[Substantiation of requirements for basic informative characteristics of tomograms].

In order to estimate the clinical efficiency of the method of layer-by-layer roentgenography a comparison of the apparatus with different translocation trajectories of the tomographic system has been made. The efficiency of the tomographs was appraised by such parameters as: coefficient of purity, thickness of the section and the degree of blurring. Investigations showed that the circular trajectory is to be preferred to the non-linear one. The adoption of such a trajectory enables it to simplify the design of the non-linear tomograph, makes it less costly and convenient in handling.

Traumatic cardiac injury following sternal fracture: a case report and literature review.

Myocardial injury used to be thought as one of the major complications associated with sternal fracture even though recent studies on injuries associated with fracture of sternum are contrary to this belief. Many authors now believe the presence of sternal fracture is no longer indicative of occult injuries to the underlying structure such as the heart. However, clinicians should still maintain a high index of suspicion for the presence cardiac tamponade in cases presented as blunt chest trauma as early diagnosis and surgical intervention is vital to the patient's survival. Presented is a case of a 38-year-old female patient transferred to our hospital after being injured in a motor vehicle accident. On arrival her blood pressure (BP) was 90/50 mmHg but it then dropped to 60/30 mmHg two hours later. Although her chest roentgenography and electrocardiography (ECG) did not reveal any significant findings, the two-dimensional echocardiography was performed and revealed a moderate amount of pericardial effusion. The chest computerized tomography (CT) scan later revealed sternal fracture and cardiac tamponade. A diagnosis of cardiac rupture resulting from sternal fracture following blunt chest trauma was made. Under midline sternotomy, her right atrial rupture was repaired. The patient was reported to be doing well during a three months, post-operative follow-up.

Early diagnosis and radical surgical treatment of Budd-Chiari syndrome.

We report a 26-year-old woman who was diagnosed with Budd-Chiari syndrome following consultation for a skin nodule in the lower extremity. Histopathological examination of a biopsy specimen showed features of erythema induratum. As part of the diagnostic work-up, chest roentgenography performed to rule out possible tuberculosis showed enlarged right lower mediastinum. Computed tomography identified a dilated azygos vein and obstruction of the inferior vena cava near the liver. Liver function tests and blood cell counts were all within normal limit and no sign of portal hypertension was noted except for mild splenomegaly. Although angioplasty by balloon catheter resulted in recanalization of the obstructed inferior vena cava, obstruction of the inferior vena cava appeared again 2 months later. One-stage surgical reconstruction of the vascular abnormalities affecting inferior vena cava and hepatic vein using autologous pericardial patch was performed 11 months after angioplasty, which resulted in normalization of blood flow. Examination of a liver biopsy obtained intraoperatively revealed hepatic fibrosis compatible with early-stage Budd-Chiari syndrome. No complications were noted postoperatively and the nodular lesion in the lower extremity disappeared after surgery.

[Syphilis lesions of the stomach].

The paper describes a syphilitic lesion of the stomach in a man who for the first time consulted therapeutist and gastroenterologist with symptoms similar to stomach tumors. A possible infection took place during diagnostic fibrogastroscopy (FGS). Reiterated FGS did not permit endoscopists to suspect syphilis. The data of roentgenography fell into the pattern of stomach cancer. After additional examination with performance of specific seroreactions, an early latent syphilis was diagnosed. The treatment resulted in full clinico-serologic regression of the disease. A timely detection of syphilitic stomach lesion is a great problem if clinical manifestations are estimated separately from anamnestic data.

Diffuse gastric polyposis: report of a case.

In diffuse gastric polyposis, all or a large part of the gastric mucosa is covered with polyps. Diffuse gastric polyposis was accidentally found in a 59-year-old female during study of her anemia. The diagnosis was confirmed by esophagogastroduodenoscopy, double-contrast barium upper gastrointestinal roentgenography, and colonoscopy. Treatment was total gastrectomy with Roux-en-Y esophagojejunostomy. Differential diagnosis of inherited gastrointestinal polyposis syndrome should be considered; malignant potential is the greatest concern.

[Experimental study of pBMP/MPTCP in bone defects therapy].

To develop a new substitute of bone graft, which is composed of porcine bone morphogenetic protein (pBMP) and the magnetic porous tricalcium phosphate (MPTCP), we investigated the effects of pBMP/MPTCP (Group A), autologous red bone marrow/porus tricalcium phosphate (BM/PTCP) (Group B), pBMP/PTCP(Group C) on the repair of bone defects created surgically in rabbits. By gross inspection, roentgenography examination, histological examination, immunohistochemistry test of osteocalcim, quantitive histological measurement of new bone formation and biomechanical test, three kinds of substitutes were proved to be biocompatible. Group A induced more new bone formation than other two groups, and showed a better biomechanical function(P < 0.01). It is believed that pBMP/MPTCP has characteristics of strong osteoinductive capacity, good biocompatibility, excellent filling of bone defects and gradual biodegradation.

