[Results of 5-year activity in a program for pregnant diabetics in south-eastern Poland. I. Methodology of a unified program and evaluation of the degree of diabetes compensation in pregnant women].

A model of diabetic-obstetrical management in pregnant diabetics from the south-eastern Poland realized in Cracow has been described. It consists in education, intensive insulin therapy and self-control with borrowed glucose meters. The study included 102 pregnant women enrolled from 1987-1991. It was found that the patients in general with poorly normalized diabetes mellitus referred late to a special unit. It was shown that with experience normoglycemia was achieved with lower insulin doses in four injections. A characteristic feature was a very small number (4) of the patients who had thought of diabetes normalization before pregnancy. Coordinated care provides a possibility of reducing failures in pregnancy, nevertheless it is necessary to develop comprehensive care both before and during pregnancy in diabetic women.

Consumer participation in community health programs: a comparative analysis of two programs.

Two model inner city health-care delivery systems are examined in terms of their organizational structure, the role of the consumer within them, their strategies for change, and their ultimate impact and effectiveness. A group practice prepayment plan in Baltimore had consumers on its governing board and, in alliance with a powerful medical institution, successfully organized around political, economic, and social issues. An Office of Economic Opportunity (OEO) grant-supported, neighborhood health center in Washington, D.C. was less effective due to its lack of community representation in the decision-making process. The Baltimore model influenced the federal, state, and local governments, while the Washington, D.C. model had stronger local, than national, effects.

[Organization of a program for the prevention of disabilities and physical rehabilitation at the center of a national program in the campaign against leprosy: practical advice].

Patients treated and cured on the bacteriological level by multidrug therapy may nevertheless present handicaps, such as deformities resulting from the disease, which have personal and social consequences. It is actually the handicap and disability from which most patients suffer and which concern populations. The number of persons suffering from such handicaps worldwide has been estimated at 4 million. Therefore, the main goal is to gradually integrate the activities of the prevention of disabilities and physical rehabilitation programme (PIRP) into the national leprosy control programme (PNL). The persons involved in the implementation of the programme outline the activities planned under the PIRP, detailed objectives, priorities, the means by which they will be implemented, the content of training programmes, assessment criteria and documents available.

[Promotion Program Health Survey (SGO). IX. Evaluation of the program section 'Toxicology'].

As a part of the SGO Health Research Promotion Programme, in collaboration with the Toxicological Research Stimulation Programme, a research programme in the field of toxicology was realized. The study ran from 1990 to 1994 (4 years) and comprised animal and clinical research into the possible adverse effects of exposure of the foetus and the neonate to polychlorobiphenyls (PCBs), polychlorodibenzoparadioxins and polychlorodibenzofurans via the placenta and maternal milk. The clinical studies in neonates revealed a slight alteration of the thyroid metabolism (still within normal limits) after increased exposure to dioxides and PCB (as measured in maternal plasma, cord plasma and maternal milk), while slight neurological abnormalities without clinical relevance were also observed. It is concluded from these study results that there are no grounds to advise against breast feeding.
