Influence of extended practice on programming time, movement time, and transfer in simple target-striking responses.

Two experiments describe the effects of extended practice on the development of motor control programs for simple target-striking responses. In Experiment 1,400 right-hand trials of simple one-target and two-target striking tasks were performed. In Experiment 2,600 practice trials were given. Overall reaction time (RT) was faster for the one-target condition in both experiments, supporting a response complexity effect. Movement time (MT) for both conditions improved linearly with practice, suggesting that development of the motor control programs was still occurring. Subjects then transferred to a three-target condition for 50 trials, performing the transfer task with the right hand in Experiment 1, and with right and left hands in Experiment 2. Transfer to the three-target conditions produced execution errors in the form of failure to contact the second target and repetitive tapping on the third target. These results suggest that extensive practice may serve to firmly entrench a response sequence, making it difficult to implement a similar, but unique, motor control program. An interpretation in terms of automaticity and enhanced priming of behavioral and neural pathways is offered to account for these results.

Investigation of photon attenuation coefficients for marble.

The total linear attenuation coefficients micro (cm(-1)) have been obtained using the XCOM program at photon energies of 1 keV to 1 GeV for six different natural marbles produced in different places in Turkey. The individual contribution of photon interaction processes to the total linear attenuation coefficients for marble has been investigated. The calculated results were also compared with the measurements. The results obtained for marble were also compared with concrete.

On a non linear describing function for bio-data.

The behaviour and handling of a flexible three variable--parameter function is presented, which is suitable for unbiased regression analysis. The function is defined by an intercept, a slope or coefficient and an exponent on the independent variable. Analog and digital problems and solutions are defined. Handling is simple and the program is run without operator intervention. The range of curves include linear, convex, concave and asymptotic forms and can be extended to include sigmoid and parabolic forms. Some application results are presented for different situations.

Collaboration Between Athletic Training Clinical and Classroom Instructors.

OBJECTIVE: The education of an athletic training student involves a balance between theory and application, which can be stated as a balance between classroom and clinical education. The instructors in these settings should work together to promote the overall educational process. Our primary purpose was to measure the observations and perceptions of physical presence, cooperation, and communication between clinical and classroom instructors and secondarily to determine if these have a perceived effect on the education of the student. SUBJECTS: Clinical instructors, classroom instructors, and athletic training students in Commission on Accreditation of Allied Health Education Programs-accredited and National Athletic Trainers' Association-approved athletic training education programs. MEASUREMENTS: Data were analyzed using various correlation techniques, a general linear model, and a one-way analysis of variance. DESIGN AND SETTING: We designed a questionnaire to measure the observations and perceptions of physical presence, cooperation, and communication between the clinical and classroom instructors. RESULTS: Of the 30 athletic training educational programs solicited for involvement in this study, 19 responded (63%). A total of 737 questionnaires were distributed, and 547 were returned (74%). Classroom instructors rated observations of cooperation between clinical and classroom instructors at a significantly higher frequency than did clinical instructors. Students rated observations of communication at a significantly lower frequency than did the clinical and classroom instructors. All 3 groups agreed that the physical presence, cooperation, and communication between the clinical and classroom instructors has a large effect upon the education of the student. CONCLUSIONS: Clinical instructors must be educators as well as care providers. At the same time, classroom instructors must make efforts to include clinical instructors in all aspects of the educational process. Also, athletic training students should be exposed to the inner workings of their educational programs, so they may have a better understanding and appreciation of how theory and application tie together.

Church-based education: an outreach program for African Americans with hypertension.

The goals of this education outreach demonstration study were to prepare a cadre of registered nurses (RN) as Church Health Educators (CHE), and to test the efficacy of a hypertension (HBP) education and support program in African American (AA) churches for persons with HBP in managing blood pressure (BP). In this two-phase study, RNs were prepared as CHEs in phase 1 and a convenience sample of 97 subjects with HBP was taught by the CHEs in phase 2. The intervention's content included the bases of HBP and HBP management strategies, and was taught in eight 1-hour sessions. Using a pre-test-post-test design, data on knowledge, social support and BP were collected at baseline (pre), post-intervention (post1) and 3 months post-intervention (post2). Major findings include: (1) there was a significant increase in knowledge scores from pre to post1 and post2 (P < or = 0.0001; F = 95.08; df = 1.79); (2) education, age and number of years with HBP explained 49% of the variance associated with HBP knowledge; (3) systolic BP (SBP) and mean arterial BP (MAP) significantly decreased from pre to post1 and post2 (SBP-p < or = 0.0001, F = 18.35, df = 1.91; MAP-p < or = 0.0001, F = 17.80, df = 1.86); (4) DBP significantly decreased from pre to post1 only (p < or = 0.008, F = 17.48, df = 1.91); and (5) relationships were found between social support and DBP, and social support and MAP. Issues that emerged from this study with implications for outreach programming include recruitment and retention, randomization, selective sampling, intervention design and use of volunteers.

