Treatment of closed tibial fractures.

Closed tibial shaft fractures are common injuries that remain challenging to treat because of the wide spectrum of fracture patterns and soft-tissue injuries. Understanding the indications for surgical and nonsurgical treatment of these fractures is essential for good outcomes. Although cast treatment of stable tibial shaft fractures has traditionally been successful and continues to be widely used, recent clinical studies have shown that intramedullary nails may be more advantageous for fracture healing and function than casting. Surgical treatment (intramedullary nailing, plate fixation, or external fixation) of closed tibial shaft fractures varies depending on multiple factors. Metaphyseal fractures are well suited for plates, although newer intramedullary nail designs provide the option of intramedullary nailing of proximal or distal metaphyseal tibia-fibula fractures. External fixators are well suited for skeletally immature patients with unstable fracture patterns or for patients with unacceptably small intramedullary canals. Interlocking intramedullary nails are the treatment of choice for most unstable tibia-fibula shaft fractures.

[Growth prognosis after para-epiphyseal tangential epiphysial injuries of the lower extremity exemplified by the distal femur: outcome after injuries of epiphyses of the distal femur].

Fractures of the distal end of the femur are rare. Premature partial closure of the physis may occur after metaphyseal fractures. After epiphyseal fractures it has a high incidence of occurrence. The cause of physeal arrest is not known. Stimulation of the physes ends in leg length discrepancy and was seen in 50% of all cases. Spontaneous correction of an axis deviation is possible after antecurvation up to an age of 5 years. But this should be only accepted during therapy of metaphyseal fractures of the distal femur. Side-to-Side deviations remodel in all cases. Aim of therapy in distal epiphyseal fractures should be an anatomical reduction and a definitive retention. Screw osteosynthesis seems to have an advantage.

[A peculiar form of neonatal adrenal insufficiency: the IMAGe association. Two new cases].

We report two siblings with an IMAGe syndrome. IMAGe is a newly reported syndrome characterized by the association of intra-uterine growth retardation, metaphyseal dysplasia, congenital adrenal hypoplasia and genital anomalies. This clinical association has only been described in five unrelated males. These two additional patients (one brother and one sister) suggest an autosomal recessive inheritance although identification of new cases will give further insight into the pathogenesis. The radiologic signs of osteopenia and metaphyseal dysplasia can suggest this diagnosis in affected individuals.

Alterations in femoral bone histomorphometry and vitamin D metabolism in neonatal streptozotocin-induced diabetic rat.

Histomorphometric examination and histological observation of femoral bone were performed on long-standing neonatal streptozotocin-induced diabetic rats (n2STZ, n5STZ) as a human model of non-insulin-dependent diabetes mellitus. The growth and strength of femurs decreased in the STZ diabetic rats. Histomorphometric parameters such as cortical bone thickness, number of metaphysical trabeculae and percent trabecular volume of metaphysical area all significantly decreased in the STZ diabetic rats. There were no significant differences in parameters between the n2STZ and n5STZ diabetic rats. Histological findings demonstrated no significant change in the number of osteoclasts in femur nor change corresponding to osteomalacia. Bone absorption in the STZ diabetic rats appeared unchanged. The plasma calcium level did not change in the STZ diabetic rats, although their plasma phosphate or A1-p levels increased. Circulating 24, 25 (OH)2D3 was significantly lower in the STZ diabetic rats than the controls. However, 25 (OH) D3 or biologically active 1, 25 (OH)2D3 was not different between the controls and STZ diabetic rats. Osteopenia is thus present in the femurs of long-standing neonatal STZ diabetic rats, due in part to abnormal vitamin D metabolism.

[Epidemiological survey of wrist fractures in children].

PURPOSE OF THE STUDY: The incidence of wrist fractures is increasing in children. We undertook an epidemiological survey in western France to determine characteristic features. MATERIAL AND METHODS: A prospective multicentric study was conducted over one year in five university hospital centers in western France. The survey included 839 wrist fractures in children aged 6 to 16 years. The following criteria were recorded: age and gender, date and energy of the trauma, side and description of the fracture. Displacement was measured on the AP and lateral radiographs. RESULTS: The boys were older than the girls at the time of the wrist fractures, generally during the summer season (May to October). The left wrist was involved in 55% of the fractures; 83% were metaphyseal fractures. Fractures with epiphyseal detachment were more frequent in adolescents and occurred after high-energy trauma. Metaphyseal fractures were more common in girls than in boys. Displacement was greater in older children, particularly in case of epiphyseal detachment. DISCUSSION: We did not observe the male predominance often reported in our country. The girls probably practice sports as much as boys.

