[Treatment of hemorrhage of the digestive system with amine alpha-adrenergic stimulants].

Noradrenaline was administered through the stomach or peritoneum in thirty patients of the Raza Hospital of the I.M.S.S. in an attempt to control severe gastro-intestinal bleeding as described by Dr. H. H. Le Veen. In 84% of the cases this was accomplished with an average elapsed time of 82 minutes. The method proved to be effective of various etiologies except in those cases in which an important size artery was involved. There were no side effects.

[Massive hemorrhage of the digestive system secondary to ulcerated colonic malacoplakia].

A case of malakoplakia of the colon in a 55-year-old female patient with a massive hemorrhage of the rectum is reported. A barium enema showed polypoid lesions (pseudopolyps). Colonoscopy revealed white-yellowish nodules simulating small pustulae all along the colon at 2 cm from the anal verge. A laparatomy disclosed lobulated intraluminal masses and yellowish lesions in the form of transcolonic plates infiltrating the duodenal wall as well as a fair amount of lymph nodes in the mesentery. We performed a proctocolectomy. The histologic study showed massive infiltrate of histiocytes and numerous Michaelis-Gutmann bodies. This disease is usually found in the urinary tract and rarely found in the colon. The importance of proper histologic examination in order to arrive at a correct diagnosis is emphasized.

[Arteriovenous and arterial-digestive system fistulas complicating isolated iliac aneurysm].

Isolated iliac aneurysms are often discovered when they rupture, sometimes into a neighbouring organ. We report 2 cases of aneurysm of the common iliac artery, one which ruptured into the right common iliac vein and the other in to the sigmoid. Diagnosis was made at laparotomy in the first case and on the CT-scan of the pelvis in the second. The first case was treated with an exclusion graft using ilio-iliac bypass with suture of the venous breach via the aneurysm. In the second case, the aneurysm was excluded by ligature of the iliac arteries with extra-anatomic crossed femoro-femoral bypass, sigmoid colostomy and drainage of the pelvis. These two cases illustrate the importance of digital examination of the pelvis and CT-scan for the diagnosis of complicated aneurysms of the iliac vessels. Rapid surgery is a determining factor in prognosis.

[Spiral computed tomography with arteriography in the diagnosis of digestive system hemorrhages of obscure origin].

Some 5-20% of gastrointestinal (GI) bleeding cases are of obscure origin because bleeding is difficult to diagnose or locate with conventional imaging methods. The authors suggest arterial spiral CT (ASCT) of the abdomen as an original tool to study GI bleeding of obscure origin. This diagnostic method consists of two subsequent phases: first, after abdominal aorta catheterization, the catheter is positioned in the celiac artery; then, CT of the abdomen is performed before and after intra-arterial contrast agent injection. In 6 of 10 patients with GI bleeding of obscure origin, ASCT depicted small high-intensity areas in the intestinal lumen, which indicate a source of bleeding. The site of bleeding was studied only with topographic criteria according to scanning planes, and the morphological features of the intestinal segments were investigated. All patients were submitted to surgery: when ASCT identified a bleeding site, angiography was limited to the involved arterial district. Angiography alone could locate the source of bleeding in 5 patients; angiography and ASCT were in agreement in 4 patients. The combined techniques located the source of bleeding in 7/10 patients and the nature of bleeding was diagnosed in 3/10 patients (angiodysplasia). Even though their experience is limited, the authors believe that ASCT can be useful in locating GI bleeding of obscure origin, as well as in guiding subsequent selective angiography.

[Study on geographic characteristics of malignant tumors in digestive system in Shanxi province].

The mortality of esophageal, stomach and hepatic cancers occupied 70% among all who died from cancers in Shanxi province. Countries which had the highest mortality of the above three kinds of cancers showed significant aggregation. Differences between countries with higher and lower mortality were obvious. The annual mean temperature and rainfall decreased with the raise of latitude in Shanxi province. So was found the mortality of these cancers. Both of them tended to be coincident, and the difference was statistically significant (P < 0.05). These results were not in agreement with reports from foreign countries which need to be further studied.
