Evaluation of a combined polymer system for use in relieving the symptoms of xerostomia.

OBJECTIVE: The aim of this series of studies on rheological, sensory, and mucoadhesive properties was to evaluate the potential of using a combined polymer system to more closely represent the adaptive nature of saliva, while providing long lasting relief from the symptoms of xerostomia. METHODOLOGY: The rheological investigations were conducted on 1% polymer solutions 24 hours after preparation, during which time they were stored in sealed glass containers at room temperature. The rheometer was controlled via peltier to 35 degrees C +/- 0.1 degrees C, and a shear rate range of 0.1 to 300 s(-1) was applied; thirty measurements were taken during each run. Eight panelists were recruited from the in-house sensory panel at GlaxoSmithKline (GSK) Weybridge to take part in the sensory evaluation. A brief training session was conducted at the start of the study in order to standardize the panelists' responses to the sensory term "tacky." For each test, panelists were given 10 ml of the respective polymer solution and instructed to swirl this solution around their mouths for 20 seconds before expectorating. The panelists were then asked to evaluate the solution in terms of "tack," measured by tapping the tongue on the roof of the mouth. A new formulation, based upon the rheology and sensory testing, was evaluated versus other commercially available sprays for the relief of xerostomia. The new formulation, named OASIS, contains a combination of Xanthan gum, carboxymethyl cellulose and a polyvinyl pyrollidone backboned polymer. To evaluate mucoadhesion of this final product, a 16 mm diameter cylindrical aluminium probe was selected. The probe and the base platform were both coated with a thin layer of artificial polyurethane membrane to more closely mimic the oral mucosal surface. The probe was set to travel downward at 2.0 mm/sec until it made contact with the base platform with a force of 1000 g. The two surfaces were then left in contact for 30 seconds before the probe receded from the base platform at 0.5 mm/sec to a distance of 10 mm. A plot of force versus distance was obtained, and the area under the curve (defined as the Work of Adhesion, energy required to separate the two surfaces) was used as a measure of the mucoadhesion of the materials. RESULTS: For rheology, the Xanthan gum solution showed high initial viscosity of 32 Pa.s at 0.1 s(-1) with rapid shear thinning to 4.0 Pa.s. By 1.2 s(-1) the viscosity continued to drop with increasing shear rate to 0.05 Pa.s at 300 s(-1). For the sensory assessment, the CMC single solution was perceived to be significantly (p = 0.04) more tacky than the CMC + PVP solution. The Xanthan + PVP solution was perceived to be less tacky (slightly/moderately tacky) than the Xanthan solution (moderately tacky). The Xanthan + CMC + PVP solution was perceived to be less tacky (slightly tacky) than the Xanthan + CMC solution (moderately tacky). Overall, there was a significant difference in favor of the inclusion of PVP across the products (p = 0.038), which was also shown to be a consistent effect across the different polymer solutions. CONCLUSION: The sensory solution work indicates that the addition of a PVP backbone polymer within an anionic polymer solution (OASIS) enables higher concentrations of mucoadhesive polymer to be included within a dry mouth formulation, while at the same time giving improved mouth-feel due to reduced tack.

[Palliative surgical indications in bronchial cancer].

Surgical palliation in bronchial carcinoma may be indicated for severe hemorrhage, putrid expectoration in large necrotising tumors, recurrent pleural effusion refractory to conservative treatment and loss of function of an entire lung due to bronchial and vascular obstruction. Alleviation of imminent asphyxia, dyspnea or malaise is the main purpose, whereas prolongation of survival remains uncertain. Palliative surgery is ranging from tumor pleurectomy to total pleuropneumonectomy. Depending on an individual situation this might be done as a primary indication or, if aforementioned complications must be anticipated, during elective surgery in advanced lung cancer instead of merely explorative thoracotomy. Bronchial obstruction with maintained lung perfusion alternatively is an excellent indication for endoscopic laser surgery, mostly combined with subsequent 192Ir afterloading therapy. Twenty-three cases of primary and secondary surgical palliation within a two-year period and 55 cases of laser/afterloading therapy in a three-year period are analyzed. Survival time is limited in the latter. Due to better general and functional state in the small surgical series in several patients not only relief from badly tolerated symptoms was achieved but also surprisingly long periods of survival with good quality of life in several cases. With proper selection operative mortality and morbidity of palliative surgery as well of laser therapy is of minor concern.

