Comment on 'The Influence of Running Velocity and Midsole Hardness on External Impact Forces in Heel-Toe-Running'.

Researchers not sufficiently sensitive to statistical power may interpret non-significant results as demonstrating that the conditions made no difference. Related issues include performer variability, sample size and effect size as they relate to experimental design. Examination of these factors suggests that caution must be exercised when interpreting the results of experimental data.

Finger reconstruction using toe transplantation.

In the mutilated hand where all the fingers are destroyed, the undamaged mobile thumb will not be capable of performing the important function of making a grip or pinch alone. Under such circumstances, it is worthwhile reconstructing the missing fingers so as to restore this simple but basic function.

Amputation, Traumatic↗

Free vascularised toe joint transfer for reconstruction of the metacarpo-phalangeal joint.

Two single and two double free vascularised joint transfers from the foot to hand have been undertaken in four patients over a period of four years. All were to replace metacarpo-phalangeal joints in young patients in whom the use of prosthetic devices was considered unsuitable. All the transfers were successfully completed, but one failed late in the second post-operative week due to sepsis. The results of the remaining three transfers compare favourably with prosthetic arthroplasty and continue to improve. Donor site morbidity in the foot has been minimal and is reviewed.

Osteoarthritis in other joints (hip, elbow, foot, ankle, toes, wrist) after sports injuries.

Osteoarthritis of nonknee joints, although less common than knee osteoarthritis, remains a significant and disabling condition for many present and former athletes. These injuries can be caused by repeated loads or following a specific traumatic event. The resulting pain and loss of motion can limit function and ability. Arthroscopic techniques in many cases enable surgeons to symptomatically treat limitations of range of motion and pain, prolonging active careers. Joint replacement remains the ultimate solution for hip osteoarthritis, and may be a viable option in ankle osteoarthritis.

A tale of two toes.

Percutaneous closure of a patent foramen ovale (PFO) is increasingly being performed for patients with suspected paradoxical embolus. We report a rare case of a PFO occluder device related infective endocarditis.