Spondylometaphyseal dysplasia, Sutcliffe type: a rediscovered entity.

The authors describe two children with spondylometaphyseal dysplasia, Sutcliffe type. This easily recognizable form of bone dysplasia is characterized by coxa vara, minimal metaphyseal changes, oval vertebral bodies and metaphyseal corner fractures later in life. It is probably the most common type of spondylometaphyseal dysplasia. Early diagnosis is important for proper management.

[Effects of ibandronate on the osteopenia process in tail-suspended rats].

Histological and histomorphometric effects of ibandronate on the metaphyseal spongiosis in tibia were studied in tail-suspended rats and their controls in a 30-d experiment. Ten- to twenty-day course of ibandronate at a dose of 6 micrograms/kg of the body mass per diem altered the architectonics of metaphyseal spongiosis and increased materially the total spongiosis volume no matter whether those were suspended rats or controls. Spongiosis growth was a result of a sharp deterioration of resorption and spongy bone remodelling due to ibandronate. The substance also exhibited a strong durable action inferring that it might be used for prevention of osteopenia for lack of loading on the musculoskeletal system.

Preventive effect of zinc acexamate administration in streptozotocin-diabetic rats: Restoration of bone loss.

The preventive effect of zinc compounds on bone loss in streptozotocin (STZ)-diabetic rats was investigated. Rats received a single subcutaneous administration of STZ (6.0 mg/100 g body weight), and 7, 14 or 21 days later the animals were sacrificed by bleeding. STZ administration caused a significant decrease in body weight and a significant increase in serum glucose and triglyceride levels, indicating diabetic condition. Femoral-diaphyseal and -metaphyseal alkaline phosphatase activity, calcium and deoxyribonucleic acid (DNA) contents were significantly decreased by STZ administration, showing that diabetic condition causes bone loss. Zinc sulfate (2.5 mg Zn/100 g) or zinc acexamate (2.5 mg Zn/100 g) was orally administered once daily for 14 days to rats received a single subcutaneous administration of STZ (6.0 mg/100 g). STZ administration-induced increase in serum glucose and triglyceride levels and decrease in body weight, femoral-diaphyseal and -metaphyseal alkaline phosphatase activity, DNA and calcium contents were significantly prevented by the administration of zinc acexamate. The preventive effect of zinc sulfate on bone components was not seen. The present results demonstrate that the administration of zinc acexamate has a preventive effect on bone loss in STZ-diabetic rats in vivo.

Childhood multifocal skeletal non-Hodgkin lymphoma is a differential diagnosis of battered child syndrome.

Skeletal lesions observed in battered children include fractures, subperiostal new bone formation, irregular metaphyses, and cone-shaped epiphyses. A 4-year-old girl presented with bone pain. Radiographs revealed multiple fractures and epiphyseal and metaphyseal lesions highly significant for battered child syndrome. Six weeks later the authors diagnosed primary multifocal skeletal non-Hodgkin lymphoma. This case report illustrates that multifocal skeletal non-Hodgkin lymphoma is a differential diagnosis of battered child syndrome.

Alteration in serum and bone component findings induced in streptozotocin-diabetic rats is restored by zinc acexamate.

The effect of zinc acexamate in streptozotocin (STZ)-induced diabetic rats was investigated. Rats received a single subcutaneous administration of STZ (6.0 mg/100 g body weight), and the animals were orally administered once daily for 14 days with zinc acexamate (2.5, 5 or 10 mg/100 g body weight). The administration of STZ caused a significant increase in serum glucose, triglyceride and calcium levels and a significant decrease in body weight, serum zinc and inorganic phosphorus levels, indicating diabetic condition. Moreover, calcium content, alkaline phosphatase activity and deoxyribonucleic acid (DNA) content in the femoral-diaphyseal and -metaphyseal tissues were significantly reduced in STZ-diabetic rats. The change in these serum and bone components of STZ-diabetic rats was significantly restored by the oral administration of zinc acexamate (2.5, 5 or 10 mg Zn/100 g body weight). The restoration of bone components was not seen by the oral administration of zinc sulfate (2.5 mg Zn/100 g) for 14 days. Moreover, when the femoral-diaphyseal and -metaphyseal tissues obtained at 14 days after STZ administration were cultured for 48 h in a medium containing either vehicle or zinc acexamate (10(-5) M), the femoral calcium content and alkaline phosphatase activity were significantly increased in vitro. The effect of zinc acexamate was completely abolished in the presence of cycloheximide (10(-6) M), an inhibitor of protein synthesis. The present study demonstrates that the oral administration of zinc acexamate has a preventive effect on STZ-induced diabetic condition in rats, and that it can restorate bone loss of STZ-induced diabetes in vivo.