[Establishment of fetal limb bud culture model of mouse].

In order to study the development of bone and the effect of each single factor on the metabolism of bone during gestation age, a self-made rotating apparatus was designed. The limb bud of fetal mouse at the 16th day of gestation was examined with the techniques of X-roentgenography and histological slices. Comparing with the uncultued control limbs, the test limb cultured for six days showed longer bone length and higher bone density. The cultured bone cells showed active differentiation, proliferation and increased bone trabecula by histological examination. It is concluded that the cultured bone continued developing and differentiating, which confirming the feasibility and reliability of the method.

Screening for pulmonary tuberculosis among military conscripts in Taiwan.

BACKGROUND AND PURPOSE: Military service is obligatory for young men who are medically fit in Taiwan. Each year, all 19-year-old men are notified to undergo a required health check to determine their eligibility for military service. This study determined the prevalence of pulmonary tuberculosis (TB) among military conscripts in 1997 and 1998 and evaluated the effect of the mass radiographic screening program for military conscripts on the reported TB case rate. METHODS: During the annual health check, all 19-year-old men in Taiwan undergo miniature (70 x 70 mm) chest roentgenography (CXR). Those who have suspicious lesions on miniature films are notified to undergo follow-up CXR (14 x 14 inch) and sputum examinations. All these data for the period from 1997 to 1998 were analyzed to determine the prevalence of pulmonary TB. To evaluate the effect of the mass radiographic screening program among military conscripts on the reported TB case rates, annual data for newly diagnosed pulmonary TB in 1997 and 1998 were obtained from the National TB Register and analyzed by age and sex. RESULTS: A total of 305, 140 men eligible for military service underwent the required examination in 1997 and 1998. Pulmonary TB was diagnosed in 237 (0.08%) of these men, and 21 (6.9/100,000) had positive bacteriologic findings. In 1997 and 1998, the annual reported TB case rates for males and females were similar in both children and young adults, except for an early peak among men aged 19 years. CONCLUSION: The screening program resulted in a significant increase in the reported TB case rate among males aged 19 years. However, a low bacteriologic confirmation rate implies the possibility of over-diagnosis with CXR. Mass radiographic screening at the health check for military conscripts is inefficient in detecting bacteriologically confirmed TB, an observation with implications for screening policy development by the national TB program.

Neural-crest tumor presenting with chronic diarrhea: a report of three cases.

The association of chronic diarrhea with neural-crest tumors is uncommon. In the past 12 years, we encountered three cases of neural-crest tumors presenting initially as chronic diarrhea. The incidence of chronic diarrhea in patients with neural-crest tumors at our hospital during this period was 3.8%. These patients (two girls and one boy aged between 15 to 28 mo) presented with a 3-week to 6-month history of chronic diarrhea of unknown cause. A posterior mediastinal mass or abdominal mass found on sonography and chest roentgenography eventually led to the diagnosis of neural-crest tumor in these patients. Pathology revealed ganglioneuroblastoma in two cases and neuroblastoma in one. All three patients received chemotherapy and underwent surgery. Diarrhea ceased postoperatively in all three patients. Although one of the patients died of septic shock during chemotherapy, the other two remained free of disease at 30 and 22 months of postoperative follow-up, respectively. A high index of suspicion is needed to identify cases of neural-crest tumor from the presenting symptom of chronic diarrhea.

Effect of zinc on bone metabolism in fetal mouse limb culture.

OBJECTIVE: To determine the effects of zinc-deficiency and zinc-excess on bone metabolism. METHODS: We developed the culture model of fetal mouse limbs (16th day) cultivated in self-made rotator with continuing flow of mixed gas for six days in vitro. The cultured limbs were examined by the techniques of 45Ca tracer and X-roentgenography. RESULTS: The right limbs cultivated had longer bone length, higher bone density than the left limbs uncultivated from the same embryo; and histologically, the right limbs had active bone cell differentiation, proliferation, increased bone trabecula, clearly calcified cartilage matrix, and osteogenic tissue. Compared with the control group, the zinc-deficient group and zinc-excess (Zn2+ 120 mumol/L) group contained less osteocalcin (BGP) and 45Ca content, and lower AKP activity; whereas zinc-normal (Zn2+ 45 mumol/L and Zn2+ 70 mumol/L) groups contained more BGP and 45Ca contents, and higher AKP (alkaline phosphatase) activity. CONCLUSION: Both zinc-deficiency and zinc-excess can alter bone growth and normal metabolism. The results indicate that the culture model of fetal mouse limbs (16th day) in vitro can be used as a research model of bone growth and development.

