Association of caries experience in 12-year-old children in Heidelberg, Germany, and Montpellier, France, with different caries preventive measures.

UNLABELLED: In Germany and France, different approaches to oral health promotion have been chosen. OBJECTIVES: It was the aim of the present study to assess whether different preventive measures were positively or negatively associated with caries experience in 12-year-old children in Heidelberg, Germany, and in Montpellier, France. METHODS: In the school year 1998/99, 12-year-old children were examined in Heidelberg (n=864) and in Montpellier (n =828). D3+4MFT, D3+4MFS (D3+4=dentinal caries), prevalence of fissure sealants and use of fluorides were recorded. RESULTS: The mean D3+4MFT scores of the pupils in Heidelberg and Montpellier were 1.56 and 1.42, the proportion of caries-free children being 43.5% and 46.9%, respectively. In 63.2% of the children in Heidelberg and in 6.8% of the children in Montpellier, at least one fissure sealed tooth was found. Fluoridated salt was used by 38.8% of families of the 12-year-olds from Heidelberg and the corresponding value was 73.3% in Montpellier. Topical fluorides were applied by 39.9% of the children in Heidelberg, but only by 5.4% of pupils in Montpellier. Multiple regression analysis revealed that, in Heidelberg, fissure sealants and the combination of topical fluorides and fluoridated salt were negatively associated with the D3+4MFT values, while in Montpellier the frequency of dental visits was positively associated with the D3+4MFT scores. CONCLUSION: A further decrease in the caries experience of 12-year-old children may well be obtainable by enlarging the share of marketed fluoridated salt in Heidelberg, by increasing the application of fissure sealants in Montpellier, and by extending the use of topical fluorides in both cities.

Dietary pattern and dental caries in 19-year-old adolescents subjected to preventive measures focused on oral hygiene and/or fluorides.

Dietary habits and dental caries were analysed in 95 19-year-old Swedish adolescents who had been subjected to different preventive measures during their late teens. A 24-hour recall was used and showed that the intakes of milk and milk products, meat, fish and eggs accorded with recommended norms but not those of vegetables, fruits and berries, and potatoes and root vegetables. Only 6% of the subjects had eaten the recommended three principal meals during the day tested. At the end of the trial, only weak or moderate correlations were found between snacks, including sucrose snacks, and selected clinical variables. Subjects who did not develop new carious lesions during the experimental 2 years had fewer risk factors, defined as the most negative values (quartile limit) for sucrose snack consumption, tooth cleaning score, lactobacillus count, saliva secretion rate and saliva buffer capacity, than those who developed one or more new lesions. During the experimental period, with an overall low caries incidence, no single factor tested for presumed risk was found to be the discriminating factor for occurrence of dental caries.

Relationship between cigarette smoking and other coronary risk factors in atherosclerosis: risk of cardiovascular disease and preventive measures.

Among the major Coronary Risk Factors (CRF) cigarette smoking has shown undoubtedly harmful effects on the heart and blood vessels either as active smoking (smoking a cigarette) or passive smoking (exposure to environmental tobacco smoke -ETS). The strong relationship between cigarette smoking and cardiovascular disease has been seen independent of the other CRF in a number of well-designated epidemiologic studies. However, a strong increase in the excess of cardiovascular risk has been defined along with the interaction of cigarette smoking and other major CRF. Thousands of pharmacologically active substances are present in tobacco smoke, and a large number of direct and indirect effects have been demonstrated. Different responses are also related to these types of exposure: active exposure or passive exposure. The cardiovascular risk increases with increasing levels of blood pressure and/or serum cholesterol and diabetes mellitus, and at each level of these three risk factors, distributed with different rates according to age and gender in individuals, the risk in active smokers or passive smokers is greater than the risk in nonsmokers. Further analytical and methodological observations are needed for better understanding of the chemical and biological synergism. Nevertheless, evidence is clear that cigarette smoking greatly increases the risk of cardiovascular diseases in individuals already at increased risk because of other CRF. Preventive measures must be absolutely conducted to prevent the CRF interaction. These are the changes in lifestyle (i.e. to give up smoking and make physical activity), drug administration, diet supplementation especially by those substances with antioxidant effects.

[Maternal-fetal transmission of HIV: preventive measures and prenatal care].

HIV can be transmitted to foetus during pregnancy, labour, and breastfeeding. An estimated 600,000 newborns acquired HIV each year through mother to fetus transmission. Without preventive interventions, approximately 25% of infants born to HIV positive mothers contract the virus. Elective delivery by caesarean section at 38th weeks, before labor and rupture of membrane, decreased the fetal risk. Anniotomy and long-standing rupture of the membranes should be avoided, as should breastfeeding. As well based on the review of literature the authors provide an overview of vertically acquired of HIV infection, preventive measures and perinatal car are discussed.

[Transfusion-transmitted malaria, preventive measures].

