Overview of an architectural approach to the development of the Johns Hopkins Hospital distributed clinical information system.

The Johns Hopkins Hospital has initiated an ambitious program to apply modern technologies to the development of a new, comprehensive clinical information system. Since many components of a system exist and since the JHH management is decentralized, a distributed, networked system is being built. To properly accomplish this task an architectural basis for strategic systems planning was developed. This paper reviews briefly the layered architectural approach.

A clinical database management system for improved integration of the Veterans Affairs Hospital Information System.

The Department of Veterans Affairs (VA) Decentralized Hospital Computer Program (DHCP) contains data modules derived from separate ancillary services (e.g., Lab, Pharmacy and Radiology). It is currently difficult to integrate information between the modules. A prototype is being developed aimed at integrating ancillary data by storing clinical data oriented to the patient so that there is easy interaction of data from multiple services. A set of program utilities provides for user-defined functions of decision support, queries, and reports. Information can be used to monitor quality of care by providing feedback in the form of reports, and reminders. Initial testing has indicated the prototype's design and implementation are feasible (in terms of space requirements, speed, and ease of use) in outpatient and inpatient settings. The design, development, and clinical use of this prototype are described.

PADS (Patient Archiving and Documentation System): a computerized patient record with educational aspects.

Rapid acquisition and analysis of information in an Intensive Care Unit (ICU) setting is essential, even more so the documentation of the decision making process which has vital consequences for the lives of ICU patients. We describe an Ethernet based local area network (LAN) with clinical workstations (Macintosh fx, ci). Our Patient Archiving and Documentation System (PADS) represents a computerized patient record presently used in a university hospitals' ICU. Taking full advantage of the Macintosh based graphical user interface (GUI) our system enables nurses and doctors to perform the following tasks: admission, medical history taking, physical examination, generation of problem lists and follow up notes, access to laboratory data and reports, semiautomatic generation of a discharge summary including full word processor capabilities. Furthermore, the system offers rapid, consistent and complete automatic encoding of diagnoses following the International Classification of Disease (ICD; WHO, [1]). For educational purposes the user can also view disease entities or complications related to the diagnoses she/he encoded. The system has links to other educational programs such as cardiac auscultation. A MEDLINE literature search through a CD-ROM based system can be performed without exiting the system; also, CD-ROM based medical textbooks can be accessed as well. Commercially available Macintosh programs can be integrated in the system without existing the main program thus enabling users to customize their working environment. Additional options include automatic background monitoring of users learning behavior, analyses and graphical display of numerous epidemiological and health care related problems. Furthermore, we are in the process of integrating sound and digital video in our system. This system represents one in a line of modular departmental models which will eventually be integrated to form a decentralized Hospital Information System (HIS).

The ultrastructure of the rabbit pineal gland after sympathectomy, parasympathectomy, continuous illumination, and continuous darkness.

The ultrastructure of the rabbit pineal gland was investigated after sympathectomy (extirpation of decentralization of the superior cervical ganglia), parasympathectomy, continuous illumination and continuous darkness. The similarity of the ultrastructural changes in the light pinealocytes occuring after sympathectomy and after continuous illumination was striking. It is supposed that these changes have a common cause, viz. the lack of free noradrenaline, the pinealotropic neurotransmitter. The smooth endoplasmic reticulum present in the terminals of the offshoots of the light pinealocytes is possibly involved in pineal indoleamine synthesis.

Provincial variation of urbanization and urban primacy in China.

Provincial variations in urbanization and urban primacy in China are analyzed for 1978 using factor analysis and regression techniques. The authors suggest that government policies have not only led to a relatively low level of urbanization but are also responsible for many of the regional differences observed. It is concluded that "low urban primacy in the eastern provinces is mainly the result of the urbanization policy of controlling the development of large cities that favours the development of small and medium cities. The spatial industrial policy of decentralizing industries from the coastal provinces to interior provinces encouraged high urbanization and urban primacy in the western interior provinces of China."

