Cytokines and matrix metalloproteinases mRNA expression in archival human tissues from failed total hip arthroplasty using in situ hybridization and color video image analysis.

Reports on the histological and biochemical nature of periprosthetic fibrous/granulomatous tissue has, to date, been largely limited to frozen tissue sections. This study reports the cytokine and matrix metalloproteinase profiles found in periprosthetic interface tissues in THA which have failed due to aseptic loosening and in capsular tissues obtained at primary surgery. The study employs immunohistochemistry, in situ hybridization, and color video image analysis on formalin fixed and paraffin embedded sections.

Diagnosis of aortic pseudoaneurysm by echocardiography.

The diagnosis of pseudoaneurysm of the ascending aorta is of paramount importance because of its propensity to rupture. As the frequency of surgical procedures involving the aortic root and valve increases, an increase in the incidence of aortic pseudoaneurysm may be anticipated. We recently studied a patient who developed pseudoaneurysm of the ascending aorta following repair of a Type I aortic dissection, utilizing a composite graft. Two-dimensional echocardiography with color flow and pulsed Doppler imaging showed a large perigraft cavity communicating with the aorta. Echocardiography provides a safe noninvasive diagnostic tool for the evaluation of the aorta postoperatively and for screening for pseudoaneurysm formation in the follow-up period.

[Postoperative iatrogenic left coronaroventricular fistula. Demonstration by color two-dimensional Doppler].

The authors report the case of an asymptomatic 67 year old patient, in whom, 6 years after aortic valve replacement, Doppler color flow mapping showed the presence of a coronary artery--left ventricular fistula. The normality of preoperative coronary angiography suggested that this fistula was created during peroperative left ventricular purging: the implantation of a needle through the right ventricle and interventricular septum. A iatrogenic lesion of a septal branch probably caused the communication between the left anterior descending artery and the left ventricle. Postoperative normalisation of the left ventricular end diastolic dimension, the absence of dilatation of the left main coronary on 2D echocardiography, the narrowness of the Doppler color jet and the absence of a significant end diastolic Doppler signal in the aortic isthmus indicated a fistula of small size and simple Doppler echocardiographic follow-up was decided upon.

Evaluation of valvular regurgitation by Doppler echocardiography.

Although most clinical decisions about timing of intervention remain based on symptoms or evidence of progressive ventricular dysfunction in valvular regurgitation, several available Doppler techniques can give confirmation of the presence of significant insufficiency. For these reasons, the 2D echocardiographic and Doppler evaluation has become an integral part of the diagnostic and clinical management of these patients. These imaging techniques are still in their infancy, however, and the exact relationship between jet appearance and clinical and hemodynamic implications remains to be defined. Newer approaches to color flow display of Doppler velocity information, such as power mode, quantification of jet momentum, and kinetic energy mapping may provide a better understanding of the dynamic nature of valvular regurgitation. In the future, Doppler recordings may provide the basis for a more sophisticated understanding of the hydrodynamics of valve dysfunction.

Hydrothermal ceramic for porcelain-fused-to-metal crowns: an initial experience report from clinical practice.

The development of hydrothermal ceramics was an important requirement for fusing hydrolysis-resistant ceramics to precious metal alloys with low melting interval. Clinical findings since 1992 confirm the wide range of application of these metal-ceramic restorations in dentistry. Initial problems concerning temperature regulation of the ceramic ovens were rectified by checking and adjusting them for lower sintering temperatures. The golden yellow color is only one advantage of the ceramic-veneered alloy. Other advantages include its nonporous, smooth surface, translucence, and suitability for use in all types of dental restorations and reconstructions. Hydrothermal ceramic veneers thus can help to substantially reduce the number of different alloys used in a patient's mouth.

Leiomyosarcoma of the inferior vena cava. Resection and reconstruction of the renal vein using the gonadal vein.

We succeeded in surgically resecting a leiomyosarcoma of the inferior vena cava (IVC), which originated at the confluent portion of the right renal vein (RRV), together with the IVC and RRV, and also were able to preserve the right kidney by reconstructing the RRV with end-to-end anastomosis using the right gonadal vein. A good blood flow of the reconstructed RRV was thereafter confirmed by color Doppler ultrasonography, and the renal function was also satisfactory. This new procedure, a reconstruction of the RRV using the gonadal vein, has not been previously reported, but is considered to be an easy and effective method which enables the surgeon to preserve the normal right kidney.