Malaria is a blood parasitic disease in the first place among the most prevalent communicable infectious diseases over the world, which leads to an increased risk of transfusion transmitted malaria. Preventive measures have been undertaken to screen blood donors such as discarding red cell donations according to the medical history, travel history and detection of malarial antibodies. However, these measures may be not sufficient and reliable to avoid the risk of transmission. Preliminary data indicates that combination of travel history, detection of malarial antibodies and antigens by commercialized kits adapted to blood transfusion centres either in endemic or non endemic areas may improve malaria transfusion risk management.

Anti-HCV antibody presence in hemodialyzed patients: assessment after one year regarding some preventive measures chosen.

The authors report on the presence of anti-HCV antibodies in a uremic chronic (UC) patient population after 6 months and 1 year with regard to certain selective preventive measures. The nonincreased number of HCV-positive subjects among the UC population could lead to the conclusion that the risk of environmental spreading of HCV is less than the risk of HBV diffusion.

Use of malaria prevention measures in Malawian households.

Information on malaria prevention practices in households was obtained in a nation-wide knowledge, attitudes, and practices survey in Malawi. Of the 1,531 heads of household questioned, 55% were able to identify mosquitoes as the cause of malaria. Use of any type of malaria prevention method was reported by 52% of respondents. Among users, 47% used commercial products (insecticide, mosquito coils, bednets), and 64% used natural measures (burning leaves, dung, or wood); 11% used both. The most common commercial measure used was mosquito coils (16%) followed by insecticide spray (11%) and bednets (7%). Increasing household income and educational level of the household head were strongly correlated with use of commercial methods to prevent malaria; households with an income ranked moderate or greater were eight times more likely to have used a purchased product. Use of natural measures was correlated with lower income and educational level. Thirty-six percent of respondents reported having heard or seen information on malaria in the previous year. Use of household malaria preventive measures in Malawi is very low and income-dependent. Educational messages are required to improve understanding and use of affordable measures.

[Report on the activities carried out in the research project of the Ministry of Instruction, University, and Research entitled "Environmental and occupational exposure to inorganic lead: assessment of toxic effects of current doses and related preventive measures"].

It is here presented the project sustained by the Ministry of Public Instruction, University and Research "Environmental and Occupational Exposure to Inorganic Lead: assessment of human health effects due to current doses and preventive measures" ruled out between 2001 and 2003. The aim of the study was to investigate about the toxic effects related to current occupational exposures to inorganic lead (particularly those effects concerning blood pressure, carcinogenic risk, nervous and immunological systems), to identify hypersusceptibility conditions, particularly the ALAD genetic polymorphism and to evaluate the role of traditional biomarker and the possibility of introducing new ones. In the present article the procedures followed during the project lasting and the contribution of each Unit are described. The results of the research, presented in detail in the current issue, do confirm the inadequacy of the biological exposure index nowadays ruled by Lex 25/2002.

Effect of preventive measures in 50-60-year-olds with a high risk of dental caries.

Salivary and microbial conditions were examined in 124 50-60-yr-old patients attending a dental clinic of the Public Dental Health Service. They were then randomly divided into a control and a test group. Patients with low salivary secretion rate and buffer capacity and high numbers of mutans streptococci and lactobacilli in the test group received special caries preventive measures during 1 yr. In the control group all patients were given prevention according to routine procedures. The incidence of dental caries between the groups was compared. The risk patients in the control group developed 1.67 new carious lesions as compared to 0.19 in the test group. The difference was statistically significant (P less than 0.05). Also the numbers of mutans streptococci and lactobacilli were significantly lower in the risk patients in the test group after 1 yr compared with baseline values. After the experimental year all patients were treated according to routine procedures. Five years after the start of the study a new examination showed increased caries activity in the risk patients in the test group and the gain which had been made during the experimental year was almost lost.

[Yersinia enterocolitica infection. Epidemiology, risk factors and preventive measures].

Yersinia enterocolitica is a significant cause of gastroenteritis in Norway. The infection may provoke a number of serious postinfectious complications, most notably non-purulent arthritis. This article reviews aspects of epidemiology, risk factors, and prevention of the disease, with special emphasis on a case-control study conducted in south-eastern Norway. The results indicate that the following preventive measures are likely to have the greatest impact on the occurrence of the infection: (a) reducing Y enterocolitica contamination of pork and pork products by changes in slaughtering procedures and improvement of hygiene during production and processing, (b) educating consumers about the importance of good hygienic practices during handling and preparation of food and sufficient cooking of meat products, and (c) providing the consumers with drinking water of adequate hygienic quality.

Control of perinatal infection by traditional preventive measures.

A 9-year review of nosocomial infections occurring in the department of obstetrics and gynecology of a major teaching hospital demonstrated the dramatic effect of strict asepsis and isolation on postoperative infection rates. With rigidly enforced preventive measures and without the use of prophylactic antibiotics, the rates of wound infection and endometritis and the relative prevalence of gram-negative aerobic organisms cultured from these infections could be reduced substantially. The role of anerobes in these infections was not established, since adequate techniques for the accurate diagnosis of anaerobic infection were not available at the time of this study. The relative importance of the numerous infection-control measures used is unclear. It is apparent, however, that by implementing all traditional methods of asepsis, virtually all types of nosocomial infection could be controlled, and the increased prevalence of gram-negative infections observed during the past decades could be reversed.

