[Iron status with particular consideration of soluble transferrin receptors in children and youth with gastritis, with or without Helicobacter pylori infection].

UNLABELLED: Role of Helicobacter pylori infection in chronic gastritis and gastric and/or duodenal ulcers is well known. Simultaneously there are some articles in literature considering H. pylori as a cause of extra-gastrointestinal illnesses such as atopic dermatitis, chronic urticaria or acne rosacea, hypotrophy, Schoenlein-Henoch disease, atherosclerosis or hypochromic anaemia. The aim of the study. was to asses iron status in aspect of plasmatic transferrin receptors concentration among children and youth with chronic gastritis with or without Helicobacter pylori infection. MATERIALS AND METHODS: Forty one patients were included as a study group. Range of age was 9-18 years. All patients were diagnosed due to chronic abdominal pains. There were 13 males and 28 females. Blood was collected from every patient for blood cell count, iron, transferrin and transferrin receptors concentration (sTfR) assessment before endoscopy of upper gastrointestinal tract. RESULTS: Concentration of sTfR was higher than age norm among 29 (71%) of patients. Among patients with higher level of sTfR 20 (69%) had normal haemoglobin concentration and in this group 10 patients had H. pylori infection. During analysis of 12 patients with nornal level of sTfR normal haemoglobin concentration was found and among five of them H. pylori infection was stated. Among 21 patients without H. pylori infection 14 had normal level of sTfR and 7 had higher level of sTfR which means that 33% had hidden iron deficiency (involuntary of normal Hb concentrations). Among 15 of 20 patients with H. pylori infection level of sTfR was higher which means that 75% patients with infection had hidden iron deficiency (involuntary of normal Hb concentrations). CONCLUSION: Level of plasmatic transferrin receptors can be good and sensitive indicator of iron deficiency and can be helpful in differential diagnosis of hypochromic anaemia and anaemia caused by chronic illness including chronic gastritis with Helicobacter pylori infection.

[Allergenic flora in the Valle de México. Relation to food sensitivity of vegetal origin (I)].

A descriptive analysis was made including the possible plants capable of producing an allergic reaction with pollen grains. We worked with 106 species from 30 families and 79 genera; only 57 species contributed to the collection with prepared pollen grains. In the same manner, morphologic descriptions of the pollen grains were conducted. Clinical charts were reviewed, selecting those patients who reacted positively to some type of pollen (1101 patients) and to some kind of food. This was corroborated by the ingestion of vegetal origin foods. The gramineous, leguminous, rosaceae, and solanaceae were the most important group of vegetal-originating foods which caused a positive reaction due to their ingestion and a skin reaction in 37.52% of the patients.

[Dermatitis in VDT operators: a review of the literature].

The paper reports studies on the relationship between work with VDT's and dermatitis in the operators. From the first observation in 1979, numerous studies have been carried out to try and explain the reason why some forms of dermatitis like rosacea, eczema seborrhoeic and erythematitis appeared more frequently in VDT operators compared to the general population. The various authors at present consider low indoor air humidity associated with a strong electrostatic field to be essential factors in the occurrence of dermatitis. Laboratory experiments have shown that the presence of a strong electrostatic field does not seem to be important in itself. Similarly, it is agreed that X, UV-A or UV-B radiations do not play any causative role in dermatitis. Further investigations are required to assess the influence of indoor climatic factors and the presence of irritant substances in the air of the working environment.

[Stability of metronidazole, prednisolone and dexamethasone in urea-containing Elacutan W dermatologic agent].

Metronidazole was investigated in the basic dermatologic agent Elacutan W to improve the topical therapy of rosacea. The suitability of that basic dermatologic agent was verified for metronidazole, prednisolone and dexamethasone by stability tests (UV-spectroscopy, pH) and by in-vitro-liberation-measurements (membrane method). The drugs are stable for a period of 100 days.

[Granulosis rubra nasi].

The Authors report a case of Granulosis Rubra Nasi, a rare early childhood dermatitis that was also defined as "Acne papulo-rosacea of the nose". The hyperhidrosis the most conspicuous features of the disease and small beads of sweat and erythema on the tip of the nose may often occur. Sometimes small red papules and comedo-like lesions may be present.

["Clown nose"--skin metastasis of breast cancer].

We report on a 74-year-old woman showing a reddish infiltration of the tip of the nose, which had appeared 3 months ago. Clinically, we considered the following differential diagnoses: sarcoidosis, rosacea, pseudolymphoma, and metastasis. Histological and immunohistological investigation proved a cutaneous metastasis of carcinoma of the breast. Our case report gives evidence of the fact that cutaneous metastases of systemic malignancies are frequently located in acral regions of the skin.

[Roaccutan in the treatment of severe forms of acneiform eruption].

