[Peribuccal esthetic surgery].

This paper concerns the procedures of remodelling the peribuccal area for aesthetic purposes; these techniques may be planned in isolation or as a complement to a face-lift. In our hands these interventions are helpful firstly to correct and excessively deep nasolabial fold and sublabial crease using dermis or fascia grafts. Secondly, are designed to correct the contour of the upper lip either by smoothening the fine vertical folds, excising a fine strip of skin along the cupid's bow associated with dermabrasion or by diminishing the labial height resecting a buffalo-horn shaped cutaneous strip at the nasal vestibular sill and at the base of the columella. The philtrum is sculptured by dissecting the philtral columns. The medial philtral depression is maintained using translabial mattress sutures. Furthermore the upper and lower lips themselves are able to be augmented by means of dermal or fascia grafts or on the contrary thinned by resecting an appropriate horizontal strip of vermilion or mucous membrane, without any lesion of the orbicularis muscle. To eliminate the sadness of sagging buccal angles, we model the subcutaneous tissue of the labial commissure.

[The contribution of genioplasty in esthetic surgery of the face].

The chin position in overall facial aesthetics is well known and it has an important influence on personality and relations with others. For most plastic surgeons, genioplasty is limited to chin advancement with alloplastic implants. Sliding horizontal osteotomy of the mandibular symphysis is not well known: the technique is simple, fast and safe. Indications are multiple (augmentation, reduction, straightening, lengthening). Improvement is both aesthetic and functional (labial gap). In deficiencies of the submental region, genioplasty may be compared to muscular transposition. Several cases of aesthetic and orthognathic surgery are presented to emphasize the value of the procedure.

Facial profile-esthetics and acceptance.

The human face influences and moulds one's personality and behaviour. The facial profile has become an important field of investigation by sculptors, artists, physiotherapists, orthodontists and maxillo-facial surgeons. The present study was conducted to understand the facial profile acceptance of 55 girls from silhouettee photographs judged by 3 orthodontists, 3 artists and 3 common men. To find out who appreciates the profile change the most, the chi-square test was applied to distribution of 3 professionally different groups for various occlusal categories. It was found that common bases for judging the profile do exist amongst the professionally different groups, however the public is more astute analyst of the face. Two profiles which got all acceptable votes were associated with malocclusion. Attempts were made to find out which areas on the facial outline alter the most between the acceptable and non-acceptable groups. It was concluded that the lip outline and the chin are chiefly responsible for profile acceptance while the nose alone, of its own, does not play important role. The only way by which the nose affects the facial profile is its relative position as compared to the position of the chin. Thus if the lip posture and the chin position can be corrected, a poor facial profile may be converted into a good and pleasing one.

[Thermal sulfur compounds and esthetic dermatology].

Sulfur compounds are involved in a variety of ways in cosmetics and topical surface treatments. The criterium to be kept in mind is the natural skin-friendliness of sulfur (Comel) and the tangible possibility to influence the morphology and function of the skin by sulfur compounds. Thermal sulfur resources--above all fanghi and baregines--appear to have the appropriate properties for use in aesthetic dermatology. Further basic and clinical studies are needed in order to find scientific support for the indications and uses that are at present contemplated with good reason.
