The case reported concerns a 76-year-old woman under treatment for a previously diagnosed "poorly differentiated endocervical adenocarcinoma". New biopsies revealed an adenocarcinomatous tumor with unexpected melanotic pigmentation. The patient underwent cesium therapy followed by colpohysterectomy with lymphadenectomy. As there were no metastases, external complementary radiotherapy was not used. Four months after surgery, a large recurrence was detected; surgical excision proved impossible but revealed a grossly pigmented tumor from which several samples were taken. The patient died 11 months after the first consultation. No autopsy was performed. Morphological study was done on the initial biopsy, on the uterine tumor and on the recurrent tumor, using histological, cytological, ultrastructural and immunohistochemical techniques. Flow cytometry and biochemical study were also carried out on the recurrent tumor. All the samples studied histologically revealed uniform tumor morphology showing a poorly differentiated adenocarcinoma with an irregular distribution of melanin pigmentation (Fontana +). Electron microscopy confirmed the epithelial nature of the tumor, showing differentiated apical poles with villosities, linked by desmosomes. Basement membranes were irregularly present. Electron microscopy also demonstrated the melanotic nature of the pigmentation with melanosomes and premelanosomes. A few membrane-bound neurosecretory granules were seen. Immunohistochemistry showed that the tumor contains no S 100 protein and that no staining was obtained with monoclonal antibodies against malignant melanoma. Hormonal secretion and chromogranin were not detected. Tumor cells contained neither GFAP nor neurofilaments. Positive staining was obtained for neuron specific enolase and synaptophysin. Tumor cells contained three types of intermediate filament proteins = Vimentin, cytokeratins and peripherin (peripherin is an intermediate filament protein identified in 1984 by Portier, of the college of France, who very kindly supplied the antiserum and was good enough to do most of the biochemical study. Peripherin is considered to be characteristic of the peripheral nervous system. This case is the first example of demonstration of peripherin in a tumor). The biochemical study gave the following results: Cytosol assays for estrogen and progesterone receptors were negative. Vimentin, cytokeratins and peripherin were demonstrated by a study carried out in the Collège de France. No GFAP was found. A study of the metabolism of melanin derivatives showed high levels of urinary dopamine, serum and cytosol L. dopa.(ABSTRACT TRUNCATED AT 400 WORDS)

The official mortality statistics in Austria fail to report the localization of uterine cancer in nearly two thirds of cases and, thus, the annual death rate of women with cervical versus endometrial cancer is not clear. These two diseases behave totally differently with regard to their aetiology, age distribution, early diagnosis, therapy and prognosis and, therefore, a separate analysis is necessary. This study presents an analysis of the incidence of uterine cancer in a series of 7,276 women who died in Vienna hospitals in 1976. Of the 151 cases (2% of all the women in the examined collective) 106 died of cervical carcinoma, 33 of endometrial carcinoma and 9 of unspecified uterine carcinoma. The average age of women who died of cervical carcinomas was 60 years, whilst that of patients with endometrial carcinoma was 71.7 years. In 12.6% of cases double carcinomata were found. The relation between deaths from cervical carcinoma to endometrial carcinoma is 3.3:1. If this ratio is applied to the 942 deaths from uterine carcinoma in 1976 in Austria, then one can estimate that more than 700 women must have died of cervical carcinoma and more than 200 of endometrial carcinoma. The 303 deaths from cervical carcinoma and 65 from endometrial carcinoma reported in the official mortality statistics are, therefore, totally misleading and ought not to be used as a basis for epidemiological investigations.

Reported in this paper is the case of a woman, 43 years of age, with malignant haemangiopericytoma. The patient had undergone radical surgery and irradiation, following diagnosis of globucellular sarcoma. Metastasis occurred after an interval of six years from surgery. Histological checks revealed a pattern typical of haemangiopericytoma. The patient died one year later. - Morphology, incidence, clinical aspects, and general importance of this rare tumour are discussed against the background of relevant literature. The point is made that histological diagnosis may be quite difficult and that there is only limited possibility to draw conclusions from the morphological pattern of the tumour as to its biological importance.
