The prevalence of green muscle disease in turkeys from the Warmia and Mazury province of Poland.

The green muscle disease (GMD), described for the first time in 1968, is a cause of significant losses in the production of turkey meat. Pathological changes of different intensity are found in smaller pectoral muscles on one or both sides of the sternal crest. GMD has been more frequently observed in Poland since 1991 in breeding turkeys of heavy and medium-heavy types assigned to slaughter after completing the reproductive period. The present study deals with the occurrence of this disease in the Warmia and Mazury District of Poland. Characteristic green foci located in small pectoral muscles were found in 4090 out of 26,169 slaughtered turkeys, i.e. in 15.63% of the carcasses investigated. It was established that 2430.5 kg of meat were confiscated what gave 0.59 kg of confiscated meat per head on the average.

Histopathological changes in green muscle disease of turkeys.

In this study, results of histopathological investigations of meat samples collected from turkeys with macroscopically recognized green muscle disease were compared with those obtained from turkeys without macroscopic signs of this syndrome. The histopathological lesions were assessed based upon the self-designed 7-grade scale of the intensity of changes. The greatest intensity of the histopathological alterations were found in the smaller pectoral muscle collected from turkeys in which macroscopical signs of the green muscle disease were observed. Similar changes were also found in the greater pectoral muscle, however, they were less intense than those determined in the smaller pectoral muscle.

Hepatitis A seroprevalence in a random sample of the Turkish population by simultaneous EPI cluster and comparison with surveys in Turkey.

This study was conducted to determine the hepatitis A virus (HAV) seroprevalence in nine provinces representative of Turkey as a whole. These provinces are representative of the country's geographical location, and demographic, economic and social characteristics. In each province, sample sizes were determined using published data on HAV seroprevalence, and sample sizes for each province and for the cluster were calculated for each group of subjects under the age of 30 for seroprevalence estimates within a 95% confidence interval. The samples were selected by a cluster method, and the planned recruitment was a total of 4,800 subjects, including 600 subjects each from five large provinces (Istanbul, Ankara, Izmir, Adana, Diyarbakir) and 450 subjects from each of the remaining four provinces (Samsun, Erzurum, Trabzon, Edirne). These numbers were distributed in accordance with the percentages for age groups in five-year increments starting from age five for the population under the age of 30 living in the rural and urban areas in each province. This study of 4,462 subjects under the age of 30 in nine provinces of Turkey identified an overall HAV seroprevalence rate of 71.3%. The distribution of HAV seroprevalences by age showed a steady increase from one year of age from 42.7% to 91.1% at 25-29 years of age. HAV seroprevalence was slightly higher in female subjects (73%) than in male subjects (69.3%). By educational status, seroprevalences were comparable except in young children under age six. Seroprevalence was notably higher in large families with six and more members (80.1%) than in small families with five or fewer members (66.7%). According to our study results, 50% of Turkish children are seropositive for HAV by the age of 10 years. We believe the date support the need for a routine primary immunization policy in Turkey and the development of effective prophylactic programs after possible exposure. Consequently, an immunization policy can be developed for each region according to its epidemiological conditions.

In vitro DNA synthesis in lymphocytes from turkeys vaccinated with LaSota, TC, and inactivated Newcastle disease vaccines.

In vitro cultures of peripheral blood lymphocytes from turkeys vaccinated and revaccinated with Newcastle disease (ND) vaccines were stimulated to transformation when exposed to the homologous and heterologous strains of ND virus. The mitogenesis was measured by the uptake of 3H-thymidine into newly synthesized DNA. There was considerable difference in DNA synthesis by lymphocytes drawn 0, 2, 5, and 10 days after vaccination and revaccination with the three vaccines. Stimulation of DNA synthesis, evident as early as the 2nd day, was highest in lymphocytes from turkeys vaccinated or revaccinated with TCND intramuscularly. Stimulation was least in lymphocytes from turkeys vaccinated and revaccinated with LaSota vaccine by aersol. Stimulation was intermediate from an inactivated vaccine given subcutaneously. DNA synthesis was greater with the homologous than with the heterologous strains of NDV. Synthesis was even greater when the same strain was used as a viral suspension in allantoic or cell-culture fluid than the commercial vaccine. The bovine paramyxovirus (PI3) resulted in a minimum DNA synthesis or completely inhibited it. A many-fold (order of magnitude) stimulatory effect was observed when PHA was used as an antigen. The stimulation of DNA synthesis did not parallel the HI antibody response.

