Survey of thorotrast-associated liver cancers in Japan.

Data on 93 autopsy cases (group A) of thorotrast-associated liver cancers were obtained from the "Annual of Pathological Autopsy Cases in Japan" from 1958 to 1979, and data on 78 autopsy cases (group B) of thorotrast-associated liver cancers were obtained from the Japanese literature from 1953 to 1980. Cholangiocarcinoma (CLC) constituted 58% of group A and 55% of group B. The curve of the cumulative numbers of patients with CLC versus year in group A was almost linear, showing an increasing risk per surviving patients with advancing time. Angiosarcoma (AGS) occurred in 25% of group A and 24% of group B. The number of patients with AGS increased significantly after 1969 in both groups (P less than 0.05). In group B, age and years after thorotrast injection of patients with AGS were statistically higher than those of patients with CLC (age: P less than 0.05; years after thorotrast injection: P less than 0.0001). Hepatocellular carcinoma (HPC) accounted for 17 and 21% of groups A and B, respectively. When yearly distribution, age, and time after thorotrast injection of patients with HPC were correlated with those of patients with other liver cancers, the only statistically significant difference between patients with HPC and patients with CLC (P less than 0.02) was in the years after thorotrast administration. Since 1977 multiple primary liver cancers including AGS developed in thorotrast-administered patients in both groups.

Resection of thorotrast-induced cholangiocarcinoma.

We report a case of successful surgical treatment of cholangiocarcinoma caused by a long standing thorotrastosis. Thorotrastosis should be recognized as a preliminary stage of malignancy. If early diagnosis of the tumor can be made, treatment may be successful.

Adenoma, Bile Duct↗

[Proliferative lesions in thorotrast-deposited livers pertaining to the development of primary cancers].

We obtained human cadavers livers with thorotrast deposits and examined them light microscopically. The bile ducts of the large portal tract exhibited many foci of intraductal papillary hyperplasia including some severe atypical foci. The proliferation of bile ductules at the periportal zone was more marked in cases with cholangiocarcinoma than in those with other diseases. Micro-peliosis, in which the number of lining endothelial cells was significantly increased, was prominent in angiosarcoma cases. We suggest that advanced micro-peliosis represents a specific proliferative lesion pertaining to the development of angiosarcoma. There were many hyperplastic lesions of hepatocytes with structural deformity that often proceeded to vague nodular growth.

Endothelial cell origin of thorium oxide-induced angiosarcoma of liver.

A 72-year-old woman was found at autopsy to have hepatic angiosarcoma approximately 30 years after systemic thorium oxide (Thorotrast) injection. Intracytoplasmic localization of factor VIII-related antigen was demonstrated by an immunoperoxidase method, confirming the endothelial cell origin of the tumor.

[Thorotrast carcinoma of the lung--contribution to the genesis of carcinoma after application of thorotrast and of old bullet wound (author's transl)].

A bronchogenic carcinoma was found in the sphere of the scar in a fifty-years-old man who suffered in 1945, 27 years before his death, from a pulmonary bullet wound caused by war influence. The histological examination furthermore disclosed considerable autoradiographic secured deposits of thorotrast in the carcinoma and scar sphere. One may suppose that in connection with the treatment of the results of this injury a bronchography or a preparation of the fistula followed. All available cases of a beginning of carcinoma after a pulmonary bullet wound as well as all cases of carcinoma of the lungs after thorotrast incorporation found in literature were collected, critically sorted out and represented in schedules. A summary of questions of the causes and pathogenesis is given.

Computed tomography in the evaluation of cervical thorotrast granuloma.

Two cases of cervical thorotrast granulomas (thorotrastomas) were evaluated with computed tomography. Computed tomography provided delineation of the precise extent of disease and the relationships of the larynx and carotid vessels to the thorotrastoma. This was helpful in planning surgical management. Computed tomography is of use in determining the extent of thorotrastomas and in planning their treatment.
