Male suicides and alcohol consumption in the former USSR.

A significant decline (34.5%) in the suicide rate occurred in 1984-1988 throughout the USSR. The decline was observed shortly after the introduction of strict restrictions on the sale of alcohol. We tested the hypothesis that the restrictive alcohol policy in the first years of perestroika (June 1985) caused the fall in suicide rates in the former USSR. Data on alcohol consumption, violent death caused by external injury and poisoning (n = 916,315), death due to accidental alcohol poisoning (n = 77,837), suicide (n = 192,305) and death undetermined whether accidentally or purposely (n = 54,253) were analyzed for all former Soviet republics for 1984, 1986, 1988 and 1990. Men were chosen for the analysis, since men are more prone to abuse alcohol than women. Regression analysis with alcohol consumption as the independent variable and suicide rates and violent death rates as dependent variables shows that suicide and alcohol consumption were positively correlated as were violet death and alcohol consumption. In the republics with high alcohol consumption (Slavic and Baltic), suicide rates were also high. In the Caucasian republics, low alcohol consumption was associated with low suicide rates. For most republics, alcohol seems to explain more than 50% of suicides. Alcohol also has considerable explanatory value for violent death. Thus, a restrictive alcohol policy might be a way to reduce suicide and violent death.

The epidemiology of epidemic parotitis in the USSR.

Morbidity from epidemic parotitis in the USSR in the period from 1958 to 1972 ranged within the limits of 266.6 and 521.7 cases per 100 000 inhabitants. A 3--4 years cyclic recurrence of rises and falls in morbidity was observed. The bulk of cases of disease occur in the winter-spring period. In towns, parotitis is registered 3 times more frequently, in the mean, than in rural districts. More than 95% of patients in the USSR consisted of children under 15, while in Moscow, children aged 3--5 years were the most affected group. In Moscow in the period from February 1, 1972 to January 31, 1973, the morbidity in men was 493.2 per 100 000 and in women 339.5 per 100 000 (ratio 1:1.45).

Investigation of some problems of virology and creation of means of specific prophylaxis of virus infections in the institute of poliomyelitis and virus encephalitis, academy of medical sciences of the USSR.

Several great problems are clearly distinguished in the development of the Soviet medical virology. The team of the Institute of Polliomyelitis and Virus Encephalitis of the AMS of the USSR actively participates in the study of these problems. They include the investigation of theoretical questions of general virology, creation and improvement of means of specific prophylaxis of virus infections, virological examination of diseases endemic for single districts of the USSR as well as the search for, discovery and exploration of new viruses, causative agents of disease. The achievements of the team of the Institute of Poliomyelitis and Virus Encephalitis in the field of liquidation of poliomyelits, creation of new effective preparations against tick-borne encephalitis, rabies and other neurovirus infections are a clear consequence of the prophylactic trend of the Soviet health care, thoughtful planning of scientific work and broad, complex conception of scientific investigations, close connection with other scientific institutions, involvement of specialists of contiguous disciplines in scientific problems, international cooperation inthe elaboration of the mentioned problems within the framework of the CMEA, the use of the most modern and most perfect methods of scientific work and the accomplishment of all socio-economic and medical measures.

[Analysis of the trends in typhoid-paratyphoid morbidity in the USSR].

Mathematico-statistical analysis of typhoid-paratyphoid infection morbidity in the USSR during 1900--1940 revealed a regular many-year cyclic recurrence of the changes in the morbidity level. The intensity of the morbidity level reduction at the periods of decline--in 1900--1917, 1920--1928, and in 1932--1939--was similar. A successive reduction of morbidity of this group of infections, by 6.45% on the average, annually, occurred in 1930--1976. The character of reduction of typhoid-paratyphoid infections morbidity level in the USSR during that period is well described by the exponential function.

Malaria research and eradication in the USSR. A review of Soviet achievements in the field of malariology.

Relatively little is known outside the USSR about the past history of malaria in that country, the contribution of its scientists to malaria research, the recent progress of Soviet malariology, or the achievements of the Soviet Union in the eradication of malaria. These achievements are of particular interest because the general strategy of malaria eradication in the USSR has many technical, administrative, and economic and social features not seen elsewhere.

[The characteristics of human immunodeficiency virus strains isolated from HIV-infected persons on the territory of the USSR].

