Diet and mortality from common cancers in Brazil: an ecological study.

A prospective ecological evaluation of mortality from common malignancies with dietary risk factors and alcohol consumption was carried out among 10 state capitals of Brazil. Regression analysis was used to examine the association of dietary intake with mortality rates of the most common cancers among adults age 30 years and older. Age-adjusted cancer mortality rates varied 2.4 to 3.3 fold across the state capitals. A positive relationship was observed between energy intake and colon, lung, and esophageal cancer (p</=0.02 for each). Colon cancer mortality was positively associated with consumption of total fat, eggs, alcohol, mate tea, cereals, and vegetables (p</=0.01). Lung cancer was positively associated with mate and cereal intake (p<0.05). Stomach cancer was associated with consumption of eggs (p=0.04); and negatively associated with consumption of high fiber foods, fruits, and vitamin A and C (p</=0.05). Esophageal cancer was positively associated with fat intake, mate and cereals (p</=0.05) and negatively associated with vitamin A (p=0.02); prostate cancer was negatively associated with vitamin C (p=0.007). Breast cancer was not associated with any of the factors studied. The marked variation in cancer mortality rates in Brazil may be partially related to the high variation in dietary components or other diet associated factors.

Attendance at day care centers increases the risk of childhood pneumonia among the urban poor in Fortaleza, Brazil.

We carried out a case-control study to investigate risk factors for childhood pneumonia in two groups of 650 children aged under two years in the city of Fortaleza, Ceará, Brazil. The cases were children recruited at the main pediatric hospital with a radiological diagnosis of pneumonia, and controls were children of the same age group recruited from the neighbourhood of the cases. In this paper we focus on variables related to childcare practices. Working mothers, proportion of time the mother had worked since the child was born, and use of day care centers emerged as important risk factors with estimated relative risks of 1.58, 1.76 and 5.22, respectively. Also important were the number of children living in the house and presence of grandparents. However, the presence of siblings under two years and the birth order were not associated with pneumonia. All analysis included adjustment for confounding by income, parents' education, and other risk factors as appropriate. This is the first study from a developing country to identify attendance at day care centers as a risk factor for increased childhood morbidity, in this case pneumonia. This finding is of significant public health importance for countries such as Brazil with growing urban populations and an increasing need by mothers to find work outside the home.

[Analysis of neonatal tetanus mortality in Brazil]

This study reports mortality from neonatal tetanus in Brazil (1979-1987), based on an analysis of death certificates. The distribution of causes of death in time and space was used as an indicator of proportional neonatal mortality from tetanus. Underreporting remains a serious problem, particularly in northern and northeastern Brazil. In most municipalities, only one death was notified during the period under consideration, thus highlighting the importance of considering the theoretical approach of "sentinel events" in the control of this disease. A proportional decrease in mortality from neonatal tetanus was observed in the states of Rondônia, Pará, Sergipe, São Paulo, Paraná, and Rio Grande do Sul. It was not possible to detect an association between immunization coverage with two doses of tetanus toxoid and neonatal tetanus mortality. The reduction in the number of deliveries taking place in the home during the period studied appears to have played an important role in the reduction of neonatal deaths attributable to this cause.

[Physical growth in stature of school children of Japanese ancestry in the city of São Paulo, Brazil]

This paper reports the results of a cross-sectional survey describing the growth in stature of a sample of school children of Japanese ancestry (three or four grandparents born in Japan) and of high socio-economic status living in São Paulo, Brazil. The sample comprises 124 individuals 7-10 years of age, of both sexes. The results show that the children present mean values of stature below the median values of the NCHS curves ("National Center for Health Statistics", U.S.A.). The values are similar to the medians of a sample of Japanese children and below those of well-to-do Brazilian children of non-Japanese ancestry studied by the PNSN ("Pesquisa Nacional sobre Saúde e Nutrição", Brazil). The findings do not support the hypothesis of uniformity of growth potential in stature, casting doubts on the utilization of a single anthropometric reference in the assessment of nutritional status.

