[Roentgenologic assessment of articular lesions in rheumatoid arthritis].

The article presents a comparative analysis of methods for quantitative assessment of roengenologic changes in the joints in rheumatoid arthritis that have come to be widely used in USA and Europe. It is noted that the Steinbroker's method widely employed by clinicians fails to give an integral assessment of the articular affection. Expediency is documented of employment of the Sharp's method in roentgenologic monitoring in clinical trials of new antirheumatic drug preparations. In a routine clinical setting the Larsen's method or its modifications are to be preferred since in spite of its low sensitivity investigation time is cut down 2.3-fold by comparison with the Sharp's method.

[Determining the cause of clicking from clinical, roentgenological, and histological results (author's transl)].

A cause of clicking is described. Results of clinical, roentgenological, and histological examinations show that clicking may be considered to be due to overstraining of the lateral ligament of the temporomandibular joint or of a muscle lying between the tuberculum articulare and the collum mandibulae, respectively. Straining is triggered by a connective-tissue and bony protuberance produced by mechanical irritation on the lateral capitulum mandibulae.

[Results of roentgenological diagnosis for revelation of patients with high surgical risk].

Examination of 1142 patients with application of roentgenological method was conducted, the increase of quantity of patients with complicated thoracoabdominal diseases was established. Authentic predominance of patients with severe disease aged 40-60 years old and older was determined. Correlative connection between age of patients and the number of cases in autumn was established. Specialized help improvement with attraction of qualified professionals was recommended, taking into account the severity of the patients state, appreciating risk of surgical intervention.

[Roentgenological characteristic of pulmonary metastases in thyroid cancer in respect of its biological behaviour (author's transl)].

In metastases of the lung which must be considered as typical in cancer of the thyroid, it is possible to differentiate roentgenologically between fine and coarse nodules, the limit being approximately 10 mm. Metastases consisting of fine nodules are by no means to be considered as representing a fatal generalisation, whereas metastases with coarse nodules are frequently accompanied by other metastases distant from the primary tumour.

Roentgenologic-pathologic correlations of miliary pulmonary metastases.

To determine the accurate localization of nodules of miliary pulmonary metastases within the secondary lobules, a Roentgenologic-pathologic study was made, using an inflated and fixed lung that was excised at autopsy from a patient who died of small cell carcinoma of the left lower lobe. Nodules in the perilobular area and nonperilobular areas were counted using 1-mm-thick specimens and their radiographs. It was found that 78% of the metastatic nodules were located in the perilobular area and 22% in the nonperilobular area. Furthermore, 13% of the metastatic nodules in the perilobular area showed tumor thrombosis in the lymphatic vessels, while none of the metastases in the perilobular area showed any tumor thrombosis. These results suggest that miliary pulmonary metastases are predominantly distributed in the perilobular area and that this finding is helpful in the diagnosis of this disease when high resolution computed tomography is used.

[Roentgenological and pathological studies on the development of discitis in canine models].

Discitis was experimentally induced in 42 dogs by intradiscal injections of bacterial suspensions and sequentially studied by X-rays and histopathology up to 24 weeks. 1. A narrowing of the serpentine intervertebral disc space was seen roentgenologically in the Pseudomonas and E. coli groups. 2. Histologically, the acute inflammation began to subside in eight weeks, at which time new bone formation started to appear, and fusion of the adjacent vertebrae became apparent in eight weeks. The degree of the disease process was more advanced in the Staph. aureus group and less severe in the Pseudomonas group. The E. coli group lay in between. 3. The inflammatory process seemed to be confined in the disc for a week after the injection, during which time the cartilagenous cells in the nucleus pulposus underwent atrophy and degeneration. This resulted in direct exposure of the cartilagenous plate to the infection, causing invasion of the inflammatory process into the vertebral body. 4. The presence of the epiphyseal line, however, seemed to act as a barrier to hinder the inflammatory process invading the vertebral body.

[Significance of roentgenologic indications of osteogenesis in the choice of tactics in distraction osteosynthesis using G. A. Ilizarov's method in rigid pseudarthrosis].

On the basis of 274 observations the authors stated the conception of staged roentgenologic course of the osseous tissue regeneration in case of close distraction of rigid false joints according to G. A. Ilizarov. In line with visual appraisal of roentgenograms there were used roentgenometry and densitometry. There are described in details the stages of reparative regeneration and in accordance with them are given the tactical recommendation as to the selection of the distraction rate and its duration, the terms of loading and immobilization of fractures by means of perosseous apparatus. The tactics of the patient control with regard to the peculiarities of osteogenesis in the process of distraction allowed to decrease the number of failures in treatment of false joints by 2 times.

[Roentgenological diagnosis of dystrophic bone cysts in children].

On the basis of examination of 58 children with diagnosed dystrophic osseous cysts there were revealed new aspects of roentgenologic characterization of dystrophic osseous cysts in children. It has been stated that high-structure roentgenography with primary magnification (apparatus Rays-D) allows to make more accurate an anatomical structure of the osseous cysts, nature of outlines as well as the state of adjacent sections of bone. Development of osseous cysts in considerable number of cases combines with the manifested symptoms of dystrophy in the surrounding osseous tissue (osteoporosis, loss of architectonics and presence of cyst-like formations).

[Clinico-roentgenological symptoms of acute adhesive intestinal obstruction].