Increased cesarean section rates in Turkey.

The aim of this study was to examine the trend in Cesarean section deliveries and the factors associated with Cesarean sections in Turkey. Data come from the ever-married women questionnaire of the 1998 Turkish Demographic and Health Survey (TDHS-98). During the decade preceding the TDHS-98, the proportion of deliveries by Cesarean section increased from 5.7% to 20.8%. When only hospital births were considered, the percentage of Cesarean deliveries for the year 1998 was found to be 26.1%. The estimated rate for the year 2001 was around 30% (i.e. double the maximum rate of Cesarean sections defined by the World Health Organization). Logistic regression analysis performed for the births occurring in the most recent period of 1993-98 revealed that the highest Cesarean section rate was strongly associated with maternal education, maternal age, place of delivery, number with prenatal care and household welfare. These findings imply that women with higher socioeconomic status are more likely to accept Cesarean section than women with lower socioeconomic status. The trend of increasing Cesarean section rates is a problem in itself, but more importantly it may indicate that Turkey is headed toward a more costly medical delivery system. For all of these reasons, the reduction of Cesarean section rates should be a priority for any reproductive health program in Turkey in order to improve the quality of prenatal care and to reduce the number of maternal deaths and morbidity.

Prevalence of asthma, asthma-like and allergic symptoms in the urban and rural adult population in Eastern Turkey.

There is no study assessing the prevalence rates for asthma, asthma-like symptoms, and allergic symptoms in Eastern Turkey. In this study, we investigated these rates in the urban and rural adult population of Elazig Region in Eastern Turkey. A questionnaire was prepared based on the standard inquiry form of the European Community Respiratory Health Survey. The survey included 3,591 consented adults who were randomly selected. Each questionnaire was interviewed face-to-face by a physician. It was found that the cumulative prevalence rates of asthma and asthma-like symptoms of the rural and the urban respondent groups were 29.6% and 25.8%, respectively. The periodic prevalence rates of the same symptoms were 27.3% and 19.3%, respectively. The percentages of the patients living in rural and urban areas diagnosed as asthma by a physician were 5.5% and 3.1%, respectively. The proportions of patients with a history of at least one allergic disorder were 36.4% and 23.7% in the rural and urban groups, respectively. Asthma and asthma-like symptoms in patients with allergic rhinoconjunctivitis were significantly higher than those without (p < 0.01). The cumulative rates of a previous diagnosis of asthma among respondents with a history of asthma-like symptoms were 18.4% in the rural and 11.2% in the urban groups (p < 0.05). Among those who had had periodic asthma-like symptoms, the rural and urban rates of previous asthma diagnosis were 18.6% and 14.3%, respectively (p < 0.01). Unexpectedly, the prevalence rates for asthma, asthma-like and allergic symptoms in adults living in the rural areas of Elazig Region in Eastern Turkey were significantly higher than the rates in the urban population.

SEROLOGIC, PATHOLOGIC, AND SYMPTOMATIC ASPECTS OF MYCOPLASMOSIS OF TURKEYS.

Adult male turkeys were exposed to Mycoplasma gallisepticum isolate 1010 by either intratracheal inoculation, intrasinus inoculation, intranasal inoculation or by contact with inoculated turkeys. The symptomatic, serologic, and pathologic responses to the different types of exposure were compared. Tracheitis occurred only in birds exposed intratracheally or by contact, and sinusitis occurred only in birds exposed via the sinuses. Antibody titers, determined by hemagglutination-inhibition and tube agglutination tests, were initially higher in turkeys exposed by intratracheal and intrasinus inoculation than in those exposed by other means. There were no appreciable differences among the four groups in incidence or severity of air sac lesions. In addition, the effects of intrasinus exposure to isolate 1010 were compared with those produced by similar exposure to a different isolate, M. gallisepticum isolate 1150. Isolate 1010 caused a slightly higher incidence of sinusitis, but a much lower incidence of tracheitis than isolate 1150. Air sac lesions did not differ in incidence or severity. The differences observed indicate differences in tissue predilection of the isolates.

Filicide cases in Turkey, 1995-2000.

