[Thermography in diagnosis and treatment efficacy evaluation of lower limbs arterial diseases].

Possibilities of thermography for diagnosis of lower limbs arterial diseases and evaluation of treatment results were analyzed. Quantitative evaluation of infra-red imaging with detection of mean temperature, area of thermo-profile and thermographic index is proposed. Possibility of this method for detection of microcirculatory disorders in the limbs before clinical manifestation is demonstrated. Therefore thermography is a reliable method of early diagnosis. Thermography permits to perform qualitative and quantitative analysis of regional blood flow and microcirculation and consequently to use this method for evaluation of treatment efficacy.

[Diagnosis of breast carcinoma with thermographic methods: tele-thermovision, plate thermography].

The diagnosis of carcinoma of the mamma with the aid of tele-thermovision and liquid-crystal-thermography A review is given on thermographic diagnosis of carcinomas of the mamma with the aid of tele-thermovision and liquid-crystal-thermography. 132 carcinoma cases were microscopically examined with cytomorphological criteria. The microscopic and thermographic results were compared. The thermographic diagnosis "suspicious" is dependent on the diameter of the tumour, the vascularisation, the essential cytomorphological criteria as mitosis, prominence and size of nucleoli as well as deviation of the nucleous/plasma ratio. There was no dependence on the distance of the tumour from the surface of the breast and even on the perifocal infiltration with lymphoid cells. The accuracy rate in diagnosis with liquid-crystal-thermography is about 74%; false negative and false positive results both are about 9%. The accuracy rate for tele-thermovision is about 10% lower. The detection rate of carcinoma stage T1 ranges around 50%.

Use of high resolution digital thermography to detect Heterodera schachtii infestation in sugar beets.

Thermography is a non-destructive method used to monitor pest and disease infestations, as it is related to changes in plant water status. Surface temperature differences of the crop canopy may be an indicator of nematode infestation as the parasitation of the root system reduces evaporation of leaves. To test the potential of high resolution digital thermography to detect Heterodera schachtii infestation, experiments using increasing nematode densities and different sugar beet varieties were conducted. From June to August 2003 the crop canopy temperature was measured with a thermal infrared camera from a helicopter. A significant correlation between canopy temperature and nematode density was observed with the susceptible cultivar Monza whereas the resistant cultivar Paulina did not show any correlation. Mean temperature comparison showed significant differences between the lowest infestation level (500 eggs and larvae/100 ml soil) and the highest infestation level (>1500 eggs and larvae/100 ml soil). At the beginning of the season canopy temperature differences between healthy and nematode infested sugar beets were higher (approximately 1 degree C) compared to later assessment dates when the water supply in the soil was limited. Since low and high nematode infestation could be clearly distinguished with the susceptible cultivar by airborne thermal images, thermography might be a useful tool for monitoring sugar beet fields.

Diagnostic value of foil thermography in urological diseases.

Authors summarize their experience of 8 years with foil thermography. Based on a total of 1113 examinations, they assess the diagnostic value of this method in some andrological and urological diseases. They found foil thermography to be an independent, but not the exclusive procedure in diagnosing varicocele. In addition, this method has proved to be highly valuable also in differentiating the inflammatory and tumorous processes of the testicles. They point out that thermography is very useful for outpatient practice, due to its inexpensiveness, simplicity and rapid applicability.

[Liquid crystal contact thermography--a new screening procedure in the diagnosis of deep venous thrombosis].

In search of a sensitive, simple, non-invasive screening method in the diagnosis of Deep Vein Thrombosis (DVT) we become aware of the recent possibility of Liquid Crystal Contact Thermography (LCCT). From October 1987 all patients over 40 years of age undergoing visceral surgery were examined on the second postoperative day by means of the LCCT Method. Furthermore patients of the traumatological clinic with high risk for DVT (hip surgery) were controlled. In case of positive thermography or uncertain interpretation a phlebography was performed and thermography repeated. Until the end of February 1988 316 patients were submitted to LCCT. In the beginning of the study pathologic and iatrogenic factors which lead to a rise in temperature of the leg, caused differential diagnostic problems. In the course of the study the LCCT turned out to be a reliable screening method in the diagnostic of DVT. In our study the LCCT has a sensitivity of 94%. The specificity was, as expected, a bit lower and is 86%. The high sensitivity and somewhat lower specificity (which is without consequence for the patients) shows that LCCT is well suited as screening method, especially since it is non-invasive. Differential diagnostic aspects are discussed.

[Analysis of errors in imaging diagnosis of varicocele. Duplex sonography, plate thermography and phlebography].

