Platelet ultrastructural morphometry for diagnosis of partial delta-storage pool disease in patients with mild platelet dysfunction and/or thrombocytopenia of unknown origin. A study of 24 cases.

BACKGROUND AND OBJECTIVE: Exact diagnosis is sometimes difficult in patients presenting with a slight bleeding diathesis, prolonged bleeding times, non-specific aggregometric abnormalities, and/or mild thrombocytopenia. The objective of this study was to evaluate the use of platelet ultrastructural morphometry in detecting a partial d-storage pool disease in such patients. DESIGN AND METHODS: Platelets from 52 patients and 15 controls were fixed immediately in glutaraldehyde in White's saline without anticoagulant and processed for transmission electron microscopy. Using computer-assisted morphometry, the size and shape of the platelets were measured, as were the size and number per platelet of the dense- and a-granules. Ultrastructural morphology of the above and other intraplatelet structures was observed. RESULTS: Twenty-four cases were diagnosed as having a partial d-storage pool disease. Mean platelet area (2.28 microm(2)) and maximum diameter (2.58 microm) were significantly greater in patients than in control subjects (1.64 microm(2) and 2. 25 microm, respectively) but discoid shape was preserved. Mean dense-granule number was decreased, both per platelet and per microm(2) of platelet area (patients 0.22 and 0.09; controls 0.42 and 0.24). Seven patients also had a marked decrease in a-granules, resulting in a significantly lower mean number of granules per microm(2 )(patients 2.43; controls 3.15). Additionally, the patients' platelets had significant increases in both lipid droplets and surface-connected canalicular system. INTERPRETATION AND CONCLUSIONS: A partial dense-granule deficiency, sometimes associated with partial a-granule deficiency, should be borne in mind faced with patients who have a slight bleeding diathesis, non-specific platelet dysfunction tests and/or mild thrombocytopenia of unknown origin. Platelet ultrastructural morphometry is useful in diagnosing this condition.

Human bone marrow fibroblasts infected by cytomegalovirus: ultrastructural observations.

Human cytomegalovirus (HCMV) is capable of infecting human bone marrow fibroblastic stromal cells (HBMF-sc). This infection is important to assess in regard to the pathogenesis of HCMV, particularly in immunocompromised patients. Stromal fibroblastic cells were infected by Towne strain of cytomegalovirus (CMV) in vitro. Several ultrastructural features of uninfected HBMF-sc were also described. The CMV-infected cells exhibited significant mitochondrial enlargement, production of dense bodies by the Golgi apparatus and cytoplasmic accumulation. Ultrastructural aspects of HCMV entry in host cells, capture by endosomes, penetration of genetic material in the nucleoplasm, assembly and formation of nucleocapsids were detected and described. Viral fusion and transit through the nuclear envelope were shown along with envelope proliferations. Trafficking of virions, maturation and completion of their cytoplasmic coating were also illustrated. Fully developed virions, defective virions, other apparently-emptied vesicles, multivesicular bodies as well as cytoplasmic dense bodies were illustrated along arrays of microtubules organized by defective centrosomes and constituted a peculiar structure that we termed 'viral field'. While some viral and dense bodies were carried to adjacent sites of the plasmalemma, in order to be extruded from the infected cells, others were concentrated into black holes--dense heterogenous bodies accumulated at the periphery of viral fields. This study further described the functional aspects of HBMF-sc and summarized the unknown aspects of ultrastructural characteristics of HCMV-infected fibroblastic stromal cells which may serve as harmful reservoir for the replication of virus. In addition, the findings of this study may stimulate further investigations about the basic cell biology and functions of the bone marrow stromal cells and may also generate some interests to bone marrow transplantation medicine as to how HBMF-sc can serve as a reservoir in the pathogenesis of CMV infections.

Helicobacter pylori in culture: an ultrastructural study.

