[Two decades of the concept of alexithymia].

In the early 1970's, American researchers Sifneos and Nemiah, using the results of there own studies and those of Ruesch, McLean, Marty and others, proposed an original concept of psychosomatic disorders, called the concept of alexithymia. It has become very popular in many research centres in the USA, Germany and Italy. The concept has been based upon the role of the so called alexithymic personality traits, which were found more often in patients with psychosomatic disorders than in other patient populations. Alexithymia (lack of words for emotions) has been defined as a set of psychological dispositions due to specific deficits in emotional and cognitive areas. Persons with alexithymic features present difficulties in perception and verbalization of emotions, cannot distinguish between vegetative feelings and emotions, and possess externalistic, outwardly directed cognitive style. Initially, alexithymia was linked to the etiology of psychosomatic disorders. Later, it was found in other pathological conditions such as somatization disorder, post-traumatic stress disorder, alcohol and substance abuse, neuroses and also in normal population. The origin of alexithymic features is seen as the dysfunction of limbic system, abnormal cerebral lateralization as well as disturbances in early learning of verbal and emotional associations. Alexithymic traits are also regarded as a specific homeostatic mechanism allowing for nearly normal functioning at expense of somatization. Twenty years after, the concept of alexithymia is still a focus of interest in many scientific centres. The most active is a Canadian research group, the authors of Toronto Alexithymia Scale, which is now considered the best measuring tool for alexithymia. Alexithymia has now been regarded a permanent personality trait and alexithymia theory can make a new psychosomatic paradigm.

General concepts of surgical treatment of severe multisystem injuries.

This article is based on an investigation of the management of 1976 patients with multisystem injuries. Three hundred twenty-five patients with multiple trauma and 981 patients with single injury were studied. The surgical treatment of severe multisystem injuries is based on 3 main concepts. The first concept is the substantiated widening of the score of indications for surgical treatment of injuries. Surgical treatment is done in 68.9% of patients with multisystem injuries. Indications for surgical intervention (according to their significance) can be categorized as emergency, urgent, postponed, and planned operations; these were 59%, 17.2%, 23.5%, and 11.3%, respectively, of total surgical procedures. The second concept is assessment of the clinical state of injured patients by means of the BIIX-CI scale, which was developed to define a reasonable time for doing a surgical procedure and to define vital function stability. The 3 levels of clinical state are graded as compensated (range, 16-30 points), subcompensated (range, 31-40 points), and decompensated (> 40 points). The third concept is the reasonable succession in surgical procedures. It is advantageous to do similar operations simultaneously at compensated and subcompensated states. Dissimilar surgical interventions should be done in succession during the course of a single anesthesia or at different times according to the grading of emergency (urgent, postponed, and planned).

The concept of hierarchy in general systems theory.

The paper reviews main ideas related to the concept of hierarchy as they are discussed in contemporary general systems theory. After presenting a dictionary definition of the concept, the author examines the intuitive idea of hierarchy quoting Mario Bunge's notion of level structure. Then relationship between two other concepts: a system and a hierarchy is characterised on the bases of Bowler's, Bunge's again, Klir's, and the author's studies. Finally, the paper is concluded that hierarchy is not an otological concept but epistemological one.

Secondary gain concept: a review of the scientific evidence.

UNLABELLED: The "secondary gain" concept originated in the psychoanalytic literature, where it was never vigorously examined. The purpose of this review is to determine if there are scientific studies that have explored the validity of this concept. DESIGN: A computer and manual literature review yielded 166 references in which primary, secondary, and tertiary gain were mentioned. Twenty-four (14.5%) of these reports were "secondary gain" studies. Fourteen "reinforcement" studies were also found. These 38 studies were grouped according to topics and reviewed in detail. SETTING: Any medical treatment setting including pain treatment was utilized in the review procedure, i.e., no exclusion criteria. PATIENTS: Any patient type, including those suffering from chronic pain, were utilized in the review procedure, i.e., no exclusion criteria. RESULTS: A significant but limited number of studies have investigated the "secondary gain" concept, and the results of some of these studies are in conflict. Results of some studies, however, are remarkably consistent in supporting the importance of "secondary gain" to behavior. Some studies have methodological flaws, usually relating to how the presence of secondary gain was established. CONCLUSIONS: Overall the results of the reviewed studies support the potential importance of the "secondary gain" concept to understanding illness behavior and underscore a need for future research in this area.