[Current and future approaches for the treatment of inflammatory diseases of inferior respiratory tract].

Among the allergic disorders we emphasize the inflammatory diseases of the inferior respiratory tract by their incidence, repercussion in daily activities, and by their high cost of medical attention. For their treatment, they require more than one inhaled or systemic drug. Current medicines tend to have adverse or secondary effects, such as: osteoporosis, type 3 diabetes mellitus, tremor or tachycardia. New medicines are being developed with less adverse or secondary effects, and much more selective and specific in the molecules involved in the allergic disease's physiopathology. Among them we find pascolizumab, which inhibits the differentiation made to Th2, as well as the citokines production. Other drugs are: mepolizumab, monoclonal antibody antiCD23, and the selective inhibitor of IgE (AJP13358) Rhu-Mab-E25. Currently, they are being developed some new drugs, such as SB-207499 (SKB) and LAS 31025, which are selective inhibitors of fosfodiesterase. Within the currently studied medicines that offer high specificity to inhibit the synthesis of interleukines we emphasize the presence of humanized monoclonal antibodies antireceptor IL-4. The anti IL-5 (SB-240563) is administered in asthmatic patients to reduce eosinofilia in expectoration. Tosilate of suplatast is a selective IL-4 and IL-5 inhibitor. Ramatroban (BAY or 3405) is an antagonist of the tromboxane A2 receptor, which reduces the inflammatory process of the nasal mucose without hemodynamic effects. Immunotherapy with peptides avoids the response of IgE by the allergen, without mast cell degranulation.

[Clinical features and surgery in children with plastic bronchitis].

OBJECTIVE: To review the clinical features and therapeutic experience in children with plastic bronchitis. METHODS: Fourteen children with plastic bronchitis were reviewed retrospectively, 12 of which were under two years old. The clinical features are characterized by sudden onset, episodes of profound hypoxia and respiratory tract obstruction. SaO2 was between 0.70 and 0.80 even with mask oxygen inhalation. Eight cases were pyretic, 4 cases expectorated jel-like bronchial casts. The chest X-ray picture showed patchy consolidation or atelectasis unilaterally (10 cases) or bilaterally (2 cases). Pulmonary marking thickening and patchy shadow were observed in 2 cases. Twelve cases underwent rigid bronchoscopy and the bronchial casts were removed. Two cases underwent endotracheal intubation. RESULTS: Eight cases of 12 children received therapeutic bronchoscopy were cured. Other 4 cases had second therapeutic bronchoscopy and bronchial casts were removed again in 3 cases, one died from pulmonary hemorrhage. Two cases who underwent endotracheal intubation died from the multiple organ failure (MOF). Pathologic results showed:the bronchial casts were composed mainly of mucus and fibrin, inflammatory cell infiltrate were observed in 6 cases (Type 1, inflammatory), no cellular infiltrate occurred in 8 cases (Type 2, acellular). CONCLUSIONS: Plastic bronchitis is a severe and dangerous disease. The branching plastic casts may obstruct part or the entire tracheobronchial, causing respiratory failure. Bronchoscopy and pathologic examination are essential for it's diagnosis and treatment.

[The last case of smallpox in Denmark--the organizing conditions in 1970].

