Double-blind comparison of halcinonide solution and placebo control in treatment of psoriasis of the scalp.

Twenty-seven patients with psoriasis of the scalp participated in a double-blind, paired comparison study between halcinonide solution (0.1%) and placebo. The therapeutic response was excellent in sixteen patients treated with halcinonide and in one patient treated with placebo. In the comparative evaluation halcinonide was superior in twenty-two patients, the placebo was superior in four patients, and both drugs were equally effective in one patient. There were no adverse reactions due to halcinonide, but one patient experienced pruritus with the placebo solution.

[Comel's linear circumflex ichthyosis and Netherton's syndrome. General conceptions based on study of 4 cases (author's transl)].

We have recently observed 4 new particularly demonstrative cases and we think it is adequate to give a general conception in order to bring together in one and a single syndrom the ichthyosis linearis circumflexa Comel and the Netherton's syndrom. We also emphasize some features:--the possible association of ichthyosis linearis circumflexa with family epileptogenous encephalopathy;--a particular type of trichorrhexis which we named "frayed pili torti";--the usual lethality in males, with possible serious viral and pyococcic superinfection;--the characteristics of ichtyosiform erythroderma in the Netherton's syndrom. At the end of this study we confirm that a unicist theory which would consider Netherton's syndrom and ichthyosis linearis circumflexa as one and a single disease should be adopted.

Red man syndrome: inadvertent administration of an excessive dose of rifampin to children in a day-care center.

A cluster of toxic reactions among children inadvertently given excessive doses of rifampin for chemoprophylaxis of invasive Haemophilus influenzae disease in a day-care center was investigated. In all 19 children, who received five times the therapeutic dose of rifampin, dramatic adverse reactions developed. A striking, "glowing" red discoloration of the skin and facial or periorbital edema were found to be the hallmarks of rifampin toxicity. These clinical signs of acute toxicity contrast sharply with the adverse side effects of rifampin reported with therapeutic doses.

Multicenter study comparing 0.05% gel formulations of desoximetasone and fluocinonide in patients with scalp psoriasis.

A double-blind, multicenter study was conducted to evaluate and compare the safety and efficacy of desoximetasone gel 0.05% and fluocinonide gel 0.05% in patients with scalp psoriasis. One hundred twenty-five patients were enrolled in this randomized, parallel-group trial. Responses based on clinical assessment in 123 patients showed that the desoximetasone gel formulation is a safe and effective treatment for psoriasis of the scalp. Although efficacy appears equivalent to that of fluocinonide gel 0.05% in treating psoriasis of the scalp, desoximetasone appears to be slightly better tolerated and better accepted cosmetically.

Localized cicatricial pemphigoid (Brunsting-Perry syndrome).

A patient who presented with scarring alopecia from a vesicular disorder was diagnosed as having localized cicatricial pemphigoid. The diagnosis was confirmed by histopathology and immunofluorescent studies. The clinical, immunologic, and therapeutic features of this entity are reviewed.

Cell kinetics of the human anagen hair follicle. Flow cytometric studies in healthy and psoriatic subjects.

Anagen hair follicles obtained from both healthy (n = 7) and uninvolved psoriatic (n = 4) scalps were segmentally analyzed for proliferative activity using DNA flow cytometry. Hair follicle kinetics were almost equal in either group except for the infundibular portion which exhibited significant increase of S-phase values in psoriatic patients. Maximum proliferation was disclosed within the bulbar segment. This study confirms that cell kinetics behavior of hair follicles from uninvolved scalp of psoriatics compared with those from healthy scalps is altered in the infundibular portion only.

Isolation of Bipolaris australiensis from a lesion of viral vesicular dermatitis on the scalp.

Bipolaris australiensis is recorded as an opportunistic pathogen of humans. The fungus was recovered from a biopsy specimen of skin taken from an area of viral vesicular dermatitis on the scalp. Hyphal elements of B. australiensis were seen in the tissues. The taxonomy of the genera Bipolaris, Drechslera, Exserohilum and Helminthosporium is discussed with particular reference to published case reports.

Identification of dander-specific and serum-specific allergens in cat dandruff extract.

Cat dandruff extract was characterized by crossed immunoelectrophoresis (CIE) and crossed radioimmunoelectrophoresis (CRIE) using sera from 27 cat-sensitive individuals. By applying a CIE/CRIE system with intermediate gel, the content of dander-specific and serum-specific allergens was established. Thirteen serum-related and eight dander-related antigens were identified in cat dandruff. Of these, eight antigens were visibly radiostained in CRIE. Five allergens were serum-specific and of these allergens cat albumin was the most important (intermediate allergen). Three allergens were dander-specific and of these three only cat allergen I fulfilled the criteria for a major allergen.