Utility of Doppler echocardiography in the evaluation of aortic homograft valved conduit function in children: need for segmental conduit interrogation.

To assess the utility of Doppler echocardiography in evaluation of aortic homograft valved conduit function, 10 consecutive pediatric patients had Doppler examination of aortic homograft valved conduits associated with follow-up cardiac catheterization. General correlation was found between the Doppler-derived peak systolic pressure gradient across the aortic homograft valved conduit and cardiac catheterization (r = 0.65, P less than 0.05, SEE = 20.5). One patient with multilevel obstruction had significant underestimation of gradient by Doppler due to incomplete segmental interrogation of the aortic homograft valved conduit. For the other nine patients with complete segmental conduit interrogation, correlation between Doppler and cardiac catheterization was excellent (r = 0.93, P less than 0.01, SEE = 10). Conduit insufficiency in nine out of ten patients was graded qualitatively using Doppler color flow mapping with excellent correlation between color flow mapping and cardiac catheterization (r s = 98). Aortic homograft valved conduit function can be accurately noninvasively assessed by Doppler but proximal, valve, and distal conduit segmental interrogation is necessary to localize site of obstruction if present, rule out multilevel obstruction, and avoid underestimation of systolic gradient.

Embolization of type II endoleaks fed by the inferior mesenteric artery: using the superior mesenteric artery approach.

PURPOSE: To evaluate the use of a superior mesenteric artery (SMA) approach to embolize type II endoleaks arising from the inferior mesenteric artery (IMA). TECHNIQUE: When reperfusion of the aneurysmal sac via the SMA occurs through the IMA, as shown by computed tomography (CT) and angiography, the IMA origin can be accessed via the marginal artery or the anastomosis of Riolan. The SMA is catheterized with a 5-F catheter, and a coaxial catheter is advanced to the leak to deliver 2- to 8-mm-diameter minicoils to embolize the IMA origin and entire aneurysmal sac. Embolization usually requires from 1 to 2 hours to complete. In our experience with this technique in 11 cases, complications have not occurred, and there has been only one very small residual leak that sealed the next day. Over a 24.5-month follow-up (range 12-39), the endoleaks have remained sealed according to serial color duplex scans. CONCLUSIONS: Successful percutaneous treatment of type II endoleak due to IMA inflow can be accomplished using an SMA access via the Riolan anastomosis or marginal artery. The procedure appears to be safe and has no adverse effects.

[Value of Doppler color echocardiography in the diagnosis of diastolic mitral insufficiency in severe aortic insufficiency. Report of 2 cases].

The authors report the value of Doppler color flow mapping in the diagnosis of late diastolic mitral regurgitation in two patients with severe post-endocarditis aortic regurgitation requiring rapid surgical intervention. Doppler color flow mapping played an essential part in the management of these cases by helping in the diagnosis of late diastolic mitral regurgitation which is known to carry a very poor prognosis in this context.

[A case of hypertrophic obstructive cardiomyopathy (HOCM) complicated by infective endocarditis (IE) and mitral regurgitation (MR)].

The case of a 56-year-old man who underwent mitral valve replacement due to HOCM complicated by IE and MR is reported. Preoperative 2D echocardiographic assessment of the mitral valve revealed systole anterior motion (SAM) and vegetation, and color flow Doppler echocardiographic examination revealed severe mitral regurgitation. The left ventricular out-flow tract gradient decreased from 140 mmHg preoperatively to 60 mmHg postoperatively. Mitral valve replacement should be considered in patients with associated IE and severe MR.

Intraoperative color Doppler sonography during renal artery revascularization.

Intraoperative color Doppler sonography is emerging as a useful procedure for evaluating renal arteries during transaortic renal endarterectomy or bypass grafting. Technical difficulties encountered during surgical renal revascularization, such as intimal defects or flaps, partial or complete thrombosis, and anastomotic stenosis of the renal artery, can be difficult to defect and evaluate. Intraoperative color Doppler sonography allows evaluation at a time when the problem is often correctable. We have been performing intraoperative sonographic studies of the renal artery for several years and have studied 151 vessels. In this pictorial essay, we present some of our observations on these cases.

[A three-year clinical evaluation of galvano-ceramic bridges made by Ni-Cr pontic bonding galvano caps].

