[The effect of mercasolyl on immunologic indicators in patients with diffuse toxic goiter].

A study of system of cellular and humoral immunity, unspecific defense in 187 patients with diffuse toxic goiter before and after treatment with mercasolyl revealed that despite the clinical effect, the agent did not essentially influence the content and functional activity of T-lymphocytes. The effect was more favourable on the content of immunoglobulins, titer of antithyroid antibodies. This was observed in mild and average severe course of the disease. It is suggested that mercasolyl effects lymphocytes synthesizing autoantibodies to the thyroid gland.

[Effect of merkazolyl on the level of thyrostimulating immunoglobulins of the blood in patients with diffuse toxic goiter].

A study was made of the level of thyrostimulating immunoglobulins (TSI) in the blood of 31 untreated patients with diffuse toxic goiter (DTG). The effect of thiamazole applied at a daily dose of 30-60 mg for 3-4 weeks on a TSI level was investigated in 18 patients. The level was assessed by an increase in a cAMP level in thyroid paired lobes after incubation with immunoglobulins of DTG patients. The basal level of TSI does not correlate with the level of blood thyroxin, thyroid iodine absorptive capacity of DTG patients, and severity of thyrotoxicosis. Thiamazole produces a different effect depending on a degree of severity of thyrotoxicosis decreasing a TSI level in patients with disease of average severity and not changing the level in severe thyrotoxicosis.

[Experience with the ultrasonic aspirator in surgery of benign goiter].

In 60 cases intracapsular resection of goiter was performed by using ultrasonic aspirator. The superior pole vessels and the inferior thyroid artery were not touched, the recurrent laryngeal nerve was not identified. Minimal hemorrhage in the intra- and postoperative period, operation time saving in comparison with conventional subtotal thyroidectomy, no temporary and no permanent damage to the recurrent laryngeal nerve and no wound infection were observed. Not suturing the remnants of thyroid optimal blood supply was obtained. There were no hypoparathyroidism and but one case of hypothyroidism.

Clinicopathologic management of tumors of the thyroid gland in an endemic goiter area. Combined use of preoperative fine needle aspiration biopsy and intraoperative frozen section.

In total, 15,325 fine needle aspiration (FNA) biopsies of the thyroid were examined in the Department of Pathology of the University of Innsbruck, Austria, between 1976 and 1985, with the cytologic results histologically verified in 3,112 cases. Since (1) it is frequently impossible to distinguish benign from malignant encapsulated follicular thyroid tumors by cytologic criteria and (2) there is a high level of follicular thyroid carcinoma in our endemic goiter area, we have adopted a diagnostic strategy that accepts a high percentage of false-positive cytologic results in order not to miss highly differentiated follicular carcinoma. To avoid unnecessarily extensive surgical treatment, 1,079 intraoperative frozen section examinations of the thyroid were performed in the same time period in (1) patients with preoperative suspicious or positive FNA cytologic findings, (2) cases with suspicious clinical and anamnestic data and (3) tumors with a suspicious macroscopic appearance without preoperative FNA or with negative or unsatisfactory cytologic findings. In 48 cases (4.5%), the frozen section diagnosis had to be revised after examination of paraffin-embedded tissue. An intraoperative false-positive diagnosis was obtained in 3 cases (0.3%) while a false-negative diagnosis was made in 45 cases (4.2%). The main effort in examining frozen sections should be concentrated on avoiding false-positive errors, which can lead to unnecessary thyroidectomies.

[Status and prospects of the problem of endemic goiter].

The paper is concerned with the clinical problems of endemic goiter control. The stabilization of prevalence of thyroid hyperplasia has been shown as a result of underestimation of other than iodine deficiency giterogenic factors. Functional essence of the disease, a tendency to hypothyrosis, have been unraveled. Organizational problems and prospects of research in this area are considered.

[Thyroid cancer in an endemic goiter region].

On the basis of an analysis of 1062 operations on the thyroid gland the authors make a conclusion that incidence of cancer of the thyroid gland in the focus of the moderate goiter endemia is 9,6%. Three stages of the development of cancer of the thyroid gland are established: a) precancer, b) latent stage of cancer (histological findings), c) stage of manifest cancer. Complex methods of treatment were used such as radical operation followed by the obligatory radiation therapy (in patients having no contraindications) and permanent treatment by thyroidine in combination with chemotherapy (on indications). Remote results followed in 92 patients during 9 years were quite satisfactory. Recurrences were found in 2 patients.

[Laboratory diagnosis of subclinical forms of hypothyroidism in an endemic goiter focus].

The paper is concerned with laboratory diagnosis of thyroid function in the endemic focus in Gornaya Shoriya of the Kemerovo region. The diagnostic value of radioimmunoassays was emphasized. The authors proposed a compensation index characterizing the ratio of the thyroid hormones and promoting the diagnosis of subclinical forms of hypothyreosis in the endemic goiter focus.

[Radiography, ultrasonic diagnosis and radioimmunologic analysis in the differential diagnosis of diffuse goiter and thyroiditis].

Radionuclide functional dynamic investigation of the thyroid with 99mTc-pertechnetate was performed in 62 patients with diffuse euthyroid and toxic goiter, thyroiditides and in 17 controls. An analysis of the results of investigations and their comparison with clinical findings, an echo- and scintigraphic picture of the thyroid as well as with the levels of total thyroxine, triidothyronine and thyrotropin of the hypophysis showed that the results of a dynamic test of pertechnetate trapping corresponded, to a large extent, to the clinical status of patients with diffuse thyroid changes, correlated with an echographic picture and made it possible to define significant radiodiagnostic signs of thyroiditides. The proposed methods made it possible to increase the volume and improve the quality of diagnostic information in functional investigations of the thyroid status raising the efficacy of diagnosis of thyroid diseases and reducing radiation exposures and the time of investigations.

