Clinical application of bioactive glass-ceramic BAS-O for filling cyst cavities in stomatology.

A description is given of the bioactive glass-ceramic material, BAS-O, and of clinical tests of this material in stomatology. Granules of BAS-O bioactive glass-ceramic were used for implantation into cyst cavities in jaw bones. Radiological examination was carried out after 3, 6 and 12 months. A total of 37 radiographs of 23 patients were recorded and evaluated. Symptoms of ossification were found in 28 cases (75.7%), while five radiographs (13.5%) did not show visible bone apposition and loss of the BAS-O granules from the operation site was found in four cases (10.8%). The authors have demonstrated the excellent healing properties and tolerance for the BAS-O material, and no significant changes were observed in biochemical and haematological examinations.

[Cicatricial pemphigoid: management in stomatology].

BACKGROUND: Cicatricial pemphigoid is a subepidermal disease of the mucous membranes. In some patients blisters involving the oral cavity may be the only manifestation. The purpose of this work was to detail the clinical features and biological findings, highlighting the diagnostic and therapeutic difficulties. MATERIAL AND METHODS: Seventeen patients, nine women (mean age 59.6 years) and eight men (mean age 67.8 years) managed between 1990 and 2000 were reviewed retrospectively. RESULTS: Mucous membranes of the mouth were the only localization in six of the seventeen patients. Other localizations (skin, eye, nose, anus) were associated, alone or in combination, in the other eleven patients. The gingivae were involved in 15/17 patients and constituted the only localization in six. Skin lesions (sometimes involving the lower limb) were variable and found in 6/17 patients. Ocular lesions were present at diagnosis in six patients, including two with severe lesions at onset. Nasal and/or laryngeal involvement was found in 6/17 patients. These lesions were often severe with major functional impact. ENT localizations were found in patients with at least three localizations and lesions extending to mucous membranes distant from the oral cavity. A biopsy was obtained from all patients and demonstrated subepidermal detachment in eleven. Direct immunofluorescence was positive in eleven. Indirect immunofluorescence, performed in twelve patients, was negative. In two patients, both histology and immunofluorescence were negative. Electron immunomicroscopy was performed in four patients and confirmed the diagnosis. Five out of six isolated oral manifestations were treated with topical steroids alone. Multiple lesions were treated case by case using daprosone and/or systemic corticosteroids associated with topical steroids in 16/17 patients. A bolus of cyclophosphamid was given for four patients with very severe lesions which did not respond to other treatments. At last follow-up (22 +/- 17 months), none of the patients was considered cured. DISCUSSION: Cicatricial pemphigoid is a complex chronic disease. Oral manifestations may be isolated or associated with lesions of the skin or other mucous membranes. The painful aspect of oral lesions explains why the diagnosis is often made in stomatology. Careful evaluation at diagnosis is essential and requires a multidisciplinary approach. Certain diagnosis can be provided by immunohistology, particularly electron immunomicroscopy which enables distinguishing cicatricial pemphigoid from other autoimmune diseases of the dermo-epidermal junction. Treatment depends on disease severity. Isolated oral manifestations are generally well controlled by topical corticosteroids which ocular, nasal, and laryngeal localizations often require more extensive treatment.

The impact of stomatological disease on oral health-related quality of life.

The clinical diagnosis of stomatological disease may indicate its cause and prognosis; however, it says little about the resulting level of impairment from the patient's perspective. The primary objective of this study was to test whether patients attending an outpatient oral medicine clinic would have worse oral health related quality of life (OHR-QoL) compared with the general population. In addition, we aimed to assess whether anxiety or depression could be predicted by OHR-QoL and to explore the relationship between clinical diagnoses, OHR-QoL and anxiety/depression. Data were collected from patients (n = 97) through face-to-face interviews using the Oral Health Impact Profile Short form (OHIP-14) to measure OHR-QoL, the Hospital Anxiety and Depression Scale (HADS) for psychiatric morbidity, and a visual analogue scale for self-rated general health. Age- and sex-matched controls (n = 388) were provided from a normative data set collected in a UK national survey in 1998. Participants had significantly lower OHR-QoL scores than the general population on all domains and overall OHR-QoL scores. Of the variance in anxiety, 55% was predicted by general health ratings and OHR-QoL domains of 'psychological discomfort' and 'psychological disability'. Of the variance in depression, 54% was predicted by general health ratings and OHR-QoL domains of 'functional limitation' and 'social disability'. Patient centred, routine assessment of OHR-QoL provides an additional dimension that may help to improve awareness of the impact of disease on the individual's life and enhance the clinical decision-making process.

Prescribing antibiotics in odontology and stomatology. Recommendations by the French Health Products Safety Agency.

In order to limit the onset of adverse effects and the increasing emergence of bacterial resistance, the prescription of antibiotics must be reserved strictly for situations where their efficacy has been demonstrated. The French Health Products Safety Agency (Afssaps) has updated recommendations concerning the use of antibiotic treatment in odontology and stomatology. The general strategy for the prescription of antibiotics proposed by the present recommendations relies on a professional consensus. The full-length, discussed and referenced text is available at the web site of Afssaps (http://www.afssaps.sante.fr) in the 'Documentation et publications' rubric'.