Smallpox contributed to many deaths in Denmark up to the introduction of the vaccination in the beginning of the 19th. Century. The last minor smallpox epidemic in Denmark was in 1924, and subsequently no doctors had special experience in smallpox. In September 1970 a Norwegian medical student died from smallpox in Copenhagen after returning from a journey to Afghanistan, where he has been hospitalized for enteritis. During the 5 days in Copenhagen before hospitalization he had had extensive contacts with many people. He was hospitalized at Blegdamshospital and was isolated, and the diagnosis of smallpox was verified on day 5. He was then totally isolated in a pavilion with 2 nurses and one doctor. The initial diarrhoea (Salmonella typhimurium), and later septicaemia with salmonella, the copious expectoration up to 1 1/2 l pr day (Streptococcus pneumoniae, Klebsiella pneumoniae, E. coli, 9-streptococci), the enormous exudation from the desquamated skin caused large problems concerning water, electrolytes and protein balance, requiring an input up to 13 1 per day. It was necessary to perform tracheotomy and artificial ventilation. He was treated as a patient with extensive burns with metal sheets and when his body temperature fell to 30 degrees C with electrical heat. He died after 25 days of smallpox with complicating extensive skin ulavs corresponding to a pathological picture of toxic epidermal necrolysis (TEN). The containment was successful in cooperation with the Danish National Board on Health, the Medical Officer of Health from Copenhagen and the county (where the patient lived), police, State Serum Institute, and Ministries of the Interior and Foreign Affairs. Vaccination of the exposed persons and the hospital staff, isolation in small groups (maximum 20 persons) of 589 primary contacts in the hospital pavilion-wards and 12 military tents were performed. No secondary cases occurred. The outbreak of smallpox in Copenhagen ended, and the city was not declared "local infected area", and we avoided a panic mass vaccination of large group of people. The article describes these activities, which are effectuated within a few days and headed by a capable and unanimous leadership, in a serious and complicated situation and with an engaged cooperation from the whole staff.

A clinico-radiological study of secondary mycoses in pulmonary tuberculosis.

Clinical and radiological differences were compared in Fungus positive (74) and negative (66) cases of pulmonary tuberculosis. Cough, expectoration, dyspnoea, and fever were marked in former group than that of latter. Anaemia, leucocytosis, raised ESR, abnormal radiological shadows and mycetoma in healed cavity were also noted in significant number in fungus positive cases.

Treatment of lower respiratory infections in outpatients with ofloxacin compared with erythromycin.

To assess the safety and efficacy of a ten-day oral course of ofloxacin (400 mg 12 hourly) as compared with erythromycin (400 mg every 6 hours) for treatment of lower respiratory tract infections, fifty-two adult outpatients with pulmonary infiltrates (pneumonia) or with a cough and purulent sputum (bronchitis) were evaluated. Expectorated sputum specimens were Gram-stained and cultured, and antibody titres to Mycoplasma pneumoniae, Legionella pneumophilia, and in most cases Chlamydia pneumoniae were measured on acute and convalescent serum samples. Patients were evaluated clinically, microbiologically and radiographically three to five days after concluding therapy; the incidence of adverse reactions was monitored throughout the study period. The ofloxacin group (N = 25) was comprised of nineteen patients with pneumonia and six patients with bronchitis. The erythromycin group (N = 27) was comprised of thirteen patients with pneumonia and fourteen patients with bronchitis. All fifty-two patients were either clinically improved or cured after therapy. Microbiological cure was documented in all fourteen cases (27%) in which causative pathogens were identified. Clinical cure was achieved with ofloxacin in 68% of patients with pneumonia and in 83% of patients with bronchitis, while clinical cure with erythromycin was achieved in 46% of patients with pneumonia and 54% of patients with bronchitis. Adverse reactions (mostly mild gastrointestinal or central nervous system symptoms) were reported by eight patients receiving ofloxacin and four patients receiving erythromycin. While the types of adverse effects were similar, ofloxacin showed a trend toward a higher rate of cure than erythromycin. Ofloxacin is a promising new antibiotic for the treatment of acute lower respiratory infections.

[Nephrobronchial fistula].