Comparative investigations were done by duplex sonography, thermography and phlebography in 142 patients with 158 varicoceles. Verification of clinical varicoceles (130/158) was performed by duplex sonography in all cases and by thermography in 93%. Measurement errors were due to dorsally located varicoceles near the radix of the penis (3/130) and by small, two-sided varicoceles (6/130). Small subclinical varicoceles (28/158) were detected by real-time sonography in 89%, by Doppler sonography in 64%, and by thermography in 53%. In three patients with normal real-time sonography, subclinical varicoceles were found by Doppler sonography alone. The combination of both sonographic methods is highly sensitive for the detection of subclinical varicoceles and should be done in all cases without therapeutic results.

[Diagnosis of breast cancer by contact thermography (author's transl)].

Epidemiological studies performed in cooperation with the Austrian National Institute for Statistic show an increase of breast cancer in the age-group of women older than 50 years. Especially in this age-group screening for breast cancer is very important and successful. Mammography in this age-group is the most useful and accurate method. In the group of women aged less than 50 years and particularly less than 45 years the use of mammography for screening is limited because of its risks for the induction of breast cancer in young women. Therefore the combination of clinical examination and contact-thermography is a very promising alternative for the detection of breast cancer. In 133 cases of histologically proved breast cancer the high accuracy of this combination of clinical examination and contact-thermography in comparison with mammography is shown. Based on this good results a recommendation for a detection-scheme for breast cancer with a reduction of radiation exposure in young women by the use of contact-thermography is presented and discussed.

[Experiences with thermography in the surgical treatment of ischemic heart disease].

Possible use of thermography (AGA Thermovision 750) in heart surgery has been investigated in animal experiments and in human extracorporeal operations as well. In six open-chest isolated dog hearts, coronary circulation had been occluded and antero- and retrograde local cooling was employed. Following coronary occlusion, effective hypothermia was achieved retrograde via the coronary sinus. 22 patients were arranged into three groups according to the operation technique used, and the possibilities for effective cardiac cooling by cold cardioplegic solution were studied. Hearts of the six patients subjected to surgical intervention for valvular lesions (four mitral and two aortic) could be safely cooled by cold cardioplegic solution infused through the aorta or the coronary arteries. The hearts of the twelve patients with coronary artery stenosis could be cooled sufficiently by additional retrograde infusion only. Employing antero- and retrograde hypothermia, in four patients the size of the aneurysm of the left ventricle, and the area which had to be resected was determined by thermography. Thermography is considered to be a suitable non-invasive method to derive information concerning: the efficacy of cold cardioplegic solution in aortocoronary bypass grafting, quantitative changes in nutritive coronary flow, the size of left ventricular aneurysmectomy.

[Evaluation of the effectiveness of preoperative local thermoradio- and thermochemotherapy of breast cancer using thermography].

The efficacy of combined preoperative local MWF-hyperthermia of breast cancer was difficult to assess because within a short period of treatment tumor remained clinically unchanged. At the same time it resulted in the death of cancer cells and blood flow blockade which had to be reflected in the time course of thermography during treatment. Thermography results in the course of thermoradio- and thermochemotherapy were compared in 27 patients. A decrease and blockade of blood flow in a tumor, the death of cancer cells manifested themselves in "colder" zones in a tumor area. The more pronounced the pathomorphological changes were, the lower was the temperature on thermograms in a tumor area as compared to adjacent tissues. Thermography can be used for assessment of the efficacy of hyperthermia of breast cancer.

[Diagnostic relevance of thermography in renal transplantation (author's transl)].

During a followup period of 18 months 75 renal transplant recipients were examined by thermography according to Tricoire. Thermography is a not invasive, quickly available and reproduceable method. Because of the high incidence of 85% exact diagnoses this investigation is a helpful additional test in kidney transplantation for evaluation of graft function as well as for diagnosis for evaluation of graft function as well as for diagnosis of pathological intrarenal or perirenal disorders. Thermography is especially recommendable for transplant patients, if postoperative haemodialysis is necessary. In these cases information can easily be obtained whether postoperative olig-anuria is caused by acute tubular necrosis or by primary insufficient vascularisation of the transplant.

Thermography in suspected deep venous thrombosis of lower leg.

Phlebography has proved to be the most reliable method to diagnose a deep venous thrombosis of a lower leg. In this study it is attempted to evaluate the accuracy of thermography in diagnosing a deep venous thrombosis. The series comprises 125 patients which were investigated with thermography and phlebography. A diagnostic agreement was 79% and false negative rate 15.9%. It was concluded that thermography is only a screening method and by using it about 40% of phlebographies are avoided.

Thermography: an alternative to angiography for vascular access survey in haemodialysis.