A longitudinal perspective on monitoring outcomes of an innovative program.

OBJECTIVES: The use of outcome assessment to evaluate the performance of programs over many years of operation is becoming an increasingly important aspect of health care management. Over a five-year period of program monitoring, this study examined changes in individual client outcomes three months after discharge from a residential work therapy program for veterans with severe substance use disorders. The study also examined the relationship between these outcomes and changing program features, such as staffing, treatment variables, and follow-up rates. METHODS: Data on admissions characteristics, services delivered during treatment, and status at discharges were collected for 3,390 veterans who were treated in 25 sites in the Department of Veterans Affairs' Compensated Work Therapy/Transitional Residence program. Follow-up data were gathered three months after discharge for 1,771 veterans (52.2 percent). Hierarchical linear modeling was used to examine the association between year of discharge, site-level measures of program staffing and follow-up rate, and individual patient-level treatment variables and outcomes. RESULTS: Over the five-year monitoring period, site staff-to-bed ratios and follow-up rates dropped substantially, and veterans attended more Alcoholics Anonymous and Narcotics Anonymous meetings and had more toxicology screens. Higher staff-to-bed ratios were associated with more positive employment outcomes, and higher follow-up rates were associated with poorer outcomes in substance abuse and lower total income. However, no significant outcome trends were observed in clinical follow-up measures after the analyses adjusted for these factors, suggesting that program effectiveness did not deteriorate during a period of program change. CONCLUSIONS: Long-term evaluations of program process and follow-up status can usefully document sustained program effectiveness over many years. Such efforts should enhance their value by examining and adjusting for the impact of factors such as changing staffing levels and follow-up rates.

Linear unsharp mask filtering of linear cross-sectional tomograms of the posterior mandible.

Linear unsharp mask filtering is a digital image processing technique which can enhance diagnostic information by subtracting from the original image the linear blur which was formed in the direction of the linear tomographic movement. It was applied to linear cross-sectional tomograms of the posterior mandible aiming at retrieving cross-sectional outline information. This study demonstrated that the current Orthophos Program 16 with a 6.72 degrees tomographic angle does not provide accurate cross-sectional outline information of the posterior mandible. Linear unsharp mask filtering of these images also failed to provide more information. A larger tomographic angle of 10 degrees was employed with the Polytome to simulate the modified Program 16 with a wider beam. The resulting tomographic images showed better defined cross-sectional outlines, and after the linear unsharp mask filtering, the outlines were more readily retrieved. In order to get cross-sectional outline information of the posterior mandible, the tomographic angle of the Orthophos program 16 should be increased to more than 10 degrees which can be achieved by increasing the width of the x-ray beam.

[Value of an algorithm of automatic adaptation of the atrio-ventricular delay to the instantaneous atrial rate in cardiac stimulation].

It has been suggested that an algorithm of automatic adaptation of the AV delay to the instantaneous atrial rate be introduced into the program of DDD pacemakers to reproduce the physiological adaptation of the PR interval to effort, characterised by progressive shortening inversely linearly related to the heart rate. In order to evaluate the potential benefits in conditions of "standard" programming (basal AV delay the same for all patients: maximal frequency of 1/1 AV synchronisation uniformly limited to 120 bpm), a haemodynamic study was undertaken in 10 patients who had permanent DDD pacemakers implanted for advanced AV block. Measurements were taken during two standardized exercise stress tests (20 W/2 mn steps from an initial load of 20 W) performed in a random order, one with a fixed AV delay of 156 ms and the other with an "automatic AV delay" allowing linear reduction from a maximum value of 156 ms at rest to a minimum value of 84 ms at the maximum heart rate of 120 bpm. At the peak of exercise the "automatic AV delay" significantly affected 4 parameters: the paced ventricular rate (p = 0.008) and rate-pressure product (p = 0.005) which increased, pulmonary capillary pressure (p = 0.03) and cycle-to-cycle variability of systolic and diastolic blood pressures (p = 0.02 < p < 0.0001) which decreased. There was a tendency (NS) to slowing of the spontaneous atrial rate and to increase in cardiac output. This increase was significant in some patients and seemed to be due to a good relationship between the individual optimal value of the value basal AV delay measured by Doppler echocardiography and the value programmed in this study (156 ms).(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of external peer review on cesarean delivery rates: a statewide program.

