Two approaches to comparing hospital charges between cadaveric renal transplant patients who received orthoclone OKT 3 sterile solution or Atgam sterile solution for induction therapy.

The objectives of this study were: (1) to compare total hospital charges for a sample of cadaveric renal transplant patients categorized according to the type of induction therapy used (Orthoclone OKT3 Sterile Solution or Atgam Sterile Solution); (2) to compare specific charge categories between the two groups; and (3) to examine the relationship between charges and a set of independent variables. A retrospective review was conducted of hospital charges associated with a sample of renal transplant patients. The overall sample for this study comprised 510 patient discharges from 22 hospitals in the United States. Comparisons between the OKT3 and Atgam groups were made for total and specific charge categories using two different approaches to help control variations in charges that were not related to the type of induction therapy used. The first approach consisted of t test or chi-square comparisons between the groups for subsets of observations that had been identified in a stepwise fashion. These judgment samples were defined to remove sources of variation in charges other than those resulting from the type of induction therapy selected. The second approach used multiple linear regression analysis to help statistically control variation in charges from other sources. The results showed that higher drug charges in the Atgam group were offset by lower charges in other categories (P < 0.05). These findings suggest that hospital formulary committees should consider all relevant costs, not just drug acquisition costs, when selecting products. However, further investigation is warranted to explore differences in charges due to: (1) between-hospital variation; (2) patients' severity of illness before receiving induction therapy; and (3) differences in side-effect profiles for the two induction therapies.

Quantitative evaluation of prion inactivation comparing steam sterilization and chemical sterilants: proposed method for test standardization.

Prions are notoriously resistant to inactivation. To prevent accidental transmission of variant Creutzfeldt-Jakob disease (vCJD), various decontamination procedures have been adopted for re-usable medical devices by the authorities of countries at risk. As the vCJD agent in humans has a wide tissue distribution, practical methods of prion decontamination urgently need to be standardized, as do other sterilization and disinfection procedures (European Committee for Standardization). This article proposes a method using a quantitative murine model, combining observations of the decrease in the infection rate, the increase in the incubation period and a simultaneously performed chemical protein fixation control. In terms of practical application, autoclaving at 134 degrees C for 18 min or 121 degrees C for 30 min, and 1N sodium hydroxide for 15 min reduced the transmission of infectivity by a factor of at least 10(6). Partial efficacy can also be identified by the methodology, particularly for liquid cold sterilants such as glutaraldehyde and peracetic acid solutions.

Female sterile (1) yolkless: a recessive female sterile mutation in Drosophila melanogaster with depressed numbers of coated pits and coated vesicles within the developing oocytes.

Ultrastructural analysis of developing oocytes produced by the recessive female sterile mutant, yolkless (yl), in Drosophila melanogaster shows that yl+ gene activity is necessary for coated pit and coated vesicle formation within these oocytes. 29 alleles of the mutation are known to exist, and they fall either within a strongly affected class or a weakly affected class. Analysis of oocytes produced by females homozygous for the strongly affected class of alleles shows a greater than 90% reduction in the numbers of coated pits and coated vesicles. These oocytes have very little proteinaceous yolk, and the females accumulate vitellogenin (the yolk protein precursor) within their hemolymph. Moreover, females homozygous or hemizygous for a given strong allele produce mature oocytes that are flaccid. Alternatively, females homozygous or hemizygous for weak alleles produce yolk-filled oocytes, but the number of coated pits and coated vesicles within these oocytes is 50% of that found in the oocytes of wild-type females. Despite the presence of yolk within these oocytes, females homozygous for weak yl- alleles remain sterile, and their mature oviposited eggs collapse with time.

Penetration of ceftiofur into sterile vs. Mannheimia haemolytica-infected tissue chambers in beef calves after subcutaneous administration of ceftiofur crystalline free acid sterile suspension in the ear pinna.

