A new method for IgE detection in nasal mucosa.

A new method for determining IgE antibodies on mucosal surfaces has been developed with the purpose of overcoming the main problems of nasal secretion sampling and standardization. The method is based on the principle of performing the incubation of the solid-phase coupled allergen with IgE antibody directly on the mucosal surface by means of a proper applicator. About two times higher values of specific IgE have been obtained with 5 min incubation on septal mucosa, behind the internal ostium, than with 3 hr in-vitro incubation with native secretion. In a study of 53 children with allergic rhinitis and asthma and 10 healthy non-atopic controls the sensitivity and specificity of this new method were evident. Because of its reliability and easy execution the new method could be widely used in diagnosis of allergic disease. Further, it seems to offer a good opportunity to study the IgE-mediated reaction in the target organ more extensively.

Low resolution electromagnetic tomography: a new method for localizing electrical activity in the brain.

This paper presents a new method for localizing the electric activity in the brain based on multichannel surface EEG recordings. In contrast to the models presented up to now the new method does not assume a limited number of dipolar point sources nor a distribution on a given known surface, but directly computes a current distribution throughout the full brain volume. In order to find a unique solution for the 3-dimensional distribution among the infinite set of different possible solutions, the method assumes that neighboring neurons are simultaneously and synchronously activated. The basic assumption rests on evidence from single cell recordings in the brain that demonstrates strong synchronization of adjacent neurons. In view of this physiological consideration the computational task is to select the smoothest of all possible 3-dimensional current distributions, a task that is a common procedure in generalized signal processing. The result is a true 3-dimensional tomography with the characteristic that localization is preserved with a certain amount of dispersion, i.e., it has a relatively low spatial resolution. The new method, which we call Low Resolution Electromagnetic Tomography (LORETA) is illustrated with two different sets of evoked potential data, the first showing the tomography of the P100 component to checkerboard stimulation of the left, right, upper and lower hemiretina, and the second showing the results for the auditory N100 component and the two cognitive components CNV and P300. A direct comparison of the tomography results with those obtained from fitting one and two dipoles illustrates that the new method provides physiologically meaningful results while dipolar solutions fail in many situations. In the case of the cognitive components, the method offers new hypotheses on the location of higher cognitive functions in the brain.

Evaluation of a new method for assessing symptoms.

BACKGROUND: Many studies in rhinology are based on results from symptom scoring. Numerous methods are used to estimate symptoms. With some of them the patients choose between various alternatives for estimating the severity of their symptoms, but the commonest method is the visual analogue scale (VAS), which is regarded as reliable. METHODS: In this study, we evaluate a new method for assessing symptoms, with which the patient uses the telephone to call a computer. We compare the results using the new method with those using symptoms estimation with VAS. RESULTS: The results are based on 487 observations in patients with birch pollen allergic rhinitis in the pollen season. A high correlation was found between the results using the new method and those using the VAS. In a questionnaire, the patients answered questions concerning the two methods. Most patients preferred the new telephone method. They found it easier to estimate their symptoms properly and easier to perform. CONCLUSION: We conclude that telephone scoring of symptoms is a reliable and adequate method when symptom scoring is used as hard data in a study.

A new method of measuring dimensional change.

A new method of measuring linear dimensional change in denture base resins is presented and its precision in determining distances between two points was investigated. Chrome steel ball bearings 1.57 mm in diameter were invested in a stone mold and were used to record measurements by a reflection from the surface of the ball bearings. The accuracy of this method was determined by performing repeated measurements between two points in a stone mold. Random measurements were made to an accuracy of 0.002 mm. The dimensional change of light-cured, heat-cured, and self-cured denture base resins was also determined with this method, followed by an analysis of variance of the data. The findings support the accuracy and reproducibility of this new method in recording linear measurements. The dimensional changes of all three resins were different from each other after the specimens were processed and stored in distilled water for 7 days at 37 degrees C.

Follow up studies on various reconstruction methods of the biliary tract including our new method (Roux Y-duodenojejunal anastomosis).

Thirty six patients with benign diseases of the biliary tract (14 patients with congenital choledochal dilatation, 15 patients with postoperative stricture and 7 patients with others) were divided into three groups: 21 patients who underwent a Roux Y (RY), 7 patients who underwent a jejunal interposition (IP) and 8 patients who underwent a side to side anastomosis between the jejunal limb of the Roux Y and the duodenum (RY-DJ). The RY-DJ was designed to decompress the Roux Y jejunal limb and to allow an inflow of bile into the duodenum. Significant complications, including cholangitis, infection, or abdominal pain, developed in 10 of the patients with RY (48 per cent), 7 of the patients with IP (100 per cent) and 1 of the patients with RY-DJ (13 per cent). None had a postoperative peptic ulcer. Simultaneous scintigraphy showed the time required for the two agents, 99mTc-IDA and 111In-DTPA, to mix at the upper jejunum, which revealed that the time taken by the patients with RY-DJ was similar to that of the patients with IP and to that of healthy controls. The time was markedly longer in the patients with RY, presumably due to a prominent stasis of the bile tracer in the Roux Y jejunal limb. Our new method (RY-DJ) for reconstruction of the extrahepatic biliary tract is more physiological and has less postoperative complications than other conventional methods.

