The effect of grenz rays on psoriasis lesions of the scalp: a double blind bilateral trial.

Sixteen patients with symmetrical psoriasis lesions of the scalp were treated with grenz rays. In a double blind fashion, one side of the scalp was irradiated with 4 Gy of grenz rays applied on 6 occasions at intervals of 1 week, and the other side of the scalp was given placebo treatment. The patients were seen before and after X-ray therapy. A significantly (P less than 0.0001) better therapeutic result was recorded on the side of the scalp which had received active grenz ray therapy. In 14 of the 16 patients there was complete healing on the grenz ray-treated side after 6 wk of treatment. Nine patients were still free of lesions of the scalp 3 months after the start of the grenz ray therapy.

Hereditary deficiency of C2 in association with linear scleroderma 'en coup de sabre'.

A 32-year-old man suffering from linear frontoparietal scleroderma was found to have low (less than 10% normal) serum classical pathway activity although C1q, C3, C4, C5, and total alternative pathway activity was normal. Addition of purified C2 led to complete restoration of the total hemolytic activity of the classical pathway. The C2 hemolytic assays showed that the patient was not totally deficient in C2. He had about 30% of the normal C2 level. Studies on his available nucleus family members in the Netherlands also showed that the deficiency was inherited; one of the patient's brothers and one of his daughters had half of the normal C2 levels. The C2 deficiency could not be corrected by a three-week regimen of danazol. To the best of our knowledge, this is the first documented case concerning an association of linear frontoparietal scleroderma with C2 deficiency.

Paraffinomas of the scalp.

Four cases of scalp paraffinoma were seen in our clinics within the past year. This unusual condition results from intradermal injection of substances containing paraffin. The presence of paraffin in excised tissue was confirmed by thin-layer chromatography and infrared absorption spectrophotometry. Our patients received their treatments 35 to 42 years ago after responding to radio or newspaper advertisements that promised a cure for baldness. Two patients were initially observed with severe scalp inflammation and two were completely asymptomatic other than having lumpy scalps. Although these cases were readily diagnosed by history, eliminating other possibilities was greatly aided by use of xeroradiography, obviating the need for biopsy in asymptomatic cases.

Alopecia and drug eruption of the scalp associated with a new beta-blocker, nadolol.

Infiltrative dermatitis and marked alopecia of the scalp appeared shortly after a new beta-blocker, nadolol (Corgard), was prescribed for the treatment of a patient with hypertension. Cessation of the beta-blocker therapy, after four months of therapy, was followed by a dramatic involution of the eruption, and total regrowth of scalp hair occurred within three months. The associated eruption and rapid regrowth of hair upon discontinuation of nadolol distinguish this alopecia from the telogen effluvium previously associated with other beta-blocker drugs, such as propranolol (Inderal) and metoprolol (Lopressor).

[Erosive pustular dermatosis of the scalp. A new entity or a variant of pyoderma vegetans].

We report on a 69-year-old woman suffering from an erosive pustular and crusted lesion of the scalp having persisted over years. There could not be found any external cause, and a large number of different physical and medicamental therapies failed. Prompt healing was achieved, however, after a low serum zinc level was corrected by a zinc preparation administered orally.-The dermatosis described closely resembles the so-called "erosive pustular dermatosis of the scalp". A search of the earlier dermatological literature supported our view that this is not a new disease, but rather a variant of pyoderma vegetans, a disease showing extremely variable clinical and histopathological features.

Perifolliculitis capitis abscedens et suffodiens (Hoffman). Complete healing associated with oral zinc therapy.

A 24-year-old man with a one-year history of perifolliculitis capitis abscedens et suffodiens (Hoffman) was treated with oral zinc sulfate. Complete healing was seen after three months and the treatment was continued for a total of six months. No relapse has occurred during a five-year follow-up. The mechanisms through which zinc therapy might influence inflammatory conditions are not fully understood.
