Intracranial aneurysms: characteristics of aneurysms by site, with special reference to anterior communicating artery aneurysms.

Data are presented from an unselected series of 212 aneurysm patients for aneurysms at three major sites: the internal carotid-posterior communicating artery junction, the anterior communicating artery, and the middle cerebral artery. More than 70% of the anterior communicating artery aneurysms occurred as single aneurysms; less than 30% of the middle cerebral artery aneurysms were single. Anterior communicating artery aneurysms showed a right-side predominance in males but not in females. Both males and females had significantly higher mean systolic and diastolic blood pressures for left-sided anterior communicating artery aneurysms than for right-side aneurysms. A brief review of factors relevant to anterior communicating artery aneurysms is presented.

Three dimensional digital subtraction angiography imaging of a ruptured aneurysm on the anterior communicating artery.

BACKGROUND: Although angiography is still gold standard for the diagnosis of cerebral aneurysms, false-negative angiograms in cases of subarachoid hemorrhage due to a rupture of an anterior communicating artery aneurysm have been previously reported. We now report a case of an anterior communicating aneurysm demonstrated by 3-dimensional digital angiography (3D-DSA) after a negative standard angiogram. CASE DESCRIPTION: A 41-year-old female was admitted in a comatose state. Computed tomography (CT) revealed intraventricular hemorrhage and a clot in the frontal interhemispheric fissure, suggestive of rupture of an anterior communicating artery aneurysm. Two days later, angiography was performed. Although standard 3-vessel angiogram with multiple views, using cross-compression on each carotid artery, was negative, 3D-DSA revealed an anterior communicating aneurysm. The aneurysm was successfully clipped based on the findings obtained from the 3D-DSA. The outcome was favorable, with mild memory disturbance. CONCLUSION: The standard angiogram sometimes fails to demonstrate aneurysms, especially those on the anterior communicating artery. In this case, 3D-DSA was useful for the diagnosis of the anterior communicating aneurysm which was not seen on standard angiogram.

Effects of physician communication style on client medication beliefs and adherence with antidepressant treatment.

The goals of this study were to examine how physician communication style impacts client beliefs and medication taking behavior during treatment for depression. The study uses a communication framework and prospective design to examine physician communication and client beliefs as treatment is initiated and again 2 months later. Two telephone interviews were conducted with 100 clients enrolled from 23 community pharmacies. Clients report that physician communication styles vary. In follow-up, 25% of the clients were not satisfied with their medication and 82% reported missing doses or stopping treatment earlier than recommended. Path analysis showed that physician initial communication style positively influences client knowledge and initial beliefs about the medication. Clients with more positive beliefs about the treatment are more likely to see the physician in follow-up and are more satisfied with treatment after attempting medication use. Physician follow-up communication style and client satisfaction are both predictive of better medication adherence.

Marinacci communication: an electrophysiological study.

OBJECTIVE: Prevalence of Martin-Gruber anastomosis, an anomalous median-to-ulnar forearm communication, is well reported in literature while Marinacci communication, the reverse of Martin-Gruber with a forearm ulnar-to-median communication is under-recognized. We systematically evaluated the presence of Marinacci communication in a series of patients referred for electrophysiological studies. METHODS: One hundred consecutive patients referred to the electrophysiological laboratory for various diagnoses were studied using standard techniques for motor, sensory and f wave studies. RESULTS: Of the 100 patients (200 arms) studied, electrophysiological features of Marinacci communication were observed in 4 patients (7 arms). Median stimulation with recording over abductor pollicis brevis (APB) revealed a pseudo-conduction block over the forearm segment while on ulnar stimulation and recording over abductor digiti minimi (ADM), the amplitude of the compound muscle action potential (CMAP) obtained on proximal stimulation was higher than that obtained on distal stimulation. Ulnar stimulation at the elbow but not at the wrist revealed CMAP from APB without initial positivity. Its amplitude was 50% of the amplitude obtained on median stimulation at the wrist. CONCLUSIONS: Marinacci communication is not uncommon in the general population. A pseudo-conduction block on median stimulation and higher CMAP amplitude on proximal than distal stimulation provide valuable clues to its recognition.

