OBJECTIVE: Our purpose was to investigate the impact of exercise and occupational activity on birth weight. STUDY DESIGN: This case-control study involved singleton infants at > or = 34 weeks' gestation without congenital anomalies. Case subjects had birth weights at <15th percentile for gestational age, and 2 control subjects were selected per case subject. Data were collected by self-completed questionnaire and analyzed by means of logistic regression. RESULTS: Relative to those who participated in structured exercise 3 or 4 times per week during the third trimester, the odds of lower birth weight were substantially increased for those who exercised > or = 5 times per week (adjusted odds ratio, 4.61; 95% confidence interval, 1.73-12.32) and modestly increased for those at the other extreme, who engaged in structured exercise < or = 2 times per week (adjusted odds ratio, 2.64; 95% confidence interval, 1.29-5.39). Other factors of importance to birth weight were maternal height, prepregnancy body mass, pregnancy weight gain, smoking in the third trimester, and nulliparity. CONCLUSIONS: Structured exercise frequency during late pregnancy is a determinant of birth weight.

Low back pain is a common complaint of athletes, and accurate diagnosis is essential, since many causes of back problems can lead to disability. If the obvious mechanical causes, such as muscle strain, "kissing spines," and leg-length inequality, have been ruled out, the more serious sources of low back pain, such as disk herniation, Scheuermann's disease (swimmer's back), compression fractures of the vertebral body, and stress fractures of the posterior elements, should be considered. Treatment goals for all these disorders include pain relief and prevention of further injury.

Two cover sprays of phosmet were applied to an orchard adjoining a camping area and a bird sanctuary with a resident goose population. Insecticide residues were monitored on orchard leaves, orchard ground cover, ground cover in the camp-site and along the adjacent lakeshore. Despite attempts to minimize drift, significant spray residues were found outside the target area. Residues on ground cover and leaves were reduced by sprinkler irrigation subsequent to spray application.
A general population sample of 5126 New Zealanders aged 15-45 was surveyed in late 1990 to determine their use of alcohol, tobacco, cannabis, and other illicit drugs. The results show that alcohol and tobacco are the most commonly used drugs. Cannabis had been tried by 43% of the sample, and this level of use appears similar to that in Australia and the United States. Hallucinogens were the next most common illicit drug and had been tried by 8% of the sample. Again the New Zealand rates appear similar to those for the United States or Australia. Much lower levels of use were reported for stimulants, opiates, tranquillizers and solvents than for other illicit drugs. The New Zealand prevalence rates for these substances are lower than those in the United States. The rates for these other classes of drugs are more similar to Australia, but may be slightly lower for stimulants, tranquillizers and solvents.

We report a case of implantation of a new design of stent which allows creation of a double-hemispheric lumen for the treatment of a bifurcational stenosis. The unfavourable outcome following the implantation of this stent is described.
BACKGROUND: Liver disease is a leading cause of death in human immunodeficiency virus (HIV)-infected women; however, risk factors for hepatitis b virus (hbv) infection in this population have not been well studied. METHODS: We describe the seroprevalence and predictors of HBV infection in a cross-sectional analysis of 2132 women with and at risk for HIV infection enrolled in the Women's Interagency HIV Study during the periods 1994-95 and 2001-02. Any test result positive for antibody to hepatitis B core antigen defined infection; those women with serological evidence of vaccine immunity were excluded from analysis. Women were stratified into those with a history of injection drug use (IDU), those with a history of noninjection drug use (non-IDU), and those with no history of illicit drug use. RESULTS: Of 1606 HIV-infected and 526 HIV-uninfected women, 7% and 12%, respectively, appeared to be vaccine immune. After exclusion of these women, 43% of 1500 HIV-infected and 22% of 461 HIV-uninfected women had HBV infection. HBV infection prevalence differed among the IDU, non-IDU, and no illicit drug use groups (76%, 30%, and 17%, respectively; P<.0001). HBV infection was strongly associated with herpes simplex virus 2 (HSV-2) seropositivity in the IDU group (odds ratio [OR], 2.9; 95% confidence interval [CI], 1.6-5.4) and with a history of syphilis in the non-IDU group (OR, 2.7; 95% CI, 1.4-5.0). DISCUSSION: We found a high prevalence of HBV infection in our cohort of women with and at risk for HIV infection. HSV-2 seropositivity and a history of syphilis appeared to be important correlates of HBV infection. Sexual transmission of HBV, particularly in those with a history of genital ulcer disease, should be a major focus of education in all high-risk groups.
Dietary habits, nutrition knowledge, and gastrointestinal complaints were evaluated in 21 female and 50 male triathletes; 30 completed hemoccult slides to determine the frequency of gastrointestinal bleeding. Triathletes trained 11 h/wk with weekly distances of 5.3, 116.5, and 40.9 km for swimming, biking, and running, respectively. Mean daily energy intake averaged 9058 and 11,591 kJ for women and men, respectively; 53.8% of the energy was from carbohydrates. Mean intakes of vitamins and most minerals exceeded the Recommended Dietary Allowances (RDAs), but many had intakes below RDAs for some nutrients; greater than 60% had low zinc and copper intakes. Because 39% took a daily multivitamin-mineral supplement, some had intakes 200-600% above the RDA. Although there were notable misconceptions about nutrition, nutrition knowledge was high. Upper-gastrointestinal complaints, reported by 50%, included bloating and abdominal gas; the incidence of positive hemoccult slides was 27%. The relation among performance, dietary patterns, nutrition knowledge, and gastrointestinal function remains to be established.

Lyme disease risk reduction through the control of the principal tick vector, Ixodes dammini Spielman, Clifford, Piesman & Corwin, has become a major issue facing public health agencies in many endemic states. Where large tracts of land are involved, established methods of I. dammini control are impractical. An aerial application of carbaryl directed against fall populations of I. dammini adults resulted in 93.8% control after 96 h. Control persisted through the following spring. The usefulness of aerial applications may be limited to areas where logistic constraints obviate the use of conventional ground applications.