Roaccutan efficacy and tolerance in severe forms of acne eruption (ball-shaped and conglobate) and in acne rosacea were under study. The treatment started with a daily dose of 60 mg, then the dose was gradually reduced. The course took 8-12 weeks. Roaccutan was sufficiently effective in severe forms of acne eruption and was well tolerated by the patients. Side effects, such as lip and skin dryness, are possible in the course of therapy.

Epidemiological studies of the influence of sunlight on the skin.

We examined and interviewed 809 randomly selected individuals (56% women and 44% men, mean age 44 years) from 4 Swedish cities; 1.7% had the sun-reactive skin type I, 26.2% type II, 61.3% type III and 10.7% type IV. Some variations in the distribution of skin types were found between the cities. One hundred subjects (12.4%), mostly women (p less than 0.001), had a history of polymorphous light eruption. People with skin type IV had significantly fewer facial skin problems than the rest of the subjects (p less than 0.05). The majority of subjects with atopic eczema, acne vulgaris or seborrheic dermatitis experienced improvement after exposure to sunlight. Individuals with rosacea also experienced improvement more often than impairment from exposure to sunlight. Facial telangiectases occurred in 57% of the subjects, unilaterally in 8.5% of the cases. The occurrence of telangiectases increased with increasing age (p less than 0.001), increasing sunbathing (p less than 0.01) and poor pigmentation ability (p less than 0.05).

Aggressive intranasal carcinoma mimicking infection or inflammation.

In a twenty-year period we have treated five patients with intranasal squamous cell carcinomas who initially were thought to have benign cutaneous disorders of the nasal skin. Advanced squamous cell carcinoma requiring total rhinectomy, often with extensive contiguous midface excision, was found in all patients. Three of these five patients died of their disease. In one patient, a red nose resembling acne rosacea was present. In the four others, chronic cellulitic or ulcerative cutaneous involvement was caused by squamous cell carcinoma. These cases emphasize the need for intranasal examination and appropriate radiologic studies to exclude intranasal carcinoma in patients with apparently unresponsive cutaneous nasal disease.

Health effects of sunlight exposure in the United States. Results from the first National Health and Nutrition Examination Survey, 1971-1974.

The dermatologic component of the first National Health and Nutrition Examination Survey (N = 20,637), conducted from 1971 through 1974, documented the deleterious effect of ultraviolet radiation on selected skin and eye conditions. Actinic skin damage was more frequent in white men with high as compared with low sunlight exposure, 36.7% vs 23.3%, respectively. Among white women, the corresponding figures were 34.1% vs 18.6%, respectively. Actinic damage was found more often in individuals with light eye color. Basal cell epitheliomas were found in 11.3% of white men aged 65 to 74 years who had severely actinic-damaged skin as compared with 1.0% of those with undamaged skin. Sunlight exposure was positively associated with localized hypomelanism, localized hypermelanism, seborrheic keratoses, senile lentigines, freckles, acne rosacea, spider nevi, varicose veins, venus star, dry skin, wrinkled skin, pterygia, arcus senilis, and a variety of minor oral lesions of the tongue, palate, and buccal mucosa. These findings suggest that a large number of dermatologic conditions, which may in part result from overexposure to sunlight, may be preventable.

[Former dietary measures in dermatology--a historical review].

A historical view will be given on forms of diet formerly used in dermatoses (especially psoriasis, tuberculosis, atopic dermatitis, acne, rosacea and seborrhoeal eczema). The use of raw food, milk, and fruit, abstinence diets and juice cures in dermatological therapy still remain just as questionable as the effects of acidified or alkalised forms of food. Their abundance and diversity leads to the assumption that it is clearly not so much the composition of food, but more in the change of food and the extensive natural state of the basic foodstuffs used, which play an essential role in the influence on the progress of dermatoses.

[Zinc and its status in some dermatologic diseases--a statistical assessment].

This report contains statistical interpretations of blood- and serum-levels of zinc, magnesium and copper, including dates of 380 patients with alopecia areata, alopecia androgenetica, psoriasis vulgaris, vitiligo, rosacea, venous ulcer and atopic eczema, compared with 31 healthy people. Significant low blood-levels of zinc were only proved in vitiligo, significant high levels were found in all patients of the psoriasis group. The serum-level of zinc was low in diagnoses as well as the blood-levels of copper. Blood- and serum-levels of magnesium were similar in all dermatological diseases; all of them were increased in comparison with the control group (significance level p = 0.05). Furthermore there were linear dependences of the variables, due to the examined diseases. It is of some interest that we could not prove a normal distribution of blood- and serum-levels of magnesium as often described in literature.

Survey of airborne pollen and fungal spores at Dehra Dun, India.