Toward an understanding of microbial communities through analysis of communication networks.

Bacteria receive signals from diverse members of their biotic environment. They sense their own species through the process of quorum sensing, which detects the density of bacterial cells and regulates functions such as bioluminescence, virulence, and competence. Bacteria also respond to the presence of other microorganisms and eukaryotic hosts. Most studies of microbial communication focus on signaling between the microbe and one other organism for empirical simplicity and because few experimental systems offer the opportunity to study communication among various types of organisms. But in the real biological world, microorganisms must carry on multiple molecular conversations simultaneously between diverse organisms, thereby constructing communication networks. We propose that biocontrol of plant disease, the process of suppressing disease through application of a microorganism, offers a model for the study of communication among multiple organisms. Successful biocontrol requires the sending and receiving of signals between the biocontrol agent and the pathogen, plant host, and microbial community surrounding the host. We are using Bacillus cereus, a biocontrol agent, and the organisms it must interact with, to dissect a communication network. This system offers an excellent starting point for study because its members are defined and well studied. An understanding of signaling in the B. cereus biocontrol system may provide a model for network communication among organisms that share a habitat and provide a new angle of analysis for understanding the interconnections that define communities.

Sural-tibial nerve communications in humans.

We investigated the occurrence of a communication between the sural and tibial nerves in 49 legs of 28 Japanese cadavers. In front of the calcanean tendon, we found the communication in 7 legs (14.3%) or in 5 cadavers (18.9%). The sural nerve gave rise to a number of medial and lateral branches, including the lateral calcanean branch at the lateral side of the ankle. The communicating branch with the tibial nerve arose from the first medial branch and pierced the deep fascia of the leg. In 4 cases, the U-shaped communication was formed between the sural and tibial nerves, and in 3 cases, the Y-shaped communication. Electrophysiological evidence of an anomalous motor function of the sural nerve has been reported recently. We consider that the U-shaped communication between the sural and tibial nerves gives a morphological basis to the motor function of the sural nerve.

The anatomy of the posterior communicating artery as a risk factor for ischemic cerebral infarction.

BACKGROUND: After the occlusion of an internal carotid artery the principal source of collateral flow is through the arteries of the circle of Willis, but the size and patency of these arteries are quite variable. Study of the anatomy of the collateral pathways in patients with internal-carotid-artery occlusion with or without infarction in the watershed area of the deep white matter may identify patterns that afford protection from ischemic infarction. METHODS: Using conventional magnetic resonance imaging and three-dimensional phase-contrast magnetic resonance angiography, we evaluated 29 consecutive patients (32 hemispheres at risk) with angiographically proved occlusion of the internal carotid artery. Four collateral pathways to the occluded vessel were evaluated: the proximal segment of the anterior cerebral artery, the posterior communicating artery, the ophthalmic artery, and leptomeningeal collateral vessels from the posterior cerebral artery. RESULTS: Only features of the ipsilateral posterior communicating artery were related to the risk of watershed infarction. The presence of posterior communicating arteries measuring at least 1 mm in diameter was associated with the absence of watershed infarction (13 hemispheres, no infarcts; P < 0.001). Conversely, there were 4 watershed infarcts in the 6 hemispheres with posterior communicating arteries measuring less than 1 mm in diameter and 10 infarcts in the 13 hemispheres with no detectable flow in the ipsilateral posterior communicating artery. CONCLUSIONS: A small (< 1 mm in diameter) or absent ipsilateral posterior communicating artery is a risk factor for ischemic cerebral infarction in patients with internal-carotid-artery occlusion.

Health communication on the Internet: an effective channel for health behavior change?

