Ontogeny of pig discrete Peyer's patches: distribution and morphometric analysis.

We investigated the development of lymphoid and non-lymphoid cells in discrete Peyer's patches (PP) of the pig using immuno-histology and image analysis. In newborn piglets discrete PP were mainly populated by CD2+, CD3+ T cells, and major histocompatibility complex class II+ cells, many of which were of macrophage and dendritic cell lineage. Four days after birth, cells were localized in defined regions: the follicle; the inter-follicular area and the dome region. Compartmentalization within the follicle started about 6 days after birth. The first signs of secondary follicles were seen from about 14 days. The pig discrete PP attained their mature structure at about 3 weeks after birth. Here we show that despite the demonstration at birth of the cell types that support antigen processing and presentation, PP did not fully differentiate morphologically until at least this time when antigen can be handled in an efficient manner.

Development of rats' maternally directed orienting behaviors from birth to day 2.

We previously observed 2- to 3-day-old (P2-3) rats perform maternally directed orienting behaviors (MDOBs)-locomotion, torso flexion, rotation to the supine orientation, and snout probing, accompanied by audible vocalizations (barking)-under surfaces having properties of the dam's ventrum, which result in close contact between the pup's ventrum and the surface. We now report on the development of MDOBs from birth through P2. Wistar rats (24 litters) were tested at one of four times: 1 hr after birth before the first nursing bout (P0pre), on the day of birth after the first nursing bout (P0post), on postnatal Day 1 (P1), or on postnatal Day 2 (P2). Pups were placed prone on a warmed platform inside a corral where, for 4 min, they encountered one of two motherlike surfaces at head height: (a) a soft cotton roof or (b) a patch of the pup's own anesthetized dam's ventrum. Control pups received no overhead stimulus. The behaviors we had identified as MDOBs and others, including limb behaviors, are easily recognized at P0pre, and, in aggregate, are more frequent then than at any later age. The differential responsiveness of most of the behaviors to the dam's ventrum compared to the cotton roof or open condition is relatively low at P0pre, but rises rapidly through P2. Behavioral structure develops quickly from a rudimentary principal component with two behaviors at P0pre to two principal components incorporating seven behavior classes by P2. MDOBs are discussed in relation to late-term fetal behaviors, early postnatal righting responses, and the oral nipple grasp, and as a model of the development of filial attachment in an altricial mammal.

Tamm-Horsfall protein as a marker of tubular maturation.

Tamm-Horsfall protein (THP), a glycoprotein with a molecular weight of 95 kilodaltons, is produced and secreted in the ascending loop of Henle. To evaluate the measurement of THP in the assessment of fetal renal development and function, we stained fetal kidney sections for THP and measured THP concentrations in 129 amniotic fluid samples from healthy pregnancies, together with other parameters such as transferrin, albumin, alpha 1- and beta 2-microglobulin. After the 16th week of gestation THP could be detected immunohistochemically in the distal tubular cells, but was not consistently detected by sandwich enzyme immunoassay until after the 20th week of gestation (detection limit 50 ng/ml). Between the 15th and 19th week of gestation THP was only detected occasionally, but after the 20th week of gestation the concentration increased significantly reaching levels of 0.4-4 mg/l at term. The THP concentration was lower in samples taken directly before birth than in the corresponding first urine after birth, indicating that THP is produced from the fetal kidney only and does not pass the placental barrier. This pattern was different from other proteins studied. Transferrin and albumin were significantly lower in the first urine voided, microglobulins remained unchanged, and the creatinine concentration increased. This indicates that maternal to fetal exchange or transport is likely for most of the other proteins. Measurement of THP concentrations, in addition to other proteins in the amniotic fluid, can improve fetal renal assessment, but because the range of THP concentrations is wide accurate predictions are still not possible.

Ontogeny of Peyer's patches of the rat.

The development of lymphoid and nonlymphoid cells in Peyer's patches (PP) of the rat was investigated using light microscopical methods (routine histological techniques, enzyme histochemistry and immunohistochemistry). In newborn rats PP were mainly populated by T lymphocytes and Ia-positive nonlymphoid cells, which most likely are interdigitating cells. At about 12 days after birth the B and T cells were localized in defined regions, the follicular (FA) and interfollicular area (IFA), respectively. Compartmentalization within the FA started about 14 days after birth. The first signs of the development of secondary follicles were seen from about 18 days onward. PP obtained their mature structure at about 4 weeks after birth. It is suggested that after PP had developed fully, cells having cytoplasmic IgA migrate via the high endothelial venules (HEV) to the lamina propria of the intestine; cIgM and IgG cells seem to develop locally within the FA.