Enzyme replacement therapy with imiglucerase in Taiwanese patients with type I Gaucher disease.

BACKGROUND AND PURPOSE: The management of Gaucher disease has been dramatically improved by the development of enzyme replacement therapy (ERT), but there have been no reports of imiglucerase treatment of this disorder in Taiwanese patients. The goal of this study was to evaluate the outcome of imiglucerase treatment for type I Gaucher disease in the first Taiwanese patients to receive this treatment. METHODS: Six patients received ERT for type I Gaucher disease at National Taiwan University Hospital. Their mean age was 36.5 years; four had undergone splenectomy before and one after the start of ERT. All received intravenous imiglucerase 60 U/kg every 2 weeks. Patients were regularly evaluated, including physical examination, laboratory tests and bone roentgenography. RESULTS: All six patients responded well to therapy. The mean length of therapy was 30.3 months. Five patients had anemia, and the mean increase in hemoglobin was 4.46 g/dL. Five patients had thrombocytopenia, and the mean increase in platelet count was 259 x 10(3)/microL. Laboratory data showed a slower response in hemoglobin than in platelet count, which may have been related to splenectomy and low initial platelet count. A decrease in serum alkaline phosphatase was the most prominent finding related to liver function. All patients had a prominent decrease in liver size. No patient had painful bony crises, but no significant improvement was observed in skeletal roentgenograms. There were no serious side effects. CONCLUSION: ERT reversed the visceromegaly and hematologic abnormalities of Gaucher disease and improved the patient's quality of life. It was effective in reducing organomegaly, correcting anemia and thrombocytopenia, improving liver function and alleviating bone pain.

Treatment of Toxoplasma brain abscess with clindamycin and sulfadiazine in an AIDS patient with concurrent atypical Pneumocystis carinii pneumonia.

Toxoplasmosis is the most common opportunistic infection of the central nervous system in patients with AIDS. The standard treatment for toxoplasmic encephalitis is pyrimethamine and sulfadiazine. There have been few reports of concurrent Toxoplasma brain abscess and cavitary Pneumocystis carinii pneumonia (PCP) in Taiwan. We report the case of a 26-year-old homosexual man with coexisting infection with Toxoplasma gondii and P. carinii who was successfully treated for brain abscess with clindamycin and sulfadiazine. The cavitary lung lesions, initially diagnosed as pulmonary tuberculosis, were proved to be PCP by lung biopsy. HIV infection and syphilis had been diagnosed 1 year before admission. He presented with general weakness, ataxia, nausea, blurred vision and fever for 2 weeks. Magnetic resonance imaging of the brain revealed multiple ring-enhanced lesions over the cerebrum and cerebellum. Chest roentgenography showed a 3-cm lesion with cavitation over the right upper lung field. Diagnostic computerized tomography-guided lung biopsy revealed P. carinii cysts. Clindamycin, sulfadiazine and trimethoprim (TMP)-sulfamethoxazole (20 mg/kg/day TMP) were given with good response. His CD4 count rose from 40 to 280/microL 4 months later. All antibiotics were discontinued after 4.5 months due to the development of a skin rash. He was well at follow-up 1 year later. This case suggests that the combination of clindamycin and sulfadiazine is an effective treatment for Toxoplasma brain abscess and highlights the importance of diagnostic lung biopsy for cavitary lung lesions, particularly in a region endemic for tuberculosis.

Potential pitfalls in the diagnosis of aortic dissection.

STUDY OBJECTIVES: The primary objective of our study was to identify clinical issues contributing to delay in the diagnosis of nontraumatic aortic dissection. DESIGN: Retrospective observational study. SETTING: A 425-bed community based, university-affiliated teaching hospital. PATIENTS: Medical records were analyzed for physiologic, clinical, and outcome variables in 32 consecutive patients who presented to our hospital with non-traumatic aortic dissection over a three-year period (1995-1997). MEASUREMENTS AND RESULTS: The mean time to diagnosis was 10.7 hours (range 0.5-72 hours). The time to diagnosis and clinical variables did not differ between type A and type B aortic dissections (8.7 vs 11.4 hours; P = 0.54). The diagnosis of aortic dissection was not entertained initially in 44% (14/32) of patients, leading to a significant delay in diagnosis compared to patients with suspected aortic dissection on admission (15.0 vs 4.5 hours; P = 0.008). Lack of a widened mediastinum on chest roentgenography (18.7 vs 6.6 hours; P = 0.026) and lack of hypertension (< 140/90 mmHg) at presentation (14.9 vs 7.1 hours; P = 0.03) were associated with a delayed diagnosis of aortic dissection as compared to presence of either finding. The presence or absence of chest or back pain, resting ECG changes, and a past medical history of hypertension did not correlate with time to diagnosis. There was a trend toward earlier diagnosis when the initial diagnostic modality was transesophageal echocardiography as compared to computed axial tomography (CAT) scan (7.7 vs 10.1 hours; P = 0.20). CONCLUSIONS: Patients with aortic dissection are frequently normotensive and may lack typical chest roentgenographic findings at presentation. A high index of clinical suspicion for aortic dissection is essential to avoid potentially fatal delays in its diagnosis.