Bone loss in regucalcin transgenic rats: enhancement of osteoclastic cell formation from bone marrow of rats with increasing age.

Bone loss was previously shown to be induced in the femoral tissue of regucalcin transgenic (TG) rats. Regucalcin is expressed in rat bone marrow cells and its expression is enhanced in regucalcin TG rats. This study was undertaken to determine the change in osteoclastic bone resorption in regucalcin TG rats with increasing age. Femoral-diaphyseal and -metaphyseal tissues were obtained from normal (wild-type) and regucalcin TG rats aged 5, 14, 25 or 50 weeks. Calcium content in the femoral-diaphyseal and -metaphyseal tissues was significantly decreased in regucalcin TG male and female rats aged 5, 14, 25 or 50 weeks as compared with the value obtained from normal rats with each age. When the marrow cells obtained from normal or regucalcin TG rats were cultured for 7 days, the number of tartrate-resistant acid phosphatase (TRACP), a marker of osteoclasts, positive multinucleated cells (MNCs) were significantly increased in the marrow culture of regucalcin TG male and female rats aged 5, 14, 25 or 50 weeks. The effect of parathyroid hormone [human PTH (1-34); 10(-7) M] or 1,25-dihydroxyvitamin D3 [1,25(OH)2D3; 10(-7) M] in stimulating TRACP-positive MNC formation was significantly enhanced in regucalcin TG male and female rats aged 14 or 25 weeks. This study demonstrates that osteoclastic bone resorption is stimulated in regucalcin TG male and female rats with increasing age.

[Combination of direct and indirect reduction techniques in periarticular complex knee fractures].

BACKGROUND: We evaluated the efficiency of treatment of comminuted intraarticular fractures of the knee, involving direct reduction and rigid fixation for articular components, and indirect reduction and biological fixation for metaphyseal-diaphyseal components. METHODS: Twelve patients (9 males, 3 females; mean age 46 years; range 22 to 71 years) with distal femoral (n=6) and proximal tibial (n=6) intraarticular fractures were included. There were seven closed and five open fractures. Intraarticular fractures were fixed directly through a lateral parapatellar incision; while comminuted metaphyseal-diaphyseal fractures were indirectly reduced and internally fixed with biological fixation methods. The patients were evaluated clinically and radiographically. Intraarticular bone and soft tissue changes were evaluated by conventional and three-dimensional computed tomography. The results were assessed according to the HSS (Hospital for Special Surgery) criteria. The mean follow-up was 43 months (range 15 to 78 months). RESULTS: All fractures healed without any refractures, implant failures, or infections. The mean time to full weight-bearing was 25.5 weeks for tibial, and 24 weeks for femoral fractures. Leg length discrepancy occurred in all the patients with femoral fractures (1-2 cm), and in two patients with tibial fractures (1 cm). One patient with a femoral fracture had a valgus deformity of 10 degrees. According to the HSS criteria, the results were good in five cases, and moderate in one case for tibial fractures; good in one case, moderate in four cases, and poor in one case for femoral fractures. Tomographic evaluations showed articular congruity in all the cases without any signs of loose bodies. CONCLUSION: Combination of indirect and direct reduction techniques is an effective method for the treatment of comminuted intraarticular knee fractures.

A comparison of the anabolic effects of rat and bovine parathyroid hormone (1-34) in ovariectomized rats.