OBJECTIVE: To assess the effectiveness of a joint-specialty society and health department statewide peer-review program to reduce cesarean rates. METHODS: Forty-five of the 165 hospitals with active delivery services were reviewed between 1989 and 1993. Differences in total and repeat cesarean rates and vaginal birth after cesarean (VBAC) rates were compared by hospital review status using Student t tests and linear regression for the years before and after completion of the program. RESULTS: Reviewed hospitals reduced their total cesarean rate by 3% and repeat cesarean rate by 0.7%, and increased their VBAC rate by 14.6% compared with nonreviewed hospitals, for which the respective reduction in rates was 1%, 0.6%, and 12.7%. Statistically, there was no difference between reviewed and nonreviewed hospitals in terms of rate changes. CONCLUSION: This joint-specialty society and health department peer review had no apparent impact on cesarean rates.

[Multivariable EDV-program for light and electron microscopic morphometry].

A computer program for morphometry and stereology in which conventional morphometric and stereological data acquisition (direct linear measurement, point counting technique) is used and which is designed for variable application of conventional and electron microscopy is presented. There are several program variants and magnification stages each designed for a different structural level. There is fixed number of essential subcellular, cellular and extracellular structures for different kinds of tissue (myocardial, liver and connective tissue), but this number can be enlarged with each stage of magnification and for each measured parameter by reserving free places for evaluating additional structures. Thanks to the extended program and to the fact that these free places are interchangeable, the program can be used independently of the kind of tissue examined and of the kind of morphological information sought. The program structure is realized by means of a master control and 4 external subprograms. The statistical analysis of the morphometric stereological parameters is calculated by image, case by case or group by group. The mean values of the calculated primary and secondary parameters, the standard errors and the standard error/mean value ratio are printed out. All common stereological parameters and the essential cellular and extracellular structures of different kinds of tissue are described in quantitative terms with the aid of the program.

An assessment of Maryland Medicaid's Rare and Expensive Case Management Program.

The Rare and Expensive Case Management Program provides intensive case management services to an especially vulnerable population comprised of primarily pediatric beneficiaries. This study examines the effect of the case management program on the pattern of costs. With its emphasis on obviating the need for acute care services and providing the necessary medical and assistive services in a timely manner, the case management program is expected to change the pattern of expenditures by category of service. Due to the low prevalence of conditions covered by the program, constituting a comparison group for the evaluation posed a major challenge. Employing an innovative methodological approach, a historical comparison group was modeled using statewide Medical Assistance claims files. The study design included a retrospective comparison of Medicaid beneficiaries enrolled in the program with a similar group before the case management program was implemented. The results bear out the anticipated pattern of shifts in cost of care from acute care to nonacute care services.

Description of a new motor re-education programme for the paretic lower limb aimed at improving the mobility of stroke patients.

OBJECTIVE: To describe and examine the feasibility of a new treatment approach for the paretic lower limb and to explore its effectiveness in one chronic hemiparetic stroke subject. DESIGN: Case report. The treatment was conducted three times per week over a period of six weeks. The mobility of the patient was assessed prior to the treatment, at the end of the treatment and at a six-week follow-up. SETTINGS: The study was carried out at the research centre of the Institut de réadaptation de Montréal. The treating therapist was an experienced rehabilitation professional as was the assessor, who worked at a different rehabilitation centre. INTERVENTIONS: The motor re-education programme was based on the use of a static dynamometer that measures the linear external forces produced at the ankle level. A computer program provided the subject with constant feedback on the direction and intensity of the applied force. In each treatment session, the subject was asked to produce several submaximal efforts in 16 specific directions. Both the intensity and the number of repetitions were gradually increased. OUTCOME MEASURES: In addition to force production measurements, three clinical assessments of mobility were used: the Timed 'Up and Go', the comfortable gait speed and a 2-minute walk test. RESULTS: The maximal static linear forces produced by the subject increased through the treatment for all directions of effort, but differences were observed amongst directions. During the treatment programme, the subject improved his performance at the three clinical assessments. Even if some of the functional gain was lost at the follow-up, the mobility was still considerably improved as compared to baseline values. CONCLUSION: This study demonstrated the applicability of the treatment programme to a stroke subject. The results seem very promising and encourage further investigation in order to assess more rigorously the effectiveness of this new approach.

A FORTRAN program for multivariate survival analysis on the personal computer.

