a few neurons in the central nervous system, in scattered endocrine cells in the midgut, and in blood cells. In the blood cell line mbn-2, calpain is associated with a granular component in the cytoplasm. The expression of this protein is more restricted than that of the corresponding transcripts, which are widely distributed in the central nervous system, digestive tract, and other tissues. The sequence of CalpA is closely related to that of vertebrate calpains, but an additional segment is inserted in the calmodulin-like carboxy-terminal domain. This insert contains a hydrophobic region that may be involved in membrane attachment of the enzyme. Differential splicing also gives rise to a minor transcript that lacks the calmodulin-like domain.

[Re-evaluation of marketed chemical drugs and biological products of therapeutic - a systematic review of literature].

OBJECTIVE: To assess the current status on re-evaluation of marketed drug in China since the promulgation of drug law in 1985. METHODS: Review of literature on Chinese pharmaceutical abstract and CBMdisc from 1985 to 2001 year was done. RESULTS: 4029 papers and 855 marketed drugs from 1985 to 2001 were included. Drugs on anti-infection agent, cardiovascular system and digestive system were the main drugs being re-evaluated, with the proportions of 27.1%, 20.1% and 11.1% respectively. The amounts of both marketed drugs and literature were increasing year by year. The method used for re-evaluation were random and non-random clinical trial. 41.4% of all the samples had a sample size of 50 - 100 research subjects. There were 13 papers with more than 5000 samples. The level on evidence based literature was assessed. 44 papers were graded as first class, and 182 papers the second, 2466 papers the third and 1337 papers the fourth. The quality of literature was improved year by year. CONCLUSION: The amount, quality as well as the sample size of literature being re-evaluated on marketed drug were increased from 1985 to 2001. However, the design and evaluation of those trials were not standardized.

[The specificity of organization of the consultative-and-diagnostic service for the adult population in a big city].

The volume and structure of the diagnostic services and of the medical aid appealability were analyzed for the Eastern Administrative District of Moscow by the example of a stage-2 diagnostic-and-consultative center. The analysis was made on the basis of 80233 receipts of patients for the period of January-April 2003. Three types of patients' flow were defined with respect to the purpose of visit: consultative aid (CA, 17.3%), consultative-and-diagnostic aid (CDA, 16.2%) and diagnostics (D, 66.5%). Differences are shown in the structure of appealability by the classes of diseases (nosological forms of diseases) and according to groups of patients. Consultations are mostly provided in cases of diseases of the endocrine system (20.9%), digestive system (20.2%) blood-circulation system (18.0%) and urogenital system (14.6%). Patients with urogenital disease (31.1%) and with blood-circulation lesions (13.5%) were found to be predominant in the CDA group. The structure of appealability is defined for the classes of diseases, their nosological forms and for medical specialties. It is pointed out that 52.1% of patients visited their general practitioners. Patients visited most often the doctors of the below specialties: oncology--16.1%, endocrinology and cardiology 12.3%, general practitioner--11.5% and gastroenterology--78%. The rate of visits was evaluated for the classes of diseases and medical fields; the estimated distribution of appealability was estimated with respect to the rate of visits. The structure of diagnostic examinations was analyzed by types, purpose, and classes of diseases (nosological forms).

The use of hypnosis in a family practice setting.

For the Family Physician/General Practitioner, there are many locales in which hypnosis is a valuable adjunct to treatment. In this chapter the author describes some of the opportunities for using hypnosis according to the site of practice, rather than in the usual pattern of describing its use in each physiological system (e.g. digestive system, reproductive system) or division of medicine (pediatrics, gynecology etc.) The areas chosen are: the office examining room, the counselling or consulting room, the emergency room, the Intensive Care and Coronary Care Units, the case room, the operating room, the home visit, at the scene of an accident, and in the palliative care unit.

Inpatient and post-discharge course of the malnourished patient.