Representation of medical concepts of the thyroid gland by physicians in anatomy and pathology.

An experimental study in cognitive psychology is described, concerning the categorization of medical concepts into specific classes, expressed by physicians specialized in anatomic pathology consultations of the thyroid gland. This study belongs to a medical computer science project, called ARISTOTLE, concerning Natural Language Processing of specialized medical reports in anatomic pathology of the thyroid gland. This research has been done for two reasons; first, to specify the characteristics of human expert categorization in an area of medical knowledge and, secondly, to validate the hierarchical organization of a prototype declarative knowledge base. In this experiment, physicians were asked to categorize 121 concepts into 10 proposed classes. These classes and concepts belong to expert knowledge represented in a conceptual graph that was constructed before the experiment. Results show variable semantic distances between concepts of a same class, and dynamic variations of these distances due to contextual representation.

The validity of the Scandinavian concept of reactive psychoses.

The historical background of the Scandinavian concept of reactive psychoses is presented. The reactivity concept consists of several elements. There has to be a traumatic life event of psychological or somatic nature but its duration can be discussed. What is the time lag between the life event and the psychotic reaction? Some say one week, others up to one year. Is a personal disposition (vulnerability) a necessary condition for the psychotic reaction? Are emotional turmoil or non-organic confusion specific symptoms of the reactive psychotic reaction? Does the psychotic reaction have a meaning as an escape or a defense? What is the duration of the psychosis in relation to the duration of the life event? Does reactive psychoses always imply full recovery (good outcome)? The various editions of the ICD- and the DSM-classification include some of these features in their definition of reactive psychoses. Scandinavian experts also emphasize various aspects in their descriptions of these psychoses. Studies have shown that reactive psychoses can be diagnosed reliably. Work on the operatinalization of "reactivity" is under way. In the Scandinavian tradition there is a diagnostic shift from reactive psychoses to schizophrenia in later admissions. Generally, the reactive psychoses have a good outcome, and the outcome is significantly better than for schizophrenia and as good as for manic-depressive psychoses. Most cases of reactive psychoses are treated with neuroleptic drugs for some months. Electroconvulsive treatment is rarely given for reactive psychoses. Scandinavian psychiatrists think that DSM-IV and ICD-10 have not taken the concept of reactive psychoses seriously, and that research work has to be done to convince international psychiatry of the value of the concept.

Principles and concept 1993 of the Systemic Cancer Multistep Therapy (sCMT). Extreme whole-body hyperthermia using the infrared-A technique IRATHERM 2000--selective thermosensitisation by hyperglycemia--circulatory back-up by adapted hyperoxemia.