Prevalence of risk factors in breast cancer patients at the University Hospital of the West Indies.

The records of one hundred and twenty consecutive patients diagnosed with carcinoma of the breast were examined between July 1996 and June 1999 in order to estimate the prevalence of established risk factors for this disease. Early menarche was present in 5.5% of patients, while 36 subjects (30%) were nulliparous and 6 (5%) had a first live birth after age 30 years. Four subjects had a previous biopsy with histological features of atypical hyperplasia. Fifteen subjects (12.5%) had one or more affected first degree relatives. Fifty-four per cent of subjects possessed none of the risk factors examined in this study while 36% had a single risk factor and 10% possessed two risk factors. There was no significant clustering of risk factors in the group of patients aged thirty-five years or less. Larger studies should be encouraged to identify the risk factors which operate in our population and the degree to which published predictive variables are applicable.

Some observations on dermis development in fetal porcine skin.

The study describes dermis development in fetal porcine skin between 40 d GA and birth. The first fibroblast arrangement occurs about 47 d GA, while between 60-65 d GA the first fibres are visible. They can be identified as collagen about 67 d GA, somewhat later the dermis shows 2 strata. At the end of median gestional life, the first birefringent lattice pattern of collagen fibre bundles is formed. The first elastic fibres cannot be demonstrated before 100 d GA.A clear increase in fibrocyte numbers occurs about 70-80 d GA, the next one at birth. The thickness development of the fetal porcine dermis is closely correlated with the increase in body weight. Additionally RNA and glycogen content developments have been studied in the fetal dermis.

[A computer filling system for patient records of recurrent and metastatic uterine cervix cancer].

This report describes a computer filing and retrieval system for patient records of uterine cervix cancer which contain a secondary status after first treatment necessitated by residual primary tumor, recurrence after 6 months or more, and metastasis. In 2 working sheets are entered registration number and birth-data, first therapy method, secondary status, topographic locations of recurrence and metastasis, policy and outcome at 5 years after secondary treatment. This information was entered in the computer filing system. The records of 549 patients who were treated between 1961 and 1976, were loaded in to the disk file and magnetic tape of our computer. The 5-year cumulative survival rate of the total cases was 11.7% (1.4% standard error); it was 26.9% (S.E. = 4.4%) for patients that had undergone primary surgery. We found that the longer the interval between first therapy and recurrence, the better was the 5-year survival after secondary treatment (mainly radiotherapy). We conclude that long-term follow-up study is important in patients with uterine cervix cancer, because our 28 cases with recurrence (10% of total cases), were found more than 4-years after the first therapy.

Maternal nutrition in pregnancy and the first postnatal year--2. After the birth.

In the second of a two-part article, a senior research scientist discusses the mother's nutritional needs during lactation and the first year after the birth. The mother's needs for energy increase during breast-feeding, but conversely studies have shown that breast-feeding mothers do not necessarily lose weight. As in pregnancy, it is recommended that all lactating women should take a supplement that provides 10 microg per day of vitamin D. Healthy diets for breastfeeding women are essentially the same as for other adults. Parents are often receptive to advice at this time and this is a good opportunity for health professionals to encourage breast-feeding and a healthy diet as this may promote future health.

Pregnancy-related mortality from preeclampsia and eclampsia.

