[Localized amyloidosis of the bladder treated effectively by occlusive dressing technique therapy using Dimethyl sulfoxide (DMSO): two case reports].

The first case is a 50-year-old female. She was consulted to our hospital for further examination of right hydronephrosis that was shown by ultrasonography. Cystoscopy revealed tumorous lesion around the right orifice. Bladder tumor was suspected and transurethral biopsy of the bladder was performed. The second case is a 54-year-old man. He was consulted to our hospital because right hydronephrosis was shown by counterized tomography. A tumorous or stenotic lesion in the lower end of ureter was suspected. Cystoscopy revealed yellowish and thickened tissue in the trigonal area. Bladder amyloidosis was suspected and transurethral biopsy of the bladder was performed. The histopathological diagnosis indicated amyloidosis and systemic amyloidosis was excluded. Both cases were treated by occlusive dressing technique therapy using Dimethyl sulfoxide (DMSO) successfully.

Use of lyophilized porcine skin to the oral wound. Clinical application and experimental evaluation as a temporary dressing material.

In order to cover the defect of the oral mucosa temporarily, we used lyophilized porcine skin (LPS) in 10 cases. The use of LPS seemed to be effective from the following points of view; alleviation of postoperative pain and as a protection against exogenous irritants. A fixation method, that is, a continuous locked suture, was devised technically to prevent the LPS from tearing by suturing. With this simple method, the patient maintained good oral hygiene and had only a slight discomfort. Histological examination using Japanese white rabbits showed no apparent difference in wound healing between this method and interrupted suture with tie-over compression. Re-epithelialization of the wound in the non-dressed (control) group was recognized earlier than that in the LPS-covered group.

[Testing the osmotic-diffusion properties for the membranous dressing Bioprocess].

In order to determine of the osmotic-diffusive properties of membranous dressing Bioprocess, which is a microfibrous network of homogeneous cellulose produced in biosynthesis by Acetobacter, the hydraulic permeability (Lp), reflection (sigma) and diffusive permeability (omega) coefficients were measured. The values of these coefficients showed that this membrane possess a low selectivity. This amount it easy permeable for solvent (water) as well as for solutions (aqueous solutions of glucose, sucrose, ethanol, NaCl or KCl). Thus, it attend a demands make for polymeric materials used in therapy of scald and ulceration.

[Multicomponent dressing media in treatment of suppurated wounds].

The multicomponent dressing medium (MDM) has been developed for treatment of festered wounds. The first layer consists of hydrofibrosed gauze with chlorhexidine being introduced into its structure in quantity of 0.2-0.5 mass %. The second layer of MDM is chlorhexidinum salt of monocarboxylcellulose with carboxyl group content of 12-22 mass % and chlorhexidine content being 1-2 mass %. The third layer is polyamide pellicle with the pores less than 1 micron in diameter. The created MDM possess significant capillar-transport, absorption, anti-adhesive and prolongated antimicrobial properties, prevents development of secondary infection and "hotbed" effect in the wound.

The influence of selected bioadhesive polymers on physical-chemical properties of stomatological dressing.

Dressings made from polyvinyl alcohol (PAV) and Hydroxypropylmethylcellulose (HPMC) with the addition of glycerol (GLY) differ in adhesiveness and the time of wash out in water, both characteristics depended on polymer and GLY concentration. The research on the release speed which is applied in topical therapy of chlorhexidine digluconate antiseptic in biopharmaceutical model proved the existence of close to rectilinear relation between the amount of released substance and release time for selected formulations.

A simple and effective method of compression dressing for skin grafts.

Skin grafting is one of the most common procedures in plastic and reconstructive surgery. Various methods of fixation and dressing, to achieve good approximation and immobilization of skin grafts and, thus, to bring about better graft survival, are described. A simple method utilizing readily available latex and staples provides good fixation and compression. This technique has been applied in 50 patients requiring skin grafts. The morbidity and complications were minimal, and the grafts have taken satisfactorily. Pressure study with an animal model has revealed adequate compression.

[Treatment of crural varicose with topical dressings of "synthetic skin"].

In Dermatological Department of the Lublin Medical Academy 22 patients with crural varicose ulcerations have been treated with topical Codogard dressings. Before this treatment the patients were given some antiinflammatory and vasodilatatory drugs. The treatment lasted from 4 to 12 weeks (mean 6). In 9 patients complete healing was reported (they had small ulcerations). In 10 cases the improvement was evident, in 2 cases the treatment was discontinued. The obtained results point to clinical usefulness of Codogard, as the drug is easy to be applied and it may play a significant role to outpatients.

[Preparation of chitosan-collagen sponge and its application in wound dressing].