The current study was designed to compare the skeletal effects of comparable doses of rat parathyroid hormone 1-34 (rPTH) and bovine parathyroid hormone 1-34 (bPTH) in ovariectomized (OVX) rats. Female Sprague-Dawley rats were OVX or sham-operated at 6 months of age and maintained untreated for 28 days after surgery. Baseline control and OVX rats were sacrificed at the beginning of treatment. Beginning 28 days post-OVX, the remaining rats were subcutaneously injected daily with rPTH or bPTH at 0, 5, 25, or 50 microg/kg/d for 28 days. Bone area, bone mineral content (BMC), and bone mineral density (BMD) of the distal femoral metaphyses were determined ex vivo using dual energy X-ray absorptiometry. Quantitative bone histomorphometry was performed on undecalcified longitudinal sections of the proximal tibia from each rat. Baseline OVX rats exhibited osteopenia as demonstrated by their significantly reduced femoral BMD and proximal tibia cancellous bone volume compared with those of baseline sham controls. Both rPTH and bPTH restored bone in OVX rats by markedly stimulating bone formation in a dose-dependent manner. However, a difference in potency between the two forms of PTH was evident. The percentage increases of BMC, BMD, cancellous bone volume, trabecular thickness, mineralizing surface, and bone formation rate in the OVX rats treated with bPTH at 5 microg/kg/d were the same as or above those treated with rPTH at the 25 microg/kg/d dose level. A relative potency analysis showed that bPTH was approximately 4- to 6-fold relatively more potent than rPTH in increasing distal femoral BMD as well as cancellous bone volume, mineralizing surface, and bone formation rate of proximal tibial metaphyses at comparable dose levels and a given time. These results may serve as a reference for in vivo study design when rPTH or bPTH are to be the agents for studies on bone anabolism.

[Intramedullary nailing osteosynthesis with epiphyseal fixation of fractures of the proximal humerus in adults. 1 to 3-year follow-up].

The authors used a medullary pinning with epiphyseal fixation in the treatment of superior metaphyseal fractures of the humerus for 41 patients aged 22 to 82 years. There were 28 cases with epiphyseal fractures and 12 with metaphyseal fractures. The follow-up was 1 to 3 years. The pins were introduced through the triceps brachii after perforation of the diaphysis, 1 cm above the ulnar fossa. Results were very good. Strength returned within 8.5 weeks. There was only one case of nonunion of a proximal diaphyseal fracture due to a technical fault (diastasis between fragments). Rehabilitation began immediately, resulting in good elbow motion in 35 cases. The shoulder motion was the same as the healthy side in 36 cases, and inferior to 90 degrees of abduction in only two complex epiphyseal fracture. The proximal humerus was never perforated by the pins. The authors find this new instrumentation to be very effective.

Intraorgan biodistribution and dosimetry of 153Sm-ethylenediaminetetramethylene phosphonate in juvenile rabbit tibia: implications for targeted radiotherapy of osteosarcoma.

UNLABELLED: Targeted radiotherapy using 153Sm-ethylenediaminetetramethylene phosphonate (153Sm-EDTMP) is currently under investigation for treatment of primary osteosarcoma. Human osteosarcoma most frequently occurs in skeletally immature individuals, and previous studies in a juvenile rabbit model demonstrated that clinically significant damage to developing physeal cartilage might occur as a result of systemic 153Sm-EDTMP therapy. The aim of this study was to determine the distribution of 153Sm-EDTMP within the tibias of juvenile rabbits and estimate the radiation-absorbed doses delivered to the physeal cartilage. METHODS: Eight-week-old New Zealand White rabbits were injected intravenously with 7.57 kBq (280 microCi) of 153Sm-EDTMP. At 21 h after injection, the biodistribution of 153Sm in the epiphysis, metaphysis, diaphysis, and red marrow of the tibia was obtained. Two-dimensional digital autoradiography was performed on 2-mm sections of tibias for qualitative comparison with the biodistribution data. Self-tissue and cross-tissue absorbed doses were calculated using absorbed fractions generated by the Monte Carlo particle transport code MCNP-4C. RESULTS: The highest uptakes (percentage injected dose per gram [%ID/g] of tissue) of 153Sm, 1.99-2.56 %ID/g, were found in the proximal and distal metaphyses, 70%-73% of which localized within 3 mm of the physeal cartilage. The second highest tissues of uptake were the proximal and distal epiphyses, at 0.33-0.62%ID/g. Digital autoradiography imaging confirmed that the majority of 153Sm deposited in the tibia localized to these tissues. Radiation-absorbed doses to the proximal and distal metaphyses were 183 and 130 mGy/MBq, respectively, and those to the proximal and distal epiphyses were 141 and 43.4 mGy/MBq, respectively. These tissues represented the only source compartments contributing to the physeal cartilage doses of 50.0 mGy/MBq for the proximal physis and 39.2 mGy/MBq for the distal physis. CONCLUSION: The 153Sm absorbed doses to the physeal cartilage were consistent with values that can cause dose-limiting damage to rapidly proliferating and differentiating chondrocytes. The pronounced uptake in the juvenile epiphysis indicates that the proliferating zone of the physis can be irradiated from multiple areas, which could increase the expression and degree of radiation damage. Further investigation of the effects of 153Sm-EDTMP on immature physeal cartilage is warranted to develop optimized treatment regimens.

