[Antibodies to Forssman antigen in patients with cancer of the gastrointestinal tract and breast].

Blood sera of 103 normal persons, 104 patients with malignant and 33 patients with benign tumors of different sites, 28 patients with inflammatory diseases of the gastro-intestinal tract and female sexual organs were examined to demonstrate antibodies to Forssman's antigen (FA) by means of the microhemagglutination test with sheep red blood cells. It was established that reduction in the amount of antibodies to FA was the most characteristic for gastro-intestinal carcinomas and to a lesser degree for breast cancers and that it was sporadically encountered in normal persons with non-cancerous pathology. The number of patients with low titres of antibodies to FA increased as the cancerous process progressed.

[The individual choice of drug preparations].

A method of superhigh-frequency dielectric measurement was proposed for control of hormone-stimulated adenylate cyclase system of human erythrocytes. The method was used for the analysis of some adrenoblockers. Differences in their effects in healthy persons and patients with cardiovascular pathology were found.

Shame in the treatment of schizophrenia: theoretical considerations with clinical illustrations.

The phenomenology and dynamics of shame have been largely overlooked in the psychoanalytic and psychological literature. The emerging literature now suggests that shame may play a vital role in autonomy and personality development, symptom formation, character pathology, and interpersonal relationships. This paper attempts to describe shame phenomena and identify shame dynamics. The role of shame in the understanding and treatment of schizophrenic individuals is then demonstrated through reference to the writings of Frieda Fromm-Reichmann and examples from the author's clinical work.

[Registration of fibular movements in vivo in relation to the tibia at the level of the ankle joint].

Fibular movements in relation to the tibia are known on extension and dorsiflexion of the ankle-joint and have been proved by experiences on cadaveric preparates using outstanding points of relation. To registrate fibular movements in vivo a special trigonometric system has been developed permitting calculation of definite movement in frontal and sagittal plane as well as a rotation around axis. Twelve healthy volunteers were examined in this way during extension and dorsiflexion of the ankle, with and without weight-bearing. The results showed that we should not consider isolated fibular movement but analyse simultaneous complexity of movements in a graphic comparison. Under these conditions we observe a very high significance of fibular movements depending each of other. The emphasis can be designed in a field covering all the points reached by fibular movement in frontal and sagittal plane of the twelve healthy volunteers during movement of the ankle; it shows that the fibula functions like a guide-stick for ankle movement. Reflecting on the designed fields of healthy persons, some patients with posttraumatic pathological alterations of the ankle-joint have been analysed.

[Topography of the contours of the inner surface of the wall of the left ventricle of the heart in systole].

Chosen at random 38 diastolic preparations of human hearts from persons having not any cardiac pathology, as demonstrate the postmortem examination, have been investigated. The left ventricle casts have been made during the first 24 hours after death according to a strictly fixed technique by means of filling the cardiac chambers with polymere mass--protacryl--under a physiological pressure of the diastolic filling. The trabecules are arranged as a spiral from the apex of the ventricle up to the atrioventricular fibrous ring, with approaching the apex the spiral step increases and the trabecules straighten. The left ventricle cast is devided into some planes, the envelopes and the trabecularity lines are measured. Average values of the shift in the trabecularity lines I, II, III and in the cross sections B, C, D, E are defined in relation to the plane A and in every case in relation to the previous plane Cn-1. The data obtained are presented in tables and diagrams. The greatest shift demonstrate the trabecularity lines I running predominantly along the posterior wall of the left ventricle in the planes B and which are situated nearer to the atrioventricular ring projection. Owing to the presence of the spiral-shaped course of the trabecules, it is possible to suppose that it influences the blood stream twisting clockwise in the left ventricle during the diastole phase. This indicates the necessity to work out some new constructions of artificial cardiac valves, securing the twisted blood stream. The condition mentioned should be taken into consideration while making prostheses of the cardiac valves.

[Effect of maternal anxiety (unrest) on the health status of the fetus].

The aim of our study was to clarify the question, to what extent the anxiety of the gravida during gestation is capable to influence the fetal condition.--For that reason, besides of a statistical analysis of a standardized interview the basal heart frequency and the reaction in a non-stress CTG over 20 min has been examined. The data of 101 women between the 36th and 42nd week of gestation were included. The age of our patients ranged from 20 to 30 years.--For assessment of the degree of anxiety we used the STAI X-1 test (questionnaire for self-assessment of anxiety as condition and characteristically personal property, resp.). Cases of pathologic gestation were excluded by history and a carefully clinical examination. No significant correlations between the level of anxiety and fetal condition could be demonstrated. We detected only a slight tendency of decreasing heart-rate variability and increasing frequency of accelerations in the CTG, as well as an increase of active fetal movements with a raising maternal level of anxiety.