The effect of Mannheimia haemolytica infection on the penetration of ceftiofur and desfuroylceftiofur metabolites into tissue chambers was studied in cattle after subcutaneous administration of ceftiofur crystalline free acid sterile suspension (CCFA-SS). Four tissue chambers were implanted subcutaneously in each of 12 calves. Approximately 45 days after implantation, two chambers were inoculated with M. haemolytica (10(6) colony-forming units per chamber) while the remaining two chambers were inoculated with sterile phosphate-buffered saline. Twenty-four hours after inoculation, CCFA-SS was administered subcutaneously in the middle third of the caudal ear pinna of each calf. Chamber fluid and blood samples were collected at predetermined times for 10 days following dosing and analyzed for ceftiofur and desfuroylceftiofur metabolites by high-performance liquid chromatography. Concentrations of ceftiofur and desfuroylceftiofur metabolites in plasma and tissue chamber fluid remained above a threshold of 0.2 microg/mL for at least 8 days. Infected tissue chamber fluid concentrations of ceftiofur and desfuroylceftiofur metabolites were significantly higher than those in non-infected tissue chamber fluid, which correlated with significantly higher total protein concentration in infected tissue chambers. These results indicate that single subcutaneous administration of CCFA-SS at 6.6 mg/kg can be expected to provide effective therapy of susceptible bacterial infections for a period of at least 1 week.

Comparison of two sterile connection devices and the effect of sterile connections on blood component quality.

BACKGROUND: Sterile connection devices (SCDs) are used to connect pieces of polyvinylchloride tubing between blood bag systems. After observing a slight decrease in inner diameter of tubing welded with the CompoDock S2 SCD, the effect of welded tubing on storage characteristics of white blood cell (WBC)-reduced red blood cells (RBCs) and platelet (PLT) concentrates was studied. Welds were made with Terumo SCD (T-SCD) or CompoDock S2, and unwelded tubing served as reference. STUDY DESIGN AND METHODS: Three WBC-reduced RBC units or 3 PLT concentrates were pooled and divided to prevent donor-dependent differences. The units were transferred 10 times over (1) tubing with a T-SCD weld, (2) a CompoDock S2 weld, or (3) unwelded tubing. RBCs were stored for 42 days and free hemoglobin (Hb) was measured; PLT concentrates were stored for 8 days and CD62P expression was measured, as markers for blood component quality (n = 10 paired experiments). RESULTS: WBC-reduced RBC units had similar hemolysis at the end of storage: 0.47 +/- 0.28, 0.47 +/- 0.35, and 0.49 +/- 0.38 percent of total Hb, for tubing with a T-SCD weld, a CompoDock S2 weld, or no weld, respectively (not significant). CD62P expression of stored WBC-reduced PLT concentrates was not significantly different between the groups: 20.3 +/- 5.1, 19.8 +/- 5.1, and 22.3 +/- 9.8 percent for tubing with a T-SCD weld, a CompoDock S2 weld, or no weld, respectively. CONCLUSION: The quality of blood components, measured as RBC hemolysis and platelet CD62P expression, is not adversely affected by the presence of a sterile connection in the tubing, made by either the CompoDock S2 or the T-SCD.

Genetic analysis of the low critical sterility temperature point in photoperiod-thermo sensitive genic male sterile rice.

It has been a long haul but photoperiod- and thermo-sensitive genic male sterile (PTGMS) rice has not been freely used in hybrid rice production because there are two perplexing problems corresponding to the critical sterility temperature point (CSTP): the uncertainty of the CSTP segregating pattern and the instability of CSTP for every originally useful line. N5088S, the most widely commercialized japonica-type PTGMS line in China, also saw that its CSTP variants have been isolated but with all other agronomic characteristics unchanged. In this report we analyzed the genetic basis of CSTP, by employing the iterated expectation and conditional maximization (IECM) algorithm on four tiller-splitting-formed sets of seven generations from N5088S and its CSTP-variant H5088S, each set treated with one temperature regime. The main results indicated that there are two dominant major genes and polygene, as well as their respective epistasis conditioning the CSTP in the 23.5 degrees C regime. Based on the results obtained, the strategy for breeding of PTGMS lines with stable low CSTP was outlined.

[Successes in the diagnosis and therapy of sterility at a regional hospital. Contribution on the organization of a sterility counseling service].

On base of diagnostical and therapeutical possibilities of a district hospital the results in the treatment of the sterility with 194 female patients were analysed. In 36,6% of all cases the treatment ended successfully. Except the general and special gynaecological examinations there were diagnostical arrangements as the simple check of the ovarial function and of the passage of tube, as well as fertility estimation of the sexual partner. In addition to the general medical advice there were therapeutically carried out vaginal sanitation, hormone therapy and the pertubation as well as ovulation induction. By experience there will be worked out a first draft for carrying out sterility hours of consultation. After this a "basic diagnostic" and a "basic therapy" will be made in the bounds of possibilities of the district hospital. Indications for the transfer of patients to special hospitals for further treatment are represented.