New method for concentration and quantitation of Mycobacterium leprae.

A new method of enumerating Mycobacterium leprae has been developed. Suspensions containing the organisms were filtered through a polycarbonate membrane filter (25-mm diameter, 0.4-micronm pore size, 10-micronm thick; Nucleopore) to concentrate the organisms. The membrane was then mounted on a glass slide and stained with a standard acid-fast stain. Finally, the membrane was treated with a small amount of chloroform to fix it to the slide and make it transparent. This method enabled us to detect M. leprae in quantities as small as 4.98 X 10(2) regardless of the total volume of the original material. Comparison with a standard method for enumerating M. leprae showed that both methods gave similar results when the organisms counted by the standard method were present in sufficient quantity for reproducibility. Because the least number of organisms that can be detected with the standard method is 10(4) ml and because the organisms detected with the new method could be concentrated on the polycarbonate filter from a large amount of infected fluid, a substantial number of suspensions were shown by the new method, but not by the standard method, to contain M. leprae.

Vaginal molds for intracavitary curietherapy: a new method of preparation.

A new method of preparing vaginal molds for afterloading intracavitary brachytherapy is described. Our technique makes it possible to obtain the most accurate individualization of therapy as far as dose distribution is concerned by taking into account the patient's anatomy and target volume.

Eliminating the gap in flexor tendon surgery. A new method of suture.

A new method of tendon suture has been designed to eliminate the development of a gap when tension is applied to the repair. It has been demonstrated to resist distraction at relatively high tensions on cadaveric tendon. The possible advantages of the technique are discussed.

A new method to test genetic models in HLA associated diseases: the MASC method.

We propose a new method to analyse data on HLA associated diseases. The method uses the simultaneous information on the marker associations and segregation with the disease. It may also take into account the differential risk of being affected for specific relatives of a patient as well as the differential HLA haplotype sharing according to the marker genotype of the patient. It is based on the principle of minimization of a sum of independent chi-squares. It allows us to test the goodness-of-fit of various models in an easy and economical way. The method is applied to a sample of 269 French IDDM patients and their relatives leading to the rejection of models with one locus closely linked to HLA with two and three alleles.

New methods for determining suggestibility.

New method for determining suggestibility is described which elicits anesthesia in the finger tips by verbal suggestion. Suggestion is applied during simultaneous tapping and the effect is evaluated by feelings in the active hand or fingers (besides changes in feeling) and by motor disturbance (inhibition). The first results are reviewed which indicate that though the method has a relatively high threshold it can disclose also some intergroup differences. We believe that this is an objective method, its objectivity is primarily due to the fact that the investigated subject cannot anticipate what kind of result the suggestion is supposed to yield.

Measurement of bronchial hyperreactivity in infants and preschool children using a new method.

We report a new method for examination of bronchial reactivity by measuring transcutaneous oxygen pressure (tcPO2) during the inhalation of histamine in stepwise incremental concentrations. The correlation between changes in tcPO2 and those in PEFR or FEV1 was high (P less than .001). When the fall in PEFR or FEV1 was more than 20% compared with baseline, the fall in tcPO2 was more than 10%. We also measured the dose of radiolabeled aerosol (99mTc-DTPA) inhaled into the lung using a scintillation camera. When aerosol is inhaled during tidal breathing, there are adequate correlations between the dose of aerosol in the lung and both the age and height of the patients (P less than .001). When aerosol was inhaled during crying, little deposit within the lung was shown. When the inhalation challenge test was done during a sleeping period for children aged under 2 years, the loading dose of aerosol in the lung was sufficient. The geometrical means of respiratory threshold of histamine (RT-Hist) among 106 asthmatic and 11 control children, aged 2 to 6 years, were 1182 and 4414 micrograms/mL, respectively. The means of RT-Hist in 17 children with bronchial asthma, nine with atopic dermatitis, five with respiratory disease, and nine controls, aged under 2 years, were 1152, 964, 544, and 3402 micrograms/mL, respectively.

Stepping frequency and stride length in animal locomotion: a new method of investigation.

A new method of investigating animal locomotion is presented which allows for the simultaneous measurement of the basic overground locomotion parameters: swing and stance durations, stride lengths, velocity of movement, gait diagrams and temporal relationships between all animal limbs. The measurements and recordings are given in detail to exemplify the application of the method in dogs.

A new method of cranioplasty.