PURPOSE: The so-called Cancer Multistep Therapy was conceived by the author in 1965. It is a combined modality treatment with 3 process steps: whole-body hyperthermia (WBH), hyperglycemia (HG) and hyperoxemia (HO). The original therapy concept was further developed into a systemic Cancer Multistep Therapy (sCMT) of high efficiency and selectivity. An outline follows of this therapy and the actual state of the sCMT concept as per 1993 (new in timing, dosage, technique). Knowledge of the synergetic-additive efficiency of various groups of cytostatics reacting to the effects of the main treatment process steps (WBH + HG + HO) led in 1974 to an extension of the sCMT concept by including chemotherapy. In addition, a radiotherapeutical treatment was included in this concept as cancer cells clearly show a higher sensibility to radiation while under the influence of sCMT. METHOD: Before commencement of whole-body hyperthermia, the blood glucose concentration is increased to approximately 27 mM in order to lower the pH values in the cancer tissues. Afterwards, the whole-body hyperthermia climaxes with a 60 ... 90 min plateau period of approximately 42 degrees C. This is achieved by means of a specially developed infrared-A irradiation machine named IRATHERM 2000. Parallel to the hyperthermia treatment, the patient inhales air enriched with oxygen at a flow rate of up to 30 l/min, which heightens the stress tolerability. Following the sCMT treatment, the patient remains under observation on an intensive in-patient basis for 24 hours before he or she is discharged for out-patient post-treatment care. During the main treatment, the body-core and skin temperatures as well as the heart and circulatory system of the patient are constantly monitored and he is subjected to a close-meshed surveillance of his glucose, lactate concentration, blood-gas, electrolyte, water and acid base values. On completion of the therapy treatment, the patient takes part in a standardised long-term dispensary programme. About 1 week before and 1 week after the main treatment day the weakened state of the patient necessitates his participation in an Oxygen Multistep Therapy with immunostimulation. RESULTS: The systemic tolerance of sCMT with minimal side-effects has been proven with several 100 patients and results have been published as part of the phase-I study [36]. A first evaluation of the efficacy of sCMT was documented in the same study. CONCLUSION: The systemic tolerability effects of sCMT on the one hand and the positive therapeutic influence on the course of the illness on the other hand, have encouraged us to carry out more research on the efficacy of sCMT, and this will be published shortly.

Clinical features of multiple conception with partial or complete molar pregnancy and coexisting fetuses.

The estimated incidence of twin pregnancy consisting of hydatidiform mole and a coexisting fetus is 1 per 22,000-100,000 pregnancies. Since 1965, nine patients with this entity have been treated at the New England Trophoblastic Disease Center (NETDC), Boston. One patient had a partial hydatidiform mole coexisting with a normal placenta and fetus. The other eight patients had twin pregnancies with a complete hydatidiform mole (CHM) and coexisting fetus. We compared the clinical outcomes in these 8 patients and 14 additional published case reports of multiple gestations composed of CHM and coexisting fetuses with a group of 71 patients with singleton CHM treated at NETDC. Twelve of the 22 patients (55%) with CHM and coexisting fetuses developed persistent gestational trophoblastic tumor, requiring chemotherapy. Five of these patients developed metastases requiring multiple cycles of chemotherapy to achieve remission. The presenting symptoms of multiple conception with CHM and coexisting fetuses were similar to those in patients with a singleton conception and complete mole. However, as compared to singleton CHM, patients having a multiple conception with CHM and coexisting fetuses were diagnosed at a later gestational age, had higher preevacuation beta-human chorionic gonadotropin levels and had a greater propensity to develop persistent tumor. These data indicate that patients with multiple conceptions consisting of CHM and coexisting fetuses are at high risk of developing persistent gestational trophoblastic tumor.

[Reactualization of the concept of unitary psychosis introduced by Joseph Guislain].

This paper reminds the concept of a unitary nosological and pathogenic process that may be traced back to Joseph Guislain (1797-1860). The "phrénalgie initiale" was regarded as the initial stage of psychic illness by Guislain (Leçons orales, Ghent, 1852). That vision inspired the work of Wilhelm Griesinger (1817-1869) who further elaborated the concept of "Einheitspsychose" (Psychose unique--Unitary psychosis). That concept partially inspired Emil Kräpelin (1856-1926). Current classification systems like ICD-10 and DSM-III-R attempt to synthesize different views and the concept of unitary psychosis is actualized in the contemporary transnosography.

[Conception ability of early mated heifers of Bohemiam spotted and black-pied lowland cattle and their crossbreeds].