[Dyspepsia among the principal diseases of the upper tract of the digestive apparatus. Analysis of the symptom in 700 cases subjected to gastroduodenoscopy].

700 cases subjected to endoscopic examination were investigated to find the relationship between dyspepsia and personal characteristics, lifestyle, family and pathological history and formulated diagnosis. Dyspepsia is taken to represent: epigastric heaviness following meals, prolonged and difficult digestion accompanied by the sensation of slow gastric emptying and fullness. Dyspepsia occurs with equal frequency in both series and is more common during the first decades of life. Dyspepsia is not affected by whether or not the patient lives in the town or the country, smokes, drinks significant amounts of alcohol or coffee. Neither a family history of gastroduodenal or intestinal complaints or gall bladder stones or chronic liver diseases have many bearing on dyspepsia. An attempt to connect endoscopic diagnosis with dyspepsia had no positive result. Dyspepsia was observed with the same frequency in subjects free from upper digestive tract diseases, subjects with organic disease and subjects with functional diseases.

[Echography in tumor pathology of the spleen: limitations and perspectives].

The authors review the symptomatology of splenic tumor pathologies in connection with 93 personal observations (71 non-Hodgkin lymphomas, 12 Hodgkin's disease, 6 metastases, 3 epidermoid cysts, 1 angioma). Nodular forms were observed in 31.2% of the cases reviewed. In most cases of lymphoma, the ultrasonographic finding was a homogeneous splenomegaly. The possibility of false negatives for splenic involvement in lymphomas when spleen volume is normal has led certain authors to suggest fine needle puncture of the spleen under ultrasonographic guidance. Such procedures are unnecessary if ultrasonography has detected an obvious lesion such as multinodular involvement of homogeneous splenomegaly. However, this moderately traumatic technique can provide valuable information until such time as ultrasonic tissue characterization has proven itself in vivo.

[Radiologic study of aberrant pancreas of gastroduodenal topography. Apropos of 11 cases].

In connection with 11 observations of aberrant pancreas in the gastro-duodenal area, the authors review the general and radiological features of this pathology. In ten of the personal cases reported on, radiological examination allowed diagnosis following discovery of an intramural image centered around an umbilication. While not uncommon, pancreatic heterotopias are generally fortuitous findings due to the usual absence of clinical symptoms. They are small, and tend to occur in the digestive tract, especially in the antro-duodenal region. Possible complications include adenomatous transformation which may or may not be insulinogenic, cystic degeneration, cystenadenoma and carcinoma. Such complications are rare, however, and are generally not discovered before surgery. Radiological examinations were negative in 23,7% of the cases reviewed; while the typical image of a rounded filling defect centered around an umbilication is easy to diagnose, the value of radiology is considerably less when dealing with peduncular, subserous or complex forms. Although certain authors recommend only surveillance following radiological and endoscopic detection of a small aberrant pancreas, the majority of authors advise exeresis.

Ventricular late potentials. Part II. Clinical importance and interpretation.

This second part of a review on the ventricular late potentials (VLP) includes most of the latest data on their clinical interpretation. A description of VLP characteristics in normal subjects is followed by an analysis of their significance in several cardiovascular diseases in which VLP presence has been almost currently investigated, i.e., acute myocardial infarction, postinfarction states, unexplained syncopes, cardiomyopathies and post-heart surgery states. Particular situations such as the congenital or acquired syndrome of long QT and VLP incidence, bundle branch blocks and VLP presence, the effect on the VLP of some anti-arrhythmogenic blockers of sodium channels are further discussed. Up to date information on the VLP significance in cardiovascular pathology and the author's personal experience on the VLP significance in patients with ischemic heart disease, syncopes, dilating cardiomyopathies or long QT interval are presented.

[Body image of the male athlete. A study of the psychological health of wrestlers and rowers of the lower weight class].

The present study reports findings concerning the body-experience of male athletes. 25 wrestlers and 59 rowers in the lower weight categories were investigated using the questionnaire for evaluation of body-experience "Fragebogen zur Beurteilung des eigenen Körpers--FBK" (Strauss u. Appelt, 1983). Due to nature of their sports, these men are particularly subject to a constant pressure to maintain a low body weight. In 31% of the subjects the FBK profiles were indicative of disturbances of body-image and body-experience. This subgroup also demonstrated multiple pathologically elevated scores in the personality questionnaire "Freiburger Persönlichkeitsinventar" (Fahrenberger et al. 1984) and the Eating-Disorder-Inventory (Garner et al. 1983). Low-weight wrestlers and rowers should be considered a high-risk male population for body-image disturbances and other psychological disorders. The casual relationship between the specific sport and the development of an body-image disorder is discussed.