[Linear growth retardation and social factors among schoolchildren from the city of Osasco, São Paulo, Brazil]

Cases and controls were selected for this retrospective investigation of the social determinants of growth retardation, from a Height Census carried out in the 1989 school year, involving children attending the first grade of all public and private schools in Osasco (in the Greater Metropolitan Area of São Paulo, Brazil). The cases, totalling 125 children entering school aged 7-8 years old, were characterized by a height-for-age index below -2 z score of the NCHS/WHO reference. The controls, totalling 139 children entering school at the same age, were characterized by a height-for-age index above -1 z score. Socioeconomíc variables such as family income, head-of-family's level of schooling, mother's schooling, environmental sanitation, and housing conditions were significant factors associated with the stunting process. Risk of linear growth retardation tended to be higher with lower social class (odds ratio = 7.3 for sub-proletariat vs. petit bourgeois; p < 0,001 for overall trend), suggesting the biological impact of Brazil's economic slowdown during the 1980s.

[Characteristics of private health care services in Salvador, Bahia, Brazil]

To study the organization of private health services in the city of Salvador, Bahia, Brazil, a survey was carried out in 1994 involving 174 facilities registered at the Brazilian Institute of Statistics and Geography - IBGE. Health services characteristics studied were the following: number of physicians, hospital beds, production and cost of outpatient services, and legislative aspects. Health services were classified according to the amount of resources each type of granting agency contributed to support outpatient care. We found that the majority (51.1%) of private health care services in Salvador do not depend on public funds. The main sources of revenue for health services are private health insurance (41.9%) and other kinds of private health plans (54%). These changes in the organization of health services challenge health planners to review strategies for municipalization of health care and the relations between public and private health services in Brazil.

Stomach cancer incidence in Brazil: an ecologic study with selected risk factors.

Contrary to many industrialized countries in which a sharp decline in stomach cancer incidence has been observed, Brazil still shows intermediate to high incidence rates. An ecologic analysis was performed to explore variables possibly associated with the development of stomach cancer. Cluster analysis, principal component analysis, and factor analysis were carried out with population data, including the following: stomach cancer incidence rates in the early 1990s obtained from population-based cancer registries in Porto Alegre, Campinas, Fortaleza, Belém, and Goiânia; and data from a Brazilian national survey on family expenditures (several diet consumption items and availability of home refrigerators) carried out in 1974-75. The results suggested that past availability of a home refrigerator, i.e. food preservation, may have played an important role in currently observed differences in stomach cancer incidence among the various populations studied in Brazil. Differences in living standards among populations in these cities also appear to have played an important role in the observed incidence differences.

Liver transplantation: expectation with MELD score for liver allocation in Brazil.

Liver transplantation represents the most effective therapy for patients suffering from chronic end-stage liver disease. Until very recently, in Brazil, liver allocation was based on the Child-Turcotte-Pugh score and the waiting list followed a chronological criterion. In February 2002 the Model for End-stage Liver Disease (MELD) score was adopted for the allocation of donor livers in the US. After that change, an increased number of patients with more severe liver disease was observed, although there was no difference in 1-year patient and graft survival. A reduction in waiting-list mortality was also observed. In Brazil, the MELD score was adopted on May 31st, 2006. Good results are expected regarding the new criterion for allocation.

Evaluation of techniques for the diagnosis of Strongyloides stercoralis in human immunodeficiency virus (HIV) positive and HIV negative individuals in the city of Itajaí, Brazil.

Human immunodeficiency virus (HIV) and intestinal parasites are common in Brazil. Previous studies have shown that infection with Strongyloides stercoralis is frequently associated with HIV infection. Strongyloidiasis is difficult to diagnosis and stool examination with conventional techniques fails to detect the helminth larvae. We made a prospective study, to test the efficacy of the agar plate technique to detect S. stercoralis in 211 HIV-positive patients and 213 HIV-negative patients in the city of Itajaí, Brazil, between September 2001 and June 2002. The feces samples of these patients were processed and analyzed according to the following methods: Lutz, formalin ethyl acetate, Baermann, Harada-Mori and agar plate culture. HIV-positive patients were more frequently infected by S. stercoralis (odds ratio= 5,.687). Among the methods used on fecal specimens, the larvae of S. stercoralis were most efficiently detected by the agar plate (69.7%) method, followed by the Baermann and the formalin ethyl acetate methods (48.5%) (P=0.01), Lutz (42.4%) (P=0.01), and Harada-Mori culture (24%) (P=0.001). Therefore agar plate culture is the most efficient method for the detection of S. stercoralis larvae and this technique should be the test of choice, especially in immunocompromised patients.

