Coverage of heel and sole defects: a new subfascial arterialized flap.

Defects of the heel and sole represent difficult reconstructive problems in terms of tissue availability and postoperative morbidity. Although local flaps offer the optimal type of coverage, their application has been restricted to defects of limited size. A study of the plantar skin and fascia, as well as its neurovascular supply, was undertaken in cadavers. Mobility of local flaps is greatest with a narrow pedicle. Dissection deep to the plantar fascia allows the inclusion of the medial or lateral plantar nerve and artery with the flap, thereby allowing considerably mobility while preserving vascularity and sensibility. Several variations of the flap are presented with clinical examples.

Contact allergy to farnesol in 2021 consecutively patch tested patients. Results of the IVDK.

Farnesol is one of the fragrances considered to be a significant contact allergen. Therefore, it was decided by the European Union to label products containing farnesol. Farnesol was tested [5% petrolatum (pet.)] together with the standard series between 1 January 2003 and 30 June 2003 in 2021 consecutive patients, 1243 females and 778 males. Of these, 22 [1.1%, 95% confidence interval (CI): 0.7-1.6%] had a positive reaction to farnesol. 147 (8.1%) of those 1825 tested to Myroxylon pereirae resin (balsam of Peru, 25% pet.) at the same time reacted positively, 143 (7.8%) of those 1823 tested to the fragrance mix (FM) (8% pet.) and 34 (1.9%) of 1831 tested to propolis (10% pet.). With regard to concomitant reactions in farnesol-positive patients, 5 of 22 reacted additionally to the FM [odds ratio (OR): 4.3; CI: 1.53-12.15] and 2 (of these 5) additionally to M. pereirae resin (OR: 1.27; CI: 0.29-5.54). The strongest association was seen to propolis (OR: 6.2; 95% CI: 1.4-27.7). Compared to those with negative reactions to farnesol, the group of patients allergic to farnesol was characterized by a higher proportion of young females and office workers, and the hand and the face were more often affected. In conclusion, farnesol is an important allergen. We recommend that farnesol should be included in a fragrance patch-test preparation and that its use should be regulated for consumer safety reasons. Furthermore, the extent of exposure to farnesol should be further studied.

[Subcutaneous phaeohyphomycosis in the state of Pará: a report of 3 cases].

Three cases of subcutaneous phaeohyphomycosis from the state of Pará, Brazil are reported. This is the first time that this form of phaeohyphomycosis is described in northern Brazil. The etiologic agents could not isolated, and the diagnosis in all cases was established only on histopathological basis, particularly through the demonstration of fungal elements in tissue sections. Hyphal fragments and vesicular cells were very numerous and conspicuous in special (Grocott and Fontana-Masson methods) stained sections, in contrast to their apparent scarcity in routine hematoxilin-eosin preparations. The difference was certainly due to the aspect in the latter of the invasive tissue forms: the thin-walled and light brown pigmented organisms stained poorly and were very difficult to distinguish from the tissue components.

Allergic contact dermatitis from chlorhexidine.

Although chlorhexidine is a widely used substance, allergic contact dermatitis from chlorhexidine has rarely been reported. Our objective was to study all cases of allergic contact dermatitis from chlorhexidine that were seen at St John's Institute of Dermatology from January 1983 to June 2002. The case records of all patients with positive patch-test reactions to chlorhexidine during the study period were retrieved. The clinical presentation, strength, and relevance of the positive patch-test results as well as possible sources of exposure were studied. There were five cases of allergic contact dermatitis from chlorhexidine during the study period; three of these patients had positive patch-test reactions to chlorhexidine that were of current relevance. The possible sources of exposure included Hibisol, Hibiscrub, a chlorhexidine spray, and peri- and postoperative antiseptic solutions. We concluded that allergic contact dermatitis from chlorhexidine is rare. However, when it occurs, it may cause a severe dermatitis reaction.

[Some clinical and paraclinical observations about the syndrome of "acropathie ulcero-mutilante" (author's transl)].

A case of "acropathie ulcero-mutilante" is reported, some features of which contribute to a better knowledge of the following controversial points: a) the occurrence of sporadic non familiar cases; b) the existence of degenerative changes in the motor pathways; c) the hystochemical picture, which has been not previously described. This case confirm sporadicity and degenerative changes in motor pathways. The histochemical picture show typical signs of denervation.

Occupational allergic contact dermatitis to silver and colophonium in a jeweler.

The aim of this study was to determine any occupationally relevant allergic contact sensitizations in hand dermatitis in a jeweler. Patch test with European Standard, vehicle, medicaments, and metal series (Finn chambers on Scanpor) was performed. Readings were taken on day 2 and day 4. Allergic positive reaction to colophonium 20% pet and silver nitrate 0.5% aq was detected. The contact sensitivities to silver and colophonium seem to be occupationally relevant in this case.

[Botryomycosis].

Four cases of botryomycosis are presented. All patients had fistulous and ulcerated lesions of the lower limbs. Diagnosis was based on the clinical and laboratory findings (presence of grains on direct microscopy examination and positive bacterial cultures). All patients had osteomyelitis. Three were treated with antibiotics or sulfonamides (orientated by the antimicrobial susceptibility test) and were cured.