[The pathogenicity factors in different representatives of the genus Citrobacter].

No trustworthy distinctions were revealed under the comparative analysis of properties of bacteria of Citrobacter genus from 3 studied ecological groups (from patients with diarrhea, from healthy persons and patients with extraenteric pathological process). It allows the finding of potentially partogenic bacteria of Citrobacter genus in patients with diarrhea to be considered the manifestation of dysbacteriosis.

[Penitentiary medicine and the Rights of Man].

Penitentiary medicine combines preventive medicine and treatment which are for the most part carried out in renovated structures. Other than the usual illnesses, these services and their personal are confronted with specific pathologies due to stress, tobacco consumption, insomnia, inactivity, distress, but also hunger strikes and self-mutilation. One particular concern is AIDS and its formidable complications, notably tuberculosis. The important role played by psychiatry and psychotherapeutic support in penitentiary medicine cannot be emphasized enough. Not only are many of the inmates alcoholics or drugs users, many of them are also starved for communication. The Ahens statement (1987), a true codicil of Human Rights, developed by the International Council of Penitentiary Medical Services, formally prohibits doctors to practice any form of torture or experimental medical operations on inmates, and puts medical priority above and beyond any administrative or judiciary considerations. This message is applicable to all prisons, regardless of countries: Europe, the Americas, Africa, Pacific or other. As President of the Council, we have distributed this message throughout the world, to all the various United Nations Agencies, thus teaching Human Rights, including prisoners' rights, in a concrete fashion.

[Characteristic features of ischaemic strokes in young adults (author's transl)].

Clinical symptoms, angiographic findings and risk factors were recorded according to a classification system in 28 women and 39 men who suffered an ischaemic cerebral stroke before the age of 40. This enabled correlations to be made within the group of young patients and the discovery--by comparison with a corresponding group of patients over 40--of statistically significant age-specific features of cerebro-vascular disease. Vascular lesions in young patients show a sex-specific distribution and a characteristic relationship to clinical symptoms. The prognosis is more favorable in young patients; risk factors which predispose to stroke and influence its further development are less frequent. It is, therefore, suggested that characteristic features of strokes in young adults can be partially explained by the fact that they are usually caused by single pathological alterations in otherwise healthy persons.

[Head injuries in sports--evaluation during admission, follow-up and routine check-ups].

The authors review head injuries during sport. Among 3,206 such injuries treated at the Regional Hospital, Trondheim, in the course of one year, 12% were injuries to head and face. Minor injuries are classified as. Mild (no amnesia or loss of consciousness), no symptoms, the activity can continue, but one week of rest is recommended. Moderate (amnesia or loss of consciousness < 1 min), medical examination, and two weeks of rest are recommended. Major (loss of consciousness < 5 min), CT scan and one month of rest are recommended. With repeated moderate or triple mild traumas, CT and the possible termination of the sporting season should be considered. Serious head injuries with intracranial pathology will generally disqualify the person from further elite competition. The possible consequences of repeated head injuries during football and other contact sports are stressed, and more regular use of neurophysiologic methods (EEG) and magnetic resonance (MR) in selected cases is recommended. The authors also point out the advantages of using helmets and protective gear, and the doctors' role in advocating such equipment.

[Detection of retrovirus in salivary glands of patients with Sjögren's syndrome].

As similar symptoms and glandular pathology are observed in certain persons infected with human immunodeficiency virus (HIV) and human T lymphotropic virus type I (HTLV-I), a search was initiated for a possible retroviral etiology in Sjögren's syndrome (SS). Sera from 33% of 15 SS patients reacted against p24 (gag) of HIV. Salivary gland (SG) biopsy specimens from 47% of the 15 SS patients contained an epithelial cytoplasmic protein, reactive with a monoclonal antibody to p24 of HIV. We also detected Mn2+ dependent, Mg2+ independent reverse transcriptase activity in the SG 3 of 10 SS patients. Target genes for HIV and HTLV-I were not found in any of the SG or peripheral blood mononuclear cells from SS patients. These data suggest the presence of an unknown retrovirus, similar to HIV, in the SG, which could contribute to the chronic inflammation of SS.
