Struma cordis. Ectopic thyroid goiter in the right ventricle.

Intracardiac thyroid goiter (struma cordis) is an exceptionally rare finding. To our knowledge, only two cases have been reported in the literature to date. A middle-aged woman was the subject of our case, in which an intracardiac tumor was clinically diagnosed and surgically resected. The mass, adhesive to the ventricular septum and obstructing the right ventricular outlow tract, histologically was formed by thyroid tissue, separated from the myocardium by fibrous tissue. The woman died of renal failure two weeks after the operation, and at autopsy the thyroid gland showed strumous changes. The similarity to previously reported cases suggests a dysembryologic origin of this ectopia.

[Cancer of the thyroid in an area of endemic goiter. "Papillarization" with prophylactic iodization].

Thyroid cancer is characterized by endemic goiter, usually where the insufficiency of iodine has lasted a long time. Prophylactic iodization produces a change in the histological distribution of the thyroid carcinomas, with a significant increase in the quantity of papillary carcinomas and a reduction in that of follicular carcinomas. In autopsies, occult carcinomas of minimal size are, with few exceptions, papillary. The change which is produced by iodization allows us to say that papillary carcinoma is the original carcinoma in the thyroid gland and that the follicular carcinoma, differentiated or less differentiated, may be evolutionary stages derived from the insufficiency of iodine. The insufficiency of iodine would be an active factor in the evolution of papillary thyroid cancer. This reasoning is maintained by the inversion of frequency of thyroid cancer in iodized areas and the remarkable regularity of findings of papillary cancer in autopsies.

An autosomal dominant form of adolescent multinodular goiter.

Eighteen members of an extended pedigree have been found to have a form of euthyroid adolescent multinodular goiter. Histological examination showed multiple adenomata with areas of epithelial hyperplasia, hemorrhage, and calcification. In two subjects there were focal areas of epithelial hyperplasia reminiscent of low-grade papillary carcinoma, but capsular and vascular invasion was not found. The pattern of inheritance appeared to be autosomal dominant, with diminished penetrance in males. Although the patients were euthyroid, the likely basis for this disorder is an abnormality in thyroglobulin structure and function.

[Quantitative composition and functional role of thyroid phospholipids in nodular hypothyroid goiter].

Correlations between change in the composition of thyroid nodal tissue individual phospholipids and intensity of oxidizing processes under the influence of various phospholipid liposomes were studied. The thyroid phospholipid fraction level was investigated in patients with nodular goiter. A decrease in the level of sphingomyelin and phosphatidylinositide by 54%, cardiolipin by 63%, phosphatidic acid by 70%, phosphatidylserine by 46%, phosphatidylcholine by 7% and a two-fold increase in the level of phosphatidylethanolamine as compared to the control one were shown. A study of a degree of reduction of oxidation metabolism by adding phospholipid liposomes to thyroid sections showed that the tissue of a node possessed weaker oxidizing activity as compared to the control one. O2 absorption and 14CO2 excretion up to 30% were lowered in a hyperthyroid node. An activating liposome effect was noted in thyroid nodal tissue, mostly in the presence of phosphatidylserine, cardiolipin and phosphatidylinositide; the weakest effect was produced by phosphatidylethanolamine and phosphatidylserine.

[Damage to the recurrent nerve during an operation for benign goiter].

Damage of the recurrent nerve was found in 40 (2.5%) of 1555 patients operated upon for benign goiter. The probability of complications was higher in wider volumes of operations. The main task of prophylactics of damages of the recurrent nerves is to identify them during operation and to observe the principle of subfascial dissection and resection of the thyroid gland.

Long-term follow-up after iodine-131 treatment for Plummer's disease (autonomous goiter).

A follow-up study is presented in 88 patients treated with I-131 for Plummer's disease (localized autonomous thyroid function, either multifocal or as a solitary nodule) one to 17 years before the present study. Studies included clinical examination, scintigraphy, and function tests. One patient was hypothyroid, seven were marginally hyperthyroid, and five still received low dose antithyroid drugs. Of 75 euthyroid patients, the thyrotropin (TSH) response to thyrotropin-releasing hormone (TRH) was absent in 16 (generally with scintigraphic evidence of autonomous function), subnormal in 20, and normal in 39. A single autonomous nodule prior to treatment was found relatively frequently in males and in patients with a normal TRH test at follow-up. Most goiters had become smaller and one third of all solitary nodules could not be detected anymore. Autonomous function at follow-up was probably due to residual rather than recurrent disease in most, if not all, cases. It is concluded that I-131 therapy is at least as satisfactory as partial thyroidectomy in the treatment of Plummer's disease; lifelong follow-up was not found to be necessary.

Parallax error in pinhole thyroid scintigraphy: a critical consideration in the evaluation of substernal goiters.

Parallax error is found to be another pitfall in pinhole thyroid scintigraphy, i.e., a substernal goiter may appear between the thyroid cartilage marker and the suprasternal notch marker. Unless one is aware of this pitfall, a substernal extension of the thyroid may not be realized or be seriously underestimated in its size and extent. We illustrated the parallax error using a thyroid phantom. An illustrative case is shown.

