The posttranslational concept of protein folding: how valid is it?

Two concepts of protein folding are known. One of them, the cotranslational concept, states that a protein folds during the synthesis of the polypeptide chain on the ribosome. According to the other, the posttranslational concept, the protein starts to fold just after the synthesis of its polypeptide chain. This article attempts to show that the posttranslational concept is hardly suited to solve the problem of protein folding. In our opinion, polypeptide chains cannot be represented as random coils. They are stiff chain-like macromolecules rather than flexible ones: the single bond rotational barriers of a polypeptide substantially exceed the accepted standard values; even in strong denaturing conditions, a protein possesses a considerable amount of residual folded structures. We believe that the popular "hierarchical" models for the protein folding mechanism are not realistic because the formation of secondary and tertiary structures of proteins occurs simultaneously and cooperatively. The time for the elongation of a polypeptide chain by one amino acid residue during biosynthesis exceeds considerably the time of the formation of alpha-helices and beta-sheets in proteins as well as the time supposed for the spatial structure formation of a native protein during renaturation. Thus, we believe that the mechanism of protein folding in vivo cannot be clarified by denaturation-renaturation experiments. In our opinion, the phenomenon of protein renaturation is no more than the restoration of native protein conformation (which initially forms cotranslationally) disrupted during denaturation, and thus denaturation-renaturation experiments cannot serve as a model to clarify the mechanism of protein folding.

[Is there an effective neuroprotective treatment of Parkinson's disease? (therapeutic possibilities against the background of etiopathogenetic concepts].

The author presents current views on neuroprotective strategies in therapy of idiopathic Parkinson's disease. They are the result of different aetiopathogenetic concepts of Parkinson's disease. The concepts of loss of nigral cells as a result of aging, apoptosis or genetic defect (alpha-synucleine) are not sufficiently proven and their role is only hypothetic. Therapeutic use of nerve growth factors seems to be a promising novel strategy but there are technical problems how to deliver them to the brain. One of the most and widely acceptable concept is the theory of the role of oxidative stress in pathogenesis of Parkinson's disease. It was a scientific basis for clinical trials with selegiline (DATATOP, SINDEPAR, PDRG) which results were unfortunately disappointing, mostly because of symptomatic effect of selegiline. Another interesting concept seems to be the excitotoxicity of amino acids (like glutamate) and amantadine is the well known drug with recently discovered antagonism to NMDA receptors. In only one retrospective clinical trial in humans its neuroprotective effect was proven, but we need now well prepared prospective trials with this drug to confirm this result. So far, despite the hopes concerning selegiline no one effective neuroprotective agent is available in treatment of Parkinson's disease.

Geographic variations in conceptions to women aged under 18 in Great Britain during the 1990s.

This article examines geographic variations in conceptions to women aged under 18. It presents data on conception rates and the percentage leading to abortion for the three countries of Great Britain, the Government Office Regions of England and local authorities within Great Britain. It provides an overview of variations between areas at each of the three geographic levels and examines whether this variation is associated with the social and demographic characteristics of local authority areas. This article is the first occasion on which conception rates for Scotland have been published on a comparable basis to England and Wales and also the first use of the revised ONS classification to examine conceptions to women aged under 18.

Complementarity is a useful concept for consciousness studies. A Reminder.

We describe the concept of complementarity and argue that it is a useful concept for consciousness and other studies at the interface of conscious phenomena and physical reality. After outlining the history and source of the concept within psychology, we describe its place as a working hypothesis for the mind-brain problem. We then point out that generalized quantum non-locality could be of importance even for macroscopic objects. If this is so, complementarity would be the key-concept to understand EPR-like correlated systems.

[Blood circulation, or the movement in the concept of motion].

The contemporary medical practice is enrolled in the rigid principles of a technological order. In order that the development and organization of the technological order would take place, and also material and objetively made concrete, it was essential that their products represented a materialization of a certain concept of human body and of health. This paper intends to be an introductory study of certain conditions of possibility for the construction of this technological order in the area of health. Although this thought includes, necessarily, a historical dimension, we do not intend here a factual reconstitution that considers dates and personalities. Its immediate goal will be to focus a moment of rupture, in which the introduction of the new concept of motion generated a new conception of human physiology. So, more important than one specific theory--of the blood circulation--or its effective results, is to understand the process of arising and/or changing of the concepts that allowed it to exist originally.