A retrospective review of the medical records of 114 malnourished and 106 non-malnourished male veterans assessed the inpatient and 1 year post-discharge dietetic care given for the treatment of malnutrition. The malnourished sample consisted of subjects discharged with a protein-energy or protein malnutrition comorbidity. Subjects in the diagnosis-matched and age-matched control sample were discharged without a malnutrition comorbidity during the same period. Data collected from progress notes included diagnoses, inpatient dietetic feedings and services provided, discharge dietetic care, intervening clinic visits, and rehospitalizations. On initial admission, 79% of the diagnoses for the malnourished group fell into five diagnosis categories: neoplasms; respiratory system diseases; digestive system diseases; endocrine, nutritional, and metabolic diseases; and mental disorders (including alcohol-related disorders). Malnourished subjects received more specialized feedings and dietetic services than did the controls on initial admission. However, the discharge and post-discharge care received by surviving members of both groups was similar. Fewer than half the members of either group received post-discharge care. Fifty-four malnourished and 54 control patients were hospitalized more than once. The findings indicate that levels of inpatient and outpatient dietetic care need to be coordinated to alleviate malnutrition.

[Multiple carboxylase deficiency].

Biotinidase deficiency and holocarboxylase synthetase deficiency are two autosomal recessively inherited disorders of biotin metabolism affecting children below the age of two years. Both cause multiple carboxylase deficiency resulting in defects of fatty acid synthesis, gluconeogenesis and amino acid catabolism. The clinical picture involves the nervous system, the skin, the respiratory system, the digestive system and the immune system, but great individual variations often makes the clinical diagnosis difficult. Early diagnosis and treatment with biotin are essential in order to prevent death from metabolic acidosis or irreversible damage to the central nervous system. Two patients with biotinidase deficiency, two patients with holocarboxylase synthetase deficiency and a review of the literature are presented. Neonatal screening for biotinidase deficiency or a higher degree of metabolic screening of the urine in children below the age of one year with seizures and unexplained clinical course are discussed.

[The extensibility and retractility of surgical margins in digestive tract cancer].

OBJECTIVE: To study the extensibility and retractility of the surgical margins in digestive system neoplasms. METHODS: The length difference of the digestive tract was measured in vivo and in vitro under different conditions. Five cm of the stomach, small intestine and large bowel and 2 cm of the esophagus were measured as standard control length in vivo just before resection. The length was measured with a ruler under pull of 500 g and 1 000 g in vivo, in fresh status in vitro, and 10% formaldehyde fixed for 6 - 8 h, 12 - 24 h and 48 - 72 h. The extension or retraction ratio was calculated. The length difference was divided by the natural length in vivo. RESULTS: Seventeen cases of the esophagus, 18 cases of the stomach, 15 cases of the small intestine and 25 cases of the large bowel were measured under pull of 500 g and 1 000 g. The esophagus extended 16.5% and 30.5%, stomach 15.0% and 22.6%, small intestine 66.4% and 120.0%, large bowel 36.0% and 56.0% respectively. In natural status ex vivo, the esophagus retracted 44.5%, stomach 13.6%, small intestine 11.4% and large bowel 15.6% respectively; they continue to retract after 10% formaldehyde fixation until 12 - 24 h later. If the length of surgical margin of the fresh specimen ex vivo was x, the natural length of margin in vivo of the esophagus would be 1.80 x, stomach 1.16 x, small intestine 1.13 x, and large bowel 1.18 x. If formaldehyde fixation for 6 - 8 h, the natural length of surgical margin in vivo of the esophagus would be 1.82 x, stomach 1.41 x, small intestine 1.22 x, large bowel 1.55 x. If formaldehyde fixation for 12 - 24 h, the surgical margin length in vivo of the esophagus would be 2.22 x, stomach 1.43 x, small intestine 1.28 x, and large bowel 1.57 x. CONCLUSION: The length of surgical margin of digestive system cancers varied under different conditions, and the evaluation of surgical margin during surgery should be performed under natural status in vivo.