OBJECTIVE: To evaluate the clinical effect of galvano-ceramic bridges made by Ni-Cr pontic fired bonding galvano caps with highly temperature-resistant adhesive based on three-year clinical trials. METHODS: A total of 29 galvano-ceramic bridges made by Ni-Cr pontic fired bonding on the galvano caps were used for 26 patients. Abutment teeth plaque indices (PI) were compared according to Silness and Löe standards and their papillary bleeding indices (PBI) according to Mazza standards before restoration and after 36 months' restoration. At the same time, the integrity and marginal fitness of galvano-ceramic bridges were observed. RESULTS: The PBI at abutment teeth were higher than those before restoration, Wilcoxon test revealed significant difference. Their PI were lower than those before restoration, Wilcoxon test revealed significant difference. All the galvano-ceramic bridges were perfect in marginal fitness and color. The rate of integrity was 89.7% in the period of observation. CONCLUSION: Galvano-ceramic bridges made by Ni-Cr pontic fired bonding galvano caps with highly temperature-resistant adhesive hold out good prospects for clinical use in fixed restoration.

Changes in photon dose distributions due to breast prostheses.

PURPOSE: Subcutaneous prosthetic implants had been routinely used for cosmetic augmentation and for tissue replacement following mastectomy over the last 15 years. The implants come in many forms as the gel filler material and surrounding shell material(s) vary significantly. METHODS AND MATERIALS: This study uses a thin window parallel-plate chamber and thermoluminescent dosimeters to quantify and dosimetric changes to surrounding breast tissue due to the presence of the prosthesis. A mammographic phantom was compared to four commercial prostheses, namely two silicon gel fillers within two different shells (silicon or silicon/polyurethane), a tri-glyceride within silicon and a bio-oncotic gel within silicon/polyurethane. The latter two implants were designed with a low-Z fill for diagnostic imaging benefits. RESULTS: Ion chamber results indicate no significant alteration of depth doses away from the implant with only minor canceling (parallel opposed) interface perturbations for all implants. In addition the physical changes to the irradiated prostheses were quantified by tonometry testing and qualified by color change. Each implant exhibited color change following 50 Gy, and the bio-oncotic gel became significantly less formable following irradiation, and even less formable 6 weeks postirradiation. CONCLUSION: The data indicates that prostheses do not affect the photon beam distribution, but radiation does affect the prostheses.

Initial clinical experience with a 48 by 48 element biplane transesophageal probe.

Recent technologic advances in ultrasound have resulted in the capability of transesophageal echocardiographic imaging in both transverse and longitudinal planes. Previous biplane probes suffered from inferior images because of reduced scan elements. We evaluated the utility of a prototype 48 X 48 element biplane transesophageal probe in 23 consecutive patients. Examinations were well tolerated with no side effects. In comparison to the single transverse plane, imaging with the longitudinal plane gave superior information on prosthetic valve pathology, atrial septal abnormalities, and pathoanatomy of the ascending aorta and mitral valve. Complementary information was provided by the longitudinal plane in patients with endocarditis and vegetations and in mitral protheses. Images obtained with this 48 X 48 element biplane probe along with color and spectral Doppler information were not perceptibly inferior to those obtained by single-plane probes. In conclusion, biplane transesophageal echocardiography with a 48 X 48 element probe indicates a great potential for enhanced three-dimensional understanding of cardiac pathology and diagnostic yield in specific pathologies.

New instruments, bone liners, and tray for finger joint arthroplasty.

Titanium circumferential grommets have been developed for finger joint arthroplasty that fit on the base of the silicone implant stems to protect the flexible hinge from tearing and fracture. To facilitate grommet insertion, new intramedullary bone rasps have been devised with a reverse cutting tooth pattern, an extended shaft for an improved view of the surgical field, and a redesigned cutting head to allow for grommet insertion. Surgical accessibility and ease of recognition have been facilitated by the development of color-coded sizers. All of the instruments necessary for finger joint surgery are available in a molded tray that has also been redesigned with a transparent lid and clearly labeled compartments for accurate determination of instruments and implant sizers.

Malignant giant cell tumor of the tendon sheath: an autopsy report and review of the literature.