Helicobacter pylori (H. pylori) is generally observed in the gastric mucus and on the epithelial cells in human, and is related to gastric disorders. Although many morphological studies have been performed on H. pylori, the precise ultrastructure of this organism has not been reported upon to the present. In this study, H. pylori (ATCC 43504) was cultured in a liquid medium for seven days and observed using scanning and transmission electron microscopic methods every day. The reproduction of the bacteria in culture was also examined daily, and the length and width of the organisms were measured with the use of Micro Computer Imaging Device (MCID) System. Spiral forms were dominant over the initial two days (the length and width: 2.04 +/- 1.12 microns and 0.49 +/- 0.12 micron on the 1st day, 1.71 +/- 0.96 microns and 0.38 +/- 0.12 micron on the 2nd day) respectively. Coccoid forms (diameter: 0.80 +/- 0.36 micron) were predominant on subsequent days. The bacterium had a cell wall membrane (15 nm thick) and a cytoplasmic membrane (10 nm thick). A hollow granule (0.15 +/- 0.06 micron in diameter) and a dense granule (0.12 +/- 0.04 micron in diameter) were observed in a cytoplasm filled with ribosomes. The flagella (51.1 +/- 6.6 nm total in diameter) with a sheath (12 nm thick) originated from a concave depression (less than 50 nm in depth) at the end of the organism which possessed a knob (less than 0.1 micron in diameter) at the distal end, which frequently attached to adjacent organisms. The coccoid form generally showed precariously membranous and intracytoplasmic ultrastructures with few flagella. A few of the coccoid forms possessed distinctive cellular ultrastructures with solid flagella similar to those found in the spiral forms.

Immunohistochemical and ultrastructural age-related changes in rat lactotroph cells.

Ageing produces alterations in some functions of the hypothalamo-pituitary axis leading to sexually dimorphic changes in the prolactin (PRL)-secreting cells. Since quantitative morphological data of these age-associated alterations are scarce, we carried out a morphometric immunohistochemical assessment as well as an ultrastructural study of the PRL cell population in male and female rats of different ages. Young (3-month-old), old (20-month-old), and senescent (31-month-old) Sprague-Dawley rats of both sexes were sacrificed by rapid decapitation, their pituitaries immediately dissected out and processed for both immunohistochemistry and electron microscopy. Analysis of different morphometric parameters revealed that the cell density (CD) and volume density (VD) significantly decreased with age in male rats. In females, while CD showed a significant age-related diminution when young rats were compared to old ones, this parameter increased in senescent animals. The VD presented higher values in senescent rats. When the data were compared between sexes, VD was found to be higher in females if old and senescent rats were considered. Finally, CD increased significantly in females when compared to males. The ultrastructure of the PRL cells from old and senescent animals of both sexes exhibited changes suggestive of an hyperstimulation state, with some prolactotrophs having the appearance of cells undergoing an involutive process. We conclude that ageing has a differential impact on the PRL cells of male and female rats with respect to the immunohistochemical and ultrastructural features of that cell population.

Ultrastructural changes in Bruch's membrane of apolipoprotein E-deficient mice.

PURPOSE: To examine the histologic and ultrastructural changes in Bruch's membrane (BM) in apolipoprotein E deficient [ApoE(-)] mice in comparison with age-matched control animals. METHODS: Two-month-old (group 1) and 8-month-old (group 2) normal control C57BL/6 mice and 2-month-old (group 3) and 8-month-old (group 4) ApoE(-) mice were studied. All groups of mice were fed a standard rodent diet. The mice were killed, serum lipid levels were determined, and the eyes were ultrastructurally examined using standard techniques to measure the thickness of BM. The area fraction of electron-lucent (EL) particles in BM was quantified using point-counting stereology. RESULTS: The serum cholesterol levels of the ApoE(-) mice were significantly higher than those of the control mice (P = 0.0001). There was a significant thickening and EL particle accumulation in BM associated with age in the control animals. Group 2 had a thicker BM and more EL particle accumulation than group 1 (P = 0.0410 for thickness; P = 0.0042 for particle accumulation). Age-related changes were not seen in ApoE(-) mice; thickness and accumulation were similar in groups 3 and 4 (P = 0.50, thickness; P approximately/= 1.0, accumulation). Significant thickening and accumulation were seen in young ApoE(-) mice (group 3) versus young control animals (group 1; P = 0.008, thickening; P < 0.0001, EL particle accumulation). Group 4 ApoE(-) mice did not have a thicker BM or more EL particles than group 2 control animals (P = 0.2910, thickness; P = 0.35, EL particle accumulation). "Membrane-bounded" material (material between two membranes) was present significantly more frequently in ApoE(-) mice. CONCLUSIONS: ApoE(-) mice exhibit accumulation of EL particles at an earlier age and have more membrane-bounded material in BM than control mice. This material has ultrastructural similarities to basal linear deposit, which accumulates in age-related maculopathy.

Quantitative ultrastructure of physiologically identified premotoneuron terminals in the trigeminal motor nucleus in the cat.