Nephrobronchial fistula is a rare pathology both in absolute terms and in relation to the complications of renal inflammatory processes. Clinical symptoms may be varied but pulmonary complications, which may mask renal symptoms, are generally predominant; cough, hemoptysis and the expectoration of calculi are rarely found. Occasionally, as in the case reported here, the most evident sign is an infective process due to the contemporaneous fistulization of subcutaneous tissues. Instrumental diagnosis is based on chest and abdomen X-ray, fistulography, retrograde pyelography and abdominal CT. CAT may be useful above all in the study of pararenal abscesses.

[Efficacy of the inhalation of warm mineral water from Moravci well MT-6 in patients with chronic obstructive lung disease].

In 20 patients with chronic obstructive pulmonary disease (COPD) the effects of inhalation of Bisolvon (BS) were compared with the inhalations of thermomineral water of the well MT.6 of thermal spring Moravci (TMW). It was found that mean maximal expiratory flow during inhalations of TMW was statistically significantly higher from that during inhalation of BS (P less than 0.05). By visually-analogous scale the patients during inhalations of TMW were found to have statistically smaller dyspnea (P less than 0.001) and after inhalation of TMW they could expectorate much better (P less than 0.01) in comparison with the period of inhalations with BS. The authors consider the inhalations of TMW to be a good method of therapy of patients with COPD.

[Detection of bronchial cancer. Realities and perspectives].

Primary bronchial cancer is responsible for at least 20,000 deaths per year in France. The treatment of cancer in the clinical phase remains disappointing. Numerous trials, which are reviewed here chronologically, have looked for a way to improve the prognosis by an earlier detection of this tumour. Unfortunately numerous methodological approaches have not been able to avoid the fact that the real value of such an early diagnosis is not always known. The only clear conclusion of these studies is that cytological examination of the expectorate does not lead to an improved survival in patients when compared to that which is obtained with the single radiographic film. However, taking account of the natural history of the disease, it is probably necessary to abandon the concept of an early diagnosis of a tumour which is already too late; it is towards a search for pre-cancerous lesions which are still reversible that the research effort should be turned.

[Study on the Chinese drug beimu. IX. Microscopic identification of the upper epidermis of the scales of Fritillaria bulls].

Beimu, a famous traditional Chinese drug, has been used as an antitussive and expectorant for a long time and it is derived from a number of species of Fritillaria. So it is difficult to identify exactly the botanical origin of the commercial crude drugs by means of macroscopic examination. This paper deals with the microscopic identification of the upper epidermis of the scales of 20 species. The result showed that these species are different from each other in the shape and size of the epidermal cells, the shape, size and arrangement of the cuticular peg. Moreover, it was found that the anticlinal wall of the epidermal cells is not bead--thickened, but cuticular peg formed by cuticle.

Sputum production during recovery from acute asthma--effect of adrenal corticosteroids.

The effect of corticosteroid drugs on sputum production was studied in 74 patients of asthma. They were alternatively allotted to two different treatment schedules-conventional bronchodilator drugs alone and bronchodilators plus corticosteroids. All the subjects recovered, bringing out sputum in the process. However, the steroid treated group produced significantly less sputum than the other group. It is concluded that (a) sputum expectoration is a regular accompaniment of recovery from acute asthma and (b) steroids inhibit sputum production in acute asthma.

[A new anti-inflammatory-analgesic-antipyretic, morniflumate, in the treatment of chronic recurring bronchitis].

To assess the therapeutic effects of morniflumate, a new non-steroidal anti-inflammatory drug, a controlled study versus imidazole-2-hydroxybenzoate, both combined with amoxicillin, and antibiotic therapy alone was carried out on 60 patients, aged 18 to 60 with flare-ups of chronic bronchitis. After administering morniflumate, all the clinical parameters assessed (objective auscultation, cough, expectoration, exertional dyspnoea, chest pain, hyperthermia) had improved. The modifications observed in this group were greater and earlier when compared to those of the control groups. No significant variations of laboratory parameters nor any particular side-effects were reported.