Diagnostic Errors↗

[Fine needle biopsy of cold goiter nodules (author's transl)].

Fine needle biopsy of hypofunctional, scintigraphically detected cold goiter nodules is being increasingly applied as a diagnostic method. Its diagnostic value is estimated by the results gained from 4,555 punctured patients, 1,076 of these nodules were histologically proven, and from the literature. The most important indication for the application of this method is a neoplastic lesion of the thyroid. 6.8% of 20,384 nodules were cytologically suspicious or malignant. Of 2,412 surgically controlled patients 85.5% were correctly diagnosed. Among 490 malignancies 92.1% were correctly diagnosed and 7.9% were false negatives.

[Tactics in the surgical treatment of nodular euthyroid goiter].

Based upon the experience with the treatment of 2759 patients the authors have established indications for surgery for nodular neoplasms of the thyroid since cancer can develop in the guise of or parallel with them. Such patients must be examined and operated upon under conditions of specialized hospitals. Grounds for indications for local and general anesthesia are given as well as for intrafascial resection of the thyroid as the operation of choice for nodular forms of euthyroid goiter.

[Operative technic in nodular goiter and cancer of the thyroid].

Based on the literature data and personal experience the author recommends the extrafascial technique of operation in cases with thyroid cancer. In nodular and polynodular goiters a subfascial operation is necessary since it prevents many complications.

Surgery of toxic goiter: indications and long-term results.

Treatment of hyperthyroidism is still debated, surgery in particular is conceived as the last step of a multidisciplinary approach. 130 patients were reviewed in a follow-up based on a double cell at fixed dates, after subtotal thyroidectomy for multinodular and diffuse toxic goiter. Clinical and laboratory tests (T3, T4, TSH) showed a rate of 9.1% of recurrent toxicity and 20.7% of hypothyroidism. These data confirm that surgery offers satisfactory results in the management of hyperthyroidism if indications are carefully followed. It is also shown that TSH has a predictive role in the screening of patients at risk of developing postoperative hypothyroidism.

Histopathologic features of the skin of hairless newborn pigs with goiter.

The skin of 15 hairless newborn pigs with parenchymatous goiter was studied histopathologically. Most of the lesions were in the hair follicles; follicles were scarce and reduced in size, contained slender hairs, and revealed a shortage of penetration into the hypodermis. Hair follicles in the immature stages, hair pegs, and germs also were recognized, with the thin epidermis lacking in keratinization. It was felt that these changes resulted from a retardation of cellular differentiation of the germinal layer. In view of these findings, skin lesions were regarded as hypoplasia of hair.

[Mode of action and tolerance of a single weekly dose of 1 mg of L-thyroxine for the prevention of recurrence of goiter].

After resection of euthyroid goiter, different doses of thyroxine administration for prophylaxis against recurrence were tested and compared. Group 1 (n = 30) received 1 mg L-thyroxine weekly; group 2 (n = 31), 0.1 mg L-thyroxine daily. The intermittent, high-dose thyroxine turned out to be equally effective as the conventional daily intake and showed no negative side effects.

Follow-Up Studies↗

[Rheoencephalography and the study of the pulsation of the cervical vascular network in the diagnosis of goiter and neck and mediastinal tumors].

The authors have applied rheography of the brain and sphygmography of the vessels of the neck and mediastinum for the diagnosis of compression of the neck vessels and organs in goiter and tumors of retrosternal and deep cervical localization. The methods allow to recognize stenosing of the vessels in early stages and facilitate the diagnosis of these conditions.

[Changes in the acetylcholine concentration and acetylcholinesterase activity in the thyroid gland tissues and in serum cholinesterase activity in goiter].

A study was made of acetylcholine concentration and of the acetylcholinesterase activity in the thyroid gland tissue, as well as of the serum cholinesterase activity in the toxic goiter. This disease was accompanied by an elevation of the acetylcholine concentration and a fall of the eacetylcholinesterase activity. As to serum cholinesterase--it changed but little in this disease.

The incidence of thyroiditis lesions in nodular goiter.

On the basis of clinical and biological data, surgical investigation and histopathological aspects, a number of 35 localized froms of thyroiditis have been given a diagnosis in order to assess their incidence in case of both nodular goiter and chronical inflammatory processes at the thyroid gland level. Three possibilities are considered: the proper inflammatory lesions--granulomatous, lymphoplasmocitary and ligneous thyroiditis, the cystic formations circumscribed by the glandular tissue with inflammatory signs as well as the association of thyroid cancer with chronic thyroiditis--seldom reported in literature--upon which 2 observations have been made.

[Protein biosynthesis in a cell-free protein synthesis system from nodular euthyroid goiter cells].

Analysis of proteins, synthesized in cell-free system using nodal euthyroid goiter, was carried out by polyacrylamide gel disc electrophoresis. Four protein farctions, incorporating labelled amino acids, were found: "complete" thyroglobulin, two its subunits and thyroalmbumin. This correlates with the concept that malignant cell synthesizes intensively proteins untypical for a normal tissue.
