[Long-term follow-up of a patient with early diagnosis of untreated stomach cancer].

A case of the patient with type IIc carcinoma of the stomach is presented. The patient was followed-up for 7 years, and did not agree to the operation. Endoscopic examination has shown that the ulcer did not change within 6 years. No data suggesting generalization of the neoplastic process were available during 7 years following the diagnosis. The authors review the literature on the kinetics of the carcinoma of the stomach proliferation in both experimental and clinical investigations.

Primary care of women infected with the human immunodeficiency virus.

AIDS is the leading cause of death among women aged 25 to 34 years in New York City. Gynecologic care should be directed toward early identification and aggressive treatment of infectious and neoplastic processes that may take a more fulminant course in immunosuppressed women. Dealing with women's contraceptive needs highlights the problem of preventing sexually transmitted diseases through contraceptive technology. Public health policies and recommendations will be of no avail without an educated and supportive medical community.

Soft-tissue causes of metatarsalgia.

A soft-tissue cause should always be considered when evaluating metatarsalgia. The history, examination, and plain radiographs can all suggest an avenue of pursuit, be it a potential dermatologic, connective tissue, vascular, neurologic, muscular, or neoplastic process.

[The procedure for treating kidney cancer patients with metastases to the bones].

Active surgical intervention in patients with kidney carcinoma and metastases in the bones helped in prolonging the life of previously doomed patients for several years. Of course, we are far from the idea that such tactics is justified in all patients. An individual approach is necessary. The fundamentals of successful treatment is thorough diagnosis. The extent of the neoplastic process cannot always be determined, errors are often made in establishing the diagnosis, which leads to faulty choice of the therapeutic tactics.

[Carcinosarcoma and papillary transitional cell carcinoma coexisting in the same bladder].

A case of synchronically coexistent carcinosarcoma and transitional cell carcinoma, with separate neoplastic processes in the same urinary bladder is presented. This tumour association is as far as we know, the second case reported in the literature. Carcinosarcoma was formed by an epithelial component represented by a high level transitional cell carcinoma and a mesenchymal component characterized by an unspecific fusocellular sarcoma with chondrosarcomatous differentiation. An immunohistochemical analysis was performed, confirming the dual nature of the carcinosarcoma.

Comparisons between endoscopic and histologic evaluation of the gastrointestinal tract in dogs and cats: 75 cases (1984-1987).

Observations made during endoscopic evaluation of the stomach, duodenum, and colon of 58 dogs and 17 cats with a history of regurgitation, vomiting, and/or diarrhea were compared with results of histologic examination of tissues obtained during the procedures. Endoscopic observations included normal mucosa, alternations in mucosal color and texture, and luminal masses. Although endoscopy alone is a useful technique for detecting alterations of the gastrointestinal mucosa, histologic assessment of tissues obtained is necessary to confirm either an inflammatory or a neoplastic process.

[Etiological role of the herpes simplex virus in causing cervical cancer].

The results of the composite virological and immunological examinations of 46 patients with cervical carcinoma (CC) attest to the association of herpes simplex virus (HSV) with neoplastic processes. HSV strains were isolated from the tumour tissue as well as from the cervical canal secretions and blood of 4 patients. A higher level of virus-neutralizing and complement-fixing antibodies to HSV was found in sera from CC patients as compared with the control group of normal women. Antibody to HSV type 2 was found in 38.8% of sera from the patients and 12.6% in the control group. Lymphocytes from CC patients induced blasttransformation reaction to HSV twice as frequently as those from healthy subjects. The virus-specific antigen was found in tumour cells of 36% CC patients in immunofluorescent examinations of smears from the cervix. The presented results are related to the determination of the etiological role of HSV in cervical carcinoma.

Fine needle aspiration cytology of talc granulomatosis in a peripheral lymph node in a case of suspected intravenous drug abuse.

A fine needle aspiration (FNA) specimen of an epitrochlear lymph node from a 22-year-old suspected intravenous drug abuser was submitted for cytologic examination, which revealed numerous granulomatous tissue fragments, histiocytes, lymphocytes and giant cells. Polarizing microscopy revealed strongly birefringent material of varying shapes and sizes, including anisotropic crystals with the Maltese cross configuration characteristic of starch. A cytologic diagnosis of talc granulomatosis in the lymph node was made. This case emphasizes the value of FNA biopsy in the differentiation of inflammatory versus neoplastic processes; this appears to be the first case of peripheral talc granulomatosis diagnosed by FNA biopsy.

