Using NCAST and the HOME with a minority population: the Alaska Eskimos.

PURPOSE: To test applicability of the NCAST and HOME assessment tools in the Eskimo population. METHOD: A cross-sectional descriptive study of Alaskan native families from two villages used the NCAST and HOME child assessment tools. Comparisons using t-test were made to normative data. ANCOVA was used to test effects of demographic differences. RESULTS: Several significant differences were found on subscales of the NCAFS, NCATS, and HOME, although the total scores of the NCAFS and the NCATS were not significantly different. Caregiver education and ethnicity were significant covariates. CONCLUSIONS: NCAFS and NCATS appears to have sufficient breadth to allow for cultural differences. The HOME scale items measure some aspects of the environment that appear to be culture specific.

Comments on this issue of Alaska Medicine--from then (1960) until now (1993).

The drugs used in frostbite injury care are: Plasma volume expanders (low molecular weight dextran); vasodilating agents (tolazoline hydrochloride); hypotensive agents (guanethidine monosulfate, reserpine); hemorrheologic agents (oxpentifylline); calcium blocking agents (nifedipine); sympatholytic agents (phenoxybenzamine hydrochloride); anticoagulating agents (heparin); thrombolytic enzymes (streptokinase, tissue plasminogen activator--TPA); an industrial solvent (dimethyl sulfoxide--DMSO); anti-inflammatory agents such as nonsteroidal drugs, and acetylsalicylic acid, Ibuprofen. As yet, no clear treatment policy has been determined for preventing injury secondary to the formation of oxygen free radicals, damaging neutrophils or reperfusion injury. The role of oxygen free radical scavengers and factors causing reperfusion injury is unclear at this date. Since that first reported series of 51 patients in 1960-61, 1,282 patients have been seen. Of that number, 1,026 had a diagnosis of frostbite; 151 were diagnosed as hypothermia; and 105 diagnosed as immersion injury.

Changing identification among American Indians and Alaska Natives.

The census-enumerated American Indian population dispersed rapidly between 1930 and 1990. Changes in ethnic classification account for most of the change. In the 1980 count, 10 states with historically large Indian populations account for 53% of births of Indians between the ages of 10 and 80, compared with 72% of the first enumerations of the same cohorts. Migration further reduced the share of these states to 46% of Indian residents in these cohorts. Study of the dispersal of the Indian population should focus primarily on the new emergence of the expression of Indian identity, rather than on migration from former population centers.