This article presents a theoretical rationale for using the Internet to conduct persuasive public health interventions. Through an examination of the conceptual bases of persuasion, it is posited that the World Wide Web and other Internet-based resources have many of the characteristics necessary for persuasive communication and may, in fact, constitute a hybrid channel that combines the positive attributes of interpersonal and mass communication. The notion that the Internet features many of the persuasive qualities of interpersonal communication makes it a prime candidate for the application of key behavioral science theories and principles to promote healthier behaviors. The broad reach that the Internet shares with many mass communication channels indicates an economy to Internet-based efforts to communicate with large audiences. It is concluded that if the Internet can be used for persuasive health communication and its reach continues to expand, it is time for public health professionals to explore the design and evaluation of Internet-based interventions directed at health behavior change.

Studies on cell communication with enucleated human fibroblasts.

Metabolic cooperation, the correction of the mutant phenotype in cells deficient in hypoxanthine phosphoribosyltransferase (HPRT-) by intimate contact with normal cells (HPRT+), represents a form of cell communication that is easily studied with radioautography. In the present study it was found that the formation of cell junctions needed for communication does not require protein synthesis nor is it under the immediate control of the cell nucleus. Enucleated normal cells efficiently communicate with HPRT- mutant cells. The effectiveness of enucleated cells as donors in metabolic cooperation provides evidence that it is the transfer of small molecules, nucleotide, or nucleotide derivatives that is responsible for correction of the mutant phenotype. Karyoplasts (nuclei with small amounts of cytoplasm surrounded by a plasma membrane) are unable to efficiently communicate with intact cells. The utilization of [3H]hypoxanthine by communicating mixtures of HPRT+ and HPRT- human cells is not significantly different than in the normal cells alone. Metabolic cooperation, as studied involves a redistribution of purine-containing compounds among communicating cells.

Research on the effects of pharmacist-patient communication in institutions and ambulatory care sites, 1969-1994.

Research on the effects of pharmacist-patient communication that appeared in the pharmacy literature between 1969 and 1994 is reviewed. The terms patients education and patient counseling were used in identifying relevant research. Many authors used these terms interchangeably; also, the terms counseling and consultation often were not clearly defined. Studies of pharmacists' history-taking in institutional settings and of other communication with patients in ambulatory care settings were examined by decade. The research questions, theories, methods, results, and limitations were analyzed. More than 30 studies examined the effect of pharmacists' communication on patient outcomes, primarily knowledge and medication compliance; generally, the results suggested that pharmacists' communication led to increased knowledge and compliance. A few researchers raised concerns about patients' knowledge as an indicator of effective communication by pharmacists, and in the 1980s a few suggested that better medication compliance could be associated with the time and attention given to patients rather than the informational content of the interaction. Little was reported about the communication theories or models on which the studies were based, and there was little indication in most studies that patients' ideas about their therapy were considered. Often, the numbers of patients and pharmacists were small, and the pharmacists may have had training or motivation exceeding that of the average practitioner. In studies of pharmacists' versus physicians' history-taking, the physicians were not well described; their involvement and their approach may not have been comparable to those of the pharmacists. Before 1990, a few researchers had examined outcomes such as pulmonary function and control of diabetes. In the 1990s, more researchers looked at outcomes such as medication-related problems and use of health care resources. The research indicated that pharmacists can increase patients' knowledge of and compliance with their pharmacotherapy; however, additional research on other outcomes may be warranted.

The Use of Visual-Tactile Communication Strategies by Deaf and Hearing Fathers and Mothers of Deaf Infants.

This article presents the results of a study comparing deaf and hearing parents in the use of visual-tactile communication strategies during interaction with their hearing-impaired children between 18 and 24 months of age. The study includes 17 deaf and hard-of-hearing children and 33 parents, covering hearing mothers (n = 12), hearing fathers (n = 11), deaf mothers (n = 5), and deaf fathers (n = 5). The four groups of parents are compared in the use of visual-tactile communication strategies during free play with their children. Overall results show that deaf mothers and deaf fathers differ significantly from hearing parents in the use of a visual communication style adapted to the developmental communication needs and abilities related to the 18- to 24-month age period. The study pays special attention to differences in visual-tactile communication strategies according to hearing status, gender, use of languages, and communication modes.