[Three-dimensional geometry of the proximal humerus and rotator cuff attachment and its utilization in shoulder arthroplasty].

PURPOSE OF THE STUDY: The aim of the study was to provide a method of measurement and data collection, based on morphologic analysis of the proximal humerus, that would facilitate precise placement of the humeral stem and would be easy to apply in clinical medicine in patients requiring shoulder arthroplasty. MATERIAL: Three groups of materials were used. Materials for the first and second groups were provided by the Department of Anatomy, First Faculty of Medicine, Charles University. The first group included 10 specimens (five left and five right humeral bones) obtained at routine anatomical dissection. The second group contained 110 so-called dry preparations from the collections of the Department of Anatomy and the third group comprised nuclear magnetic resonance (NMR) scans of 20 patients. To show spatial relationships between the proximal and the distal humerus, another coil was applied to the epicondylar region in addition to the one placed over the proximal humerus. METHODS: The first group material was used to study proximal humerus morphology and to determine reference points for the other two groups. The points were constructed to make seven planes perpendicular to the axis of the proximal humeral metaphysis. Based on the reference points, parameters of the proximal humerus were assessed in the defined planes also in the other two groups. We measured angles between the reference points and the transepicondylar line or the humeral head axis. The vertex of each angle was always placed in the point of intersection of the metaphyseal axis and the given transverse plane. Reference points of the greater tubercle were marked on the medial margin continuous with the intertubercular groove, on the lateral margin of the lesser tubercle and in the "deepest" place of the intertubercular groove. We also measured humeral head retroversion and the position of maximal bony mass of both the greater and the lesser tubercle (this parameter can be used with advantage for optimal insertion of screws in proximal humerus reconstruction). RESULTS: The angle between the medial margin of the greater tubercle and the humeral head axis was on average 164.8 degrees on the left side and 163.2 degrees on the right side; the angle between the great tubercle margin and the transepicondylar line was 137.0 degrees on the left humerus and 137.7 degrees on the right humerus. The lateral margin of the lesser tubercle and the humeral head axis formed on average an angle of 124.4 degrees and of 122.6 degrees on the right and the left side, respectively. The intertubercular groove/ humeral head axis relationship was 143.4 degrees and 144.8 degrees for the left and the right humerus, respectively, and the intertubercular groove/transepicondylar line angle was 115.6 degrees for the left and 119.5 degrees for the right humerus. The humeral head axis and the transepicondylar line made an angle of 27.8 degrees for the left and 25.3 degrees for the right humerus. These values corresponded to the angle of the humeral head retroversion. The reference point of maximal bony mass of the greater tubercle and the humeral head axis made an angle of 181.1 degrees and of 180.2 degrees for the left and the right humerus, respectively; between this point and the lesser tubercle was an angle of 120.2 degrees for the left and 126.9 degrees for the right humerus. DISCUSSION: One of the most important parameters in restoring shoulder function by alloplasty is humeral head retroversion. If this is not correct, ventral or, less frequently, dorsal instability of the shoulder may result. The correct setting of retroversion is guided by the transepicondylar line or several specific landmarks on the greater and the lesser tubercle of the humerus. Another important factor is the correct reconstruction of anatomic position of the greater and the lesser tubercle in relation to the insertion of rotator cuff muscles into the humeral head. Relationships of diaphyseal, metaphyseal and humeral head axes have been reported in the relevant literature dealing with proximal femoral morphology. None of the reports, however, has dealt with tubercular angles and position of the maximal body mass, which is a decisive factor for insertion of screws fixing the prosthetic stem. CONCLUSIONS: The values provided here can be used for a more precise construction of implants for shoulder replacement. The method of three-dimensional presentation of the proximal humerus may aid in a more exact implantation procedure during shoulder arthroplasty. An optimal position of the implant can also be based on parameters obtained from the healthy contralateral shoulder joint.

[Scintigraphic detection of osteoblast activity after implantation of BAS-0 bioactive glass-ceramic material into long bone defects].

