An 18-year-old man was admitted to our hospital for further examination of fever of unknown origin and ascites. Ga-67 scintigraphy revealed diffuse increased uptake throughout the abdomen. He was diagnosed with tuberculous peritonitis and began the treatment for tuberculosis (rifampicin, 450 mg/day orally and isoniazid, 300 mg/day orally, and 0.75 g of streptomycin by intramuscular injection 2 times a week). One year after starting the treatment, Ga-67 scintigraphy revealed accumulation of radioactivity in the upper abdomen, but the diffuse accumulation in the abdomen decreased. A specimen obtained by tumor biopsy under ultrasonic guidance revealed a tuberculous granuloma. Percutaneous injection was performed in the tumor with 1.0 g of streptomycin. On Ga-67 scintigraphy performed 2 weeks after the injection of streptomycin, the accumulation of radioactivity in the upper abdomen had disappeared. These findings suggest that Ga-67 scintigraphy is useful for diagnosis and observation during treatment of tuberculous peritonitis.
A 46-year-old woman with multiple myeloma presented with neurological symptoms in the right upper extremity. After MR imaging of the cervical spine failed to show the cause of her symptoms, Ga-67 scintigraphy demonstrated increased uptake in multiple areas including the right supraclavicular region and bilateral lower extremities. Histology of the specimen obtained from the left thigh proved soft-tissue involvement of myeloma, and extensive extramedullary involvement was diagnosed. Radiotherapy to the right supraclavicular mass relieved her symptoms. Although Ga-67 scintigraphy is generally considered of limited value in multiple myeloma, this modality contributed to the development of an appropriate strategy in this patient with extensive extramedullary involvement.
UNLABELLED: The objective of our study is to determine whether 67Ga SPECT can supplement CT and/or MRI diagnostic information by visual comparison of the two separate data sets in patients with head and neck tumors. METHODS: A total of 50 patients with head and neck tumors (benign: 19, malignant: 31) were entered in the study. Three board-certified radiologists who had practical experience in interpreting both head and neck CT/MRI and 67Ga SPECT images, participated as readers. All of the CT and/or MR images of each patient were shown to each reader first, who after they had finished interpreting them were shown the 67Ga SPECT images. They were asked to score each image on a 7-point scale for the likelihood of the presence or absence of malignancy. Histological or cytological evaluation was done in all cases, and the radiologic studies were correlated with these findings. RESULTS: Improvement of all three readers' performance was from 70.7% to 83.3% in the mean accuracy and from 0.790 to 0.921 in the mean Az value (p = 0.033, 0.163, 0.105 in the Az values) after they were shown the 67Ga SPECT images. CONCLUSIONS: 67Ga SPECT should substantially increase confidence in the diagnosis of head and neck tumors when CT and/or MRI do not permit differentiation between benign and malignant disease.

The effect of FeCl3 on the uptake of 67Ga by inflammatory and normal tissues was studied to clarify the role of transferrin in 67Ga uptake by inflammatory tissue. The administration of FeCl3 5 min before the injection of 67Ga decreased the uptake of 67Ga by liver and spleen, but had little effect on the uptake of 67Ga by the inflammatory tissue. These results suggest that 67Ga is taken up by normal tissues in a transferrin-bound form but in an unbound form by inflammatory tissue. On the other hand, when FeCl3 was simultaneously injected with 67Ga, the uptake of 67Ga by liver and spleen was markedly increased but the uptake by inflammatory tissue was decreased.
A case of non-Hodgkin's lymphoma with pleural effusion was presented. Pleural effusion is not a common manifestation of this disease and 67Ga-citrate scintigraphy gave the clue for the diagnosis. The possibility of malignant lymphoma should be considered in cases with pleural effusion, and we showed that 67Ga-citrate scintigraphy can be useful in a case with pleural effusion of undetermined cause.
A rare case of adult intussusception caused by malignant lymphoma (ML) of the cecum was reported. In spite of the cecum lesion, strong 67Ga accumulation was located in the right epigastrium. This finding was useful in suggesting the presence of intussusception due to ML.

UNLABELLED: Coincidence-detection 18F-FDG-PET (PET) and 67Ga whole-body and SPECT (Ga) were compared in children and young adults with newly diagnosed Hodgkin's disease (HD). MATERIALS AND METHODS: Thirty patients with histologically confirmed HD underwent PET with attenuation correction 1 h after injection of 150-220 MBq 18F-FDG and whole-body and SPECT imaging 72 h after injection of 250-370 MBq 67Ga citrate. Two experienced readers retrospectively reviewed PET and Ga scans, grading 13 anatomic regions from one (normal) to five (abnormal). Numerical stages were assigned based on Ann Arbor classification. Comparison was made with disease sites (established by biopsy or two or more of the following: physical examination, conventional imaging studies, radionuclide studies, and follow-up studies) and clinical stages. Sensitivity, specificity, and accuracy were calculated and significance of differences determined using McNemar's test. RESULTS: PET detected 120/138 (87%) disease sites and Ga 109/138 (79%). PET and Ga were concordant for 103/138 (75%) sites. Accuracies were not significantly different for supradiaphragmatic disease. PET was more accurate than Ga for detecting splenic (0.91 vs 0.61, P = 0.012), infradiaphragmatic (0.89 vs 0.75, P = 0.042), and all disease sites combined (0.95 vs 0.91, P = 0.039). PET stage agreed with clinical stage in 79% of patients and Ga in 71%. CONCLUSION: PET was superior to Ga for evaluating children and young adults with newly diagnosed HD.