[Sphincter-sparing operations in rectal cancer].

Data on 564 patients who underwent operation for cancer of the rectum are analysed. Amputation of the rectum was resorted to most frequently because of the low localization of the neoplastic process in the majority of patients. Mortality was higher in amputation than in anterior resection of the rectum. Criteria for sphincter-preserving operations are determined. Preference is given to anterior resection of the rectum as a more physiological operation which was also performed on elderly patients.

[Surgical and combined treatment of esophageal cancer].

The authors analyse 37-year experience in the treatment of patients with esophageal carcinoma by various methods at the All-Union Cancer Research Center. The immediate and long-term results of radiotherapy and surgical and combined treatment of 1,850 patients are shown. Bearing in mind the considerable extension of the neoplastic process by the time of the operation, the authors substantiate the expediency of one-stage operations with the formation of intrapleural esophagogastric anastomoses. The mortality in the recent years after 218 Lewis' operations and 36 Herlock's operations was 9.0%. After analysing experience in combined treatment, the authors failed to reveal any particular advantages over solely surgical treatment, but in view of the lack of other alternatives for improving the results of management of esophageal carcinoma to-date, they believe it expedient to continue study and improve various combinations of preoperative radio-(chemoradio)therapy and operation in large research centres.

[Cancer of the pancreas. VI. The characteristics of the localization and metastasis of tumors of the gland in both sexes].

Particular attention is called to the localization and metastasizing of the tumor in different organs and systems of the body of males and females. To this end, a rather ample postmortem case material was used. Included in the study were 25,589 autopsies performed over a period of 25 years (1963-1987). Pancreas cancer was detected in 419 autopsies--274 males (65.4 per cent) and 145 females (34.6 per cent). The neoplastic process was analyzed by sex in two directions: metastasizing in near and remote organs and study of the connection between cancer localization in the pancreas and metastasizing in the body. Difficulties are pointed out, encountered with cancers in advanced biological development, when metastases in the gall bladder from primary cancer of the pancreas and from primary cancer of the bile ducts are to be differentiated. It follows from this analysis that metastases of pancreas cancer were most frequent in the regional lymph nodes, with metastases in the liver and lungs ranking next in both sexes. Metastases in other organs were relatively seldom encountered.

[Studies on fibrinolysis and ascites accumulation associated with peritonitis carcinomatosa--effects of protease inhibitors (PI) on MM2 ascites tumor growth, ascites accumulation and fibrinolysis].

The effects of protease inhibitors(PI), t-AMCHA, gabexate, aprotinin and heparin on the growth of mouse MM2 ascites tumor (MAT) and on several components of fibrinolysis were studied. The drugs were administered intraperitoneally one time daily for 12 days, one day after the tumor transplant. The volumes of ascites, total packed cell volume (TPCV) and fibrinolytic parameters (FDP, whole plasmin, plasminogen activator (PA)) were measured on the 8, 10 and 12th days of therapy. Fibrinolytic activity was assayed by the lysin sepharose affinity chromatography-radio caseinolytic method. Fibrinolytic activity in the ascites increased during the tumor growth. The ascites accumulation as well as levels of FDP, whole plasmin and PA in the drug treated group were significantly decreased when compared to the control group. In these drug-treated groups, MAT cells agglutinated in the abdominal cavity, but in contrast to this, no agglutination was observed in the control group. It was uncertain whether PI directly inhibited tumor growth. The fact that PI inhibited the ascites accumulation and also decreased fibrinolytic activity suggest the involvement of protease in the neoplastic process and indicates another therapeutic approach to malignant ascites tumors.

Cancer-associated alterations of blood group antigen expression in human colorectal polyps.