Little is known about the ultrastructure of synaptic boutons contacting trigeminal motoneurons. To address this issue, physiologically identified premotor neurons (n = 5) in the rostrodorsomedial part of the oral nucleus (Vo.r) were labeled by intracellular injections of horseradish peroxidase (HRP) in cats. The ultrastructure of 182 serially sectioned axon terminals from the five neurons was both qualitatively and quantitatively analyzed. In addition, the effects of the glycine antagonist strychnine, GABA(A) antagonist bicuculline, NMDA antagonist 2-amino-5-phosphonovalerate (APV), and non-NMDA antagonist 6-cyano-7-nitroquinoxaline-2,3-dione (CNQX) on Vo.r-induced postsynaptic potentials in trigeminal motoneurons (n = 11) were examined to evaluate potential signaling substances of the premotor neurons. Labeled boutons made synaptic contacts with either jaw-closing or -opening motoneurons. All the boutons contained pleomorphic vesicles, and most formed a single symmetric synapse either on the somata or on primary dendrites. Morphometric analyses indicated that bouton volume, bouton surface area, apposed surface area, total active zone area, and mitochondrial volume were not different between boutons on jaw-closing and -opening motoneurons. Vesicle number and density, however, were higher for boutons on jaw-closing motoneurons. The five morphological parameters were positively correlated with bouton volume. Vesicle density was the exception, which tending to be negatively correlated. Intravenous infusion of strychnine or bicuculline suppressed Vo.r-induced inhibitory postsynaptic potentials (IPSPs) in jaw-closing motoneurons. Abolition of Vo. r-induced excitatory postsynaptic potentials in jaw-opening motoneurons with APV and CNQX unmasked IPSPs. The present results suggest that premotor neurons in the Vo.r are inhibitory and that positive correlations between the ultrastructural parameters associated with synaptic release and bouton size are applicable to the interneurons, as they are in primary afferents.

Ultrastructural and molecular analysis of Bowman's layer corneal dystrophies: an epithelial origin?

PURPOSE: Two mutations (R555Q and R124L) in the BIGH3 gene have been described in anterior or Bowman's layer dystrophies (CDB). The clinical, molecular, and ultrastructural findings of five families with CDB was reviewed to determine whether there is a consistent genotype:phenotype correlation. METHODS: Keratoplasty tissue from each patient was examined by light and electron microscopy (LM and EM). DNA was obtained, and exons 4 and 12 of BIGH3 were analyzed by polymerase chain reaction and single-stranded conformation polymorphism/heteroduplex analysis. Abnormally migrating products were analyzed by direct sequencing. RESULTS: In two families with type I CDB (CDBI), the R124L mutation was defined. There were light and ultrastructural features of superficial granular dystrophy and atypical banding of the "rod-shaped bodies" ultrastructurally. Patients from three families with "honeycomb" dystrophy were found to carry the R555Q mutation and had characteristic features of Bowman's dystrophy type II (CDBII). CONCLUSIONS: There is a strong genotype:phenotype correlation among CBDI (R124L) and CDBII (R555Q). LM and EM findings suggest that epithelial abnormalities may underlie the pathology of both conditions. The findings clarify the confusion over classification of the Bowman's layer dystrophies.

Ultrastructural expression of primary ciliary dyskinesia after ciliogenesis in culture.

During the period 1990-1999 84 PCD patients were identified and characterized. The expression of inherited abnormalities in primary ciliary dyskinesia after ciliogenesis was investigated in 41 patients with dynein deficiency, 6 patients with absence of the central pair of microtubules and 24 PCD patients with normal ultrastructure. In patients with dynein deficiency, the outer dynein arms counts were 1.9 +/- 1.0 in the biopsies and 1.6 +/- 0.7 after ciliogenesis. Secondary abnormalities were found in 15.8 +/- 20.4% of the transverse sections of cilia and only in 1.0 +/- 1.3% after ciliogenesis. Ciliary orientation was 28 +/- 11 degrees and 24 +/- 10 degrees respectively in biopsies and cultures. In patients with absence of the central pair this was found in 15 +/- 16% in biopsies and 21 +/- 19% after ciliogenesis. The values for the outer dynein arm were 8.4 +/- 0.3 and 8.7 +/- 0.2 and for the secondary abnormalities were 11.7 +/- 7.3% and 0.5 +/- 1.3% in the biopsies, respectively after ciliogenesis. In patients with normal ultrastructure the scores for the dynein arms were similar. Secondary abnormalities were found in 12.2 +/- 11.7% in the biopsies and 0.6 +/- 0.9% after ciliogenesis while ciliary orientation was respectively 21 +/- 7 degrees and 25 +/- 8 degrees. In conclusion, inherited abnormalities in primary ciliary dyskinesia are expressed after ciliogenesis, while secondary abnormalities are virtually absent, thereby facilitating the ultrastructural diagnosis.