AIM: To determine socio-demographic features and criminal liability of individuals who committed filicide in Turkey. METHOD: The study involved 85 cases of filicide evaluated by the 4th Specialized Board of the Institute of Forensic Medicine in Istanbul in the 1995-2000 period. We assessed the characteristics of parents who committed filicide (age, sex, education level, employment status, and criminal liability) and children victims (age, sex, own or stepchild), as well as the causes of death. RESULTS: There were 85 parents who committed filicide (41 fathers and 44 mothers) and 96 children victims. The mean age of mothers who committed filicide (52% of filicides) was 26.5-/+7.7 years, and the mean age of fathers (48% of filicides) was 36.1-/+10.0 years (t=-5.00, p<0.001). Individuals diagnosed with psychiatric disturbances, such as schizophrenia (61%), major depression (22%), imbecility (10%), and mild mental retardation (7%), were not subject to criminal liability. Almost half of parents who committed filicide were unemployed and illiterate. CONCLUSION: Filicide in Turkey was equally committed by mothers and fathers. More than half of the parents were diagnosed with psychiatric disorders and came from disadvantageous socioeconomic environments, where unemployment and illiteracy rates are highly above the average of Turkey.

West Nile virus infection among turkey breeder farm workers--Wisconsin, 2002.

In 2002, Wisconsin public health officials were notified of two cases of febrile illness in workers at a commercial turkey breeder farm (farm A) in county A. The Wisconsin Division of Public Health (WDPH) initiated an investigation that found a high prevalence of West Nile virus (WNV) antibody among farm A workers and turkeys. An associated high incidence of febrile illness among farm A workers also was observed. This report summarizes the results of this investigation, which indicate possible nonmosquito transmission among birds and subsequent infection of humans at farm A. Because the mode of transmission in this outbreak is unknown, turkey handlers should take appropriate precautions, including use of DEET--containing mosquito repellents, protective clothing and gloves, respiratory protection, and proper hand hygiene. Suspected occupationally acquired WNV infections should be reported immediately to local and state health departments.

Echinococcosis multilocularis in south-eastern Anatolia, Turkey.

OBJECTIVE: To quantify the human prevalence, elucidate possible risk factors and present other epidemiological parameters of human alveolar echinococcosis in the south-eastern region of Turkey. DESIGN: A retrospective study. SETTINGS: South-eastern region (Anatolia) of Turkey by two medical centres: Dicle University Medical Faculty in Diyarbakir and Harran University Medical Faculty in Urfa. SUBJECTS: Forty seven cases of human alveolar echinococcosis were presented between the period of 1980 and 2000. METHODS: For the majority of our patients, personal anamnestic, clinical data and addresses were available from their files, and we visited their villages to investigate the risk factors for human alveolar echinococcosis. Epidemiological parameters were evaluated. RESULTS: The 47 cases consisted of 18 (38.3%) males and 29 (61.7%) females. The average age was 34.6 years. In all cases, lesions occurred in the liver. No village or district had more than one case. The majority of patients (73.8%) were farmers and 33 had a history of dog ownership. CONCLUSION: This region has a higher incidence than Turkey as a whole, and domestic dogs and free-ranging dogs seem to be the main source of infection.

Acute methanol poisonings reported to the Drug and Poison Information Center in Izmir, Turkey.

The demographics, sources and outcomes of methanol poisoning have not been described in Turkey. Our study identified the profile of acute methanol exposures reported to Drug and Poison Information Center (DPIC) in Izmir, Turkey, from 1993 to 2002. Data analysis included patient demographics, sources of methanol, reason for the exposure, clinical effects and outcomes of methanol poisoning. The DPIC recorded 30,485 calls concerning poisoning; 996 (3.3%) alcohol poisonings were recorded and 113 (11.3%) of them were methanol poisonings. There were 91 (80.5%) males and 22 (19.5%) females with a mean age of 34.7+/-1.3 y (range 19-65) and 4.8+/-0.9 y (range 1-18) in adults and children, respectively. The sources of methanol were eu de cologne (72.6%), spirits (10.6%) and antifreeze (2.7%). Accidental poisoning occurred in all children between 0 and 12 y old, abuse (55.7%) and intentional poisoning (27.3%) were predominant in adults. Clinical signs in all cases were central nervous system symptoms (45.1%), metabolic acidosis (23.0%), visual symptoms (21.2%) and gastrointestinal symptoms (10.6%). Sixteen patients (14.1%) died, 63 (55.8%) had complete recovery and 1 (0.9%) had irreversible visual problems. Most patients with methanol poisoning may die or present serious morbidity without appropriate treatment in a health care facility. Methanol for producing cheap "eu de colognes" in Turkey is the principal reason for severe poisoning and deaths. Public education about colognes and legislative control of cologne production are important in preventing methanol poisoning.