The conception rate of heifers was studied in dependence on their age and body weight at the first mating. The trial included 163 heifers of the Bohemian Spotted breed, 72 heifers of the Black-Pied Lowland breed, and 101 crossbreds between these two breeds. The heifers of the Bohemian Spotted breed mated for the first time at the age of 12 months had a lower weight than required by the standard (336 kg -- standard requires 360 kg) and a lower weight than the heifers of the other breeds under study. In the heifers of the youngest age categories (111/2-15 months) the conception rate after the first insemination ranged from 56.5 to 58.8% in the Bohemian Spotted heifers, from 57.5 to 77.7% in the Black-Pied Lowland cattle, and from 52.17 to 66.0% in the crossbreds. There were no greater differences from the heifers of higher age categories (15-18 and more months). These tendencies are proved also by other indices such as the average time from the first insemination to getting in-calf and, in particular, the insemination index. Somewhat better indices of the conception rate of early-mated heifers were found in the Black-Pied Lowland breed. The conception rate of early-mated heifers was only slightly influenced by the season. In the summer months (June to August) the pregnancy percentage was lower but the remaining indices showed no greater differences. On an average, the length of gravidity was longer by five days in the heifers of the Bohemian Spotted breed as compared with the heifers of the Black-Pied Lowland cattle, and longer by two days as compared with the crossbred heifers.

The concept of fractals in the pharmaceutical sciences.

An old problem is sometimes better understood when it is viewed from a new perspective than by investigating it repeatedly with classical methods. Such a new perspective is provided by the concept of fractals. Briefly, this term defines a spread class of geometric shapes, whose subunits replicate the structure of the larger unit in accordance with the formalization of the idea of self-similarity. This concept was introduced by Mandel-brot in respond to the need for a more sophisticated explanation of numerous phenomena commonly encountered in nature. In this review we will provide a short introduction into the concept of fractals and a concise overview of those studies where fractal geometry has been employed so far in pharmaceutical research. We believe that a review of the most recent findings in the application of fractal geometry to problems encountered in the pharmaceutical sciences might be helpful also to those people who are involved in the search for novel drug substances, as well as in the investigation of their performance in vitro and in vivo. A few suggestions will be made where this concept probably could be helpful in the future and instructions will be given on how the fractal dimension of rugged objects can be estimated practically.

Children's concept of death and sibling death from trauma.

Factors influencing children's concept of death (their understanding of finality, inevitability, and universality) were examined. A bereaved group of 29 siblings (5 to 12 years of age) of children who died from trauma were interviewed 13 to 17 months after their sibling's death. A comparison group (n = 29) of nonbereaved siblings who had not experienced a sibling's death was matched for age, race, gender, and sociodemographic background. Demographic data, a Piagetian developmental assessment, and a concept of death assessment incorporating vignettes were used to collect data. The experience of sibling death from trauma did not significantly influence acquisition of an accurate concept of death; developmental level (p = 0.0001) and age (p = 0.0003) were significant predictors. Of the 5-year-old subjects in this study, 45.7% had an accurate concept of death, as did > 60% of 6-, 7-, and 8-year-old subjects, 100% of 9-, 11-, and 12-year-old subjects, and 90% of 10-year-old subjects. This reflects an understanding of death at a much younger age than reported by previous researchers.

[Inpatient psychological management in pediatric oncology: the concept of liaison management].

Concepts for counseling and long term support have been the major interest in the psychosocial care of pediatric oncology patients and their families. Similar methods for psychosocial care on the ward are still rare. This paper presents the psychological care of the oncology patients during their stay on the ward, complimentary to the medical treatment. The concept is based on the process of coping with the phases and specific situations (L.P.; B.M.A.; diagnosis; medication ect.) of the cancer therapy. This process contains on the part of the patients and their parents the need for information, active involvement in treatment regimes and relaxation (before, during and after the stressful situation). The concepts of informed consent, adherence and recovery-counseling are related to the coping methods of the patients and their parents. The aims of our concept are to create and utilize diagnostic tests and psychological methods to enable the families to help themselves. To reach this goal, the technique of semantic and pragmatic information is very important because this kind of social communication helps us to make the medical implications of the treatment understandable and suitable for the patients and families.

Dental plaque removal in adults using a newly developed, 'Concept 45 degrees' toothbrush.