Differential morphometric and ultrastructural remodelling in the left atrium and left ventricle in rapid ventricular pacing-induced heart failure.

BACKGROUND: Heart failure induced by rapid ventricular pacing (RVP) is associated with left atrial (LA) but not left ventricular (LV) hypertrophy. OBJECTIVE: To determine whether differences in wall tension correlate with the differential ultrastructural remodelling in the LA and LV chambers, changes in ultrastructure, systolic function and wall tension (an index of wall stress) were compared in dogs after RVP (n=7) and with no RVP (n=9). RESULTS: Compared with dogs with no RVP (controls), dogs with RVP had increased collagen volume fraction (5.3% versus 8.3%), myocyte cross-sectional area (245 versus 366 microm(2)) and hydroxyproline (222 versus 323 microg/mg protein) in the LA (all P<0.05), but not in the LV. The increase in systolic wall tension produced by RVP was greater in the LA (five versus 43 units, P<0.0004) than in the LV (227 versus 290 units, P<0.01) chambers and correlated closely with the collagen volume fraction (r=0.87), which in turn correlated with myocyte cross-sectional area (r=0.98). In the left atrium, wall tension correlated with wall stress (r=0.99). CONCLUSIONS: The results suggest that differential wall tension may provide the stimulus for differential ultrastructural remodelling (with more hypertrophy and collagen) between LA and LV chambers in RVP-induced cardiomyopathy.

[Ultrastructural characteristics of glaucomatous trabecular meshwork].

PURPOSE: To compare the normal and glaucomatous trabecular meshwork ultrastructure and to relate the observed changes with the intraocular pressure increase characteristic of the primary open angle glaucoma. METHODS: 21 non glaucomatous trabecular meshworks, aged 23 to 99 years, and 5 from patients diagnosed of primary open angle glaucoma, aged 40 to 65 years, were fixed by Karnovsky's solution and processed and observed by transmission electron microscopy and their morphological characteristics were qualitatively compared. RESULTS: Ultrastructural changes of glaucomatous trabecular meshworks are similar, but much more intense, than those observed in the aged normal trabecular meshworks. These changes are loss of endothelial cells, thickening of basal membranes and trabecular beam central nucleus changes such as an increase of electrodense plaques and collagen degenerative processes. CONCLUSIONS: Ultrastructural changes observed in glaucomatous trabecular meshworks are comparable to an early aging of them. These changes can be related with the mechanisms that increase the intraocular pressure in primary open-angle glaucoma.

[Ultrastructural characteristics of rat neuromuscular junctions after physical load].

It is interesting to ascertain the adaptive reaction of rat neuromuscular junctions (NMJ) of muscle fibers of different types to a chronic physical load. We examined ultrastructural changes in NMJ following both static load (pre- and postnatal ontogenesis of Wistar rats till a 2 month age took place under a constant rotation on the centrifuge at hypergravity conditions 2G), and after three kinds of dynamic loads (1/run on treadmill with a speed 35 m/min for 6 wks, 10-60 min/day; 2/swimmings, each 10 hrs/day for 10 days; 3/strength exercises on a vertical treadmill with load for 6 wks). Differences in NMJ reaction of muscle fibers of the same type to various loads were established. A low secretory activity of axonal terminals of type I muscle fibers of m. soleus was shown after the static load. The dynamic load (run) is accompanied with a high secretory activity of axonal terminals in m. soleus type I muscle fibers and of some axonal terminals of m. quadriceps femoris IIB type muscle fibers after strength exercises; the secretory activity of axonal terminals of m. quadriceps femoris IIA and IIB types muscle fibers is expressed in a lesser degree after swimming. The NMJ ultrastructure remodelling (terminal renewal) of type I muscle fibers of the 2 month old control rats increases after static and dynamic (run) loads. Some correlations between different kinds of physical load, muscle fiber type and the degree of NMJ ultrastructure transformation have been shown.

Effects of interaction between cadmium and selenium on heart metabolism in mice: the study of RNA, protein, ANP synthesis activities and ultrastructure in mouse heart.

