[Medialization of the paralyzed vocal cord by cartilage chips and "wing door thyroplasty"].

Unilateral recurrent laryngeal nerve paralysis with atrophic vocal cord and persisting glottic insufficiency as well as hemiparesis of the larynx caused by lesions of the upper parts of the vagal nerve have to be managed surgically. Instead of the nowadays often applied Teflon-injection the authors prefer the following techniques of surgical medialization. 1. Medial shifting of the paralyzed vocal cord by subperiostal implantations of autogenous cartilage through a window in the thyroid ala. 2. "Wing door thyroplasty" with medialization of a door like part of the thyroid cartilage which is locked by a cartilage chip in the desired position. The results of the surgical treatment of 19 patients (13 paresis of the recurrent laryngeal nerve, 6 lesions of the upper part of the vagal nerve) showed an obvious improvement with regard to voice and aspiration. There have been no postoperative complications. These methods can be an alternative to Teflon-injections.

Alpha radiation detection using silicon memory chips--preliminary studies.

Alpha radiation dosage is an important occupational health factor in the mining of uranium and mineral sands. Alpha radiation induced errors in the data of silicon based memory chips provide the foundation for a new type of sensor, with the potential for affordable and prompt measurement of personal alpha doses. With particular reference to Dynamic Random Access Memories (DRAM) this paper introduces the operating principle of a memory based radiation sensor, which is the error mechanism in silicon integrated circuits.

[Bacterial culture of chip tissue of enucleated prostates].

To assess the prevalence of infection and colonization of the prostate by bacteria, chip tissue samples from 166 patients undergoing retropubic prostatectomy were submitted for bacterial tissue culture. In 28 patients with an indwelling catheter before surgery, E. coli, Klebsiella, Pseudomonas and Enterobacter were the commonest species encountered, the first the most common. In only 7 patients (20%) who didn't have an indwelling catheter before operation was the culture positive. We confirmed that the longer the time the catheter was indwelling before surgery, the greater the likelihood of positive cultures. However, postoperative outcome and morbidity were not related to culture results. We conclude that even though it is worth trying to sterilize the urine and prostate before prostatectomy, the effect on the postoperative outcome is minimal when proper antimicrobial therapy is given perioperatively.

A further application of buried chip skin grafting.

Buried chip skin grafting used to close a granulation wound, that cannot be kept at rest, or else is in some way irritated, is described. This requires an extremely small amount of skin. Although it takes a month to heal, irritation or contamination of the grafted area does not affect or have any influence on the take of graft survival. Therefore, the patient need not be kept at rest after surgery, and can continue with daily living or rehabilitation exercises.

Cells on chips.

Microsystems create new opportunities for the spatial and temporal control of cell growth and stimuli by combining surfaces that mimic complex biochemistries and geometries of the extracellular matrix with microfluidic channels that regulate transport of fluids and soluble factors. Further integration with bioanalytic microsystems results in multifunctional platforms for basic biological insights into cells and tissues, as well as for cell-based sensors with biochemical, biomedical and environmental functions. Highly integrated microdevices show great promise for basic biomedical and pharmaceutical research, and robust and portable point-of-care devices could be used in clinical settings, in both the developed and the developing world.

Chembio extraction on a chip by nanoliter droplet ejection.

This paper describes a novel liquid separation technique for chembio extraction by an ultrasonic nanoliter-liquid-droplet ejector built on a PZT sheet. This technique extracts material from an aqueous two-phase system (ATPS) in a precise amount through digital control of the number of nanoliter droplets, without any mixing between the two liquids in the ATPS. The ultrasonic droplet ejector uses an acoustic streaming effect produced by an acoustic beam focused on the liquid surface, and ejects liquid droplets only from the liquid surface without disturbing most of the liquid below the surface. This unique characteristic of the focused acoustic beam is perfect (1) for separating a top-layer liquid (from the bulk of liquid) that contains particles of interest or (2) for recovering a top-layer liquid that has different phase from a bottom-layer liquid. Three kinds of liquid extraction are demonstrated with the ultrasonic droplet ejector: (1) 16 microl of top layer in Dextran-polyethylene glycol-water ATPS (aqueous two-phase system) is recovered within 20 s; (2) micron sized particles that float on water surface are ejected out with water droplets; and (3) oil layer on top of water is separated out.

Single-molecule detection sensitivity using planar integrated optics on a chip.

We present a fully planar integrated optical approach to single-molecule detection based on microfabricated planar networks of intersecting solid and liquid-core waveguides. We study fluorescence from dye molecules in liquid-core antiresonant reflecting optical waveguides, and demonstrate subpicoliter excitation volumes, parallel excitation through multiple pump waveguides, and single-molecule detection sensitivity. Integrated silicon photonics combined with single-molecule detection in solution create a compact, robust, and sensitive platform that has applications in numerous fields ranging from atomic physics to the life sciences.