Effects of Social versus Musical Antecedents on Communication Responsiveness in Five Children with Developmental Disabilities.

The present study involved a comparison of social versus musical antecedents on communication responsiveness in five children with developmental disabilities. During the social antecedent condition, the teacher presented opportunities for the children to greet, name objects, and request materials. In the musical antecedent condition, these same opportunities were embedded within a music/singing activity. A reversal design was used to compare the percentage of opportunities with appropriate communication responses across the two conditions. For three of the five children, the musical antecedent condition was associated with higher percentages of appropriate communication responses. For the other two children, the two conditions were associated with approximately equal rates of appropriate communication. Across both conditions, appropriate responses were more likely during opportunities for greeting and requesting than during opportunities to name objects. The results suggest that embedding communication opportunities within a musical activity may lead to increased appropriate communication responses for some children with developmental disabilities.

Organizational theory and the stages of risk communication.

The evolution of risk communication has been described as a series of communication strategies. This article suggests that organizational theory provides another dimension to understanding the evolution of risk communication, and that risk communication can be seen as an organizational adaptation of chemical manufacturers to external pressure. Following the tragedy in Bhopal the chemical manufacturing sector's loss of legitimacy led to destabilization of its authority and to increased uncertainty in its external environment. Risk communication was one means to increase legitimacy, thereby decreasing uncertainty and potential impact on resources. However, although risk communication may evolve from crises of legitimacy, the concept of "isomorphism"--conformance to norms within a corporate sector--predicts this need not be the case.

Ventriculo-coronary micro-communications in pulmonary atresia and sequential changes of coronary arteries.

In the present paper, numerous micro-communications between myocardial sinusoids and small branches of coronary arteries are studied which existed in a rudimentary right ventricle of a female neonate with pulmonary atresia and an intact ventricular septum, who was born on the 40th week of gestation. Cardioangiography revealed a large fistulous subepicardial communication. At autopsy, myocardium of the right ventricle appeared spongy with extensive sinusoid slits. In addition to the large communication which was connecting a deeply extending sinusoid of the right ventricle to the main trunk of the anterior descending artery, numerous micro-communications were found between myocardial sinusoids in the rudimentary right ventricle and intramural small branches of the coronary artery. Furthermore, morphometrical analysis demonstrated that the medias of the intramural arteries in both ventricles of the present case were significantly hypertrophic, as compared with those in the control cases without communications (p less than 0.05). These facts indicate the significance of hemodynamic factors on the persistence of embryonic sinusoid-coronary communication.

Linking the practice environment to nurses' job satisfaction through nurse-physician communication.

PURPOSE: To investigate direct and indirect relationships among the practice environment, nurse-physician (RN-MD) communication, and job satisfaction, as is posited in the nursing role effectiveness model (NREM). DESIGN: Survey. METHODS: Surveys were sent to a random sample of 500 hospital nurses throughout Michigan, and 332 (66%) responded. Main study instruments were the Conditions for Work Effectiveness Questionnaire-II (CWEQ-II), the Practice Environment Scale of the Nursing Work Index (PES-NWI), the ICU Nurse-Physician Questionnaire, and the Index of Work Satisfaction (IWS), Part B. Inferential statistical tests included multiple regression, t tests, and one-way analysis of variance. FINDINGS: Practice environment (PES-NWI) and empowerment (CWEQ-II) scales explained 20% of the variance in RN-MD communication. The combination of both environment scales (PES-NWI and CWEQ-II) and RN-MD communication explained 61% of the variance in nursing job satisfaction scores. RN-MD communication was also a significant mediating variable in the relationship between structure (practice environment and empowerment scales) and outcome (nursing job satisfaction). CONCLUSIONS: Factors in the practice environment contributed both directly to nursing job satisfaction and also indirectly through RN-MD communication. Study findings showed that a practice environment favorable to nurses improved both nurses' perceptions of their communications with physicians and their job satisfaction.

Transcatheter coil occlusion of residual interatrial communications after Fontan procedure.