Fifty-seven HIV-1 strains were isolated from peripheral blood lymphocytes of 102 HIV-infected persons involved in epidemic outbreaks in different cities of the USSR. The effectiveness of isolation was 29.1% in asymptomatic infection, 51.7% in cases with generalized lymphadenopathy, and 82.6% in persons with severe clinical manifestations. Identification of the isolates by indirect immunofluorescence, ELISA, reverse transcriptase activity, Western blot, electron microscopy, and polymerase chain reaction showed them to belong to HIV-1 type. Reproduction of the isolates in cell cultures was accompanied by cytopathic effect and syncytium formation. The isolated strains can be divided into two groups: (1) the poorly growing isolates with low or negative RT and ELISA results and (2) the isolates with high infectivity, broad spectrum of cell tropism, and high levels of RT and ELISA. These data show the correlation of biological properties of HIV-1 strains isolated in the USSR with those of HIV strains previously isolated in Europe, USA, and Africa.

[A program of prevention of hereditary lysosomal diseases in the USSR].

The organization of genetic counselling for the families of patients with lysosomal storage diseases (LSD) was based on the interaction of the genetic counselling units of this country with a laboratory of inherited metabolic diseases of the National Research Center of Medical Genetics, USSR AMS. All the patients from 705 families at risk were examined using biochemical techniques and methods of somatic cell genetics. In total the loci differentiation was performed for 309 patients with mucopolysaccharidoses, glycoproteinoses, mucolipidoses, sphingolipidoses and other LSD. 53 families at risk (of 277) were prenatally diagnosed. 66 fetuses were diagnosed for mucopolysaccharidoses, type I, II, III, A and B, VI, Tay-Sachs disease, Sandhoff's disease, GM1-gangliosidosis, metachromatic leukodystrophy, mannosidosis, and multiple sulfatidosis. In total 18 affected fetuses were diagnosed and aborted. All the prenatal diagnoses were verified. The prevalence of mucopolysaccharidoses in two Central Asian republics was evaluated as 1:15,000. An Uneven ethnic distribution of different mucopolysaccharides in the USSR has also been shown.

[Epidemiologic analysis morbidity with meningococcal infections in the USSR].

The authors present the analysis of the incidence of epidemic cerebrospinal meningitis in the USSR from 1937 to 1974, and of meningococcus infection from 1965 to 1974. A rise of the meningococcus infection incidence from 1969 to 1974 was recorded 28 years after the elevation of 1940-1942 and was 1.5 times below this latter rise. The rise in 1969-1974 was characterized by marked signs peculiar to the infection with the droplet transmission mechanism; among those who contracted the disease prevalence was seen among children aged under 14 years (63-72%). A marked affection of juveniles was noted. Three types of the dynamic of the meningococcus infection incidence in the republics located in different climatic-geographical zones of the USSR were noted: slow, gradual increase of the level, interrupted and explosive. Such character was determined in the dynamics of the seasonal elevations of morbidity. Antiepidemic measures including a complex of nonspecific measures could not be assessed as sufficiently effective. This finds reflection in the natural course of the epidemic process of the meningococcus infection which remains uncontrolled. From the patients with generalized form of the disease meningococci of group A were isolated in 80-100% of cases, from the number of those typed. The group-specific reference of the nasopharyngeal strains depended on the epidemic situation: strains of serological group A prevailed at the period of the morbidity elevation, and other serological groups (particularly of C and B) increased at its decline.

METHODS OF LABORATORY DIAGNOSIS OF GONORRHOEA USED IN THE USSR.

In this review of gonorrhoea diagnostic methods in current use in the USSR, the author stresses first the value of the examination of smears for the detection of the typical forms of the gonococcus. Where atypical L-forms are present, as may occur after treatment with antibiotics or sulfanilamide, microscopy may be misleading and a combination of laboratory methods is called for, including the use of culture techniques. Transport media for the maintenance of material for culture have been little used in the USSR so far, but experiments with the Stuart medium and modifications of it have shown promise.Although the complement-fixation test has lately fallen rather into disuse for gonorrhoea diagnosis, the author considers the inherent value of this serological method to warrant its further use provided that new and improved antigens and techniques can be developed capable of yielding specific reactions with both typical and atypical forms of the gonococcus.

JAPANESE ENCEPHALITIS IN THE USSR.

The author sketches the history of Japanese encephalitis in the USSR, where it has been thoroughly studied since it first occurred in 1938. After a brief outline of its epidemiology, he describes the pathogenesis, the signs and symptoms, and the pathophysiological mechanisms that make this form of encephalitis so dangerous. He also discusses the diagnosis and the methods of treatment and prevention practised in the USSR.

TICK-BORNE ENCEPHALITIS IN THE USSR.

The author gives the history of the study of tick-borne encephalitis in the USSR, following it up with a description of the etiological and epidemiological aspects and of the pathology, symptomology, diagnosis, treatment, and prevention of the disease. He stresses that other ticks as well as the main vectors of the virus, Ixodes, transmit the disease, in particular Dermacentor and Haemaphysalis. A detailed account of tick-borne encephalitis in the USSR may be of value in the study of the rather similar forms of the disease now being encountered more and more frequently in countries of South-East Asia as well as of northern and eastern Europe.