Heavy metals tend to occur in increasingly many aspects of human activities. Studies of cadmium (Cd) have revealed that it is extremely toxic in its effects. It is known that selenium (Se) may suppress deleterious effects of Cd. We investigated the effects of dietary Cd-intoxication on the incorporation of precursors of RNA, protein and ANP (atrial natriuretic peptide) granule synthesis in mouse cardiocytes and compared them with the results of Se interaction with Cd-intoxication. Functional condition of the heart was evaluated on the basis of the number of ANP granules synthesized in cardiocytes of the right atrium in the mice exposed to the tested elements. The experiment was conducted on 100 male white Balby mouse during the period of three months. The animals were divided into four groups. The control group I (C) was fed a standard Murigran diet. Group II (Cd)--received 50 ppm Cd as cadmium acetate in drinking water. Group III (Se) received a standard diet supplemented with 5.0 mg Se/kg DM/24 h as acid sodium selenate. The experimental animals in group IV (Cd + Se)--were fed a diet supplemented with Cd and Se in the same amounts as the above groups. Our results revealed that after 3 months long intoxication with Cd, 3H-uridine and 3H-alanine uptakes to cardiomyocytes were decreased by 33% and 40%, respectively, and fewer ANP granules were synthesized when compared with the controls. Ultrastructure of myocytes proved slightly distorted. Se-intoxicated cardiomyocytes indicated diminished incorporation of RNA synthesis precursors by 17% and 27% in the ventricle and atrium, respectively, in comparison with the controls. Some Se-induced structural changes were observed. Finally, after Se in interaction with Cd intoxication, the uptake of 3H-uridine and 3H-alanine to cardiocytes was higher and the number of ANP granules increased. The values approximated those in the controls. We concluded that: prolonged Cd intoxication disturbed intercellular metabolism by damaging ultrastructural elements and suppressing the incorporation of precursors of RNA, protein and ANP granule synthesis. Se in interaction with Cd revealed protective effects against Cd toxicity. After Cd-Se-intoxication neither metabolic activity nor cardiocyte ultrastructure showed significant differences from those in the controls.

Contractility of smooth muscles and ultrastructure of their microfilaments in restriction of physical activity.

BACKGROUND: Depending on the intensity of hypodynamics (as a factor of ischemic heart disease) the aim of our investigation was to determine the contractility of smooth muscles of thoracic aorta with and without endothelium by biological active substances (prostaglandin F2 alpha and acetylcholine) and to compare with ultrastructural alterations of microfilaments. MATERIAL AND METHODS: A hypodynamic stress of 48-days duration was provoked via permanent and periodically recurrent intervention (when the hypodynamic state was altered with physical activity) in Chinchilla rabbits by placing them in metal hutches (according to B. M. Fiodorov). The relaxing effect of endothelium--dependent vasodilatator agonist (acetylcholine in concentration 10(-7) M, 10(-6) M, 10(-5) M, 10(-4) M) on smooth muscles (treated with vasoconstrictor prostaglandin F2 alpha) was detected by means of a mechanotron 6 MXIC in isometric regime. The contraction force was expressed in percentage. One hundred percent was the maximum contraction that developed after administration of 2 x 10(-5) M prostaglandin F2 alpha solution to the smooth muscles preparation. For ultrastructure studies of microfilaments semithin sections of smooth muscles were examined with electron microscope 'Philips--300'. RESULTS: The research findings show that under the influence of acetylcholine the relaxing of endothelium of thoracic aorta smooth muscles (treated with prostaglandin F2 alpha) is more expressed in periodically recurrent intervention. CONCLUSION: Permanent hypodynamic stress of 48 days duration caused strongly expressed contractility of smooth muscles with is associated with ultrastructural damage of microfilaments (such as irregular formation, drop out in flakes, microfilaments becoming shorter and thicker).

Ultrastructure of nuclear compartment in cardiomyocytes during regenerative and plastic insufficiency of the myocardium.

We studied ultrastructure of nuclear compartment in cardiomyocytes during regenerative and plastic insufficiency of the myocardium induced by anthracycline antibiotic daunomycin. A peculiarity of ultrastructural organization of cardiomyocyte nuclei under these conditions is almost complete disappearance of the heterochromatin lumps. The earliest changes in nucleoli under conditions of disturbed DNA-dependent RNA synthesis are segregation of the granular and fibrillar nucleolonema components. Deep alterations in the nucleoli manifested by fragmentation and annulation correlate with pronounced changes in cardiomyocytes ultrastructure, intensive lysis of the myofilaments, reduction of the organelles, and enhanced autophagocytosis.