[Use of AFLP analysis for the molecular biological fine typing of Campylobacter jejuni strains from turkey flocks].

One of the most promising genotyping methods for microorganisms is the AFLP (amplified fragment length polymorphism)-analysis, originally developed to genotype plants. The AFLP method has a high discriminatory power and an excellent reproducibility. Thus it is suitable for solving taxonomic and epidemiological questions. AFLP analysis was used to reveal the genetic diversity of Campylobacter (C.) jejuni strains from turkeys. The aim was to detect and genotype C. jejuni strains on individual turkeys throughout the complete slaughter process. A broad spectrum of genotypes was detectable on the turkey skin at the beginning of the slaughter line. At the end of the slaughter process-after chilling, only a diminished spectrum of genotypes was detectable by AFLP-analysis, suggesting, that slaughter specific environmental stressors may exert a selective pressure on the strain diversity. Our results proved, that AFLP analysis is an excellent tool for subtyping large numbers of Campylobacter strains.

Effect of hypoxia and diet on spontaneous turkey cardiomyopathy (round-heart disease).

The purpose of this study was to examine the effect of hypoxia-induced hypoxemia and rate of growth on spontaneous dilatory cardiomyopathy in turkey poults. Turkey poults grown in a hypobaric chamber at an atmospheric pressure of 592 mmHg (calculated partial pressure of oxygen: 124 mmHg; calculated altitude and O2 equivalents: 2054 m and 16.3%) on a rapid-growth diet developed a mainly right ventricular dilatory cardiomyopathy typical of the acute form of spontaneous turkey cardiomyopathy (STC). Poults grown in a hypobaric chamber on a slow-growth diet had a much lower incidence of STC. Control poults at atmospheric pressure, altitude 295 m (calculated atmospheric pressure: 735 mmHg; partial pressure of oxygen: 154 mmHg, 20.26% O2), on a rapid-growth diet had an incidence of STC similar to that of the slow-growth hypobaric group, while those on a slow-growth diet had a lower incidence.

Turkey macrophage and heterophil bactericidal activity against Pasteurella multocida.

Bactericidal activity of turkey macrophages and heterophils was demonstrated in an in vitro colorimetric bactericidal assay. Two vaccine strains and one field isolate of Pasteurella multocida A:3,4 and a single isolate each of Escherichia coli and Staphylococcus aureus were compared for susceptibility to the bactericidal activity of turkey macrophages and heterophils. Only P. multocida A:3,4-strain M-9 (the least virulent strain) was susceptible to macrophage bactericidal activity in the absence of specific immune serum, whereas all three P. multocida A:3,4 organisms were killed when opsonized with specific immune serum. E. coli was susceptible to the bactericidal activity of macrophages, and S. aureus was resistant. All bacteria tested were highly sensitive to the bactericidal activity of intact turkey heterophils, regardless of the opsonin treatment. Electron microscopic findings suggested that heterophils may kill extracellular P. multocida. Only S. aureus and E. coli were killed by lysed heterophils.

Furazolidone-induced cardiomyopathy in turkeys. Association with a relative alpha1-antitrypsin deficiency.

A naturally occurring cardiomyopathy (round heart disease) which is potentiated by inbreeding and a cardiomyopathy produced by furazolidone, a nitrofuran derivative, were studied for an associated alpha1-antitrypsin deficiency in two flocks of turkeys (one inbred for round heart disease and a commercial flock). At 4 weeks of age, the furazolidone-fed birds of both flocks demonstrated a marked increase in mortality and cardiac dilatation associated with disordered hepatic metabolism when compared with controls. Although PAS-positive, diastase-resistant globules were observed in the livers of both strains of turkeys fed furazolidone, these globules were present in lysosomes and not in the rough endoplasmic reticulum as in alpha1-antitrypsin deficiency. The control inbred birds with round heart disease did not demonstrate histologic or biochemical evidence of an alpha1-antitrypsin deficiency. It is proposed that furazolidone in the turkey produces primary hepatic damage that is reflected in lowered total serum proteins, including trypsin inhibitory capacity, and that the alterations produced by furazolidone are superimposed on round heart disease in the inbred flock.