OBJECTIVE: To examine the role of preeclampsia and eclampsia in pregnancy-related mortality. METHODS: We used data from the Centers for Disease Control and Prevention's Pregnancy Mortality Surveillance System to examine pregnancy-related deaths from preeclampsia and eclampsia from 1979 to 1992. The pregnancy-related mortality ratio for preeclampsia-eclampsia was defined as the number of deaths from preeclampsia and eclampsia per 100,000 live births. Case-fatality rates for 1988-1992 were calculated for preeclampsia and eclampsia deaths per 10,000 cases during the delivery hospitalization, using the National Hospital Discharge Survey. RESULTS: Of 4024 pregnancy-related deaths at 20 weeks' or more gestation in 1979-1992, 790 were due to preeclampsia or eclampsia (1.5 deaths/100,000 live births). Mortality from preeclampsia and eclampsia increased with increasing maternal age. The highest risk of death was at gestational age 20-28 weeks and after the first live birth. Black women were 3.1 times more likely to die from preeclampsia or eclampsia as white women. Women who had received no prenatal care had a higher risk of death from preeclampsia or eclampsia than women who had received any level of prenatal care. The overall preeclampsia-eclampsia case-fatality rate was 6.4 per 10,000 cases at delivery, and was twice as high for black women as for white women. CONCLUSION: The continuing racial disparity in mortality from preeclampsia and eclampsia emphasizes the need to identify those differences that contribute to excess mortality among black women, and to develop specific interventions to reduce mortality from preeclampsia and eclampsia among all women.

Maternal smoking and birth weight.

To determine whether it is the smoker or smoking per se that explains the observed reduction in birth weight of infants of mothers who smoked during pregnancy, data collected on 1016 pairs of successive births which occurred between 1946 and 1963 in Washington County, Maryland, were studied. The mean birth weight difference between first and second members of pairs of consecutive live births of the same mother was examined in relation to her smoking habits during both pregnancies. A multiple regression technique was used to simultaneously adjust for the effects of maternal smoking habits during pregnancy, infant sex, maternal age, and birth order on birth weight difference. There were no significant differences in mean birth weight difference among pairs in which the mother smoked throughout both pregnancies, pairs in which the mother was a nonsmoker during both pregnancies, and pairs in which the mother smoked during the second pregnancy of the pair, but not the first. When birth weights of only first members of the pair were studied, the mean birth weight of infants of women who were nonsmokers throughout both pregnancies was higher than that of infants of women who smoked during both pregnancies. Infants of first members of pairs in which the mothers smoked only during the second pregnancy tended to have birth weights which were lower than that of infants of nonsmokers and higher than that of infants of smokers. These findings neither confirm nor deny the hypothesis that the smoker rather than the smoking per se causes a reduction in birth weight.

Lack of association between calf morbidity and subsequent first lactation milk production in 25 New York Holstein herds.

The association of owner-diagnosed calfhood diseases with first lactation milk production was evaluated using data collected prospectively for 728 heifers from 25 New York dairy herds. Cows diagnosed with one or more occurrences of dullness, respiratory disease, or scours within 90 d of birth had first lactation milk production that was similar to that of unaffected herdmates. Estimates for the effects of dullness, respiratory disease, and scours on second test milk production were -1.0, 1.0, and .4 kg/d, respectively (adjusted for season and age at calving, times milked per day, study month on birth, sire PTA for milk production, and the other two calfhood diseases). Estimates for the effects on 305-day mature equivalent production were -139, 123, and 158 kg for dullness, respiratory disease, and scours, respectively (adjusted for study month of birth, sire PTA for milk production, and the other two calfhood diseases). These results were based on data of heifers that were selected as replacements and remained in the herd long enough to have milk production recorded. There was no significant detrimental effect of owner-diagnosed calfhood disease on first lactation milk production, but affected heifers had higher mortality before calving and were less likely to enter the milking herd.

Case-control study of borderline ovarian tumors: reproductive history and exposure to exogenous female hormones.

Borderline ovarian tumors possess many of the same morphological features as their malignant counterparts, but they do not destructively invade the ovarian stroma, and the women in whom they develop generally have a favorable prognosis. Female residents of three urban counties of western Washington diagnosed with serous and mucinous borderline ovarian tumors between 1980 and 1985 (n = 116) were interviewed regarding past reproductive events. A random sample of women from the same counties (n = 158) was identified through random digit dialing and were interviewed. The risk of these ovarian tumors among women who had given birth to 1 or 2 children and to 3 or more children was, respectively, 0.7 and 0.4 that of nulliparous women. There was no consistent influence of increasing age at first live birth. Adjusting for parity, a history of lactation was associated with a 50% reduction in risk. Among nulliparous women, a further increase in risk was present in those who reported a history of infertility. Use of oral contraceptives was associated with a 60% reduction in risk. However, the size of the association was not dependent on duration, age at first use, or years since last usage. In conclusion, borderline tumors appear to have similar epidemiological patterns with regard to reproductive events as their more malignant counterparts.