Production of monoclonal antibody to subtype 9 of Neisseria meningitidis and the distribution of this subtype in Brazil.

A new monoclonal antibody (5F81A4P1.9), which is specific for subtype 9 antigen of meningococci, was studied. The antibodies were raised against a previously non-typable (NT) serogroup B strain from Brazilian patients and were found to react with the subtype antigen of prototype reference strains for subtype 9 (M982), as well as with those of homologous strains. The subtype 9 epitope was found in 6.8% of serogroup B strains among 602 strains of Neisseria meningitidis case isolates, including representative isolates from Brazilian states. Subtype P1.9 was predominantly related to serogroup B in Brazil among the isolates collected during the N. meningitidis epidemic in 1992. No significant differences were observed in the occurrence of subtype P1.9 among strains isolated from several Brazilian states. Fluorescence-activated cell-sorter analysis showed that 5F81A4 MAb recognized a 46 kDa protein on the surface of a homologous strain of N. meningitidis (B:4:P1.9). These results, in association with a bactericidal activity assay for 5F81A4, and with experimental passive protection in mice, demonstrated the importance of subtype 9 class 1 proteins of N meningitidis in Brazil. Serotyping is essential for the development of vaccination strategies.

Estimated costs of treatment of benign prostate hyperplasia in Brazil.

INTRODUCTION: The treatment of benign prostate hyperplasia (BPH) presents 2 options: medical or surgical, and there are doubts about what is the best treatment since 80% of patients who undergo surgery become asymptomatic and 10 to 40% of those under medical regimen undergo surgery within a 5 years period. It is difficult to assess the actual costs of treating BPH in Brazil due to several factors, among them regional particularities and the scarcity of current statistical data. PATIENTS AND METHODS: Recently, in the Ribeirao Preto area, Sao Paulo, Brazil, the IPSS (International Prostatic Symptoms Score) and quality of life were verified in 934 volunteers. It was determined the percentage of individuals with ages ranging from 40 to 79 years with moderate symptoms (score 8-19) and with severe symptoms (score 20-35), values for which are indicated medical and surgical treatment, respectively, according to the Brazilian Society of Urology consensus on BPH. Data on Brazilian population in that age range were obtained from the Brazilian Institute of Geography and Statistics referent to the year of 2000. It was determined the number of patients, according to the criteria above, subjected to either one of the treatments mentioned. Surgical costs of prostate transurethral resection were researched according to Unified Health System - SUS tables (173 US dollars) and of Brazilian Medical Society - AMB with a mean cost in 3 hospitals of 933 US dollars. Drug costs were calculated by the annual mean price (355 US dollars) of 4 alpha-blockers (tamsulosin, alfuzosin, doxazosin and terazosin). RESULTS: The estimated population for medical treatment was 5,397,321 individuals, with a cost corresponding to 1,916,489,055.00 US dollars. The estimated population for surgical treatment was 2,040,299 men, what would represent a cost of 353,291,204.00 US dollars based on the SUS table and of 1,904,279,066.00 US dollars based on AMB with hospital expenses included. CONCLUSION: All theses facts induce us to predict that the treatment of BPH in a not-so-far future can become a public health problem for Brazilian society, since the current estimate would be, approximately, costs around 2.26 - 3.83 billion dollars, added by the yearly increase in the risk population (24.99%) for the group under medical treatment and over the non-operated amount of the surgical group.

The federal ergonomic standard in Brazil: its social historic process.

