[Surgical procedure in non-Hodgkin's lymphomas of the digestive system--a status quo analysis].

A total of 329 patients were registered in the Münster multicenter study, "Gastrointestinal Lymphomas", including 118 different centers since October 1992. Primary surgical intervention was performed in 135 cases. In contradiction to the prospective results of other oncological centers, the preoperative diagnosis of non-Hodgkin lymphoma was only known in 44.4% of cases. R0 resection was achieved in only 62 of the 135 patients (46%). This study reveals the realistic situation of operative treatment of non-Hodgkin lymphomas in everyday practice.

The selective recognition of antibody IgY for digestive system cancers.

Biological methods for cancer therapies are very important. A small and efficient target carrier is the key component for anti-cancer drugs. In our laboratory, the antibody IgY was extracted from egg yolk of a SPF hen. The SPF hen was immunized with an antigene of P110 protein which was purified from human stomach cancer MGC-803 cells. Results indicated that the antibody IgY can specifically recognize gastrointestinal system cancers. It may become an important carrier for antitumorigenic drugs.

[Possibilities of drug therapy of acute hemorrhage of the upper digestive system].

Early detection of bleeding site by immediate endoscopy is the key of effective treatment in massive upper gastrointestinal (GI) bleeding. Upper GI endoscopy gives useful information about the risk, re-bleeding and mortality. Algorithm of treatment is also based on findings upon early endoscopy. Meta-analysis of prospective, randomized, multicenter clinical trials assessing H2-receptor antagonists and proton pump-inhibitors in the treatment of peptic ulcers suggest that these drugs cannot be justified for stopping bleeding or prevent re-bleeding. Other drugs, such as somatostatin, might be effective, but further studies are needed to prove their effectiveness. Both vasopressin and somatostatin are also successfully employed the treatment of bleeding related to portal hypertension, and a recent meta-analysis found significant benefit for beta-blockade in the prevention of recurrent bleeding. Although beta-blocker therapy does not improve survival, it reduces re-bleeding rate, therefore, it can be used as prophylactic therapy for esophageal varices. As for the treatment of erosions, none of the drugs currently employed are effective in reducing or preventing clinically significant bleeding.

[Joint manifestations of carcinoid tumors of the digestive system].

The joint manifestations of carcinoid tumours become integrated into the carcinoid syndrome, the totality of the symptoms occurring at a distance from the tumour and linked to the metabolites secreted by the tumour. These arthropathies have been the subject of only a very limited number of publications. They consist of arthralgia, sometimes with swelling, predominantly in the extremities of the upper limbs, and linked to activity. The radiological picture includes a "band" of demineralization, and less frequently destructive joint lesions. The sedimentation rate is always increased. These arthropathies can be related to their origin only by knowledge of, and demonstration of, the other elements of the carcinoid syndrome and of the tumoro-metastic syndrome. Laboratory diagnosis rests on determination of the 5-hydroxyl derivatives of tryptophane.