Human colorectal carcinoma tissues may exhibit several patterns of altered blood group substance (BGS) expression: reappearance of A, B, H, or Lewisb antigens in distal colon; deletion of BGS in the proximal colon with or without precursor substance accumulation; and incompatible BGS expression in proximal or distal colon. The present study evaluated these cancer-associated alterations in colorectal polyps with different malignant potential. With respect to ABH antigens, hyperplastic polyps (HPs), considered to have no malignant potential, did not exhibit incompatibility and only a few cases demonstrated BGS reappearance or deletion. Adenomatous polyps (APs) however, frequently reexpressed ABH antigens or expressed incompatible BG-A or B in 27% of polyps; one specimen demonstrated BG-B deletion. Precursor expression was not found in HPs but was frequently observed in APs. Reappearance of ABH in distal polyps was significantly correlated with increasing grade of dysplasia, but was not significantly correlated with polyp size or histological type. With respect to Lewis antigen expression, Lewisb reappearance occurred in almost every distal polyp, and Lewisa-Lewisb coexpression was also quite common. Lea deletion was frequently noted, especially in HP, but the significance of this finding is unclear. This study indicates that several antigenic alterations that occur in colorectal cancer tissues also appear in premalignant polyps, and often in early stages of the neoplastic process. The observation that incompatible expression of BG-A or B occurs only in AP and cancer tissues (as well as mucosa adjacent to cancer) but not in fetal colonic mucosa, adult normal colonic mucosa, or HP, suggests that this may be a cancer-specific phenomenon.

Clinical applications of the immunohistochemistry of filamentous proteins.

The past few years have seen great progress in our knowledge of the biology and the chemistry of intermediate filaments and other filamentous proteins. This has translated into the development of antibodies that, together with new, highly sensitive immunohistochemical methods, have resulted in much improved diagnostic accuracy. New, more specific antibodies to these and other useful markers will no doubt be developed in the future. More efficient techniques for screening of these antibodies will contribute to their rapid evaluation. Libraries of antibodies that identify subclasses of filamentous proteins will very likely become routine tools in the histopathology laboratory in the near future. Moreover, as the chemical structure of the proteins that form the complex family of intermediate filaments becomes better understood, and the DNA sequences coding them are determined, it may become feasible to use molecular genetic approaches such as in situ hybridization to gain further insight into neoplastic processes of unclear histogenesis and to develop more precise diagnostic and taxonomic instruments. Nevertheless, it is worth repeating that application of these molecular tools cannot be successful unless it is backed by a thorough understanding of the principles of histopathologic diagnosis that have been painstakingly established by generations of anatomic pathologists.

Keratinizing corneal intraepithelial neoplasia.

Corneal intraepithelial neoplasia (CIN) is the term applied by some authors to the spectrum of disease ranging from mild dysplasia to carcinoma in situ. Such lesions usually are associated with dysplastic or neoplastic processes at the limbus or adjacent conjunctiva; isolated corneal dysplasia is rare. Clinically, CIN appears as a geographic, gray, translucent thickening of the epithelium with fimbriated or scalloped borders and lesions often contain scattered white dots. We report a case of intraepithelial neoplasia limited to the cornea that had the unusual clinical appearance of a white plaque, which prompted the misdiagnosis of a calcific scar. Histopathologic examination of the debrided tissue revealed intraepithelial neoplasia and marked hyperkeratosis as the cause of the opacification.

[Diagnostic possibilities of laparoscopy in the assessment of the stage in stomach cancer].

The authors employed laparoscopy as a method of final examination for identifying the stage of a neoplastic process in 259 patients. Obvious signs of generalization of the process were found in 110 patients (42.5 +/- 6.0%). An unnecessary additional trauma--diagnostic laparotomy was avoided in this case. The informativeness of laparoscopic examination was 97.3 +/- 3.6% in metastases to the liver, 96.1 +/- 5.3% in metastatic involvement of the parietal peritoneum, and 87.7 +/- 8.5% in ascites. The diagnosis was verified morphologically.

[Inflammatory pseudotumor of the liver].

Certain rare, reactive, non-neoplastic processes are defined under the term inflammatory pseudotumor, which is constituted by the proliferation of connective tissue and chronic inflammatory infiltrate of circumscribed location. Its etiopathogenesis ins unknown, although it is thought to have a possible infectious nature. It has been described various organs, but location in the liver is exceptional. We present a new case of inflammatory pseudotumor of hepatic location, related to an "hour-glass" abscess adhered to diaphragm, this being the first case of this relation. We reviewed the literature concerning etiopathogenic, histopathologic, clinical and diagnostic aspects, and our findings are discussed.

[A periventricular tumor associated with acute kidney failure and bilateral renal cortical necrosis].

The authors present a case in which the clinico-evolutive particularity was an association between a periventricular tumour (multiform glioblastoma) and acute renal failure. Considerations are made on clinical signs determined by the neoplastic process (apraxia-agnosia hyperthermia, etc.), and on the disseminated intravascular coagulation syndrome related to the acute renal failure.