Total intravenous anesthesia with remifentanil, propofol and cisatracurium in end-stage renal failure.

PURPOSE: To compare recovery parameters of total intravenous anesthesia (TIVA) with remifentanil and propofol, hemodynamic responses to perioperative events, and pharmacodynamic parameters of cisatracurium in 22 end-stage renal failure and 22 normal renal function patients. METHODS: Anesthesia was induced with 2-3 mg x kg(-1) propofol and 1 microg x kg(-1) remifentanil and maintained with 75 microg x kg(-1) x min(-1) propofol and propofol initial infusion of 0.2 microg x kg(-1) x min(-1) propofol. Arterial pressure and heart rate were maintained by remifentanil infusion rate adjustments. The first twitch (T1) was maintained at 25% by an infusion of cisatracurium. RESULTS: There was no difference in the time to maintenance of adequate respiration, date of birth recollection, first analgesic administration, between the renal failure (4.8+/-2.5, 7.8+/-3.2, 12.3+/-5.3 min respectively) and the control group (5.2+/-2.8, 8.1+/-3.1, 12.7+/-5.5 min): nor were there any differences in the time to 25% T1 recovery, T1 recovery from 25% to 75%, or cisatracurium infusion rate between the renal failure group (32.1 +/-10.8 min, 18.2+/-5.5 min, 0.89+/-0.29 microg x kg(-1) min(-1) respectively) and the control group (35.9 (7.9 min, 18.4+/-3.8 min, 0.95+/-0.22 microg x kg(-1) x min(-1)). CONCLUSION: End-stage renal failure does not prolong recovery from TIVA with remifentanil and propofol, or the recovery from cisatracurium neuromuscular block.

Cumulative pregnancy and live birth rates after gamete intra-Fallopian transfer.

To evaluate the efficacy of gamete intra-Fallopian transfer (GIFT) the Kaplan-Meier life table method was used to analyse a patient cohort treated with GIFT between 1991 and 1994. In a tertiary referral centre for reproductive medicine, 1628 women with a median age of 33 years and various causes of infertility were included to calculate cumulative pregnancy and live birth rates. Age and cause of infertility were main factor variables and the study was based on a total of 2941 consecutive GIFT cycles, leading to a first clinical pregnancy, and 3052 cycles, leading to a first live birth. The cumulative pregnancy and live birth rates were 49.6 and 38.8% respectively, after three initiated cycles and 64.1 and 52.0% respectively, after five initiated cycles. The multiple pregnancy rate was 22.6%. The implantation rate of 13.1% after GIFT demonstrates that the developing embryo benefits from a period of exposure within the environment of the Fallopian tube. The present results indicate that approximately 50% of couples will have at least one live baby after five initiated GIFT cycles. Advancing age was a major negative prognostic factor for the cumulative live birth rate because of higher cancellation rates, lower implantation rates and higher pregnancy failure rates.

Clinical detection of low upper body blood flow in very premature infants using blood pressure, capillary refill time, and central-peripheral temperature difference.

OBJECTIVE: To determine the accuracy of blood pressure (BP), capillary refill time (CRT), and central-peripheral temperature difference (CPTd) for detecting low upper body blood flow in the first day after birth. METHODS: A prospective, two centre cohort study of 128 infants born at < 30 weeks gestation. Invasive BP (n = 108), CRT (n = 128), and CPTd (n = 46) were performed immediately before echocardiographic measurement of superior vena cava (SVC) flow at three, 5-10, and 24 hours after birth. RESULTS: Forty four (34%) infants had low SVC flow (< 41 ml/kg/min) in the first day, 13/122 (11%) at three hours, 39/126 (31%) at 5-10 hours, and 4/119 (3%) at 24 hours. CPTd did not detect infants with low flows. Combining all observations in the first 24 hours, CRT > or = 3 seconds had 55% sensitivity and 81% specificity, mean BP < 30 mm Hg had 59% sensitivity and 77% specificity, and systolic BP < 40 mm Hg had 76% sensitivity and 68% specificity for detecting low SVC flow. Combining a mean BP < 30 mm Hg and/or central CRT > or = 3 seconds increases the sensitivity to 78%. CONCLUSIONS: Low upper body blood flow is common in the first day after birth and strongly associated with peri/intraventricular haemorrhage. BP and CRT are imperfect bedside tests for detecting low blood flow in the first day after birth.
