Effects of using systematic assessment instruments on patient and nurse satisfaction with nursing care.

To determine the effects of using systematic assessment instruments on patient and nurse satisfaction with the nursing process, a single-group, pre- and postintervention quasi-experimental design was used in a three-phase study. In the pre- and postintervention control phases, nurses used a routine clinical assessment instrument to implement the nursing process; in the intervention phase they used the Derdiarian Behavioral System Model (DBSM) self-report and the DBSM-O (observational) instruments. The Comprehensiveness of Nursing Assessment and the System of Practice instruments were used to evaluate the comprehensiveness and quality of care respectively; the Patient-Satisfaction and Nurse-Satisfaction instruments were used to evaluate patient and nurse satisfaction with care. The findings showed that the increase in patient and nurse satisfaction was statistically significant when the DBSM and DBSM-O instruments were used.

Too important to miss out? Documentation of care in critical care nursing.

1. In critical care settings, time may be limited for the documentation of care in the care plan. 2. Implementation of the nursing process involves responsiveness both to clients' clinical needs and their informed choices. 3. The care plan is not wholly indispensable to individualised, holistic care, but is integral to the full professional implementation of the nursing process.

[Experiencing the educational process in nursing with high risk pregnant women and their companions].

The present study is a picture of the educational process lived by hospitalized high-risk pregnant women and also by their companions, conducted at the University Hospital of the Federal University of Santa Catarina. It has been developed as a joint process with the clients, through dynamic interaction, as a set of actions in an effort to socialize knowledge and experiences. The goal is to offer greater autonomy to the clients, allowing the high risk pregnancy to be a peaceful, safe and joyful experience, beyond the fact of contributing with the construction of new knowledge about the being-and-living process of these women.

The Verbal Protocol: a research technique for nursing.

This paper discusses the Verbal Protocol technique as presently used in the fields of psychology and computer sciences. It proposes that the technique is an appropriate design tool for studying clinical reasoning in many nursing contexts. An example is given, taken from a pilot study of a larger research project, of the use of the technique to demonstrate the nurse's thinking while assessing patient problems. Results show that the Verbal Protocol technique can reveal useful information concerning problem diagnosis in nursing and has the potential for further research into the cognitive behaviour underlying other stages of the nursing process.

Regulating: the social control process registered nurses use to teach psychiatric patients about their medications.

In light of rising costs of health care, trends toward community based care, and greater demands by patients for control over their health, patient teaching has become a necessary part of the registered nurse's role. The purpose of this study was to describe how nurses teach patients with mental illness about their medication regimens. Twenty-nine nurses from three mental health settings in Australia participated in semistructured interviews and participant observation. The constant comparative method was used to analyze the data. Findings from this study indicated that the process of regulating, a social control process, was used by nurses to teach mentally ill patients about their medication regimens. Implications for practice and education are drawn from the findings of this study.

Quality assurance programme for nursing.

This article describes a conceptual and methodological framework for evaluating nursing care quality. The conceptual framework is based on Orem's theory of self-care which emphasizes the importance of decisions in the selection of nursing process based on patient status and problems. The primary outcome criteria is self-care. The methodological framework is based on Greenfield's criteria maps methodology for linking patient status (problems or diagnoses) to decisions about nursing interventions and linking these specific nursing intervention criteria to desired outcomes for self-care. The article further describes a multiagency quality assurance programme for nursing. The programme includes: a definition of criteria and standards across agencies; nursing care management protocols that define standards of care; continuing education courses for nursing based on management protocols; the maps method of auditing actual and recorded nursing process and patient outcomes as defined in the management protocol.

Evaluating quality nursing care through peer review and reflection; the findings of a qualitative study.

The purpose of this study was to examine the everyday methods by which nurses evaluate quality care. The method was qualitative; specifically, a grounded theory approach was used. The sample comprised 18 registered nurses from a large hospital in the south of England. Data was collected through tape recorded interviews and the constant comparative method used to analyze the data. The findings revealed that although nurses used objective methods such as evaluating planned care as part of the nursing process, they also used more subjective methods such as peer review and intuition. The study seeks to explain the value of these less formal methods of evaluation and recognises how difficult they may be to substantiate in light of the recent health care reforms. It is suggested that the use of a more formalised process of peer review using reflection as its foundation would enable nurses to satisfy managerial concerns for a measurable outcome to quality.