[Medical ethics in the clinical context -- a new concept by the physician Richard C. Cabot].

Richard C. Cabot was a professor of medicine and philosophy at Harvard University. He developed a very practical system of medical ethics structured by the hospital, its patients and care givers. Clearly he differentiated between a concept from an inside perspective and a concept born from an outside perspective. Thus, he opposed to the orientation to ethical codes which the American Medical Association established since 1847 (the outside perspective). The code-tradition was oriented by principles without professional context, while Cabot focused his concept of ethics rather on duty, which can not be reflected without its context (the inside perspective). This paper displays the essentials of his concept of ethics: 1) communication as the substantial core of medicine, 2) the basis in cases, 3) the multi/-interdisciplinarity, 4) the focus on the structure of therapy, i.e. care giving, 5) the respect of the patient, and 6) the obligation by truth.

[Diagnosing complicated congenital heart disease with two-dimensional echocardiography and three-segment concept].

OBJECTIVE: To apply three-segment heart anatomy concept to two-dimensional echocardiography (2DE) in the diagnosis of complicated congenital heart disease (CHD). METHODS: 920 cases of CHD were diagnosed with 2DE and three-segment heart anatomy concept. 822 out of the 920 cases underwent angiocardiography, and 584 cases were confirmed by operation. RESULTS: According to three-segment heart anatomy concept of atrial position, 855 cases were situs solitus (including 89 cases of left superior vena cava and 31 cases of complete anomalous pulmonary venous drainage), 53 cases were situs inversus, and 12 cases were situs ambigus. As to atrial-ventricular valve, 747 cases were both opening, 112 cases had one-side atresia, and 61 cases had one together valve; As to ventricle position, 606 cases were dextro-loop, 111 cases had levo-loop, and 203 cases had single ventricle (including 24 cases), As to cor biloculare of great artery relation, 304 cases had normal relation of great arteries, including 35 cases of pulmonary atresia, 219 cases had right-transposition of great arteries, 240 cases had left-transposition of great arteries, 32 cases were mirror to normal, and 25 cases had persistent truncus arteriosus. 823 cases were diagnosed with 2DE, with a total accurate rate of 89.5%. CONCLUSION: Applying three-segment heart anatomy concept to 2DE to diagnose complicated CHD and analyzing the heart segment by segment will make the diagnosis simple and systematic, providing a concise basis for heart surgeries.

[Integrating non-university institutions into clinical research. The example of implementing the concept of quality of life into regional management of cancer patients].

Implementation is the stepwise introduction of a concept with the goal to change the behaviour of health care providers. The concept of quality of life (QL) was implemented in order to improve the regional care for cancer patients. The following steps were taken: creating a quality circle, teaching the concept QL, listing QL-enhancing therapy options, graphically presenting individual patients in the form of QL-profiles. In the course of an implementation study QL-profiles of individual patients were sent to their respective practitioners. Doctors judged these profiles as comprehensible (100%) and informative (55%). QL-profiles influenced doctor-patient communication (42%), but had no impact on therapeutic decisions (0%). Acceptance of QL-profiles was higher in doctors treating breast cancer patients (67% response rate) than in doctors treating rectal cancer patients (25%). Reasons for this difference as well as driving and restraining forces for implementing the QL-concept were analysed through a barrier analysis.

Integration of nursing assessment concepts into the medical entities dictionary using the LOINC semantic structure as a terminology model.

Recent investigations have tested the applicability of various terminology models for the representing nursing concepts including those related to nursing diagnoses, nursing interventions, and standardized nursing assessments as a prerequisite for building a reference terminology that supports the nursing domain. We used the semantic structure of Clinical LOINC (Logical Observations, Identifiers, Names, and Codes) as a reference terminology model to support the integration of standardized assessment terms from two nursing terminologies into the Medical Entities Dictionary (MED), the concept-oriented, metadata dictionary at New York Presbyterian Hospital. Although the LOINC semantic structure was used previously to represent laboratory terms in the MED, selected hierarchies and semantic slots required revisions in order to incorporate the nursing assessment concepts. This project was an initial step in integrating nursing assessment concepts into the MED in a manner consistent with evolving standards for reference terminology models. Moreover, the revisions provide the foundation for adding other types of standardized assessments to the MED.

Old species and new concepts in the taxonomy of Pomacea (Gastropoda: Ampullariidae).