[Evaluation of contact and care centers, a low-threshold daytime source in the city of Zurich].

Contact centres for drug addicts in the city of Zurich exist since 1988 to provide decentralized low threshold meeting places and contact centres for persons who are poor and neglected. The main objective of these survival aids is to counteract further impoverishment of the addicted in order to actively contribute to their subsistence. Since its foundation the project as a whole has been submitted to numerous structural changes which had to be taken into consideration by the evaluation. Subject of the examination was the description of the target group and the staff, the use of the services and their effect on the users as well as the environment. Only quantitative methods were used. The target group of the centers consisted of drug consumers who at the best are partially socially integrated, but usually completely desintegrated. The evaluation revealed that the different contact centers services were extensively used and met the focal needs of their users. However, the high fluctuation of personnel within the project teams turned out to be a permanent problem for the project management and the operators. As main reasons for the fluctuation the authors discuss the great dependency on political decisions and the problematical demanding clientele. Due to these structural turbulences some staff related offers are only accepted to a limited degree, and the contact centers tend to become simple supply services. Therefore the authors endorse the redefinition of long term objectives of the institution under consideration of the necessary framework.

Universal health insurance in Canada: history, problems, trends.

This paper describes the universal health insurance program in Canada and identifies the historical events and social values leading to its adoption. Universal hospital insurance was adopted in 1958, ten years before medical insurance, as a result hospital-based patterns of practice were solidified. Through cost sharing, the federal government influenced the provinces to enact relatively uniform universal plans. From 1951 to 1971 health care expenditures rose rapidly to 7.3% of the gross national product (GNP), but have since decreased and stabilized at about 6.9%. In contrast, health care in the United States represents 8.6% of GNP. Hospital use also increased rapidly in Canada to 1970 but appears to have stabilized and decreased slightly in this decade. Physician incomes rose rapidly before 1971, but since then the increases have slowed and relative incomes of physicians have fallen. Althouth the percent of GNP spent for health care has leveled, there are still substantial annual increases in expenditures that are paid for by government. Two federal initiatives, Bill C-37 and the Lalonde Report, have their roots in cost containment; Bill C-37 transfers greater taxing authority from the federal government to the provinces. To meet the goal of containing costs, provincial governments are moving in the direction of regionalization, decentralization, and greater coordination. In the short term, the provinces have limited hospital budgetary increases to percentages less than the rate of inflation. Cost constraints may be long overdue. Imposing fiscal limits encourages rational planning. It does not appear that the health of Canadians will be adversely affected or essential benefits curtailed by present budgetary restrictions or reorganization.

On the inter-relationship of cognition and affect: fantasies of deaf children.

A few projective studies focusing on the impact of cognition on affect in the deaf have reported strikingly disparate results with little systematic inquiry. A new and previously unreported play situation (with the deaf) was devised to elicit a detailed story. Fantasies were obtained in a uniform fashion from four groups of five-year-old boys comprising middle-class deaf and hearing children and lower socio-economic-class deaf and hearing children. Results suggest that: (1) The impact of deafness on cognitive functioning in these children is akin to--but apparently no worse than--the cultural deprivation phenomenon experienced by low socio-economic-status children. (2) There is no paucity of fantasy life in the deaf or deprived child, despite the absence or relative lack of verbal skills. (3) A problem common to both disadvantaged children and the deaf children tested in the intrusion of anxiety into autonomous ego functions, so that practice necessary for the firm development of cognitive schemata is interfered with. (4) The clinical impulsivity secondary to these emotional pressures may perpetuate itself by interfering with cognitive decentration in time and space in relation to others.

[Demonstration of reticulocytes and three-dimensional presentation of blood cells by means of reflexion contrast (author's transl)].