Intravenous therapy: current practice and nursing concerns.

The field of intravenous (i.v.) therapy has been subject to major change, with increasing numbers of nurses taking on the high profile, technical aspects of care. The transfer of previously medicalized tasks such as cannulation has been welcomed by nurses who are keen to develop practical skills in order to embrace the concept of holistic patient care. This literature review aims to clarify the role of the nurse in i.v. therapy, exploring cannulation as a specific issue. Legal and professional aspects are discussed in terms of extended/expanded practice and practical aspects in terms of i.v. access and maintenance. Discussion focuses on a team approach to the management of i.v. therapy. Finally, the nursing process is applied to an i.v. therapy scenario. Exploration of the nursing issues enables practitioners to justify the expansion of individual practice in order to deliver holistic care and improve standards of service. The review concludes that nurses cannot afford to lose sight of the caring component of their role as a result of immersion in the culture of technical skill acquisition.

Cultivating effectiveness in your organization.

To be truly effective, the occupational and environmental health nurse must possess skills as both a manager and a leader. Effective management results in programs and projects that are likely to be successful, achieve established goals, and meet expectations of the intended recipients. Effective leadership results in individuals who feel valued for their opinions, empowered to act independently, and accountable for setting and achieving personal goals. When these individuals come together to form teams, they create an organization in which the group benefits from the commitment and mutual desire to work toward a shared mission and vision. The nursing process provides an excellent framework for the occupational and environmental health nurse to approach the tasks associated with leading and managing in a rapidly changing, challenging environment. Techniques to be employed are those acquired in formal management training programs and those passed down from experienced mentors. It is incumbent on all occupational and environmental health nurse managers to perfect and use leadership and management skills to positively contribute to organizational effectiveness and, ultimately, employee health and well being.

Development of the nursing information system based on the standardized nursing language system for gastric cancer patients.

The purpose of this study was to develop the nursing information system based on the Standardized Nursing Language System for gastric cancer patients according to the framework of the nursing process. This study also tried to evaluate the usability by applying it to the gastric cancer nursing practice. The system was developed based on the System Development Life Cycle. Therefore, it is considered that the gastric cancer nursing information system developed in this study can contribute to the improvement of clinical usability under the nursing process and to the enlargement of the range of nursing records. In addition, the system developed in this study was based on the Standardized Nursing Language System, for effectively controlling data related to gastric cancer nursing. The gastric cancer nursing information system in this study can be applied effectively in the quality control of nursing care, follow-up care and research for the whole nursing field as well as in cancer nursing, and can promote the use of the Standardized Nursing Language System.

Nursing in pigs: communicating needs and distributing resources.

When nursing, the sow has a problem to solve different from that of most other mammals because she gives birth to a large number of precocial young. The sow has to incorporate elements into its nursing process that enable it to distribute milk evenly among its young in spite of their being able to fight with and suckle independently of their littermates. This paper described the different features of the nursing process, discussing the functions of a long initial massage before ejection, the grunting of the sow during the first phases of nursing, the postejection massage, and nonnutritional nursings in the context of a process of communication. Through tactile stimulation of the udder by the pigs, the sow releases a number of pituitary, gut, and pancreatic hormones. The physiological response of the sow is suggested to be quantitatively related to the tactile input given by the pigs. Evidence is presented to support the idea that a fault in the communication process disturbs the nursing-suckling interaction and, thus, the transfer of milk from the sow to its pigs. Also, the importance of some environmental factors that affect this process is discussed.

Testing of the Individual Nutrition Rx assessment process among nursing home residents.

The purpose of this article is to describe the Individual Nutrition Rx (INRx) assessment process and to report preliminary findings on baseline nutritional status, common nutrition problems, and specific individualized interventions. The INRx utilizes a predictive model for participant identification, a systematic assessment process, and evidence-based interventions to improve the nutritional status of at-risk residents. Nineteen percent of participants had abnormal serum albumin levels, and 32.8% had abnormal serum prealbumin levels, indicating diminished protein stores and protein-calorie malnutrition. Frequent nutritional problems identified through the INRx assessment process are appetite change, poor positioning while eating, and oral status concerns.