The taxonomic history of the South American genus Pomacea Perry, 1810, and some shifts of systematic concepts during recent decades are briefly reviewed. Too many pre-evolutionist, shell-defined species created a gibberish, the only acceptable solution of which being perhaps a conventional, somewhat authoritarian decision based on expertise. The addition of other sources of morphological, biochemical, ecological or genetic information should not solve the problem if it is not accompanied by a sound reappraisal of the species concepts. Since the assumptions of each concept differ, any correspondence between them is irrelevant, and may drive to incompatible results. The shell variability of Pomacea canaliculata was acknowledged for most authors throughout more than a century. A recent insight into its life-history traits demonstrated they are as variable as the morphology. These findings stress the need of determining the ecological identity of any pest apple-snail population at a local scale, because its invading ability may be not exactly correlated to its taxonomical identity. Probably, all the canaliculata-like apple snails constitute a single, very variable "species" in most senses, even though different subsets may be recognized under other incommensurable concepts.

Why is the concept of spondyloarthropathies important?

The concept of spondyloarthropathy was recognized first by clinicians based on the aggregation of several diseases occurring either sequentially in the same patient or simultaneously in a family. This concept was thereafter confirmed by the higher prevalence of the HLA-B27 antigen, not only in the group of patients suffering from an axial involvement of ankylosing spondylitis but also in other diseases belonging to the concept of spondyloarthropathy, i.e. psoriatic arthritis, reactive arthritis, inflammatory-bowel-disease-related arthritis and/or other clinical manifestations such as acute anterior uveitis. Recognition of the concept of the spondyloarthropathy is of great importance not only for research purposes but also in daily practice because such recognition has at least a threefold effect: (a) it permits earlier diagnosis, (b) it facilitates patients' education and monitoring, and (c) it has prognostic implications

Darwinian psychiatry and the concept of mental disorder.

In this paper, we discuss the concept of mental disorder from the perspective of Darwinian psychiatry. Using this perspective does not resolve all of the quandaries which philosophers of medicine face when trying to provide a general definition of disease. However, it does take an important step toward clarifying why current methods of psychiatric diagnosis are criticizable and how clinicians can improve the identification of true mental disorders. According to Darwinian psychiatry, the validity of the conventional criteria of psychiatric morbidity is dependent on their association with functional impairment. Suffering, statistical deviance, and physical lesion are frequent correlates of mental disorders but, in absence of dysfunctional consequences, none of these criteria is sufficient for considering a psychological or behavioral condition as a psychiatric disorder. The Darwinian concept of mental disorder builds from two basic ideas: (1) the capacity to achieve biological goals is the best single attribute that characterizes mental health; and (2), the assessment of functional capacities cannot be properly made without consideration of the environment in which the individual lives. These two ideas reflect a concept of mental disorder that is both functional and ecological. A correct application of evolutionary knowledge should not necessarily lead to the conclusion that therapeutic intervention should be limited to conditions that jeopardize biological adaptation. Because one of the basic aims of medicine is to alleviate human suffering, an understanding of the evolutionary foundations of the concept of mental disorder should translate into more effective ways for promoting individual and social well-being, not into the search for natural laws determining what is therapeutically right or wrong.

Novel monitoring concepts to acquire new water quality knowledge.

Two novel water quality monitoring concepts were developed: the HPLC-fingerprint for the monitoring of yet unidentified pollutants and the HPLC-Toxprint for the recognition of (unknown) toxic or genotoxic compounds. The paper describes applications of both concepts. The HPLC-fingerprint is used for the evaluation of the overall water quality in addition to the monitoring of individual pollutants. Based on their occurrence (frequency, concentration, location) a listing of unknown priority pollutants is set up. Participating waterworks monitor for these compounds using a dedicated HPLC-DAD library that contains the required compound data (UV-absorbance spectrum, retention time index). In five years of experience with this concept, the HPLC-fingerprint was also found very suitable for the retrieval of new priority pollutants in existing HPLC-fingerprint databases, providing historic data on these new compounds. The HPLC-fingerprint concept was also used as an Early Warning System for accidental spills or sabotage. The HPLC-Toxprint was successfully applied in identifying genotoxicity (in the umu-test) in various waste water samples. By the application of LC-MS/MS genotoxicity could be assigned to acridine-derivatives in one of these wastewaters. To enable the evaluation of drinking water resources, the sensitivity of the HPLC-Toxprint was improved, now allowing the detection of pollutants with a 10% genotoxic potential as compared to 2-aminoanthracene (the positive control compound) at concentrations as low as 0.1 microg/l.