Recently, it was demonstrated that endogenous relaxin promotes growth of the vagina during the second half of pregnancy in rats and that administration of porcine relaxin promotes growth of the uterus in nonpregnant or early pregnant gilts. This study examined the effects of circulating relaxin on growth of both the vagina and uterus during the last two thirds of the 114-day gestation period in gilts. Furthermore, this study employed an in vitro immunohistochemical localization technique to determine whether the vagina and uterus in pigs have specific relaxin-binding sites. Three groups of pregnant gilts were used: sham-ovariectomized controls (group C; n = 8), ovariectomized progesterone-treated (group OP; n = 6), and ovariectomized progesterone- plus relaxin-treated (group OPR; n = 7). Gilts were either sham ovariectomized or ovariectomized on day 40 of gestation. Hormone replacement therapy with progesterone (group OP), progesterone plus relaxin (group OPR), or hormone vehicles (group C) began on day 38 (progesterone) or day 40 (relaxin) and continued until day 110. On day 110, the vagina and uterus were collected, and wet weight, dry weight, and percent hydration were determined. Small pieces (2-3 cm3) of the vagina and uterus from groups C and OP were frozen and cryosectioned for the immunohistochemical localization of relaxin-binding sites. Relaxin promoted growth of both the vagina and uterus. The wet weights of both the vagina and uterus in relaxin-deficient gilts (group OP) were lower (P < 0.05) than those in controls (group C), and relaxin replacement therapy (group OPR) restored the wet weights of both tissues to values that did not differ from those in controls. The mean dry weights and percent hydrations in the vagina and uterus did not differ among treatments. Immunohistochemical localization studies in the vagina and uterus demonstrated that specific and saturable binding of relaxin was localized in the same cell types of both tissues, namely epithelial cells (luminal in vagina, and both luminal and glandular in uterus), smooth muscle cells (both circular and longitudinal in vagina, and myometrial in uterus), and cells associated with blood vessels. In conclusion, this study provides evidence that circulating relaxin promotes growth of both the vagina and uterus during pregnancy in the pig. Furthermore, this study provides evidence that both the vagina and uterus contain specific and saturable relaxin-binding sites in epithelial cells, smooth muscle cells, and cells associated with blood vessels. We conclude that these cells probably initiate relaxin's effects on the vagina and uterus of the pregnant pig.
The effects of histamine and its analogs 2-(2-pyridyl) ethylamine (PEA) and 4-methylhistamine (4 MH) have been studied on uterine preparations obtained from estrogen-primed guinea pigs and rats. (1) Histamine and 4 MH, a specific H2-receptor agonist produced relaxation in estrogen-primed rat uterus, whereas these agonists produced contraction in the estrogen-primed guinea pig uterus. (2) PEA, a specific H1-receptor agonist produced contraction in the guinea pig uterus but had no effect on the rat uterus. (3) Metiamide blocked responses to histamine and 4 MH in the rat uterus, whereas mepyramine blocked responses to histamine and PEA in the guinea pig uterus. (4) Propranolol produced competitive antagonism with histamine in the rat uterus, whereas it had no significant effect on the histamine or PEA responses in the guinea pig uterus. (5) Reserpine pretreatment completely abolished the responses to histamine and 4 MH in the rat uterus but did not alter the response in the guinea pig uterus. (6) Our data suggest that in rat uterus only H2-receptors are present and they are indirectly through the release of noradrenaline. In the guinea pig uterus both H1- and H2-receptors are present and are excitatory and directly acting.
Rhythmic contractions occur in the sheep uterus at oestrus and during pregnancy from about 65 days to term (145 days). To define factors responsible for these contractions we have examined and quantified the degree of synchronization of electrical activity in the uterus and isolated segments of myometrium in 3 types of experiments. In the first, a segment of myometrium was totally separated from the uterus. After a period of 9-16 days the isolated tissue developed a typical pattern of uterine activity which showed no significant degree of synchronization with EMG bursts in the body of the uterus. During labour, the isolated tissue showed changes in activity similar to those observed in the uterus. In the 2nd experiment, the tubal end of one of the uterine horns was severed from the uterus, but a connection was retained with the uterus via the oviduct and ovarian blood vessels. Activity in the partly isolated segment remained in synchrony with the uterus. In a 3rd experiment, impulse propagation through nerves and smooth muscle to the tip of a horn was disrupted by severing 'the tip' from the uterus while its blood supply from the ovarian vessels was retained. The blood vessels were momentarily frozen, and denervation confirmed by monoamine histofluorescence. In 5 out of 6 animals the operated tissue showed activity that was not synchronous with the rest of the uterus. These data indicate that: (1) isolated uterine muscle in vivo has rhythmicity resembling that of intact myometrium and (2) systemic or local circulating factors are not responsible for synchronizing uterine activity before parturition, although circulating factors do play a major role in increasing the uterine activity which occurs at parturition and at oestrus, and (3) hydraulic continuity between different regions of the uterus is not essential for maintaining co-ordinated activity.