The application of the International Classification of Diseases to dentistry and stomatology.

The Application of the International Classification of Diseases to Dentistry and Stomatology (ICD-DA) takes from the parent volume (the International Classification of Diseases) all those items of specific interest in regard to oral pathology whether they be common or rare conditions, or whether the pathology originates in the oral cavity or is a manifestation of a systemic disease. The initial advantage is convenience in finding the numerical classification of disease without searching through the large parent volume. However, an even greater advantage is the fact that these conditions are not sufficiently subclassified in the parent manual whereas in the ICD-DA they are classified down to the five-digit level to make differential diagnosis meaningful within the practice of dentistry.

[Preparation trauma in stomatology].

In this paper authors deal with the causes of preparation trauma in stomatology. They have studied effects of high temperature on human cells cultured in vitro. Based both on literature data and on their own experience they summarize basic principles of preparation which prevent preparation trauma. They summarize how to eliminate as much as possible factors that damage hard dental tissues and pulp.

Assistance with generalized net models to the differential diagnosis of some pathologic somatic muscular and neurologic conditions in stomatology.

Some somatic pathologic muscular and neurologic conditions in stomatology are difficult to differentiate. For example, the myofascial pain dysfunction syndrome, trigger point muscle pains and a number of temporomandibular joint related pathoses often cause diagnostic difficulties. With the purpose to assist general clinicians and students to understand and differentiate more easily these difficult occasions, the authors try to offer a model simulating the decision-making process in some of the aforementioned pathoses. The model is based on the Generalized Nets Models which represent extensions of Petri's nets and their modifications.

[Experience with clindamycin in stomatologic diseases].

In 114 patients suffering from dental inflammatory diseases (periostitis, gingivitis, sinusitis, periodontitis etc.) and in 41 cases of dental surgical procedures (implantation, dentoalveolar surgery etc.) 3 x 300 mg/day clindamycin was applied during 7 days in the framework of GCP phases IV. In 150 cases (97%) the product proved to be effective and only in 2 cases a mild side effect was observed. On the base of the results clindamycin can be advised for the treatment of stomatological diseases.

[Corticotherapy and stomatologic oncology].

Long-term use of corticoids had proved beneficial in the treatment of advanced stomatological cancers, the effective dose being 40 mg of prednisone per day. In so far as it is possible to distinguish any specific action on individual symptoms, its action on pain was obvious in 40% of cases, on inflammation and infection in 52% of cases on fever (independent of infection) in 14% of cases, on oedema in 34% of cases. It was almost invariably favourable on the general condition. The method of using it emphasize how easy it is to use corticoids by injection which is of particular value in our speciality in view of: dysphagia, administration of large doses, deficiencies in the general condition, sometimes a matter of urgency. The counter-indications usually recognized for corticoids need to be modified in the case of cancer patients. Incidents occur surprisingly rarely and are minor relative to the advantages of the therapy, provided the patient is kept under strict supervision.

[Symbol-documentation of stomatological data--a basis for discussion].

The author puts on discussion symbols for the rapid and informative recording of stomatological findings and for the marking of the most important therapeutic measures. The present condition should be marked, respectively, in blue and black; and the plan of treatment, in red. Any therapeutic measure achieved will be retraced on the chart in blue or black. The actual condition of treatment will thus been read in the chart at a glance.

[Disinfection and sterilization in stomatologic practice from the viewpoint of the district health inspection].

Disinfection and sterilization measures are of paramount importance in the prevention of iatrogenic infections in the medical consulting-room. They are parts of the hygiene schedule in the office. The disinfection and sterilization measures are largely determined by law. Their securing is incumbent on the head of the respective institution. The control of this securing devolves on the State Hygiene Inspectorate. With regard to the stomatological work place, special conditions arise in enforcing observance of the legal provisions. The author tries to compile some of these conditions from the viewpoint of a district hygiene inspectorate.

[Treatment of salivary gland tumors in the past and at the present time at the 1st and 2nd Stomatology Clinics of the Charles University Medical School in Bratislava].

The retrospective study analyzes a group of 1021 patients (484 men and 537 women) with salivary gland tumours treated at the 1st and 2nd Departments of Stomatology in Bratislava within the years 1951-1996. This time period is divided into the past (1951-1980) and recent (1981-1996) periods. The trends in the diagnosis and treatment with the main emphasis on surgical procedures are mentioned. The most common operation were lateral parotidectomy (409 cases = 40.1%) and conservative parotidectomy (105 cases = 10.2%), enucleations were indicated rarely. Authors stress the necessity of team approach and cooperation of maxillofacial surgeons, radiologists, histopathologists, otorhinolaryngologists and oncologists. (Tab. 1, Fig. 2, Ref. 6.)

Epidemiological study of oral mucosa pathology in patients of the Oviedo School of Stomatology.