[Data on the spread of Culex pipiens on the territory of the USSR].

A map of Culex pipiens molestus distribution over the USSR was made using data of sanitary epidemiological stations of various regions of the country, literature and the authors' own data. It was shown that during the last decades (1978-1990) the Cx. p. molestus breeding occurring predominantly in flooded basements of comfortable houses, especially in new built towns and cities, was recorded throughout the European part of the USSR including some subarctic regions. Cx. p. molestus is also widespread in the Asiatic part of the country including Western and Eastern Siberia, southern regions of Far East and the Sakhalin Island and Kamchatka. In the permafrost areas, most of new houses are built on piles without basements and cellars, and it prevents Cx. p. molestus breeding. The dates of appearance of these mosquitos in various parts of the country due to the development of transportation are discussed.

Misoprostol and dalargin for the inpatient treatment of duodenal ulcer in the USSR.

In the USSR, uncomplicated duodenal ulcer is usually managed in an inpatient setting. A double-blind, randomized, double-dummy, multiclinic, comparative trial examined the safety and efficacy of oral misoprostol (200 micrograms qid) and intramuscular dalargin (1 mg bid) in duodenal ulcer healing. Dalargin, an enkephalin peptide with gastric antisecretory and cytoprotective properties, is commonly used in the USSR for treating peptic ulcer. The trial characteristics and monitoring were performed in accordance with US standards. Consenting patients of either sex, aged 18 to 70 years, who had a duodenal ulcer of 0.3 to 2.5 cm in diameter were enrolled. Endoscopy, physical examination, and laboratory assessments were performed on entry and after 4 weeks of treatment. The effects of the treatments on complete ulcer healing, dyspeptic symptoms, antacid consumption, adverse drug reactions, and safety were determined. Both treatment groups were well matched for demographic characteristics. The majority of patients were smokers and alcohol users and had a history of recurrent peptic ulcers. Misoprostol was significantly more effective than dalargin in inducing duodenal ulcer healing, both in the intent-to-treat cohort (P = .001) and in the evaluable cohort (P = .0001). Both regimens were well tolerated, and no patients were withdrawn from the study because of adverse reactions. Mild, self-limiting diarrhea was reported more frequently in patients receiving misoprotol (9.4% incidence) than those receiving dalargin (1.9%). The disparity in ulcer healing between the two groups suggests that hospitalization alone is not an optimum treatment in this high-risk patient population.

The Sabin live poliovirus vaccination trials in the USSR, 1959.

Widespread use of the Sabin live attenuated poliovirus vaccine has had tremendous impact on the disease worldwide, virtually eliminating it from a number of countries, including the United States. Early proof of its safety and effectiveness was presented in 1959 by Russian investigators, who had staged massive trials in the USSR, involving millions of children. Their positive results were at first viewed in the United States and elsewhere with some skepticism, but the World Health Organization favored proceeding with large-scale trials, and responded to the claims made by Russian scientists by sending a representative to the USSR to review in detail the design and execution of the vaccine programs and the reliability of their results. The report that followed was a positive endorsement of the findings and contributed to the acceptance of the Sabin vaccine in the United States, where it has been the polio vaccine of choice since the mid-1960s.

[Comparative characteristics of nutrition of the male population in some cities of the USSR (a cooperative study)].

The state of nutrition was studied in men of varying age groups (20-29, 30-39, 40-49, 50-54 years) living in different cities of the USSR. The standard method of food ration "daily reproduction" was used to investigate 25% of representative subsample in each age group. It has been found that the daily ration of men living in the European part of the USSR (Moscow, Kiev, Tallinn) is characterized by a rather high caloric value and by a significant imbalance in the consumption of the main food substances.

[The isolation of Vibrio fluvialis on the territory of the USSR].

For the first time V. fluvialis strains were detected on the territory of the USSR. The taxonomic position of these vibrios was determined by their nucleotide DNA composition (the content of guanine + cytosine was 49.3-51.0 mole%) and the characteristic features of their phenotype. The individual features of the strains consisted in their capacity for agglutination with cholera antisera, groups 01 and Inaba, in diagnostic dilutions in the presence of differences in genomes and phenotypes with cholera vibrios. Molecular hybridization DNA-DNA also gave no confirmation of their relationship to cholera vibrios (23-26% homology). The comparative study of V. fluvialis strains from the USSR and other countries by a broader set of their phenotypical signs confirmed their identity.