Brazil has a Federal Ergonomic Standard enacted in 1990 that attracts the attention of practitioners in occupational health and safety fields because it is viewed symbolically as a political gain and because of its technical advances. The 1990 ergonomic standard modified a former one that was issued within a set of 28 occupational health and safety regulations established in 1978. This article focuses on the social and historical steps in a persistent workers' struggle for a healthier work environment in the late 1980s that resulted in this federal standard as a "command-and-control" regulation pioneering a wide tripartite process of policy-making in Brazil.

[Breastfeeding indicators in the city of Feira de Santana, Bahia, Brazil]

OBJECTIVE: To evaluate breast-feeding indicators surveying current alimentary status of under- two-year-old children.METHODS: During Vaccinations day, on June 15th, 1996, 3,898 mothers were interviewed in urban areas of Feira de Santana, Bahia, Brazil. The methodology proposed at WHO/CED/SER 91.14 was used. Rates were calculated for the appropriate age groups: breast-feeding exclusive and predominant, persistence of breast-feeding one and two years, median duration of breast-feeding, timely complemented breast-feeding, bottle-feeding and pacifiers utilization.RESULTS: The main results were exclusive breastfeeding rate of 45.6%, in children from 0-3 months old; breastfeeding medium duration of 9 months; and persistence of 45.4% of breastfeeding in children from 12-15 months old. CONCLUSIONS: These results pointed that children in Feira de Santana are benefitting from breast-feeding more than in other areas of Bahia and other states of the Brazilian Northeast reflecting correct orientation. The methodology revealed ease to applicate providing significant and replicable samples. Wider utilization of standardized methodologies will contribute to improve monitoring of breast-feeding indicators in Brazil.

[Risk factors of stillbirths in Fortaleza-Brazil: a case-control study].

Stillbirths are a common event in areas where reproductive health care is poorly delivered, such as the Northeast region of Brazil. This case-control study aimed to identify risk factors associated to foetal deaths occurred in a major obstetric facility of Fortaleza, 1.7 million inhabitants, Northeastern Brazil. 125 stillborn foetus over 20 weeks of gestation (cases) were compared to 250 healthy newborns (controls), in relation to socioeconomic, reproductive, behavioral and morbidity characteristics of their mothers. Crude and adjusted Odds Ratios were then calculated. After adjustment for confounders, the following characteristics of the mother remained as risk factors for stillbirths, with OR statistically significant at the 95% level: attending <5 antenatal consultations during pregnancy (OR=3.30; CI=1.92 - 5.07 ), illiterate mother (OR=3.30; CI=1.84 - 5.92 ), mother's age above 19 (OR=2.73; CI=1.42 - 5.24 ), monthly family income of 1 minimum wage or less (OR=2.12; CI=1.03 - 4.35 ) and severe illnesses or complication during pregnancy (OR=1.75; CI=1.01 - 3.03 ). Inadequate attendance to antenatal care consultations was the risk factor most strongly associated to stillbirths. Similarly, it was the condition most amenable to change in a short term, among those identified as risk factors.

[Health and work in rural areas: sugar cane plantation workers in Ribeirão Preto, São Paulo, Brazil]

This study is based on an understanding of health and rural labor as a social process related to the characteristics of the agrarian issue in Brazilian society, focusing on sugar cane cutters in northeastern São Paulo State, Brazil, and attempting to identify patterns of attrition and reproduction in such labor. Based on direct observations, interviews with laborers and other agents involved in production, and a related bibliographical review, the analysis points to daily exposure of cane cutters to physical, chemical, and biological hazards resulting in various diseases, traumas, and accidents: dermatitis, conjunctivitis, dehydration, cramps, dyspnea, respiratory infections, high blood pressure, and wounds, besides aggravating the biopsychic burden underlying attrition patterns showing up in the spinal column, chest, head, and lumbar pains, nervous breakdowns, and other kinds of psychosomatic manifestations. This study of the work process pointed up its unhealthy working conditions and helped provide an overview of the conditions and means by which capital asserts itself in Brazil - in its self-reproduction process - especially in the agro-industrial sector.