Reticulocytes and erythrocytes are distinguishable in Giemsa-stained blood smears by means of reflexion contrast, and adjunct to a reflected-light microscope. The surface topography of red cells can be quantitatively analyzed; it is directly visible in oblique illumination after decentralization of the central diaphragm. Reticulocytes are about 15% larger and show less deviations from the circular form and a less deep deviations from the circular form and a less deep central depression than mature erythrocytes. After supravital staining with brilliant cresyl blue, 72% more red cells can be diagnosed as being reticulocytes in reflexion contrast than in bright field.

Migration cost externality and interregional equilibrium.

"This paper will investigate the characteristics of population allocation between two regions in the presence of migration cost. It will also examine both populations and the non-migration range of the initial population in which migration does not occur, in social optimum and market equilibrium with central government intervention, to reveal migration cost externality, and to propose a remedy for it." The author finds that "migration cost gives the social planner an additional burden of population reallocation, and it has an important effect upon an individual's decisions on migration in a decentralized market mechanism."

Prehospital and hospital coronary care.

This article reviews the current state of services for coronary care. Since the majority of deaths from coronary heart disease occur early and outside the hospital, the importance of the prehospital phase is emphasized. The delay in this period, which is very dangerous for the patient, should be reduced and mobile coronary care units (MCCU's) are one possibility to reduce the rate of sudden coronary death. Different systems of MCCU's are discussed: (1) those based on a hospital coronary care unit, usually accompanied by a nurse and/or a doctor and (2) those integrated into an already existing decentralized emergency system (e.g. fire department) run by paramedics. Although long-term survival of patients resuscitated from ventricular fibrillation is not so good, the results of many of these units are remarkable.

Pain and symptom control for cancer patients at the University Hospital in Essen: integration of specialists' knowledge into routine work.

At the University Hospital of Essen, supportive care for patients with cancer and other painful diseases is carried out by an interdisciplinary ambulant pain clinic supported by a pain conference with delegates from all departments involved in the care of cancer patients as permanent members. More than 600 in- and outpatients per year are treated by this institution. This model tends to integrate supportive care into the overall therapeutic concept and routine work and to improve education in this field by bedside teaching and training of local specialists in every department. Therefore, when hospitalization becomes inevitable it is realized within the department mainly concerned with the underlying disease, where the registrar and the physician belonging to the palliative care team work closely together. Scientific research in the field of palliative care, including supportive care, is a further concern of the pain clinic. Evaluation of the model shows that the concept has been realized within a decentralized and interdisciplinary setting; it must, of course, be borne in mind that the staff of the pain control clinic are deeply committed to their work. In conclusion, the way supportive care is realized in Essen can be recommended for large hospitals, and especially for medical schools.

Differential effects of sympathetic activity on AV junctional automaticity and AV conduction.

Rapid ventricular response during episodes of supraventricular tachycardia are often followed, on abrupt cessation of the tachycardia, by prolonged pauses terminated by a sluggish and sometimes erratic escape of a supraventricular pacemaker. Such chronotropic-dromotropic paradoxes are readily reproduced in the animal laboratory following elimination of the sinus node and bilateral decentralization of the stellate ganglia and vagi. This study examined whether left stellate stimulation (0.5, 1, 2, 4, 8 and 16 Hz) or lack thereof differentially affected AV junctional automaticity and AV conduction. In the absence of any sympathetic neural activity (maximal sympathetic deficit), the AV junctional rate averaged a mere 22 +/- 2 percent of its peak performance, whereas under the same conditions, anterograde AV conduction averaged 73 +/- 5 percent and retrograde VA conduction 56 +/- 13 percent of their respective peak performances. On comparing the response curve (normalized responses) for AV junctional automaticity with that obtained for anterograde AV conduction the differences were significant at all frequencies between 0 and 4 Hz. Retrograde VA conduction (as assessed by the fastest ventricular pacing rate still conducted 1:1 to the atria) was always significantly less than anterograde AV conduction (as assessed by the fastest atrial pacing still conducted 1:1 to the ventricles). These results indicate that AV junctional automaticity is considerably more affected by sympathetic deficit than are either anterograde or retrograde AV conduction. In other words, AV junctional automaticity is far more dependent upon sympathetic input than AV conduction. While sympathetic influence is critical to the escape and maintenance of AV junctional automaticity both anterograde and retrograde AV conduction are remarkably resilient even under conditions of severe sympathetic deficit.

