Prospectively, 125 patients were examined with 99mTc-perfusion scintigraphy, 89mKr- or 127Xe-ventilation scintigraphy and chest radiogram prior to major surgery. Postoperative therapy-demanding pulmonary complications occurred in 18% of the patients. A statistical association could be demonstrated between all the preoperative tests except ventilation scintigraphy and the frequency of complications. However, the predictive values of each of the tests, or even the combined results, were not significantly different from the frequency of complications among all the patients. It is concluded that the predictive values of perfusion- and ventilation scintigraphy, spirometry and radiogram of the chest are too low to be of any practical use.

Ventilation-perfusion scintigraphy is an established procedure for the investigation of lung disease. Perfusion scans are commonly obtained with a gamma-camera following injection of 99mTc-labelled microspheres. For the assessment of regional lung ventilation, a number of techniques are now being employed, i.e. 133Xe and 127Xe single breath/washout studies, continuous inhalation of 81mKr and inhalation of radioactive aerosols. The latter two methods are now gaining consideration in clinical practice. Lung perfusion scanning is highly sensitive for detection of regional abnormalities of blood flow; the diagnosis of pulmonary embolism remains the most important clinical application of the technique. In this context, the use of a ventilation scan is required in order to increase the specificity of the procedure. In general, lung ventilation-perfusion scintigraphy is of great value for the management of patients with both primary lung disease and heart disease, by providing pathophysiological information of importance for the diagnosis, follow up and the functional evaluation of the patient.
The purpose of this study was to examine the relationship between the pulmonary distribution of inhaled radioaerosol, bronchial responsiveness, and lung function in children and adolescents. The participating subjects (n = 39) were divided into three groups: (1) 14 asthmatics with bronchial hyper-responsiveness (BHR), (2) five non-asthmatic subjects with BHR, and (3) 20 controls without BHR. Pulmonary distribution of [99Tcm) albumin radioaerosol, maximal expiratory flow when 25% of forced vital capacity remain to be exhaled (MEF25), and bronchial responsiveness to inhaled histamine were measured. Twenty subjects (52%) had irregular central distribution and 19 subjects (48%) had regular distribution of radioaerosol in their lungs. No difference in distribution of radioaerosol was found between the three groups of children. The median MEF25 among non-asthmatic subjects (80% predicted) was lower than that found in controls (92% predicted) but higher than that found in asthmatic subjects (55% predicted). A relationship was found between reduced flow at the peripheral airways, as indicated by MEF25 and the degree of central distribution of radioaerosol. Furthermore, subjects with irregular central distribution of radioaerosol had an increased degree of bronchial responsiveness. In conclusion, children and adolescents who have flow rates in the peripheral airways or increased degree of bronchial responsiveness tend to have abnormal distribution of radioaerosols.