Ultrastructural localization of calcium deposits in rat myocardium after loud noise exposure.

In previous studies we demonstrated that loud noise exposure induces ultrastructural alterations in the rat myocardium together with an increase in noradrenergic activity and in mitochondrial calcium influx. To verify the causal relationship between myocardial calcium entry and ultrastructural alterations induced by loud noise, in the present study we coupled routine electron microscopy with cytochemistry specifically dedicated to visualize calcium accumulation (revealed as antimonate deposits). We observed that the ultrastructural alterations occurring in both atrium and ventricle after 12 h of noise exposure, were densely packed with antimonate deposits. In particular, enlargements of the sarcoplasmic reticulum and dilution of the mitochondrial matrix, observed during routine electron microscopy, were markedly positive for calcium accumulation when observed by using antimonate. The present data strongly suggest that calcium entry results in accumulation of this ion at myocardial subcellular level. Moreover, the present results joined with previous evidence indicate that calcium accumulation is the final common pathway responsible for noise-induced myocardial morphological alterations.

An electron-microscopic study of lymph node cells in patients with lymphogranulomatosis has shown that the ultrastructure of these cells undergo some changes consisting in increased variability of the form and size of cellular organoids, and also being manifested in reticular cells to a greater degree than in lymphoid ones. A compparative study of cell ultrastructure at different stages of maturation and the analysis of literature data on ultrastructural changes in the process of differentiation of various cell types enabled to pinpoint further a conception of the morphological basis of differentiation at a subcellular level and to represent it schematically.

Focal ischemia in the cerebral cortex has an effect on the neurohypophysis. I. Ultrastructural changes in capillary vessels of the neurohypophysis after focal ischemia of the cerebral cortex.

OBJECTIVES: In our investigations we have reported that photochemical reaction leading to brain ischemia can also be precipitated with visible light from a non-coherent light source. It was revealed that focal cerebral ischemia after photochemical reaction cause the alterations in the capillaries ultrastructure and perivascular spaces of the barrier-competent regions of the brain. The purpose of this study is to first characterize the ultrastructural morphological consequences of photochemically induced ischemia in the cerebral cortex on the capillaries of neurohypophysis as the barrier-free region of the brain. METHOD: We used a model of ischemic brain damage due to obliteration of microvessels following the photochemical reaction. Rats were treated with an intravenous injection of rose bengal and irradiated from a halogen lamp source through an intact cranium to precipitate microvascular damage. Material for electron microscopic studies were sampled from the neurohypophysis 1 and 4 days after irradiation (4 animals in each group) in experimental group and 1 and 4 days after a rose bengal injection in control group. RESULTS: Investigations in transmission electron microscopy revealed platelet aggregation on the endothelium preceded by its early ultrastructural damage. In the capillaries of the neurohypophysis, one and four days after irradiation, numerous microthrombi adhering to the damaged endothelium were present. The capillary vessels contained a continuous, rather than a fenestrated endothelium. The basement membrane was thickened, blurred and locally multiplicated. CONCLUSION: Our results show that experimentally-induced thrombosis of cortical microvessels leads to alterations in the capillaries of neurohypophysis.

Chronic heat-induced skin lesions (erythema ab Igne): ultrastructural studies.

Erythema ab igne (EI) is an uncommon skin lesion caused by mild and repeated exposure to infrared sources. The aim of this study was to investigate the ultrastructural alterations in this condition. The ultrastructural study was carried out on 5-outpatients who presented typical EI of their exposed sites. Skin punch biopsies were processed for standard electron microscopy. The epidermis was hyperpigmented, with focal regressive changes of basal keratinocytes. An apparent functional activation of melanocytes with numerical increase of dendritic processes was also observed. The dermis showed abundant melanophages and occasional elastic fiber alterations similar to actinic elastosis. No alterations consistent with preneoplastic skin conditions were observed. The ultrastructural findings associated with EI seem to be nonspecific and consistent with moderate regressive changes of keratinocytes as well as a consensual melanocytic activation and elastic fiber modifications. Similar alterations can be observed in chronic actinic skin damage. This condition is presumably more benign than the ultraviolet exposure. The association of EI and premalignant skin lesions, though occasionally described, seems relatively infrequent.