OBJECTIVES: The objectives of the present study were: 1) to determine the frequency of oral mucosa lesions and to analyze their relationship with toxic habits (tobacco and alcohol), and 2) to determine the prevalence of the oral pathology in patients who are denture wearers, to establish necessities for treatment and the opportune preventive measures. STUDY DESIGN: The study was carried out in patients attending the Oviedo Stomatology School to receive treatment for periodontal or prosthodontic dental pathology; but not for oral mucosal disorders. 337 patients were examined (140 males and 197 females). The ages ranged from 30 to 85 years. A clinical protocol was elaborated which included lesions previously reported in epidemiological studies. RESULTS: The number of patient with lesions was 198 (58,75%). The pathology diagnosed with more frequency was Melanin Pigmentation (n=83; 24,6%), followed by Frictional Keratosis (n=39; 11,5%), Linea Alba (n=34; 10,7%), Cheek Biting (n=23; 6,8%) and Traumatic Ulcer (n=16; 4,7%). In the patients who were denture wearers, the most frequent pathology was Hyperplasia (n=17; 5%) and Stomatitis (n=9; 2,6%). A statistically significant relation was observed between denture wearers and Traumatic Keratosis, Leucoplakia and Candidosis Pseudomembranous; tobacco habit and Leucoplakia presence, Frictional Keratosis and Melanin Pigmentation; and between alcohol habit and Traumatic Keratosis. CONCLUSIONS: Our results have shown the necessity to periodically examine denture wearers, and to establish the opportune preventive measures to eradicate the pathology that this causes. The observation of the association between Traumatic Keratosis, and two factors related with the etiology and prognosis of oral cancer should awake maximum interest in its correct diagnosis and treatment.

[The French Journal of Stomatology and Maxillofacial Surgery: what is the future?].

UNLABELLED: A scientific journal reflects the dynamics of a specialty and should allow wide diffusion of clinical as well as fundamental research findings in order for all practitioners to keep up with changes in their specialty. These objectives are totally different from a journal designed to meet teaching objectives where general reviews, technical notes or clinical cases are in the forefront. The purpose of this work was to determine the current role of the Revue de Stomatologie et de Chirurgie Maxillo-faciale in an attempt to determine future perspectives. MATERIAL AND METHODS: A Medline research was conducted for the period 1998-2000 to ascertain what French papers in the field of stomatology and maxillo-facial surgery had been published in indexed international reviews devoted to the specialty or related areas. The results obtained were compared with articles submitted for publication in the Revue de Stomatologie et de Chirurgie Maxillo-Faciale in 1999. RESULTS: Nineteen French papers were published in indexed international journals. These papers included 10 original articles, 8 cases reports and one technical note. The 10 original articles issued from 8 different centers and the 19 papers from 12 centers. During 1999, 96 papers were submitted to the Revue de Stomatologie et de Chirurgie Maxillo-Faciale for publication. Before acceptation, these papers included original articles (25%) case reports (45%) and technical notes or reviews (30%). In all, 18 original articles issuing from 10 different centers, the same centers that had published in the indexed international journals, were published. DISCUSSION: Considering that the substance of a scientific review lies in the abundance of its original articles, whether the work reports fundamental or clinical research, but more particularly clinical research for a surgery review, one is obliged to observe that the volume of French publications produced annually is insufficient to fullfil the objectives of a truly scientific journal. The 45% submission rate of case reports is in line with this observation. The number of international publications clearly does not explain this situation since globally, over a 2.5 year period, the number of original articles published was to order of one-third of the number published in the Revue de Stomatologie et de Chirurgie Maxillo-Faciale in the single year of 1999, and since the teams involved were the same.

[Stomatological studies on board of an "airspace" station].

The status of the oral organs was evaluated in normal subjects exposed for a long (30-262 days) time to conditions simulating an airspace flight in an on-land experimental complex (Sphinx-99 experiment). Stomatological studies were carried out in 15 volunteers aged 28-48 years. The initial (before "flight") and post-flight examination included clinical examination of the maxillofacial area and oral organs, evaluation of the intensity of caries by the CDL index, evaluation of the periodontal tissue status by the PMA and SPITN indexes. Based on the data, we singled out periods of adaptation characteristic of the oral organs under conditions of long enclosure. The results allow prediction of the course of diseases of the oral organs under extreme conditions and recommend methods of purposeful prevention in certain terms.

The activity of dental caries in students of the Faculty of Stomatology examined by using microbiological and biochemical tests--Dentocult LB.

The aim of the study was to examine the relation between the value of Dentocult LB and numbers of DMF, DMFs and Ds in the 20-25 age group. Sixty-six randomly chosen students of the Faculty of Stomatology, Medical University of Lublin were included in the study. In all of them, their dental state was assessed by using average numbers of DMF, DMFs and Ds. In the microbiological examinations, a ready-made medium from the Dentocult LB set was used. Statistically significant, directly proportional dependence of the number of surfaces with Ds active caries and the LB number was found.