[Determination of spleen size by roentgenographic and nuclear medicine methods (author's transl)].

Knowing the splenic size is important in many medical diseases. This paper describes methods how to measure the size of the spleen by palpation, scan and a roentgenogram with soft tissue technique. In 486 normals medium splenic length was 12,8 +/- 1,9 cm measured roentgenographically. In 60 patients with splenomegaly the correlation coefficient between roentgenologically determined size and scintigraphically measured splenic length was 0,89, between radiologically determined size and estimated splenic weight by scan was 0.69. Nuclear medicine methods are important for exact evaluation of the size of the spleen, roentgenography is adequate in screening and control examinations.

[Treatment of advanced metastatic breast cancer with exemestane, a multicenter randomized controlled study of 195 cases].

OBJECTIVE: To observe the efficacy of exemestane for postmenopausal advanced metastatic breast cancer, and to assess its side effects. METHODS: A randomized, double-blind, and parallel controlled study was conducted among 195 patients with postmenopausal advanced metastatic breast cancer from December 2001 to June 2002. Except for the 4 cases who were lost to follow-up, the remaining 191 patients were divided into two groups: study group (n = 96, treated with exemestane capsule 25 mg and one model tablet of letrozole orally q.d. for 8 weeks), and control group (n = 95, treated with letrozole 2.5 mg and one model capsule of exemestane orally q.d. for 8 weeks). Physical examination, roentgenography and CT were conducted to observe the outcome of the tumor and the level of estrogen was tested 2 weeks before and 4 and 8 weeks after the beginning of treatment. RESULTS: The effective rate was 44.8% in the study group and 45.3% in the control group (P = 0.971). The level of estradiol was 5.17 +/- 6.68 x 10(4) pg/L and 4.19 +/- 3.06 x 10(4) pg/L in the study group and control group respectively; and was 3.08 +/- 2.80 x 10(4) pg/L and 2.76 +/- 1.98 x 10(4) pg/L in the study group and control group respectively 8 weeks after the beginning of treatment, both decreased by 43.7% in comparison with those before treatment (both P < 0.001), however, there was no significant difference between the study group and control group (P = 0.141). The side effects of exemestane included thirst, giddiness, and nausea. CONCLUSION: An effective hormonal medicine, exemestane has good therapeutic efficacy in postmenopausal advanced metastatic breast cancer with only mild side effects.

[BMP-2 gene modified tissue-engineered bone repairing segmental tibial bone defects in goats].

OBJECTIVE: To evaluate the effectiveness of the tissue-engineered bone substitute loaded with adenovirus mediated human bone morphogenetic protein-2 gene (Adv-hBMP-2) transfected bone marrow derived mesenchymal stem cells (BMSC) in the repair of diaphyseal segmental bone defect of large animal. METHODS: The right tibial bone defects (2.6 cm) model of 26 goats were established and divided into 5 groups: I. Adv-hBMP-2 transfected BMSC/calcined bone (CB) group (n = 9); II. adenovirus-beta-galactosidase (Adv-betagal) gene transfected BMSC/CB group (n = 6); III. untransfected BMSC/CB group (n = 6); IV. single CB group (n = 3); VI. untreated group (n = 2). The above tissue-engineered bone substitutes were implanted in the bone defects respectively except group VI. Roentgenography, histomorphometrical analysis and biomechanical measurement were studied at various times. RESULTS: X-ray: at 4 - 8th weeks after implantation, more bony callus was found in the bone defects of group I. The complete healing rates of group I, II, III, IV, and V were 5/8, 1/5, 0/5, 0/2, 0/1 respectively at 26th week after implantation. Histomorphometrical analysis showed much more new bony callus including cortical bone formed in group I than those of other groups. The compression strength of the implanted bone substitute of group I is significantly higher than those of group II and III. CONCLUSION: The tissue-engineered bone substitute loaded with human BMP-2 gene transfected BMSC can repair diaphyseal segmental bone defect of large animal (goat).