Quantitative determination of four antihistamines in combination by high-pressure liquid chromatography.

A procedure used to assay four antihistamines--brompheniramine maleate, chlorpheniramine maleate, pheniramine maleate and pyrilamine maleate in combination--is discussed. The method requires a high-pressure liquid chromatograph and an intermediate polarity column. The method is accurate and appears to be stability-indicating. The four antihistamines can be separated and assayed accurately. There is no interference from the two commonly used decongestants, phenylephrine hydrochloride and phenylpropanolamine hydrochloride, from the expectorant potassium guaiacolsulfonate, from the preservative propyl paraben or from two other antihistamines, diphenhydramine hydrochloride and bromdiphenhydramine hydrochloride. Guaifenesin, methylparaben and sodium benzoate interfere only with pheniramine maleate. Methapyrilene hydrochloride interferes only with pyrilamine maleate.

[Intravenous chemotherapy and galvanization of damaged lung regions in patients with newly diagnosed pulmonary tuberculosis].

The results of a combined treatment of 179 patients with newly diagnosed destructive pulmonary tuberculosis are presented. Patients of the main group (89 subjects) were given intermittent intravenous chemotherapy with simultaneous galvanization of the affected pulmonary zone, while patients of the control group (90 subjects) received the same treatment but without galvanization. In pulmonary tuberculosis patients who had undergone intracutaneous electrophoresis, the body temperature normalized significantly more rapidly (within 2.2 +/- 0.2 weeks), weakness and weakness disappeared (within 1.2 +/- 0.2 months) and cough ceased (1.6 +/- 0.2 months); sputum expectoration disappeared or substantially reduced (within 1.5 +/- 0.1 months); leukocytosis came to an end (within 1.5 +/- 0.1 months). The basic parameters of a spirogram improved in a shorter period and to a greater degree. Bacillary excretion ceased more rapidly (within 2.2 +/- 0.4 months), so did the infiltrative phenomena resolute (within 3.0 +/- 0.1 months) and pulmonary destructive changes disappear (within 3.3 +/- 0.2 months).

Effect of Paxirasol aerosol on the clinical condition and objective functional respiratory parameters of obstructive respiratory tract disease patients.

Paxirasol aerosol applied in daily 3 x 5-puff doses was well tolerated by the examined patients. In the course of the 21-day therapy the laboratory findings did not change-haemopoetic or liver injury was not observed. On two-thirds of the patients the amount of sputum decreased in response to Paxirasol aerosol but the symptoms accompanying expectoration did not change notably. Among the objective respiratory parameters FEV1 values increased and Rtot values decreased. These changes may be observed during the first third of therapy. There was a continuous and linear increase in FEV values characteristic of respiratory pressure conditions. This phenomenon is not characteristic alone of the changes of values of obstructive diseases patients. The authors recommend the use of Paxirasol aerosol for the treatment of respiratory tract disease patients.

[Evaluation of 5 selection criteria for sputum as a valid sample for culture].

We compare five selection criteria to discriminate between good quality expectorated sputum and specimens contaminated with upper respiratory tract secretions. We employed two microscopic methods (Bartlett, Heineman) which rely upon the number of squamo-epithelial cells and polymorphonuclear leucocytes; and a macroscopic method (the Medical Research Council) which evaluates the gross appearance of sputum, with three different criteria for admission established by our own. Three hundred and thirty two sputum were included. We evaluated sensitivity, specificity and predictive values as compared with isolation or not of a potential pathogen. We obtained 72% and 45% sensitivity, and 81% and 86% specificity for Bartlett and Heineman methods respectively. Most restrictive criterion for macroscopic method provided 57% sensitivity and 82% specificity; least restrictive showed 97% sensitivity and 17% specificity. Macroscopic method is a quick, valuable screening for discarding sputum contaminated with upper respiratory tract secretions.