[Overweight and obesity in children and adolescents and preventative measures].

The prevalence of overweight and obesity in children is increasing rapidly. This is alarming because obesity is associated with severe chronic diseases, such as type 2 diabetes mellitus. Obesity at young age is related to obesity at adult age. Consequently, the prevention of overweight from childhood onwards is an important issue. Apart from diabetes mellitus type 2 there is an increased risk of orthopaedic complications, respiratory problems, fertility problems, cardiovascular diseases and psychosocial consequences in the form of a negative self-image, emotional and behavioural problems and depression. Environmental and behavioural factors are regarded as the most important causes of the rapid increase in the prevalence of overweight and as the most important starting points for prevention. Most prevention programmes are still in the initial stages. Prevention programmes aimed at stimulating breast feeding and daily physical activity (playing outside) and the restriction of sweetened drinks and watching TV are very promising. With such preventive measures the involvement of both the school and the parents is important.

[Epidemiological studies on viral hepatitis among long-term sojourners in the developing countries and evaluation of preventive measures].

It is known that acute viral hepatitis is very common among sojourners in developing countries. In order to conduct effective health control, epidemiological studies were made on viral hepatitis which occurred among Japanese staying in developing countries, and evaluations were carried out on preventive measures. The subjects of present study were a group of Japanese people staying in developing countries for two years. Mid year population of the group was 1732 in 1988. Period of the present study is ten years from 1979 to 1988. The study was conducted based on the reports from offices in each country, survey trips and serological studies on the subjects. In 1979, frequency of hepatitis A (HA) was very high showing 79% of total hepatitides. However after starting of inoculation of human immune serum globulin (ISG), the frequency of HA declined remarkably. Statistical significance was recognized in the efficacy of ISG. Among 35 cases of hepatitis B (HB) (34 males and 1 female), 2 derived from HBe antigen carrier while the remaining 34 were regarded as infected during their stay in developing countries. No cases of HB were recognized among those who received injections of HB vaccine. The rate of people whose HB marker turned positive during their stay is showing a tendency of increase (4.2% in 1987). The frequency of infection with HB virus is especially high in Asian and African countries where the carrier rates of native people were also high. Ten cases of non-A non-B hepatitis were recognized. Among them 7 were infected in Asia, 1 in Africa and 1 in Central America.(ABSTRACT TRUNCATED AT 250 WORDS)

Health care coverage: traditional and preventive measures and associations with chronic disease risk factors.

Physician counseling of patients on health related activities is an essential component of chronic disease prevention, however this requires patients to have ready access to health care providers. Previous studies have explored access to health care in terms of health plans and cost without accounting for the lack of preventive coverage inherent in many insurance policies. This study compares two measures of health care access, one using an assessment of cost and health plan availability, and a new coverage measure including preventive services. Data was collected from 2574 adult respondents to the 1991-92 Missouri Behavioral Risk Factor Surveillance System Surveys. Odds ratios were generated for demographic variables, health related behaviors and preventive screening and the two coverage measures. Using health plan and cost 22% lacked full coverage, however including availability of preventive coverage almost 60% lacked full coverage for preventive care. For both coverage measures significant associations were found with age, exercise, marital status, routine checkup and mammography screening. Using the measure of coverage of preventive services, rural residents and those who had never had cholesterol screening were more likely to lack coverage. Inclusion of preventive care in measures of health care coverage may alter previously reported associations with socio-demographic and health related factors. Policy makers should realize that including preventive services in health care coverage greatly increases the number of individuals lacking adequate coverage, and that those lacking adequate coverage are the least likely to undergo preventive screening.

[Health effects of occupational exposure to cadmium and its compounds and proposed preventive measures].

The paper presents effects of cadmium compounds upon human organism. Special attention was paid to disturbances resulting from chronic occupational exposure to those compounds. In addition, the description of toxic effects was supplemented with suggestions to carry on pre-employment and periodic examinations that would enable undertaking appropriate preventive measures for the population occupationally exposed to cadmium compounds.

Early complications in second toe to hand transplantation-the occurrences and preventive measures.

The operative experience of twenty cases of second toe to hand transplantations was discussed. Although there was only one total failure, there were frequent complications. The factors leading to the high rate of complications (just over 1/3 of cases) were analysed. A treatment scheme, which took into consideration a large number of precautions and preventive measures against early complications was carefully outlined.

MRSA in The Netherlands: preventive measure raises a moral issue.

In the Netherlands the incidence of methicillin resistant Staphylococcus aureus (MRSA) infections in hospitals is surprisingly low when compared to that of neighbouring countries. It is believed that this favourable condition is caused by stringent precautionary measures such as complete isolation of the patients. In one case the nurse taking care of such patients was herself MRSA positive. This condition changed and she became MRSA negative after removal (at her request) of her tonsils. The question is raised how far one should go to protect patients when the preventive measures taken may endanger the health of those who take care of them.