OBJECTIVE: To assess the use of detachable coils as an alternative method to occlude interatrial communications after Fontan operations. DESIGN: Descriptive clinical study of selected patients after Fontan operation with interatrial communications inappropriate for transcatheter umbrella occlusion. SETTING: Tertiary paediatric cardiac referral centre. PATIENTS: Seven patients after Fontan operation with residual interatrial communications of various types producing a right to left shunt. INTERVENTIONS: Transcatheter placement of detachable coils with a diameter of 3 or 5 mm within the interatrial communication. RESULTS: A total of 14 coils were successfully placed within persistent patent fenestrations of the interatrial baffle, residual leaks at the suture line between the patch material and the right atrial wall, and unusual venous interatrial communications. The mean (SD) aortic oxygen saturation increased from 88 (1.1)% (range 86-89%) to 92 (1.3)% (range, 89-93%; p < 0.001) and the mean (SD) right atrial pressure rose from 9.7 (2) mm Hg (range, 6-11) to 10.6 (2.4) mm Hg (range, 6-13; p < 0.05) after coil implantation. In five patients, complete obliteration of the interatrial shunt was shown by angiography after coil implantation. At a mean (SD) follow up of 10 (4) months (range, 3-15) a residual interatrial shunt was detected by Doppler colour echocardiography in only one patient, and oxygen saturations ranged from 90% to 95% (mean, 92%). There were no late coil embolisations, thromboembolic events, or haemolysis in any patient. CONCLUSIONS: Detachable coils can be used successfully to occlude residual interatrial communications after the Fontan procedure. In selected cases, in whom intended transcatheter umbrella occlusion of residual interatrial leaks is not possible, the use of detachable coils might offer a safe alternative method to eliminate interatrial right to left shunting after the Fontan procedure.

Communication between noncontacting macromolecules.

Molecular interactions are the language that molecules use to communicate recognition, binding, and regulation, events central to biological control mechanisms. Traditionally, such interactions involve direct, atom-to-atom, noncovalent contacts, or indirect contacts bridged by relatively fixed solvent molecules. Here we discuss a third class of molecular communication that, to date, has received less experimental attention, namely solvent-mediated communication between noncontacting macromolecules. This form of communication can be understood in terms of fundamental, well-established principles (coupled equilibria and linkage thermodynamics) that govern interactions between individual polymers and their solutions. In contrast to simple solutions used in laboratory studies, biological systems contain a multitude of nominally noninteracting biopolymers within the same solution environment. The exquisite control of biological function requires some form of communication between many of these solution components, even in the absence of direct and/or indirect contacts. Such communication must be considered when describing potential mechanisms of biological regulation.

Vagal communicating branches between the facial and glossopharyngeal nerves, with references to their occurrence from the embryological point of view.

A rare and hitherto not reported case in which a branch of the vagal nerve communicated simultaneously with the facial and the glossopharyngeal nerves was encountered in the body of a Japanese male cadaver in an anatomy class. This vagofacial-vagoglossopharyngeal (X.VII-X.IX) communicating branch was found to issue from the vagal nerve truck in close association with the pharyngeal branches (rami pharyngei nervi vagi), bifurcating soon into a vagofacial (X.VII) and a vagoglossopharyngeal division (X.IX). The X.VII division coursed forward and reached the posterior belly of the digastric muscle; after entering this muscle, this division broke up into filaments to communicate with the ramus digastricus of the facial nerve which was found to play an equivalent role in making the vagofacial ansa. The X.IX division, in contrast, took its course medially to reach the stylopharyngeal muscle. After entering this muscle, the X.IX division communicated with the stylopharyngeal branch of the glossopharyngeal nerve, which was found to be the equivalent to the X.IX division; these two form together the vagoglossopharyngeal ansa. Therefore, it could be concluded that the X.VII-X.IX communicating branch constitutes the vagal moieties of the vagofacial as well as the vagoglossopharyngeal ansae. The background of the appearance of the communicating branches observed in this report is discussed in the text from the developmental viewpoint on the basis of the findings obtained in chick embryos stained in whole mounts with anti-neurofilament protein antibody.