PURPOSE OF THE STUDY: The aim of the study was to demonstrate, by three-phase bone scintigraphy, radionuclide uptake at the site of defects in long bones filled with the non-resorbable bioactive glass-ceramic material BAS-0 at a long follow-up. MATERIAL: Twenty patients, 14 men and 6 women, operated on between 1990 and 2000 for benign bone tumors or tumor-like lesions localized in the femur, tibia or humerus were comprised in the study. Their average age at the time of operation was 14 years (range, 8 to 24). The diagnoses based on histological examination included juvenile bone cysts in 11, aneurysmal bone cyst in five, non-ossifying fibroma in two, and fibrous dysplasia in two patients. The lesions were localized in the femur, humerus and tibia in 11, five and four patients, respectively. The metaphysis was affected in eight and the diaphysis in 12 patients. METHODS: Clinical, radiological and scintigraphic examinations were carried out at 2 to 12 years (7 years on average) after surgery. The clinical evaluation included subjective complaints and objective findings. Radiographs were made in standard projections and the osteo-integration of glass-ceramic material was investigated. Three-phase bone scans were made and the healthy and the affected limbs in each patient were compared by means of an index. Radionuclide uptake was considered normal when the index value was equal to 1.0, mildly increased at an index value of 1.2, moderately increased at 1.2-1.5 and markedly increased at an index value higher than 1.5. RESULTS: The clinical evaluation showed that, in the patients with glass-ceramic filling of metaphyses, six had no subjective complaints and two reported transient pain. In the patients with implants in diaphyses, subjective complaints were recorded in nine and no complaints in three patients. No inflammatory changes in soft tissues were found. No restriction in weightbearing of the limb treated was reported by any of the patients. On radiography, 18 patients were free from any disease residue or recurrence. Two patients had a residual defect. The bioactive glass-ceramic material BAS-0 was completely incorporated in all patients. On three-phase bone scans, radionuclide distribution on the flow phase and soft tissue phase was symmetrical in both limbs of all patients. For the metaphyseal location of implants, the delayed images demonstrated physiological radionuclide distribution in one patient, mildly increased distribution (index up to 1.2) in four, increased uptake (index up to 1.5) in two patients, and highly increased uptake (index above 1.5) in one patient. For the diaphyseal location of implants, the delayed scans demonstrated slightly increased radionuclide distribution in two, markedly increased in two and highly increased uptake in eight patients. DISCUSSION: The tissue during incorporation of a non-resorbable synthetic material is influenced by stress-shielding. This changes local mechanical signals, which has a negative effect on the adjacent bone tissue. Stress accumulating at the interface of a rigid implant and bone tissue may result in pain, and is detected by scintigraphy as an increased nucleotide uptake, particularly in diaphyseal grafts. CONCLUSIONS: This paper presents problems associated with implantation of the non-resorbable bioactive glass-ceramic material BAS-0 in the treatment of diaphyseal defects of long bones. The results show that, for filling of the defects described herein, non-resorbable glass-ceramic materials are not suitable.

Reliability of vascularized fibula in maintaining arthrodesis following extra-articular wide excisions of malignant musculoskeletal tumors.

OBJECTIVE: To evaluate the reliability of vascularized fibula transfer in maintaining arthrodesis following wide excision of malignant tumors located at the metaphyseal ends of long bones. METHODS: Fourteen patients underwent wide extra-articular excisions followed by arthrodesis with vascularized fibula and its variants at the Department of Orthopedic Surgery and Traumatology, University of Hacettepe Medical School, Ankara, Turkey, in the period from 1996 to 2003. There were 9 males and 5 females. Their mean age was 28 years (ranging 12-52 years). The mean follow-up time was 68 months (range 28-110 months). The most common diagnosis was malignant giant cell tumor (6), followed by Ewing's sarcoma (3), hemangioendothelioma (2), osteosarcoma (2) and chondrosarcoma (1). Three tumors were located at the distal end of the radius, 4 at the proximal humerus, 3 at the distal femur, one at the proximal tibia, one in distal tibia, one in distal humerus and one in proximal ulna. We used fibula as a vascularized graft in 4 patients; fibula osteoseptocutaneous flap in 4 cases; bilateral vascularized fibulae in 3; fibula with autoclaved autograft in 4 and free osteoseptocutaneous fibula in conjunction with a structural allograft and a circular external fixator in one patient. RESULTS: In 4 cases, implant failure developed and revision was required in 3. One case had local infection around the distal interlocking screw. In one case, clawing of first and second toes were developed at the donor side. Two patients with metastatic Ewing's sarcoma had metastatic disease. We achieved union within 9 months in 12 cases. In 2 cases with implant failure, bony consolidations were maintained at 11th and 13th months. CONCLUSION: Vascularized fibula transfer is reliable to achieve arthrodesis following extra-articular excision of malignant tumors from the metaphyseal ends.