[Radionuclide and ultrasound studies in nodular (colloid) goiter].

The informative value of scintigraphy and ultrasonic investigation (USI) was analyzed in 110 patients with nodular colloid goiter. The patients were operated upon, and diagnosis was made on the basis of histological findings. Scintigraphy was more preferable for the assessment of atypical position of the thyroid and function of its different parts but USI was better for the detection of its nodular, diffuse and other structural changes. Both methods contained no specific information pertaining to histology.

[Antibodies to the plasma membranes of thyrocytes in patients with diffuse toxic goiter during treatment with mercazole].

Antibodies to thyroid cell membranes (ATCM) were studied in sera from 49 patients with diffuse toxic goiter (DTG) using dot-immunobinding prior to treatment with mercazolyl and 1, 4, 12 and 24 mos. later. Among the DTG patients ATMC before treatment were detected in 75.5%; 4 mos. after the start of mercazolyl therapy ATCM were detected in 49% of the patients. Among patients in a long-term remission after mercazolyl therapy ATCM were detected in 37.5% pointing out to immunologic disturbance in these patients.

[Status of the hypothalamo-hypophyseal system in patients with diffuse toxic goiter].

Disorder of function of the hypothalamus-thyroid system in patients with diffuse toxic goiter is accompanied by change of the hormonal activity of the hypothalamohypophyseal-adrenocortical and hypothalamohypophyseal-ovarian systems. It is particularly marked in persons with heredity aggravated by thyroid diseases or/and diabetes mellitus.

[Several aspects of the pathogenesis of disturbances of hemostasis in patients with diffuse toxic goiter].

Components of the kallikrein-kinin system of the blood and some hemostatic indices were studied in parallel in 32 patients with diffuse toxic goiter. A decrease in the levels of prekallikrein, kininogen, the activity of kallikrein and kininase inhibitors in the blood plasma of the examinees resulted in raised activity of the kallikrein-kinin system in decompensated thyrotoxicosis. Hemostatic changes were characterized by signs of chronic disseminated intravascular coagulation. It was shown that kinin formation and blood coagulation were correlated. A certain parallelism in the increment of the activity of the kinin system and disorders of hemocoagulation was noted with an increase in a degree of severity of thyrotoxicosis. A tendency to improved indices was also noted after thyrostatic therapy.

[Lipids of blood and erythrocyte membranes in healthy persons and in patients with diffuse toxic goiter, residing in the European North of the USSR].

A study of the blood plasma content of total cholesterol in 575 healthy persons and 675 patients with diffuse toxic goiter (DTG), and the content of total lipids and triglycerides in 52 healthy persons and 132 DTG patients showed the development of adaptation hyperlipidemia in persons living long in the North. It was preserved in DTG patients irrespective of the gravity of thyrotoxicosis, stages of its therapy and the patients' age. Basing on the analysis of the level of lipid indices of erythrocytic membrane malonic dialdehyde and their refractoriness to peroxide hemolysis in 52 healthy persons and 53 DTG patients, a conclusion was made of excessive stable activation of lipid peroxidation biomembranes which can underlie the development of visceral complications.

[Status of goiter prevention in East Germany from an interdisciplinary viewpoint].

The endemic goiter occurs still in many countries of Europe. Alimentary iodine deficiency could be proved as essential cause of the increased prevalence. Comprehensive epidemiological investigations refer to the medical importance of this appearance for the GDR. 200 million marks might have to be spent annually for diagnostics and therapy. Also in the production of animals sequelae of iodine deficiency play a great part. Nitrate containing and glucosinolate containing feeding stuffs (e. g. rape extraction groats) here under the conditions of the iodine deficiency additionally develop goitrogenous effects (so-called indirect or relative iodine deficiency), which lead to decrease in vitality and clinical symptoms in the agricultural useful animals cattle, pig and sheep. The iodine content of animal products was frequently low. In view of the ecological connection of these problems an interdisciplinary iodine commission was founded which in accordance with the responsible state organs elaborates adequate recommendations for the prophylaxis and control of efficiency. From 1986 was begun with the iodisation of the largest part of packet salt (32 mg KJO3/kg) und the delivery of iodized mineral mixtures for the production of animals. Thus is to be reckoned with an improved direct supply of iodine for the population by iodized table salt or an indirect one over animal product. The iodisation of the entire common salt with modified iodine concentration is striven for. Measures for the control of efficiency are introduced.

[Autonomous intrathoracic goiter. Apropos of 2 new cases].

An autonomous intrathoracic goiter (AIG) is a thyroid gland formation located in the thorax or, more precisely, the mediastinum. It is not a metastasis of thyroid cancer, and it has no parenchymatous or vascular connections with the cervical thyroid gland. It is fed by thoracic vessels and is observed in the absence of previous thyroidectomy. Its multiple appellations, and the fact that clinical reports often lack precision and detail, complicate investigations concerning AIG, a rare variety of mediastinal goitre (slightly more than 100 cases published). AIG is essentially caused by an abnormal embryonic development of the thyroid gland. It must be distinguished from migratory goitres partially resected and forgotten in the thorax after cervicotomy. This fully mediastinal tumour is usually removed by the thoracic approach.

Cerebrovascular circulation "steal" syndrome secondary to a voluminous retrotracheal goiter.

A case of recurrent transient ischaemic attacks in the brain resolved after removal of a voluminous retrotracheal goiter is presented. The presence of congenital atretic left vertebral artery, associated with impairment of left carotid artery blood flow by a very large inferior thyroid artery, probably caused the episodes to transient ischaemic attack.