Centralized preparation of hazardous drugs. A choice between isolator and laminar airflow.

In 1987, the manager of the Saint-Joseph Hospital (Paris, France) requested a reorganization of cytotoxic drug preparation. Protection for staff who handle hazardous drugs was the main concern. The conclusions drawn from a first analysis emphasize the advantages of a centralized reconstitution unit against a decentralized system. Subsequently, a workload study and an economic study (investment, maintenance, supplies, staff costs, comparative balance sheet and a 5-year simulation) were carried out, but to choose between a laminar airflow in aseptic room and an isolator in a conventional room. The selected isolator is the first of the conception: the central half-suit uses as a server, and four sleeves located on one side allow two technicians to work in a sterile and closed area without sterile garments.

The effects of diazoxide on total lung resistance in the anesthetized paralyzed guinea-pig.

The initial effects of diazoxide on dynamic pulmonary compliance (CL) and flow resistance (RL) were measured in paralyzed, anesthetized guinea-pigs. Diazoxide (10, 20 or 40 mg/kg) caused a dose-related increase in RL and decrease in CL. The increase in RL could be selectively abolished by glossopharyngealotomy or decentralization, and by pretreatment with mepyramine (0.1 mg/kg) or disodium cromoglycate (10 mg/kg). Pretreatment with indomethacin (0.1 mg/kg) or acetylsalicylic acid (1 mg/kg) selectively eliminated the decreases in compliance observed after diazoxide. It is concluded that diazoxide initially constricts both large (RL) and small (CL) airways. The constriction of the large airways may involve a release of histamine, whereas the constriction of the small airways may be mediated by a prostaglandin or prostaglandin-like substance.

Evidence for spontaneous neuro-melanophore activity in Pseudopleuronectes americanus (Teleostei; Pleuronectiformes) during total darkness.

In total darkness, melanophores of much of the integumentary pattern of Pseudopleuronectes americanus display intermediate melanosome distribution, whilst the alpha-adrenoceptor antagonist phentolamine evokes their complete dispersion. The intermediate condition is not attained in total darkness in locally decentralized melanophores. It is proposed that, in the absence of photic stimulation, spontaneous neural activity sustains a partial melanosome aggregation in this species.

[Humanizing the work place - concepts of work psychology].

Recent research on central life interests shows that the position of work within the hierarchy of values is actually changing. One of the reasons is the increasing gap between school and vocational education on one hand and the level of qualification of many work activities on the other hand. Work psychology concepts of humanization of work intend to overcome this gap by different forms of autonomy oriented work organization. Therefore it is necessary to add to the traditional criteria of evaluation and measures of work organization the criteria of personality development. This implicates on one side the deduction of the author's principles of differential and dynamic work organization from this criteria, on the other side it requests the distinction between corrective, preventive and prospective strategies of work organization. In addition, trends of autonomy oriented work organization are thought to consider also the options of decentralization and delegation of decision which have been made possible by recent developments in technology, at the same time as they can include stress-reducing and personality developing elements.

[Treatment of fractures by traditional healers in Morocco].

Traditional traumatologists, specialized in the treatment of fractures, practise throughout Morocco. Their original technique is described and its efficiency evaluated through twenty or so cases studied during three years of medical work in the Great South of the country. Theories are propounded to explain the population's preference for these treatments to the detriment of the medical methods, which have a bad reputation. In the present situation, the medical infrastructure is unable to replace the benefits of these bonesetters, neither in quality--with the exception of the main centres-nor, above all, in quantity. Their activities are decentralized, socially integrated and economical for both the population and the state. These considerations lead to the conclusion that their practice should be maintained, improving it if need be.