[Tuberculosis in the elderly: concept analysis].

The goal of this study is to define the concept of tuberculosis in the elderly expressed in the literature covering the 80's and 90's. We have used concept analysis with emphasis on "essential attributes", "preceding events, and "consequential events", which build up the core sense of the concept to be defined. The elderly tuberculosis is expressed as the recrudescence of infection inactivated for a long time. The elderly is predispose to endogenous and exogenous reinfection. Among the most important "preceding" events are the institutional housing as a contact disease source. The "consequential" events are related to the need of a differentiate treatment and diagnostic in elderly due clinical conditions. We arrive at the conclusion that new studies about the concept analysis need to be developed in order to contribute to the knowledge about the diagnosis, treatment and care to elderly patients.

[Clinical results with the "open lung concept"].

Elements of the "open lung concept" are being increasingly included in clinical ventilatory strategies. Despite encouraging experimental investigations to date, relatively few studies exist that examine the clinical application of the complete concept. The aim of this study was to prove that with effective recruitment maneuvers and titrated PEEP levels this concept is applicable in clinical settings. We sought to determine if it was possible to achieve a significant improvement in oxygenation and also to examine what side-effects resulted. Twenty consecutive patients who had had an acute lung injury (ALI) for less than 72 hours, with an oxygenation index (P/F-Ratio = quotient from arterial partial pressure of oxygen [PaO2] and the inspiratory fraction of oxygen [FiO2]) of less than 200 torr, and with a PEEP > or = 10 cmH2O were treated using a recruitment manoeuvre (RM). A PEEP was titrated to keep the lung open, and the patients were kept under pressure-controlled ventilation. The P/F-Ratio increased while using a recruitment pressure of 66 +/- 13 cmH2O from 137 +/- 41 to 381 +/- 150 torr (p < 0.001). The titrated PEEP which kept the lung open after recruitment was 17 +/- 3 cmH2O. One patient developed a pneumothorax. The dose of norepinephrine was increased in ten patients from 0.24 +/- 0.12 to 0.31 +/- 0.1 microgram/kg/min. Due to elevated liver enzymes within the first 48 hours, titrated PEEP had to be decreased in three patients. The clinical application of the "open lung concept" demonstrated a quick and effective improvement in oxygenation in many patients. Side-effects in some patients limited the use of high PEEP levels.

[Minimal invasive dentistry. A revolutionary concept?].

This article describes the mutual relations between three important dental concepts. 'Minimal invasive dentistry', 'adhesive dentistry' and the 'dynamic treatment concept' are concepts which have changed restorative dentistry substantially during the last decade. The ultimate goal of restorative dental care, which is the maintenance of a healthy and functional dentition for life, is unchanged and as applicable as ever. To achieve maximal results from applying minimal invasive dentistry, a clear understanding of adhesive dentistry as well as the implications and goals of the dynamic treatment concept is needed. Dentists must realize that as invasiveness of interventions is decreasing, the need for monitoring and aftercare of restorative work is increasing. It is important to involve the patient in this process by providing sufficient information and achieving informed consent.

Unintended conception and unwanted fertility in Gondar, Ethiopia.

This is a report on unintended conception and unwanted fertility in Gondar, Northwestern Ethiopia. The purpose of the study was to explore the extent of the problem and to inquire whether there is a need for fertility regulation. To this end, relatively numerous questions on reproductive history of women aged 15-49 have been raised. Results showed that the proportion of abortion to delivery was 0.3984 to 0.6016 and the ratio of unintended conception to intended conception was 0.4043 to 0.5957. The proportion of unwanted fertility to wanted was 0.4099 to 0.5901. Mean desired interpregnancy interval was substantially higher than actual interpregnancy interval. The relatively high rate of abortion, high proportion of unintended conception and unwanted fertility combined with the wide difference between desired interpregnancy interval and current interpregnancy interval all point to the need for a policy that increases the capacity to regulate fertility.

Visual mapping for medical concepts.

Concept relationships are traditionally defined in human-generated vocabulary lists such as the Medical Subject Headings (MeSH). This poster describes a prototype system that automatically generates concept relationships from the medical literature. The system is directly connected to the PUBMED search engine. For any given medical concept, the system will generate two styles of visual maps from MEDLINE in real time. Users can use the maps to explore concept relationships or construct better search queries interactively.