A case of malignant giant cell tumor of the tendon sheath of the right hip, which developed in a 72-year-old Japanese woman, is described. The tumor exhibited histological similarities to a benign giant cell tumor of the tendon sheath (localized nodular tenosynovitis). The resected tumor, measuring 9 x 9 x 11 cm, was located in the adductor muscle and invaded the proximal femur and acetabulum. The nodule was encapsulated with a thin membrane which was soft and gelatinous in consistency and varied in color from yellow to brown. The synovium of the hip joint was normal. The primary lesion was composed of plump polyhedral and spindle-shaped cells. The nuclei were large, irregular and hyperchromatic, and contained prominent nucleoli. A moderated number of multinucleated giant cells was scattered throughout the lesion. There was little stromal collagen. In the majority of the specimens, pseudoglandular or alveolar spaces were predominant. An ultrastructural study demonstrated three cell types: fibroblast-like, histiocyte-like and an intermediate. The patient underwent reconstructive surgery with a Dacron fabric-enveloped alumina ceramic pelvic prosthesis and total hip components after resection of the primary lesion. Unfortunately, because of a local recurrence, a hemipelvectomy was required 10 months after the initial operation. At that time the intestines were involved with the recurrent tumor, and the patient subsequently died of perforative peritonitis. An autopsy revealed distant metastases to the right pelvis, urinary bladder, right ureter, ilium, mesenterium and lungs.

Assessment of valvular heart disease by magnetic resonance imaging.

MRI has developed very rapidly and now provides anatomic and functional information in cases of valvular heart disease. MRI has several important attributes that make it advantageous for the evaluation of valvular heart disease. First, the natural contrast between flowing blood and surrounding cardiovascular structures provides sharp delineation of endocardial and epicardial borders without the need for contrast media. This feature in combination with the essential three-dimensional nature of this imaging technique allows precise quantification of cardiac volumes, function, and mass without the use of any assumed formulas or geometric models. Second, blood flow-sensitive GRE techniques are able to identify areas of turbulent flow caused by stenotic or regurgitant valves. With this technique regurgitant jets can be visualized and semiquantitative grading can be performed as with color Doppler. Third, recently developed velocity-encoded techniques permit measurements of blood flow velocities across stenotic native and prosthetic heart valves and retrograde flow caused by regurgitation. Moreover, the close interstudy reproducibility of measurements of cardiac dimensions and valvular regurgitation suggests a role in assessing the effect of therapeutic interventions.

[Diagnostic value of transesophageal echocardiography in diseases of the heart valve system].

Due to high resolution transesophageal echocardiography has been proven to have additional diagnostic benefits in valvular heart disease. The quantification of aortic and mitral valve insufficiency is possible by the use of semiquantitative methods. In aortic valve insufficiency the relation of the regurgitant jet width to the left ventricular outflow tract and in mitral valve insufficiency the relation of the regurgitant jet area to the left atrium are the parameters with the most accurate sensitivity. The main advantage of the method in this point is the rapid detection of morphological pathology at the valves. The development of multiplane transducers enhanced the quantification of aortic valve stenosis which is performed by planimetry of the orifice area in an ideal cross-section of the valve reached at a mean angle of 46 degrees. The high resolution image of mitral valve anatomy leads to the intraoperative use of the method during valve repair identifying inadequate surgical correction, which can be revised instantly. Visualization of the valvular and subvalvular mitral apparatus enables a more reliable indication for valvuloplasty by detecting the presence or absence of atrial thrombi and assessing the severity of calcification, the guide catheter and the balloon placement. Transesophageal echocardiography is the method of choice in the detection and measurement of vegetations and abscesses in infective endocarditis. Imaging an increasing extent of a vegetation during follow-up investigations can lead to the necessity of surgical intervention for preventing embolic events. The transesophageal approach especially in combination with color-doppler flow imaging is superior to the transthoracic method in the detection of abscesses and secondary complications like communication to adjacent structures or implication of the mitral valve. In this point the use of multiplane probes permits a more accurate assessment of the extent and the spatial configuration of additional masses. Concerning valve prostheses the detection rate of vegetations, abscesses as well as degenerations of bioprostheses is higher using the transesophageal method than the transthoracic approach. Abscesses located near the right coronary sinus or in the ventricular septum are difficult to detect due to shadowing caused by prosthetic artefacts. The visualization of bioprostheses allows the measurement of the thickness of the leaflets which is necessary for the diagnosis of valve sclerosis or degeneration. In combination with color-doppler flow imaging the method allows the differentiation of transvalvular and perivalvular regurgitation. The movements of the occluder can be imaged leading to a more reliable diagnosis of obstruction in combination with the trans-thoracic use of doppler-methods.