[Questions about re-sterilization and reuse of sterile disposable devices].

Authors deal with the problems of restilization and reuse of sterile disposable devices on the basis of literary data, their own experiences, and the standpoints offered on the Fourth Symposium of the Canadian Office for Health Technology Assessment. They state, though the reuse of sterile disposable items is possible to work out, it needs a very sophisticated technology, its thriftiness is often doubtful, and the related legal and ethical problems are mostly unsettled. The personal, material and legal preconditions assuring a safe reuse do not exist in Hungary at present.

[Survey of using conditions of sterilized and non-sterilized disinfectants in Kyushu University Hospital].

We surveyed the using conditions of sterilized and non-sterilized disinfectants in Kyushu University Hospital by questionnaires and direct hearing. The kinds and concentrations of disinfectants used were generally proper for their respective purposes. However, there were also some inappropriate cases, such as the cleansing of mucosal membranes with chlorhexidine (Hibitane) gluconate and uses of the concentrations different from those described in the package insert. The present survey was very useful for proper uses of disinfectants.

[The implantation of lyophilized and gamma-sterilized allogenetic incus and of lyophilized and gamma-sterilized xenogenetic malleus (calf) in rabbits and humans (author's transl)].

The histological reactions to the implantation of allogenetic and xenogenetic lyophilized and gamma-sterilized material into the middle ear of rabbits and men is described. In contrary to the behaviour of allogenetic material the xenogenetic shows a clear minor tendency of revitalisation. The reason is the antigen-antibody-reaction which takes place at the capillary network of the donor and leads to a hylinosis with following obliteration of the vascular rete. Therefore the angiogenetic osteogenesis cannot develop. The clinical usuability of such material is discussed.

Application of electrolyzed acid water to sterilization of denture base part 1. Examination of sterilization effects on resin plate.

Bactericidal activities of electrolyzed strong and weak acid waters for acrylic denture base resin were evaluated in order to discuss the applicability of these waters for sterilization of denture base. Only 1-minute immersion in the electrolyzed strong or weak acid water could completely eliminate the attached bacteria, Staphylococcus aureus 209P, on the resin plate. When the resin was relined with tissue conditioner, 5-minute immersion or 1- to 2-minute ultrasonic cleaning reduced the number of the bacteria from 10(5)/cm2 level to 10(1)/cm2 and no surviving bacteria could be detected after 10-minute treatment. These findings suggest that both the electrolyzed strong and weak acid waters are well applicable to the disinfectant for acrylic denture base showing excellent bactericidal activities in a significantly shorter treatment as compared with the conventional denture cleaning.

The "sterile" debate: the effects of radiation sterilization on polymers.

All medical devices have to be sterilized, but the process of killing bacteria and other organisms can also have some deleterious effects on the devices themselves. This article discusses the mechanisms by which gamma irradiation can cause polymer damage and the potential consequences of this.

Tubal sterilization with Filshie Clip. A multicentre study of the ICMR task force on female sterilization.

Female sterilization by the application of Filshe Clip Mark IV has been studied, on a prospective multicentric basis in 869 women for a period of two years. Sixty involuntary pregnancy following ligation were reported giving a failure rate of 8.6 per 100 women at the end of 24 months. Method failures were observed to be evenly distributed throughout the period of observation except in puerperal ligation in whom majority of the pregnancies occurred after six months following ligation. Failure rate was significantly higher in post-abortal ligations as compared to interval ligations. Due to unexpectedly higher failures rate further recruitment of study subjects was discontinued. Information on the status of the clip was available in only 30 of the sixty failures and the clips were in situ in only 4 cases, indicating improper application of clips. In view of high failure rates attributable to improper clip application, the method does not appear to be suitable for programmatic conditions in our country.

[Sterilization in family planning. The psychology of voluntary sterilization (author's transl)].

What are the motives, what effects must be considered when voluntary sterilization is undertaken for reasons of family planning? About 70 psychological/psychiatric serial follow-up histories available are evaluated. The prognosis, psychic consequences and side effects of the operation are discussed. It is shown that the psychological prognosis can be markedly improved by a well-considered decision following a thorough explanation.