We have prepared a wound dressing made from chitosan and collagen. Its clinical curative effect was detected. Chitosan solution was put into purified collagen solution. Then, the solution became sponge by means of freeze drying, and it was subjected to a series of toxicology tests, including acute toxicity, stimulation test, allergic and hemolysis tests, as well as the clinical test of openning trauma in orthopedics. All of the results of toxicology tests were negative. The chitosan-collagen sponge could not only accelerate the speed of curing but also restrain the extravasate. Therefore, the chitosan-collagen sponge has good biocompatibility and clinical curative effect. It is a prospective security-biomaterial for medical use.

[Not a tourniquet, but compressive dressing. Experience from 68 traumatic amputations after injuries from mines].

In autumn 1991, 68 patients with traumatic amputations after injuries caused by mines were evacuated to the United Nation's field hospital in the demilitarized zone between Iraq and Kuwait. Most were seen during a three week period when civilians harvested mines. During the first days of this period, continuous bleeding distally to applied tourniquets was frequently observed. Orders were issued to remove any tourniquets and cover the wounds with a very tight elastic bandage. Prehospital intravenous infusions were decreased. Three out of 18 patients died prior to the change of routine compared with one out of 50 afterwards. The new directives led to visibly less haemorrhage. Haemoglobin on admission was mean 8.6 g/100 ml during the first part of the observation period compared with mean 10.5 g/100 ml with the new routine. 23 patients received blood transfusions. Fewer patients needed transfusions after the use of tourniquets was discontinued. A tourniquet should not be used in the treatment of bleeding extremity injuries. In extensive crush injuries and traumatic amputations a compressive dressing should be used, applied from the end of the extremity in a proximal direction.

A prospective, randomized trial of Unna's boot versus Duoderm CGF hydroactive dressing plus compression in the management of venous leg ulcers.

Leg ulcers caused by chronic venous insufficiency plague an estimated 500,000 Americans, but there have been few improvements in conservative treatment in this century, and Unna's boot continues to be a mainstay of therapy. A recent report suggests that Duoderm CGF dressing provides greater patient comfort and enhanced compliance, but Duoderm alone (without compression) resulted in slower healing compared with Unna's boot. We enrolled 30 patients (30 ulcers) in a clinical trial to compare Duoderm CGF plus compression (Coban wrap) to Unna's boot. No significant difference was observed between the two groups with respect to age, sex, initial ulcer area, ulcer duration, or extent of venous insufficiency by duplex scan. Eight of 16 ulcers (50%) in the Duoderm group healed completely versus 6 of 14 ulcers (43%) in the Unna's boot group (p = 0.18). Healing rates (square centimeters per week) correlated significantly with initial ulcer area and initial ulcer perimeter for both groups but best correlated with initial ulcer perimeter (r = 0.88 with Duoderm, p less than 0.0001; r = 0.80 with Unna's boot, p less than 0.002). After adjusting for differences in initial ulcer perimeter, healing rates were significantly faster for patients on Duoderm than patients on Unna's boot during the first 4 weeks of therapy (0.384 +/- 0.059 cm2/wk/cm perimeter for Duoderm versus 0.135 +/- 0.043 cm2/wk/cm perimeter for Unna's boot; p = 0.002). At 12 weeks patients on Duoderm again appeared to heal faster than those on Unna's boot, although the result did not reach statistical significance (0.049 +/- 0.007 cm2/wk/cm perimeter for Duoderm versus 0.020 +/- 0.017 for Unna's boot, p = 0.11).(ABSTRACT TRUNCATED AT 250 WORDS)

Hemostatic efficacy of hydrophilic wound dressing after transradial catheterization.

Here, we evaluate the efficacy of the Clo-Sur PAD nonwoven hydrophilic wound dressing (HWD) on hemostasis in an arterial-access site after transradial percutaneous coronary angiography compared with the RadiStop compression device (CD). Eighty patients who had undergone transradial coronary angiography with or without intravascular ultrasound were randomly assigned to the HWD or CD group. The time required to achieve hemostasis was measured, and the incidence of vascular complications was assessed. No significant differences in clinical and procedural characteristics were observed between the HWD group (n = 40) and the CD group (n = 40). A significant reduction in the time required to achieve hemostasis (58.7 +/- 32.6 minutes versus 131.3 +/- 59.1 minutes; p < 0.001) was associated with the use of HWD. The incidence of vascular complications was similar in both groups (5% for HWD versus 2.5% for CD; p = 0.500). No major complications, such as large hematoma or acute radial occlusion, occurred in the HWD group. In conclusion, HWD represents a safe alternative to the compression method. Hemostasis can be achieved more quickly using HWD, with no increase in access site complications, as compared to CD.

[Biophysical properties of membrane dressing made of bacterial cellulose].

In the paper, review of papers devoted to biophysical properties of membrane dressing made of bacterial cellulose was done. These properties were determined on the basis of studies on osmotic and diffusive transport through pure (non modified) bacterial cellulose membrane form called Bio-Fill. The measures of these properties are values of membrane transport parameters resulted from Kedem-Katchalsky's theory and interferograms of near-membrane regions made laser interferometric method.