[Breastfeeding and feeding patterns in two cohorts of children in southern Brazil: trends and differences]

Breastfeeding is fundamental for infant health. However, its median duration in Brazil is very short, and national campaigns since the mid-1980s have attempted to revert this situation. In the present paper, data on breastfeeding are compared for two population-based cohorts of children born in 1982 and 1993 in the city of Pelotas in Southern Brazil. All hospital births in both years were studied and samples of these children were visited at home around 12 months of age. Median duration of breastfeeding increased from 3.1 to 4.0 months in this period. This rise suggests an impact by the national campaigns. In both cohorts, there were interactions between family income and the percentages of children breastfed at different ages. In the early months, breastfeeding was more prevalent in high-income families, but from nine months onwards it was more common among the poor. Low birthweight babies enjoyed shorter duration of breastfeeding. Despite the progress observed during the decade, duration of breastfeeding is still far short of international recommendations, justifying further campaigns prioritizing low birthweight babies and those from low-income families.

Serologic Testing for Chagas' Disease and HIV in Counseling Programs and Blood Banks in Midwest Brazil.

For many years it has been the practice in Brazil to question donors and to test blood to be used in blood banks for HIV, Chagas' disease, hepatitis B and C, and syphilis. Counseling and blood testing programs have recently been established to assist in identifying and treating those with HIV. In a blood bank center in Campo Grande, Mato Grosso do Sul State (MS), we evaluated the frequency and age of patients with positive tests for T.cruzi in order to determine whether the frequency and age distribution indicated that this infection should be added to the counseling and testing programs. A group of 476 first time blood donors were enrolled to answer a questionnaire and to be tested serologically for Chagas' disease, HIV, hepatitis B and C, and syphilis. A standard ELISA and an IFA test were used for Chagas' disease patients. Five patients (1.1%) were serologically positive for T.cruzi infection. All five were male between 22 and 35 years of age. None of the five had been born in Mato Grosso do Sul State. Four donors donated blood in order to obtain serologic test results, only one donated blood as a social responsibility. One was HIV positive and related previous STD, and one had received a blood transfusion between 1980 and 1986. Each had had only one sexual partner in the last six months. The age range and the frequency of 1.1% suggest that it would be beneficial to include this disease in counseling and testing program in Brazil. The limitations of this approach are the relatively low attendance at such centers in Mato Grosso do Sul (1.7 people/day compared to a national average of 2.9 people/day), the slow reporting time for results (1 week compared to 1 day for the blood bank reporting), and the absence of a standard disease-specific testing procedure. We recommend increasing the use of serologic tests for T.cruzi and designing appropriate questionnaires in counseling and testing programs now used primarily for HIV, assuming the existing limitations can be overcome.

Human Fascioliasis in The Metropolitan Area of Curitiba, Brazil Evaluation of The Foci of Infection and Report of Nine Cases Treated With Triclabendazole.

In Brazil, only 35 cases of human fascioliasis have been reported. Several drugs have been used to treat Fasciola hepatica in humans, including praziquantel, and, more recently, triclabendazole. After three patients were diagnosed as having human fascioliasis by routine stool parasitological examinations done at Hospital de Clínicas UFPR, a study was done to determine the frequency of the infection in people and cattle from the surrounding area. Stool samples from 185 people and from 20 bovines were examined. Nine bovines' exams were positive for Fasciola hepatica and six new diagnoses of human fascioliasis were made. After taking a medical history, physical examination, routine blood examinations and biliary imaging in the 9 patients, therapy was given with praziquantel 75mg/Kg daily for 5 days. After 30 and 60 days stool examinations were made to evaluate the therapy's efficacy. All patients were asymptomatic. Hematological examinations were normal, and only one patient had an abnormal biliary system on the image study (echography). In the nine patients, praziquantel did not eliminate the fasciola eggs. Triclabendazole (a benzimidazole licensed for veterinary use) was therefore tested, 10mg/Kg one oral dose. Triclabendazole was effective and well tolerated in all patients. We conclude that fascioliasis is more common in some regions of Brazil than previously reported, and that single dose triclabendazole is effective and well tolerated in treating human fascioliasis.