The clinico-roentgenological signs of acute commissural ileus were analysed in 226 patients. Of them, 192 were operated on. In 6.4% of the patients in plain roentgenography, the signs of acute ileus were absent, in 27.7%--they were not prominent. Study of the barium suspension passage through the small intestine, as the authors consider, is indicated in unmarked clinical features of the disease, and as well in its short duration (less than 6 h) with the aim of establishing a degree of ileus (complete, or partial).

[Roentgenological manifestation of ossification of the posterior longitudinal ligament in the cervical spine causing severe spinal canal stenosis--a group comparison with and without marked spinal cord dysfunction].

Relationship between the degree of clinical symptoms and the lateral roentgenograms of the cervical OPLL causing severe spinal canal stenosis was studied among patients whose roentgenological minimum antero-posterior diameters of the available spinal canals were under 9mm. Lateral roentgenograms of 28 clinically mild patients were compared with those of 20 clinically severe patients. The type of ossification, the extent of OPLL, the narrowest level of the available spinal canal, and the minimum a-p diameter were not statistically significant parameters. The degree of canal stenosis at the 1cm cranial and 1cm caudal levels from the narrowest level, and the axial extent of canal stenosis around the narrowest level were less significantly in the group with mild symptoms than in the group with severe symptoms.

[Clinico-roentgenological characteristics of the changes in ischemic Volkmann's contracture].

In the article is presented the study of clinical aspects and roentgenograms of 91 children with Volkmann's contraction. The causes of contraction with 46 patients were supracondylar fractures of arm and with 32--forearm bone fractures. All fractures were associated with massive injures of soft tissues, some of the persons have injures of brachial artery. Tardy or incompletely dissected circular plaster bandage turned out to be a factor of complication. All patients demonstrated typical clinical aspects of acute circulatory disturbance of extremity, in some of them with skin and subcutaneous fat necrosis. Clinical aspects of contraction during residual period were notable for variety of manifestations and depended upon the depth of tissue affection (muscles, tendons, articulations, vessels, bones). The gravity of disease has been aggravated by affection of nerve trunks (66 patients). Roentgenologic picture revealed changes of mutual positioning of articular ends of jointed bones and trophic changes within the bones. Trophic changes were revealed by osteoporosis of different extent, growth disturbances as well as perversion of ossification rate of epiphysis and apophysis of tubular and carpal bones.

[Studies on the roentgenological differentiation between complex and simple bowel obstruction].

We studied 81 cases of organic bowel obstruction. In complex obstruction 7 cases out of 25 (28%) were diagnosed on the plain film before operation, although in simple obstruction 54 cases out of 56 (96%). But in retrospective evaluation of the plain films, we noticed that in some cases of complex obstruction, marked small bowel obstruction and ascites were seen from the onset, which were never seen in simple obstruction. We considered this finding as the key point for the roentgenological differentiation between complex and simple obstruction.

[Clinico-roentgenologic diagnosis of disseminated and diffuse lung diseases].

The results of clinical and x-ray investigations of 398 patients with disseminated pulmonary lesions were analyzed. Two groups were identified: patients with interstitial type of lesions (101) and patients with focal type of lesions (297). Proceeding from roentgeno-morphological correlations, the authors presented specified roentgeno-semiotics of 10 nosological entities with the syndrome of pulmonary dissemination. The possibilities of optimization of differential diagnosis of disseminated pulmonary lesions were shown basing on combined (clinical, functional and roentgenological) investigation.

[Clinico-roentgenologic diagnosis of neck lipomas].

The results of clinical roentgenological investigations of 52 patients with cervical lipoma were analyzed. The authors describe the radiographic appearance and the investigation techniques for circumscript lipoma in the subcutaneous tissue, in the muscles and for a large diffuse symmetric lipomatosis. In the diagnostics of cervical lipoma x-ray investigations have a leading role.

[The roentgenologic diagnosis of cor triatriatum sinistrum].

Varied in compliance with the presence and size of the septal foramen, existence and location of the atrial shunt and abnormal pulmonary venous connection, the haemodynamic status of the reported 10 cases of cor triatriatum sinistrum might be classified into four types: Pulmonary venous hypertensive, polyemic, cyanotic polyemic and normal. Accordingly, the roentgenologic features of this anomaly manifested differently and mimicked mitral stenosis or atrial septal defect.

[Clinico-roentgenologic characteristics of patients with pulmonary tuberculosis and damage to segment VI and results of their treatment].

The clinicoroentgenological pictures of pulmonary tuberculosis located in segment VI in 90 patients and the results of their treatment were analyzed. Among the clinical forms, only infiltrative tuberculosis (88.9 per cent) and tuberculoma (11.1 per cent) were diagnosed. In 92.7 per cent of the patients the destruction phase was stated. Pneumonia was detected in almost half of the patients with acute and subacute processes of the disease. Tuberculosis in them was confirmed within the periods from 1 to 6 months. This is indicative of late diagnosis in some of them. Diagnostic mistakes were mainly associated with the features of the process localization and defective bacteriological and roentgenological examinations since the clinical signs and histories were typical of tuberculosis. The treatment results were estimated in 79 patients who stayed in hospital for at least 2 months. Closure of destructive cavities was observed in 73.7 per cent of the patients against 81.1 per cent in the control group. Abacillation was recorded in 96.5 per cent of the patients.