A new method of cranioplasty is described in which the skull defect is exposed and multiple angled holes through the outer cortical bone are made. Methyl methacrylate is molded into the defect continuous with the perforating holes. The major advantage of this technique is the resulting good cosmetic appearance, in addition to the strength and firmness of position it provides.

A new method for quantifying encephalization in the growing individual.

Here we describe a new method for quantifying encephalization in the growing individual and provide a worked example of the methods. The new method is based on the use of conditional SD scores derived from brain and body growth references. These encephalization SD scores control for age, sex and body size effects on brain size, and therefore, control for the confounds associated with allometry as well as growth differences between the brain and body and between the sexes. The methods also control for distribution skewness. Encephalization SD scores derived from pre- and post-natal data may be directly compared and changes in SD score over time assessed. These methods may be applied to a broad range of data where relative size during growth is to be quantified. Derived SD scores may also be applied to correlation and regression analyses where statistical relationships with other variables are of interest.

Assessment of childhood intracranial pressure recordings using a new method of processing intracranial pressure signals.

BACKGROUND: Intracranial compliance may be more reliably predicted by the pulsatile component (pulse pressure) than the steady (mean pressure) component of intracranial pressure (ICP). A new method of processing continuous ICP signals assessing both components of ICP is described and applied to the ICP recordings of 6 pediatric cases. METHOD: The new method was applied to each subsequent 6-second time sequence window of a continuous ICP signal. For time sequence windows including single ICP waves, the following time sequence (TS.x)-related parameters were computed: (a) mean ICP (i.e. TS.MeanP) was computed according to the currently used and known technology; (b) the mean ICP wave was computed according to the new method, characterized by mean wave amplitude (i.e. TS.MeanWavedP) and mean wave latency (i.e. TS.MeanWavedT). Cases No. 1-4 were treated for hydrocephalus and cases No. 5 and 6 for craniosynostosis. RESULTS: In 5 children, clinical intracranial hypertension was associated with elevations of mean ICP above 15-20 mm Hg of variable durations. The ICP recordings of the 5 children with intracranial hypertension and successful outcome after surgery revealed mean wave amplitude values above 5 mm Hg. Mean wave latency was more variable, ranging between 0.10 and 0.25 s. CONCLUSIONS: In the children with intracranial hypertension and successful outcome after surgery, mean wave amplitude was variably above 5 mm Hg. It is suggested that mean wave amplitude may be a useful parameter by more directly predicting cerebral compliance than mean ICP.

Variable pressure infrared diode laser spectroscopy: a new method for trace-gas monitoring.

A new method for measuring trace concentrations of atmospheric pollutants by infrared diode laser spectroscopy has been devised. This method relies on the increase of the signal as the pressure inside the cell increases, while the frequency of the diode is stabilized on the line, even if it is unresolved. Performances of this method were tested with N2O and with 1,3-butadiene. As an example of application, we measured the butadiene emitted by car exhausts. Sensitivity and rapidity of this method are equivalent to the usual scanning method in which the whole line is described, but this new method benefits from its simplicity and robustness.

Clamp delay: an effective new method of nonsurgical delay.

This study introduces an effective new method of nonsurgical delay. In this new method, a special clamp that compressed a bipedicled skin fold along the sides of a proposed flap was glued to rat dorsa. The study also used a control group of untreated flaps and a group of flaps delayed by the conventional surgical procedure involving conventional parallel incisions. Eight days later, 1 x 6 cm reverse McFarlane flaps were isolated from the wound and raised. After 5 days, the survival length of the flaps was measured. Viability of the flaps delayed by the clamps [40.5 +/- 2.0 mm (mean +/- standard error); n = 10] did not differ from that of surgically delayed flaps (41.3 +/- 3.6 mm; n = 8) and was significantly higher (p < 0.001) than the survival of control flaps (26.3 +/- 0.6 mm; n = 10). Clamp delay can be useful in flap research that explores the relative significance of the vessel obstruction and biochemical processes that follow the surgical delay procedure. This method also offers a new perspective by introducing the concept of nonsurgical delay into clinical practice.

The radioulnar ratio: a new method of quantifying distal radioulnar joint subluxation.

Computed tomography was used to image the distal radioulnar joint (DRUJ) for instability. Four methods were used to quantify subluxation of the DRUJ: the Mino criteria, the epicenter method, the congruency method, and a new method called the radioulnar ratio (RUR). Validity of the various methods was evaluated in clinical and laboratory situations. Rheumatoid patients with symptomatic DRUJ pathology had significantly more abnormal RUR values (100% vs 73% [epicenter method] and 88% [Mino criteria]). The RUR detected instability sooner in a progressive laboratory-induced instability model. The intraobserver and interobserver reliability of the RUR was high, with intraclass correlation coefficients of 0.89 and 0.87, respectively. The RUR demonstrated superior performance in the diagnosis of DRUJ subluxation.