A two by two factorial designed study was conducted to compare the effectiveness of the newly designed 'Concept 45 degrees' toothbrush on removal of dental plaque, to a standard conventional toothbrush using either the roll technique or the Bass technique of toothbrushing. One hundred male soldiers, aged 21-22 years were stratified into four balanced groups according to baseline plaque scores obtained from a pretest period. They received oral prophylaxis and were randomly assigned into one of the four experimental groups: (a) standard conventional toothbrush with the roll technique, (b) standard conventional toothbrush with the Bass technique, (c) the 'Concept 45 degrees' toothbrush with the roll technique, and (d) the 'Concept 45 degrees' with the Bass technique. The results showed that the 'Concept 45 degrees' toothbrush using the Bass technique removed dental plaque more effectively than the standard conventional toothbrush with either technique.

Effects of breast feeding after resumption of menstruation on waiting time to next conception.

We investigate the association between breast feeding after resumption of menstruation and the duration from resumption of menses to the next conception. Data from a survey, "Breast-Feeding and Its Effect on Fertility," conducted in 1987 under the auspices of the Centre of Population Studies, Banaras Hindu University, India, were used. We used hazard models of conception rates after the return of menstruation with breast-feeding duration as a time-dependent covariate. The interaction of breast-feeding duration after resumption of menses and postpartum amenorrhea have a significant effect on the rate of conception after return of menses. After resumption of menstruation, when there is no breast feeding, the risk of conception increases with the increase in postpartum amenorrhea. However, breast feeding attenuates the effects of postpartum amenorrhea. These results suggest that breast feeding beyond the resumption of menstruation plays a significant role in the proximate determinants to reduce fertility.

The UMLS Metathesaurus: representing different views of biomedical concepts.

The UMLS Metathesaurus is a compilation of names, relationships, and associated information from a variety of biomedical naming systems representing different views of biomedical practice or research. The Metathesaurus is organized by meaning, and the fundamental unit in the Metathesaurus is the concept. Differing names for a biomedical meaning are linked in a single Metathesaurus concept. Extensive additional information describing semantic characteristics, occurrence in machine-readable information sources, and how concepts co-occur in these sources is also provided, enabling a greater comprehension of the concept in its various contexts. The Metathesaurus is not a standardized vocabulary; it is a tool for maximizing the usefulness of existing vocabularies. It serves as a knowledge source for developers of biomedical information applications and as a powerful resource for biomedical information specialists.

[Life control, the core of being healthy. Theoretical-empirical definition of the concept "life control"].

In this article the concept 'life control' is being analysed and defined. A need to define the concept 'life control' emerged during a research that deat with a human being's descriptions being healthy. 'Life control' turned out to be the central category and the main concept organising the whole data. The data were collected by focused interviews (N = 60). Those interviewed represented five different styles of 'life control'. One of these styles that were discovered in the research was chosen as the basis of the conceptual analysis. This style was characterised by 'laissez faire' attitude towards life. Hybrid model was used as the theoretical frame of reference in the conceptual analysis. Thorough study of Finnish and foreign literature about 'life control' leads into an explicit definition of 'life control' describing the data collected by focused interviews: 'Life control' is an individual's basic belief concerning the possibilities to control the course of human life and the extent of this control. An individual either controls his/her life quite independently and completely or only to a certain extent, or it is controlled by factors beyond the influence of a human being. The definition of the 'life control' -concept in the sense that it displays basic beliefs should be examined by studying people's way of thinking with the help of interviews, essays, questionnaires or analysis of autobiographies.

[Illness concepts of children: an overview of current research].

The present paper tries to summarize the main research topics on concepts of children about health and illness. The present research in this area is allocated to two theoretical positions: (a) cognitive-structural and (b) knowledge related research orientations. After describing the theoretical positions the paper turns to consequences of illness-related conceptions for the experiencing and behaviour of children. The main topics are interactions between illness-related conceptions and emotional experiences and relations of illness-related conceptions to behaviour (compliance, coping and preventive behaviour). The final sections are related to consequences of the present research results for dealing with children.