A survey of airborne pollen and fungal spores was carried out using a gravity settling device at Dehra Dun for two consecutive years (1980-81). The daily, seasonal, and annual variations recorded for the dominant pollen and spore types are analysed in detail. Two distinct peak pollen periods, February-April and August-October, have been recognized although pollen was found present in the atmosphere throughout the year. In contrast, fungal spores did not exhibit any distinct seasonal pattern. July through October has been observed as the period of higher spore catch as compared to other months. Pollen grains of Pinus are recorded as the most dominant type at Dehra Dun followed by Broussonetia, Rosaceae, Poaceae, Debregeasia, Rumex, and Morus. Among fungal types, Cladosporium, Alternaria, smut spores, Curvularia, Ascospores, Nigrospora, Aspergilli/Penicilli, and Epicoccum dominated the atmosphere in order of their prevalence.

[Haber's syndrome. First French family (2 cases)].

Haber's syndrome is a genodermatosis first described by Sanderson and Wilson (17) in 1965. Pursuing Dr. Haber's work, these authors reported three cases from one single family presenting with the dermatosis. A second family was reported by Seiji and Otaki (17) in 1971; Izaka described two cases (pedigree unknown, 13) and two more by Kikuchi (10, 13) in 1981 and 1983. We report here two new cases (brother and sister) discovered in 1984 (7) and representing the first French family. The dermatosis is characterized by clinical, genetic and histological criteria. It is transmitted as an autosomal dominant trait (fig. 1 and 2). Clinically, the face is affected by a rosacea-like dermatitis beginning in childhood and proceeding with pustular flare-ups. The facial lesions are frequently aggravated by exposure to the sun. The patients have pigmented keratotic lesions of the trunk resembling seborrhoeic warts. These lesions are very numerous and begin, on average, during the second decade of life. Some patients present with lesions that are diagnosed as Bowen's disease at histological examination (7, 17). Xerosis cutis is also present. Microscopically, the facial lesions consist of a necklace of basaloid cells around the hair and sebaceous follicles. The keratotic lesions have been reported as either intra-epidermal epitheliomas (17), or seborrheic warts without signs of malignancy (7, 10, 13), or equivalents of the facial lesions (19).

[Differential diagnosis of facial skin swellings (author's transl)].

Enlargement of the cheeks may be due to a multitude of disorders, congenital, neoplastic, and in particular inflammatory. Congenital facial anomalies include cutaneous (and osseous) hemihypertrophy of the face and unilateral angiomatous malformations (e.g. Sturge-Weber-Krabbe Syndrome). Buccal enlargement due to dermal tumours include localized haemangiomas and lymphangiomas, lipomas and other benign connective tissue neoplasms, generalized disorders of the lymphatic or reticuloendothelial system including mycosis fungoides, reticulum cell sarcoma and other soft tissue malignancies, and cutaneous manifestations of malignant haemoblastoses, in particular chronic lymphatic leukaemia. Within the very large group of inflammatory skin swellings of the face a review is made of some bacterial pyodermias, severe forms of acne vulgaris, herpes zoster, lupus vulgaris, erysipelas, rosacea, steroid dermatitis, lupus erythematosus (discoid and systemic), toxic dermatitis, allergic eczema, urticaria, Quincke's oedema, and the Melkersson-Rosenthal syndrome. The importance of prevention and early detection of steroid-induced dermatitis is emphasized. This disorder, which is a pseudo-inflammatory disfiguring complication of prolonged topical steroid abuse, ranks in frequency with the skin problems most often seen in dermatological practice.

Phytophotodermatitis.

Several plant families contain species liable to cause phytophotodermatitis. They include the Umbelliferae of which Ammi majus has been known for centuries and the Rutacea to which Citrus bergamia belongs. Phototoxic plants are also found in, e.g., the Moraceae, Leguminosae, Rosaceae and Compositae plant families. The photoreactivity of the plants depends on their furocoumarins (psoralens) content. These cause skin damage by phototoxic mechanisms, when activated by longwave ultraviolet radiation (UVA). The results are clinical changes such as erythema, bullae in the skin 24-72 h later, followed by hyperpigmentation. For the strongest phototoxic plants the content of the major phototoxic furocoumarins like bergapten or xanthotoxin have been calculated at approximately 0.5 g/100 g dried plant weight, but it varies between plant parts and also with the age of the plant. Repeated contact may occasionally lead to photocontact allergy.

The occurrence and seasonal distribution of C50-C60-polyprenols and of C100-and similar long-chain polyprenols in leaves of plants.

Large amounts of fully unsaturated, mainly-cis, higher isoprenoid alcohols consisting of 17-30 isoprene units were found in several plants of the Rosaceae family. They occur as mixtures of several prenologues with either C85 - or C100 - prenol dominating in the form of acetates. The highest level of these polyprenols (0.5-1.0% of wet weight) were found in Crataegus crus- galli , Cotonoaster lucida, Prunus serotina and Sorbus suecica (intermedia). Their content increased with increasing age of the leaves. The dynamics of this rise is different from that observed in the case of accumulation of free C50- C60 - prenols (up to 0.5% of wet weight) in leaves of various plant species.