In this paper a FORTRAN program is presented for multivariate survival or life table regression analysis in a competing risks' situation. The relevant failure rate (for example, a particular disease or mortality rate) is modelled as a log-linear function of a vector of (possibly time-dependent) explanatory variables. The explanatory variables may also include the variable time itself, which is useful for parameterizing piecewise exponential time-to-failure distributions in a Gompertz-like or Weibull-like way as a more efficient alternative to Cox's proportional hazards model. Maximum likelihood estimates of the coefficients of the log-linear relationship are obtained from the iterative Newton-Raphson method. The program runs on a personal computer under DOS; running time is quite acceptable, even for large samples.

Construction of restriction maps.

A computer program is described, which constructs maps of restriction endonuclease cleavage sites in linear or circular DNA molecules, given the fragment lengths in single and double digestions with two enzymes. The algorithm is based upon a partition method and a very simple rule to chain fragments. The program is written in Prolog II.

[New horizons in medicine. Chaos and its laws].

We have synthesized the fundamentals that modern technology offers in all areas of research, especially in the field of biomedicine. The theory of systems, cybernetics, synergetics, boolean algebra, communication science (according to modern laws of signal transmission and translation), the solution of non-linear equations by computer science, applied principles of reduction in biological survey, fractal analysis as a representation of dynamic, chaotic, non-linear systems, defined attractors as conditioning elements of biologic function, are just a few of the many instruments that modern science offers as a revolutionary approach to research programming. Borrowing the laws of mathematics, we have defined the fundamental characteristics of linear and non-linear homeostatic systems along with the concept of predictable behavior of a system as a function of its complex structure. Lastly, we have documented, based on personal research and recent findings in biomathematics, and despite current and strong opposition, how the functional death of any dynamic system is identified by the system's absolute state of equilibrium. The operative errors at times caused by different stimuli acting on specific organs and apparatus, are interpreted not as an index of altered function but as an expression of a chaotic response of the deterministic type and therefore an indication of the system's adaptability to the specific functional requirements in that precise moment.

Prevalidation statistical design to assess analytical methods. Example of a quick liquid chromatographic assay of itraconazole in serum.

(A) An analysis-of-variance spreadsheet program is presented which allows to readily test and/or quantitate in a single run analytical linearity, matrix effect on recovery, repeatability of measurement and of extraction and the ruggedness of these features for up to three sessions. Owing to napierian logarithmic transformation, ANOVA mean squares directly read as relative standard deviations and checking linearity is straightforward. (B) A quick assay for therapeutic drug monitoring of itraconazole and its main metabolite was devised with the help of the program, and subsequently validated according to current quality control recommendations. The assay involves acetonitrile demixing extraction, reversed-phase HPLC and UV detection and shows acceptable performance from 0.06 to 5.0 mg/l (limit of detection about 0.03 mg/l). The prevalidation design fairly predicted precision and accuracy, was more informative about matrix effect and was even more demanding about analytical linearity.

Community education for cardiovascular disease prevention. Morbidity and mortality results from the Minnesota Heart Health Program.

The Minnesota Heart Health Program was a community trial of cardiovascular disease prevention methods that was conducted from 1980 to 1990 in three Upper Midwestern communities with three matched comparison communities. A 5- to 6-year intervention program used community-wide and individual health education in an attempt to decrease population risk. A major hypothesis was that the incidence of validated fatal and nonfatal coronary heart disease and stroke in 30- to 74-year-old men and women would decline differentially in the education communities after the health promotion program was introduced. This hypothesis was investigated using mixed-model regression. The intervention effect was modeled as a series of annual departures from a linear secular trend after a 2-year lag from the start of the intervention program. In the education communities, 2,394 cases of coronary heart disease and 818 cases of stroke occurred, with 2,526 and 739 cases, respectively, being seen in the comparison communities. The overall decline in coronary heart disease incidence was 1.8 percent per year in men (p = 0.03) and 3.6 percent per year in women (p = 0.007). For stroke, there were no significant secular trends. The authors recently published findings showing minimal effects of sustained intervention on risk factor levels. In the current report, there was no evidence of a significant intervention effect on morbidity or mortality, either for coronary heart disease or for stroke.

Identifying the 3'-terminal exon in human DNA.

MOTIVATION: We present JTEF, a new program for finding 3' terminal exons in human DNA sequences. This program is based on quadratic discriminant analysis, a standard non-linear statistical pattern recognition method. The quadratic discriminant functions used for building the algorithm were trained on a set of 3' terminal exons of type 3tuexon (those containing the true STOP codon). RESULTS: We showed that the average predictive accuracy of JTEF is higher than the presently available best programs (GenScan and Genemark.hmm) based on a test set of 65 human DNA sequences with 121 genes. In particular JTEF performs well on larger genomic contigs containing multiple genes and significant amounts of intergenic DNA. It will become a valuable tool for genome annotation and gene functional studies.
