[Prediction of effectiveness of electroconvulsive therapy in major depression with routinely collected data].

BACKGROUND: Little is known about the possibility to predict response to electroconvulsive therapy (ECT) in patients with major depression. AIM: To create an indexforthe prediction ofresponse to ECT in an individual patient. METHOD: Fifty-three depressive patients referred for ECT were included. Poor response was defined as a decrease in Hamilton Rating Depression Scale less than 50%. With multivariable analyses a simple index of independent predictors was constructed. RESULTS: Thirty-one patients (58%) showed poor response. The index comprised age < 65 years, psychotic depression, refractory to antidepressant medication, and personality disorder. It discriminated poor response patients reasonably well with an area under the receiver operating characteristic curve of 0.76 (95% confidence interval 0.63-0.89). CONCLUSION: Response to ECT may be predicted using an index with four patient characteristics. Before implementation, however, validation of the index in future patients is mandatory.

[Manometric data collection].

During the recent years, intraluminal esophageal manometry has developed into an important method for detecting motor function of the esophagus and pharynx. High fidelity instrumentation is now available to perform adequate recording of pressure phenomena of the esophagus. It consists of water-filled catheters systems, perfused by minimally compliant hydraulic system and of intraluminal strain gauge probes. A sleeve catheter assembly is suitable for recording lower sphincteric pressure for long period, despite the respiratory and deglutitive axial movements of the sphincter along the catheter. The pressure events of interest are phasic pressure wave along the esophageal body (usually peristaltic) as well as resting tone and relaxation of upper and lower esophageal sphincters. Recording instrumentation available allows accurate detection of esophageal motor activity both in healthy subjects and in patients.

Part I: The NGRI Registry: initial analyses of data collected on Connecticut insanity acquittees.

The NGRI Registry is a comprehensive census database collected by the Law and Psychiatry Division of the Department of Psychiatry at the Yale University School of Medicine. This longitudinal database was compiled following a systematic search of all available docket books from the superior courts and mental health records from the state hospitals in Connecticut beginning in January 1970. Detailed life span information is available for 364 insanity acquittees identified during the search. Comparative analyses with four other locales suggested that there were regional differences in diagnoses of and crimes committed by the acquittees. These initial analyses demonstrate the promise of this registry becoming one tool for collaborative research on issues relevant to law and mental health.

[Non-polio enterovirus in France from 1974 to 1985. Data collected from a system of 30 hospital virology laboratories].

A total of 12,097 isolates of NPEV was reported in France from 1974 through 1985 by 30 volunteers laboratories (71% of all virology laboratories). The 14 most common types which accounted for 75% of all isolates are echoviruses 4, 6, 7, 9, 11, 14, 30 and 33, coxsackieviruses A9, B1 to B5. Among them, 12 are equally reported as the most common types isolated in the United States from 1970 through 1983. Echoviruses 14 and 33 are specially found in France; Echoviruses 3 and 5 in the United States. We can estimate that those 16 types are the most common EVNP in human pathology. The pattern of each type is different in the United States and in France: it seems not possible to assign an epidemic character specific to each of them. Yet epidemic types accounted for 13 to 27% of isolates in March, April, May. Among clinical manifestations, aseptic meningitis are the most frequently reported: 37% of 3,619 cases. Echoviruses are significantly most often associated with meningitis than coxsackieviruses (p less than 0.001) and coxsackieviruses more often associated with other neurological manifestations, myocarditis and pericarditis. From these results wa can suggest to simplify the typing procedures of NPEV for routine diagnosis.

[Collecting data on the state of health of the elderly by mail survey of family doctors (author's transl)].

In connection with a gerontological research project data on the physical of the persons being investigated were, amongst other sources, also obtained through a mail survey of family doctors. How far survey shortcomings have a distorting effect was studied. The results indicate that a mail survey of family doctors may yield appropriate data even if some of the physicians named by the persons being investigated do not respond.

Predicting energy requirements: is energy expenditure proportional to the BMR or to body weight? An analysis of data collected in rural Gambian women.

It has been suggested that energy requirements are either proportional to the BMR (FAO/WHO/UNU, 1985) or to body weight (WHO, 1973). The present analysis sought to distinguish between these two possibilities by determining whether, in rural Gambian women, energy expenditure was more nearly proportional to the BMR or to body weight. For nine of eleven common daily activities energy expenditure expressed per kg body weight was significantly lower in heavier compared to lighter individuals, indicating that energy expenditure was not proportional to weight. Since, in a preliminary analysis, it was found that in Gambian women BMR was roughly proportional to weight raised to the power 0.5, energy expenditure was also compared per kg. For ten of the eleven activities there were no significant differences between different weight groups in the energy costs expressed per kg, and it follows from this that in Gambian women the energy cost of common daily activities was more nearly proportional to the BMR than it was to body weight. Possible reasons for and implications of these results are discussed.

[Association between Trichomonas vaginalis and some types of lesions of the uterine neck based on data collected in the quinquennium of 1974--1978].

In our statistic the number of cases of vaginal trichomoniasi in which discariosi, displasie, metaplasie are found are represented in about 40%. While 37,5% of women affected with neoplastic lesions had Trichomonas vaginalis. It is certain that in the presence of flogosi from Trichomonas vaginalis we meet more frequently than normal, atipie cellulars and that often an exact valutation of this lesion is put back to an exam after the recovery of the Trichomonas vaginalis, because fortunately, more than often these disturbances disappear.

[Computer-assisted obstetric data collection and processing--trial with a system solution].

A system is introduced for the registration and utilization of obstetrical dates. It is based on a new birth journal and a file for date collection in agreement to that. With the aid of a bureau computer a short delivery report will be given for the out-patient department of post-clinical medical attendance, as well as a label with the most important obstetrical dates for the postnatal care will be printed. An extensive birth report can be requested. The voluminous obstetrical statistics will be concentrated in form of charts.