Assessment of goiter prevalence, iodine status and thyroid functions in school-age children of rural Yusufeli district in eastern Turkey.

According to previous studies, Turkey has generally been accepted as a moderate endemic iodine deficient country. However, it has recently been reported that there are regions in Turkey where iodine deficiency is more severe than previously known. The current study was aimed at ascertaining the goiter prevalence by thyroid volumes, iodine status and thyroid functions in school-age children living in an area which is suspected to have moderate or severe iodine deficiency. Overall goiter was found in 47.6% of children, in 22.8% of girls and in 24.8% of boys. Mean thyroid volumes did not differ significantly according to sex. Significant correlation was found between thyroid volume and body surface area and age. There was a negative correlation between the urinary iodine concentration and thyroid volume (r = 0.45, p < 0.01). Median urinary iodine concentrations in subjects with and without goiter were 20 microg/dl and 5.2 microg/dl, respectively. While median urinary iodine levels of the subjects with goiter were consistent with severe-moderate iodine deficiency, levels in subjects without goiter were comparable to moderate-mild iodine deficiency. None of the subjects had the signs or symptoms of hyper-or hypothyroidism. The differences in the mean values of thyroid hormones and TSH levels between subjects with or without goiter were not significant (p > 0.05). No correlation was found between urinary iodine concentrations and thyroid hormone levels. A weak correlation was found between urinary iodine concentration and TSH levels (r = 0.12, p = 0.05). Individuals with goiter were investigated etiologically: biochemical hypothyroidism was detected in 2%, compensated hypothyroidism in 12.6%, autoimmune thyroiditis in 2%, nodular goiter in 3% and isolated high TSH level with autoimmune thyroiditis in 0.08%. In conclusion, although a salt iodization program has been started in Turkey, our study indicates that some regions with severe iodine deficiency are still present. This research suggests that this program should be re-evaluated for remote areas with self-contained economic systems, and should be expanded and more effectively applied nation-wide.

[Histomorphometric study of the superficial pectoral muscle of the turkey. Relationship with the meat discoloration syndrome].

Scallop discoloration affects about 30% of turkey carcasses, causing important economic losses. Histological and histomorphometric analyses of the superficial pectoral muscle were performed in order to show distinctive aspects of discolored scallops and predictive criteria for the phenomenon. Pectoral muscles of live male turkeys were biopsied 8 d before slaughter. Twenty four h after slaughtering, the same animals were classified into 2 distinctive classes by reflectometry (pale and dark muscles) and muscle samples were collected in each group. Superficial pectoral muscle was totally composed of type IIB, fast switch and glycolytic fibers. Pale and dark muscles exhibited no significant differences 8 d before slaughtering, but glycogen level was higher in pale muscles 24 h after slaughtering. These results do not support the current hypothesis of accelerated glycolysis and low ultimate pH to explain the meat discoloration syndrome in turkeys.

[Tobacco, alcohol and substance use prevalence among elementary and secondary school students in nine cities of Turkey].

OBJECTIVE: Previous studies in Turkey regarding substance use have failed to take into consideration the elementary school students. The purpose of the present study is to determine prevalence of tobacco, alcohol, and substance use among elementary and secondary school students in Turkey. METHOD: The study was conducted in 9 big cities that were selected from different geographic regions of Turkey. According to the student population size of the cities, a sample of 1250 or 2500 students were selected from each city. The questionnaire was administered to 11.989 elementary school students, and 12.270 secondary school students. In elementary school students' questionnaire, for the substances other than tobacco, alcohol, inhalants and cannabis a general term "drug" was used. RESULTS: Overall, Izmir had the highest rate of substance use. The prevalence of tobacco use at least once in life time was found to be 16.1% in the elementary school. The lifetime prevalence was found to be 15.4% for alcohol use and 1.7% for inhalants and for general term "drug". The prevalence of tobacco use at least once in life time was 55.9%, for alcohol use it was 45.0%, for cannabis use 4.0%, for inhalant use 5.1%, and for heroin and ecstasy use 2.5% among secondary school students. A significant difference was found between males and females in terms of tobacco, alcohol and other drug use both in the elementary and the secondary school. The risk for substance use was found to be greater in private schools than public schools. CONCLUSION: Although the prevalence of substance use among elementary and secondary school students is low when compared to the findings from other countries. It was observed that especially the prevalence of tobacco use was quite high.