Results obtained by thermography and angiography in surveying vascular access have been compared in 33 haemodialysis patients, including 23 with Cimino arteriovenous fistulae and 10 with homografts and heterografts. In 23 of 30 patients, blood flow stealing through collateral veins, or venous stenosis was detected by thermography and confirmed by angiography. Thermography seems a quick and simple method in routine conditions, restricting angiography to selected patients for whom a surgical revision of the fistula or of the graft is needed.

Thermography as a tool for evaluating the local tolerance of i.m. administration of drugs.

Until today the local tolerance of i.m. administration of various drugs has been possible to register only by asking the patients about their subjective feelings. In this study using a new galenic form of trimethoprim/sulfamethoxazol (Bactrim) developed for i. m. administration and ampicillin we could find that thermography can be used as a very exact method to measure the local tolerance of i.m. administered drugs. In fact, the thermography gave reaction before the patient reported any pain and in cases where pain existed the thermography findings correlated well with the reports of the test persons.

[The value of mammary thermography. A report on 655 cases (author's transl)].

A total of 655 patients were investigated by both mammography and contact thermography, and 63 cancers of the breast were detected. Thermography was slightly more sensitive (0.82) than mammography (0.75). They were of equal specificity (0.96 and 0.97). A critical analysis of the experimental conditions, however, leads the authors to propose thermography as the essential investigation completing clinical examination and mammography. It may also have a certain prognostic value, but this remains to be demonstrated.

[The value of infrared and plate thermography in the early diagnosis of mastocarcinoma (author's transl)].

The inspection and palpation of the breast remains the basis of every breast examination. However, genuine early diagnosis of carcinoma of the breast cannot be achieved by clinical examinations without any supporting investigation. It must be borne in mind that such early diagnosis is the aim of every medical checkup for cancer. This fact has resulted in the development of novel examination methods employing suitable apparatus. The value of mammography is beyond dispute. Since it is the safest of all examination methods as far as accurate results are concerned, mammography makes a decisive contribution toward improving the success rate of early diagnostics. The present paper aims at emphasizing the value of thermography in diagnostics, especially in the early diagnosis of mastocarcinoma. Such a valuation becomes possibly by checking all thermographic findings with the help of mammography. As a result, it must be stated that both infrared thermography and plate thermography can only be additional examination methods in support of mammography in order to discover clinically occult carcinomas within the framework of preventive care examinations.

Inflammation of the lacrimal drainage system--assessment by thermography.

Thermography is an investigative tool without side effects. To our knowledge thermography has not been evaluated in the assessment of lacrimal passage inflammation and obstruction. It can give an objective assessment of presence and degree of inflammation. An infrared scanning device was used to detect surface temperatures in 29 patients with known disorders of the lacrimal drainage passage. Thermal images were compared for bilateral symmetry and against the normal thermographic appearance of the face. Acute dacryocystitis and canaliculitis showed gross hyperthermia and asymmetry. Forty-two percent of patients with a picture of chronic dacryocystitis and 40% of patients with only tearing and no clinical evidence of inflammation showed temperature elevation on the side of involvement, whereas 58% and 60% of these respective groups showed symmetrical thermal patterns, suggesting no active inflammation. In a few cases there were discrepancies between clinical assessment and objective thermographic evaluation of the degree of inflammation. The implications of our results and possible applications of thermography are discussed.

[Diagnostic relevance of contact thermography in renal transplantation (author's transl)].

102 renal transplant recipients were checked by contact thermography according to Tricoire for 2 1/2 years. Diagnostic value of this non invasive, quickly available and reproduceable method was investigated. The grafted kidney reveals on the thermographic screen its size, site, and vascularisation. The thermograhic pattern of a well functioning transplant shows warm areas in green, blue and violet colour. Onset of acute or chronic renal rejection leads to impaired heat conduction to the body surface either by oedema or by diminished blood flow. By photographic documentation in natural colour spotted or diffuse cold regions of brown, maroon and orange are seen. In the very early posttransplant period up to two months thermography is helpful in differential diagnosis for those recipients requiring initial haemodialysis treatment. Information is available between non functioning grafts with diminished renal blood supply and transplants with acute tubular necrosis. Impressive thermograms are found by rupture and subrupture of the kidney respectively. Superficial perirenal changes lead to topical temperature elevation as well. The high reliability of 92% correct diagnoses depends on exact application of the thermosensitive film and on determination of the basic individual skin temperature in reference to repeated examinations of the grafted area. Temperature measurement is influenced by subcutaneous abdominal fat distribution and muscle thickness as well as by deep position of the transplant or asymmetry of the lower abdominal region. In the wide field of diagnostic procedures necessary for transplant recipients with complications thermography by Tricoire is recommended.
